Trying to put pad duster back on track and he reached for a track jack and hurt his back. **Possibly contesting accident**

While taking a flat car around a curve, the car derailed. Injured attempted to uncouple the car but it was in a bind and would not uncouple. Injured used a slate bar to pry the pin loose. Draw head uncoupled violently and caused the slate bar to strike him on the right side of his face and neck

Employee has been diagnosed with Occupational Pneumoconiosis. The safety department received notification of his diagnosis on August 18, 2015.

An unintentional roof fall has occurred at the Old #6 Headings #5 Entry spad 6538. The fall was approximately 20ft x 20ft x 6ft. No injuries were reported and no equipment was involved.

While hooking up cathead, Employee had right hand under cathead. When moving cathead to latch, the latch pin bent and caused cathead to smash finger between cathead and power center (Fracture - Seven (7) Sutures).

Employee was operating a Damascus 3-wheeler when the wheel hit the track rail causing the steering wheel to spin out of control. Employee tried to hold onto steering wheel injuring left wrist and thumb. Employee was treated at MedExpress.

A timber fell striking employee's left hand and mashing his ring finger against belt rail resulting in 8 sutures.

An unplanned roof fall occurred on 6A belt in the #5 entry. It fell in an intersection measuring 20' long X 20' Wide X 3 1/2 X 4 feet high.

Employee was tramming a scoop when he ran over a crib block. The impact resulted in pain to his lower back. He had previously had back surgery to the same area to repair three discs.

Injured was pulling a piece of belt structure from the supply hole. The area was wet and muddy and the structure was tangled. His feet stuck in the mud while pulling and he felt pain in his back.

Beltman/fireboss was helping men calibrate the scales on the South Portal punchout tail piece. He was using a 3/4"" open end wrench to remove a bolt. The bolt came loose suddenly and he lost control of the wrench. It struck his right cheek bone resulting in a contusion. He is off work for treatment.

Electrician was bending over the side of a Eimco scoop and changing the oil filter when he felt a strain in his lower back. He sought medical attention on the following day. Doctors gave him time off for a strained back.

Employees were changing out a cross under guard on the #5 Belt at break 18. When taking the guard loose, a belt roller bracket snapped off, pinching the end of one employee's right index finger. The cut required six stitches. He missed no work.

Area was developed 6/2011 and roof support consisted of 5' Torque Tension roof bolts Fall Measured 20'LX20'WX6'High. fall will not be rehabilitated. Federal and State Agency's were notified immediately.

An unplanned roof fall occurred in the #6 entry 100' inby spad #998. This fall is 6500 feet from active face. Fall measured 20' long x 20' wide x 5' thick. No equipment or injuries. Roof fall did not affect ventilation, passage of persons was affected. The fall will be rehabilitated. Area was initially supported with 48"" resin bolts.

An electrician was walking down two steps from the mine office when he twisted his left ankle. He did not slip or fall. He underwent surgery at CAMC to repair a broken tibia and fibula at the ankle.

Two workers were cleaning gob. Co-worker picked up a rock about 12""x12"" to throw into scoop bucket when a larger rock (3'x2'x5"") slid from top of the 4' pile onto injured's right hand. He received lacerations to the middle and ring finger. He refused medical treatment & finished his shift. He would not fill out a report and didn't report to work the next day.

Electrician was replacing a wheel unit on the Low-Trac. He lifted the wheel unit to install in and felt pain and a protrusion in his lower abdomen. He was diagnosed with a hernia and surgery is scheduled. He has continued to work.

Engineering rodman was climbing an incline covered with heavy grass and weeds. Grass was wet. He was cutting brush with a machette and slipped. The blade hit his left knee. Laceration required eight stitches.

EE was helping move a new lose shuttle car U.G. The men were pulling cable out of the conveyor of shuttle car when they reached their destination extra parts from Joy were stored in conveyor in plastic. EE picked up one to shift off cable and the parts inside smashed his ring finger.

Fall measures 80'L X 20'W X 6-8'H. Area was supported by 48"" resin Bolts. All entries have be breakered off. State/Fed agencies notified.

An unplanned roof fall occured in the #3 entry near spad #107 along #5 belt. The fall measured 30' long, 20' wide and 5' thick. There was no equipment involved. Ventilation and passage of persons was not affected. The fall will be rehabiltated. The fall was breakered off and both agencies notified immediately.

