Struck by flying object

Employee was beating a bearing off the shaft on the dryer with a sledge hammer while prying with a bar. While beating the bearing a piece of metal went into EE's eye.

Flash burns (electric)

Checking damaged wire on transformer balcony. Arc flash occurred. Fell off platform to the ground below it. Approx 3 feet.

Three men attempting to remove a large rock from raw coal belt. Two were lifting the rock, one was using a slate bar to pry. Slate bar slipped from beneath the rock striking one of the lifters in the left eye.

Accident type, without injuries

On 9/18/2019 a fire was discovered at the clean coal bin. This was an off shift and no employees were on property. Mingo 1 Plant only operates on the day shift. There was a security guard on site at the time the fire was discovered. Fire department was called and the fire was extinguished. no persons were injured.

Struck by flying object

Employee stated that EE was attempting to unlatch EE's seat belt when the seat belt buckle struck EE in the mouth chipping EE's left front tooth. The employee had EE's tooth fixed the next day, but has not missed any scheduled work due to the incident.

Truck driver lost control / brakes while traveling down grade on slate haul. Driver went into ditch below the No. 6 curve bursting the slurry line and damaged truck.
