While taking a flat car around a curve, the car derailed. Injured attempted to uncouple the car but it was in a bind and would not uncouple. Injured used a slate bar to pry the pin loose. Draw head uncoupled violently and caused the slate bar to strike him on the right side of his face and neck

An unintentional roof fall has occurred at the Old #6 Headings #5 Entry spad 6538. The fall was approximately 20ft x 20ft x 6ft. No injuries were reported and no equipment was involved.

While hooking up cathead, Employee had right hand under cathead. When moving cathead to latch, the latch pin bent and caused cathead to smash finger between cathead and power center (Fracture - Seven (7) Sutures).

Employee was operating a Damascus 3-wheeler when the wheel hit the track rail causing the steering wheel to spin out of control. Employee tried to hold onto steering wheel injuring left wrist and thumb. Employee was treated at MedExpress.

Struck against a moving object

Employee was tramming a scoop when he ran over a crib block. The impact resulted in pain to his lower back. He had previously had back surgery to the same area to repair three discs.

Over-exertion in pulling or pushing objects

Injured was pulling a piece of belt structure from the supply hole. The area was wet and muddy and the structure was tangled. His feet stuck in the mud while pulling and he felt pain in his back.

Beltman/fireboss was helping men calibrate the scales on the South Portal punchout tail piece. He was using a 3/4"" open end wrench to remove a bolt. The bolt came loose suddenly and he lost control of the wrench. It struck his right cheek bone resulting in a contusion. He is off work for treatment.

Electrician was bending over the side of a Eimco scoop and changing the oil filter when he felt a strain in his lower back. He sought medical attention on the following day. Doctors gave him time off for a strained back.

Employees were changing out a cross under guard on the #5 Belt at break 18. When taking the guard loose, a belt roller bracket snapped off, pinching the end of one employee's right index finger. The cut required six stitches. He missed no work.

An electrician was walking down two steps from the mine office when he twisted his left ankle. He did not slip or fall. He underwent surgery at CAMC to repair a broken tibia and fibula at the ankle.

Two workers were cleaning gob. Co-worker picked up a rock about 12""x12"" to throw into scoop bucket when a larger rock (3'x2'x5"") slid from top of the 4' pile onto injured's right hand. He received lacerations to the middle and ring finger. He refused medical treatment & finished his shift. He would not fill out a report and didn't report to work the next day.

Over-exertion in lifting objects

Electrician was replacing a wheel unit on the Low-Trac. He lifted the wheel unit to install in and felt pain and a protrusion in his lower abdomen. He was diagnosed with a hernia and surgery is scheduled. He has continued to work.

Engineering rodman was climbing an incline covered with heavy grass and weeds. Grass was wet. He was cutting brush with a machette and slipped. The blade hit his left knee. Laceration required eight stitches.

EE was helping move a new lose shuttle car U.G. The men were pulling cable out of the conveyor of shuttle car when they reached their destination extra parts from Joy were stored in conveyor in plastic. EE picked up one to shift off cable and the parts inside smashed his ring finger.

Area was in with 4' resin bolts. 6 cribs had already been installed prior to fall. Ventilation and passage were not affected. State and Federal agencies were notified immediately.

While loading waterline into scoop bucket, employee lost his footing and grabbed the scoop bucket for balance. At that time, the scoop operator pulled the ram in and mashed the middle finger of left hand. He was wearing gloves. He did not stop working and sought medical aattention later. He received stitches on the end of his finger.

EE was helping to put a mantrip in a side track. The trip was jumping the point rail. Operator backed the mantrip up and the switch handle sprang up hitting him on the right knee. He continued to work. (This was reported as file only and in July 2010 he had orthoscopic surgery on knee).

An unplanned roof fall occurred in the #2 entry in the Big Daddy Mains, 75' inby Spad 634. Fall measured 320' long x 20' wide x 8-10' thick. No equipment or injuries, passage of person was affected. Area was developed 11/02 and roof support consisted of 4' resin bolts. State and Federal agencies were notified immediately.

Struck by falling object

Member was working in the #4 entry in the intake near the elevator when he tripped and grabbed top brow as he fell, a rock measuring 32"" X 21"" X 8"" thick fell and struck him in the lower back / pelvic area.

Fall from machine

Exiting a 15 ton Brookeville locomotive in an area where the track was approx 48' high from bottom of floor, as he climbed out, he stepped on loose blocking causing him to lose his footing. Tried to catch himself & landed on both hands, injuring his middle finger of right hand. Refused any med treatment & continued to work his shift.EE advised his finger was fractured.

At approximately 2:15 pm a roof fall was discovered in track entry of slope, 11,000' from section, measuring 60' long, 22' wide and 8' thick. Ventilation was not affected, but passage of men had to re rerouted. Area originally mined in 2001. Fall cleanup plan posted using 12/12 miner, supervision with certified personel, no apprentice miners.

An unplanned roof fall occured 1 Bk. inby spad 304, # 3 entry adjacent to Glory Hole # 1 belt. Fall was 6000 ft from the face. Fall measured 20 ft length, 20 ft wide and 5 ft high. Roof was supported with 48 in resin bolts. Mixed stratam no coal rider seam present. Fall was breakered off, not rehabilitated. WVOMSA inspector discovered fall while checking belts.

EE was helping to chain hoist a new ranging arm jack to the top of the shear. He had his foot inside the frame trough. The chain hoist slipped on it's load and employee jumped out of the way. In doing so, he twisted his right ankle resulting in fractures to his tibia and fibula.