An unplanned roof fall occurred in the #3 entry 40 feet outby spad #113 on the #5 beltline. There were no injuries or equipment involved. Fall area will be rehabilitated. Area was in with 4' resin bolts. 6 cribs had already been installed prior to fall. Ventilation and passage were not affected. State and Federal agencies were notified immediately.

While loading waterline into scoop bucket, employee lost his footing and grabbed the scoop bucket for balance. At that time, the scoop operator pulled the ram in and mashed the middle finger of left hand. He was wearing gloves. He did not stop working and sought medical aattention later. He received stitches on the end of his finger.

An unplanned roof fall coourred in the #5 Entry 1 Break Inby spad 2515 near the Glory Hole. Fall measured 80' long x 20' wide x 8-10' thick. No equipment or injuries. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Area was initially supported with 48"" and 60"" resin bolts. No active section at this location.

EE was helping to put a mantrip in a side track. The trip was jumping the point rail. Operator backed the mantrip up and the switch handle sprang up hitting him on the right knee. He continued to work. (This was reported as file only and in July 2010 he had orthoscopic surgery on knee).

An unplanned roof fall occurred in the #2 entry in the Big Daddy Mains, 75' inby Spad 634. Fall measured 320' long x 20' wide x 8-10' thick. No equipment or injuries, passage of person was affected. Area was developed 11/02 and roof support consisted of 4' resin bolts. State and Federal agencies were notified immediately.

Member was working in the #4 entry in the intake near the elevator when he tripped and grabbed top brow as he fell, a rock measuring 32"" X 21"" X 8"" thick fell and struck him in the lower back / pelvic area.

Exiting a 15 ton Brookeville locomotive in an area where the track was approx 48' high from bottom of floor, as he climbed out, he stepped on loose blocking causing him to lose his footing. Tried to catch himself & landed on both hands, injuring his middle finger of right hand. Refused any med treatment & continued to work his shift.EE advised his finger was fractured.

At approximately 2:15 pm a roof fall was discovered in track entry of slope, 11,000' from section, measuring 60' long, 22' wide and 8' thick. Ventilation was not affected, but passage of men had to re rerouted. Area originally mined in 2001. Fall cleanup plan posted using 12/12 miner, supervision with certified personel, no apprentice miners.

An unplanned roof fall occured 1 Bk. inby spad 304, # 3 entry adjacent to Glory Hole # 1 belt. Fall was 6000 ft from the face. Fall measured 20 ft length, 20 ft wide and 5 ft high. Roof was supported with 48 in resin bolts. Mixed stratam no coal rider seam present. Fall was breakered off, not rehabilitated. WVOMSA inspector discovered fall while checking belts.

EE was helping to chain hoist a new ranging arm jack to the top of the shear. He had his foot inside the frame trough. The chain hoist slipped on it's load and employee jumped out of the way. In doing so, he twisted his right ankle resulting in fractures to his tibia and fibula.

An unplanned roof fall occurred 1200' from the face on #2 section in #4 entry 100' outby spad 5720. The fall measured 20' length, 20' width, and 6.5' in height.

An unplanned roof fall occured one break outby spad 2335 near # 23 Seal. Fall measured 10' thick, 17' wide.

While checking a gob switch at the glory hole, employee put his hand on a metal guard over the dump roller. The guard had a hole worn in it which cut the tip off his finger. Received six stitches. No bone loss.

Splicing conveyor belt and cut hand with belt knife.

Employee was operating a dozer on B-1 stockpile when he backed up too far and his dozer backed over a five foot wall into the ditchline. Doctor tole him he had spurs on his vertebrae due to occupation. Doctor on 9/23/08 said he had contusions & scheduled therapy.

EE was walking around the track motor at IA switch. The area was wet and muddy, EE slipped on a stick causing him to cut his small finger on jagged rock. EE refused medical treatment, but sought medical attention later, he recieved six sutures. EE worked his next regualr shift.

An unplanned roof fall occurred at the #6 belt. Fall measured 50' long, 20' wide and 4' thick. No persons were involved and fall reportedly fell on a belt that was out of service.

An unplaned roof fall occurred in the #4 entry of Spad # 1603. From longwall face to 10 feet outby. Fall measured 10 foot long, 20 foot wide. Ventilation was not affected. No persons or equipment were involved. The fall will not be rehabilitated but should mine through fall in about 3 passes.

