June 2nd, EE was working with another person on the bottom of a Cat 785B truck. The bed was turned upside down exposing the bottom side of the bed. EE was walking down the length of the bed & was stepping over of the bolsters. Due to moisture on the bed, EE's foot slipped & EE fell awkwardly backward on right leg.

Employee stated while installing a left hand turbo on an off road truck they felt a sharp pain in right shoulder didn't see a physician until 3/27 and was placed on light duty - non available off work as of 3/28/18

The truck the injured employee was operating, was being loaded and dust came through the window that was rolled down halfway, and employee felt dust go into left eye.
While removing a cutting edge. The employee was pushing a carriage nut back into the slot that keeps it from turning with left hand while the impact was still on the bolt, EE was attempting to pick up on the impact with right hand when inadvertently activated the impact causing the nut to rotate at a high rate speed resulting in a laceration to left middle finger.

On 9/7/2012 Employee lost balance while on a 5 foot step ladder and jumped to the ground, landing on his feet. He continued to work until 2/11/2013 when he was taken off for surgery to repair ligament damage to his knee.
Employee was lifting an acetylene tank to place back into storage area and felt pain in his back. He continued to have pain and on 6/14/2012 he sought medical treatment and was prescribed physical therapy.
Employee was operating a DllR dozer in shot material, while backing up he trammed over a rock causing a pain in his neck. Employee continued to work until 2/6/2012 when he was take off work due to increase in pain.

MEMBER WAS IN THE PROCESS OF LIFTING A 5-GALLON BUCKET OUT IN THE SUPPLY YARD WHEN HE SLIPPED & FELL. AS HE FELL MEMBER REACHED & GRABBED A PIECE OF SHEET METAL TO CATCH HIMSELF RESULTING IN A LACERATION TO HIS MIDDLE FINGER ON HIS RIGHT HAND.
EE WAS WORKING ON A FOOT SHAFT WHILE THE MINER WAS DOWN. HE WAS UNDER THE HEAD ABOUT 3:00 A.M. WHEN HIS LOWER BACK BEGAN TO TIGHTEN UP. HE CONTINUED TO WORK UNTIL IT WAS UNBEARABLE AROUND 5 A. M. HE LEFT THE JOB WITHOUT FILLING OUT AN ACCIDENT REPORT. AFTER TALKING TO HIS CREW THERE WAS NO INCIDENT LEADING TO THIS SITUATION.
EMPLOYEE WAS HELPING LOAD BLAST HOLES WHILE OPERATING THE HYDRAULIC CONTROLS, A HOSE RUPTURED SPRAYING THE EMPLOYEE WITH FLUID. HE REACTED BY JUMPING AWAY FROM THE TRUCK RESULTING IN A STRAIN OF HIS LEFT HIP. SAW PHYSICIAN ON 8/5/02.

EE WAS IN THE PROCESS OF HOOKING UP AN ADD CAR WHEN HE SLIPPED ONT HE WALKWAY SURFACE OF THE LAUNCH PAD. WHEN EE FELL HE LANDED ONT HE LEFT SIDE OF HIS BODY INJURING HIS LEFT ELBOW.
CHANGING TIRE ON CAT ROCK TRUCK AND TOOK THE WHEEL APART AND WAS ROLLING THE (SIDE RING) WHEEL PART AND IT HAD A BURR STICKING OUT AND CUT THE RT. HAND PALM AREA.
EE WAS HOLDING UP A CABLE WHILE STACKER BELT WAS BEING PUSHED INTO PLACE. THE STACKER BELT HIT A SET OF STEPS. THE STEPS STRUCK AND LACERATED HIS KNEE, REQUIRING 8 STITCHES. NOTE: EE COULD HAV E RETURNED NEXT SHIFT. HE WORKS 6 DAYS AND OFF 3. HIS SCHEDULED DAYS OFF WERE 4-2-02, 4-03-02 AND 4-4-02. HE RETURNED NEXT SCHEDULED SHIFT ON 4-5-02.
EMPLOYEE WAS PUTTING MOTOR IN AN ADD CAR. IN THE PROCESS HE WAS PRYING WITH A SLATE BAR. AS HE WAS PRYING THE BAR SLIPPED CAUSING HIM TO TWIST HIS RIGHT WRIST.
MEMBER WAS HELPING COMPLETE A BELT SPLICE IN THE BELLY BELT OF THE HIGHWALL MINER. HE WAS UNDER THE LAUNCH ON HIS KNEES WHEN HE FELT PAIN AS HE STOOD UP.

MEMBER WAS IN PROCESS OF ADDING ANTIFREEZE TO THE HIGHWALL MINER GENERATOR WHEN HE TOOK THE CAP OFF CAUSING HOT ANTIFREEZE TO HIT HIM IN THE FACE CAUSING MILD BURNS.
MEMBER STUMBLED AND HIT HIS HEAD AGAINST THE SIDE OF A DML DRILL KNOCKING HIS HARD HAT OFF CAUSING A LACERATION TO THE SIDE OF HIS HEAD.
EE WAS CHECKING AND CLEANING ADD CARS OF HIGHWALL MINER. A PIECE OF ROCK FELL FROM HIGHWALL AND STRUCK HIM ON THE HEAD.
VICTIM WAS REPAIRING SHOVEL BUCKET. WHEN HE BEGAN TO DISMOUNT HE SLIPPED AND FELL TO GROUND BREAKING HIS LEG.