While checking a gob switch at the glory hole, employee put his hand on a metal guard over the dump roller. The guard had a hole worn in it which cut the tip off his finger. Received six stitches. No bone loss.

Employee was operating a dozer on B-1 stockpile when he backed up too far and his dozer backed over a five foot wall into the ditchline. Doctor tole him he had spurs on his vertebrae due to occupation. Doctor on 9/23/08 said he had contusions & scheduled therapy.

EE was walking around the track motor at IA switch. The area was wet and muddy, EE slipped on a stick causing him to cut his small finger on jagged rock. EE refused medical treatment, but sought medical attention later, he recieved six sutures. EE worked his next regualr shift.

EE was putting panels on top of overcast walls while sliding the panels together, EE's left hand was caught between two panels causing a laceration to his left thumb, requiring 7 stiches. Accident accurred at the mouth of headgate 25.

Employee stated he was attempting to lift one side of 3 top rollers banded toogether, when he felt pain in his lower back, resuting in a strained lumbar muscle.

Due to continued pain resulting from a mantrip accident that occurred on 12/03/2007, EE went back to the doctor on 03/21/2008 and was diagnosed with two ruptured discs C-5 & C-6. He is currently off attending therapy 3 days a week and awaiting approval for surgery.

Member was hanging fire hose over discharge roller, when he cut his little finger on his right hand, on a guard that covers shaft. Member received 5 stitches on little finger on his right hand.

Employee's injury is the result of continuing pain caused by a mantrip accident that occurred on July 27, 2007. Due to increased pain he required surgery to repair tendon in his left shoulder. He was off from 3-4-2008 to 3-24-2008.

Member was working with the outby crew to lead 5'belt from the roll into a sled that was on a longwall car. When the pulled the car, member stepped up onto the car & proceeded to ride the car through the Aframe belt stairs. His left foot & leg were caught between sled & aframe.

Member was operating a 15 ton motor pulling shield cars outside, when he lost control of the motor & wrecked causing a laceration to his head which needed 5 staples to close.

EE was fueling up a welder when gasoline splashed on the hot manifold and ignighted the welder which was mounted on a truck. The welder and a 1989 Ford 350 4X4 pickup burned to the ground.

Employee was waiting for crew to bring in a set of doors to install. He heard motor approaching. As he got up, he stepped on a piece of cinderblock and twisted his ankle.

Employee was located on the offside of the Fletcher Roof Ranger II when a piece of rock fell from between the last row of bolts & the ATRS. The rock slid down the canopy & struck the EE on the right leg. Rock measured 18""X 14""X 4"".

Member was in the process of advancing shields, had hooked up 1"" pull chain to drag shield forward, when one of the hooks bent, causing chain to ties and strike him in the right arm and side. He received a compound fracture which required surgery to correct.

Member was walking down the steps of an overcast when he stepped on a broken piece of cinder block that was at the base of the steps, causing him to twist his right ankle.

Member had his left hand positioned on the belt rail and was reaching for the sheave rope with his right hand while pulling slack out of the take-up when the rope became hung between top structure and bottom roller. Bottom roller moved causing his middle finger to be pinched between roller bracket & top rollar causing laceration that was sutured.

Fall from machine

Member was standing on scoop batteries to hang bolter cable in the #2 entry at spad #4531. As he was climbing down off scoop, right knee gave out causing him to fall and twist left ankle. Member continued to work until 5/23/06 when due to swelling, he was told to remain off work to see specialist.

EE had just cleaned his knife blade in his utility knife, bent down to help electrian when his knife blade cut his wrist, knife was hanging off his belt by a clip and apparently the blade had slipped out. Sutured with eight stitches.

member & 2 0thers were unloading a piece of track rail approx 6'long off of a car, when as it was coming off it struck a longer rail causing it to come off the car and bounce mashing his left index finger in between the 2 rails. Member fractured tip of index finger and rec'd 7 stitches.

Member was removing backboard from face pan on longwall. Backboard slipped out of his hand and pinched his left middle finger against pan, causing laceration to middle finger which needed 2 stitches to close.

EE was in the process of drilling an 8 ft hole in the mine roof. As he was putting the 4 ft finisher and pushed together, the steel bowed as he raised the drill pot causing the steel to spring apart and strike the EE in his right thumb. X-rays revealsed he dislocated his thumb, but there were no fractures. Lacertaion was sutured closed with 3 stitches.

Member had just finished re-fueling the IT28 endloader and was in the process of climbing back in the operator's compartment. As he opened the door, the door swung back to close. He was reaching to grab the door to keep it from closing when the door shut catching his left thumb in the door causing a fracture.

Contact with electrical current

EE was helping assist the foreman in hanging a D-Box cable whe EE grabbed the cable he came into contact with a bad place in the cable and was shocked. EE was checked by Ambulance personell and was deemed okay with no medical problems.

EE was operating Fairchild Scoop and had his L arm outside of the deck with his left hand resting on the stop block, when he turned to the scoop to the right, his middle index finger between the stop block and the frame fracturing his middle finger and a laceration.

Member was operating 35c scoop to clear roadway. Member started to feel shaky, heard a noise, raised up to see if something had fell off the back of scoop. He then passed out and fell out of scoop at which time the scoop rolled over left leg breaking his tibia. He is a diabetic & scoop was on an incline.
