Employee was beating a bearing off the shaft on the dryer with a sledge hammer while prying with a bar. While beating the bearing a piece of metal went into EE's eye.

Checking damaged wire on transformer balcony. Arc flash occurred. Fell off platform to the ground below it. Approx 3 feet.

Three men attempting to remove a large rock from raw coal belt. Two were lifting the rock, one was using a slate bar to pry. Slate bar slipped from beneath the rock striking one of the lifters in the left eye.

On 9/18/2019 a fire was discovered at the clean coal bin. This was an off shift and no employees were on property. Mingo 1 Plant only operates on the day shift. There was a security guard on site at the time the fire was discovered. Fire department was called and the fire was extinguished. no persons were injured.

Employee stated that EE was attempting to unlatch EE's seat belt when the seat belt buckle struck EE in the mouth chipping EE's left front tooth. The employee had EE's tooth fixed the next day, but has not missed any scheduled work due to the incident.

EE was climbing up hill and felt sharp lower back pain on the right side of spinal area.

Employee was closing door latches on empty rail road cars felt tear in left bicep.

Bent down and felt something pop in his knee.

Occupational Pneumoconiosis

Truck driver lost control / brakes while traveling down grade on slate haul. Driver went into ditch below the No. 6 curve bursting the slurry line and damaged truck.

EE was checking the hydraulic oil level; he lost his balance and fell from the dozer platform.

Employee bent over, to move a dozer roller, and felt something pull loose in his chest, in the front, on the right side.

Occupational hearing loss.

Employee was walking towards warehouse slipped on ice covered walkway twisting left ankle.

Employee was helping to unload chemicals from tractor trailer, driver dropped pallet on right hand.

Injured employee was stepping off bulldozer and twisted foot on dozer track, pulling a ligament in his right knee.

Taking gate up on #2 and #3 bin, slipped and struck right knee on rail.

Employee was removing a bearing from a centrifugal dryer when the bearing slipped, employee caught the bearing resulting in a cut that required stitches.

Injured employee was removing the chute from a coal dryer and twisted his right knee.

Injured employee was operating an excavator on the slate. He swung the bucket around and caught a rock and jerked his lower back.

The driver backed up in stockpile to dump coal and turned over.

Tripped over crossbeam support and while falling reached for beam to stop fall, resulting in pulled muscle in right arm.

Injured emplyee was carrying a barrel, stumbled, almost dropped the barrel and injured shoulder and neck.

As he was leaving office slipped & fell down steps & hurt back.

EMPLOYEE WAS WALKING DOWN STEPS TO CHECK ENGINE OIL, HIT KNEE ON ACCESS PANEL HANDLE.

LIFTING SAMPLE BAG HE STRAINED HIS BACK.

ANOTHER EE WAS USING AN AIR MAILER AND HIT A GLANCING MAIL ON WOOD FRAME, THE STRAY NAIL FLEW THOUGHT THE AIR AND STRUCK EE ON RIGHT CHEEK ON HIS FACE.

EE WAS DIAGNOSED WITH CARPAL TUNNEL SYNDROME 5-23-01. DOI SHOWN BY WORKERS COMP. EE THEN HAD SURGERY ON 10-22-01. HE WAS OUT OF WORK FROM 10-22-01 THRU 11-5-01 FOR SURGERY ON LEFT ARM.

KNOCKED PIN OUT OF DOZER ARM, PIECE OF METAL FLEW OUT AND HIT HIM IN THE CHEST, CAUSING A CUT.