EE was putting panels on top of overcast walls while sliding the panels together, EE's left hand was caught between two panels causing a laceration to his left thumb, requiring 7 stiches. Accident accurred at the mouth of headgate 25.

Employee stated he was attempting to lift one side of 3 top rollers banded toogether, when he felt pain in his lower back, resuting in a strained lumbar muscle.

Due to continued pain resulting from a mantrip accident that occurred on 12/03/2007, EE went back to the doctor on 03/21/2008 and was diagnosed with two ruptured discs C-5 & C-6. He is currently off attending therapy 3 days a week and awaiting approval for surgery.

An unplanned roof fall occurred in the #4 entry, left return of old 3 belt. Fall measured 80 ft long, 19 ft wide and 8 1/2 ft high. This area is worked out. No equipment was involved, no personal injuries reported. Ventilation & travelways were not affected. All approaches have been breakered off.

Employee was in the process of cutting a piece of cable when the knife slipped and cut his left index finger.

Member was cutting belt at the tailpiece when knife came out of belt cutting his right knee. About 1"" cut resulted in 3 stitches.

An unplanned roof fall occurred at parallel mains #3 Entry 1 crosscut inby spad #673. No injuries reported and no equipment involved. Fall measured 200' length, 18' width and 4' to 5' height. Fall was in the intake escapeway. Production was stopped unti escapeway was made passable. Area has been rehabilitated.

Member was hanging fire hose over discharge roller, when he cut his little finger on his right hand, on a guard that covers shaft. Member received 5 stitches on little finger on his right hand.

Employee's injury is the result of continuing pain caused by a mantrip accident that occurred on July 27, 2007. Due to increased pain he required surgery to repair tendon in his left shoulder. He was off from 3-4-2008 to 3-24-2008.

An unplanned roof fall occurred at the glory hole mains in the #4 entry at spad #3191. No injuries reported. No equipment involved. Ventilation and passage of person not affected. Fall measured 100' x 19' x 2' to 10'. All entrances to fall have being breakned off. Areaa will be rehabilitated. Fall was 12 ft fromt he active face. MSHA and slate were notified immediatetely.

Member was working with the outby crew to lead 5'belt from the roll into a sled that was on a longwall car. When the pulled the car, member stepped up onto the car & proceeded to ride the car through the Aframe belt stairs. His left foot & leg were caught between sled & aframe.

Member was operating a 15 ton motor pulling shield cars outside, when he lost control of the motor & wrecked causing a laceration to his head which needed 5 staples to close.

EE was fueling up a welder when gasoline splashed on the hot manifold and ignighted the welder which was mounted on a truck. The welder and a 1989 Ford 350 4X4 pickup burned to the ground.

Employee was waiting for crew to bring in a set of doors to install. He heard motor approaching. As he got up, he stepped on a piece of cinderblock and twisted his ankle.

Employee was walking on the headgate 2 section outby when he stepped in a hole and twisted his right ankle. He was diagnosed with a sprained ankle.

An unplanned roof fall occurred on the #1 section in the #3 entry at spad #5044 approximately 180 feet from the active face. No injuries were reported, no equipment was involved. Ventilation and passage of persons were affected. Fall was measured at 114' length x 20' width x 10' heighth. Fall will be rehabilitated.

Employee was located on the offside of the Fletcher Roof Ranger II when a piece of rock fell from between the last row of bolts & the ATRS. The rock slid down the canopy & struck the EE on the right leg. Rock measured 18""X 14""X 4"".

Member was in a hurry crossing the line of the longwall when he ran into one of the longwall shields in front of him that he did not see due to his head being down. He struck his right shoulder against the shield causing pain in his back.

Member was in the process of advancing shields, had hooked up 1"" pull chain to drag shield forward, when one of the hooks bent, causing chain to ties and strike him in the right arm and side. He received a compound fracture which required surgery to correct.

Member was using a standard utility knife to square the belt to make a splice when the knife slipped and struck him in the left forearm causing a laceration which took 2 sutures to close.

Member had finished loading supplies into bucket of scoop and was backing around corner into #2 entry when he ran over a piece of 6"" Ni-Pak line not in use. End of pipe rose up and entered scoop deck striking member in his left arm causing a fracture.

An unplanned roof fall occured 6000' from the active face of the # 1 section in the #4 entry at spad #4283. Fall measured 30' length 18' width x 6' heightno equipment was involved and ventilation and passage of persons was not affected . No accidents were reported due to this accident.

An unplanned roof fall occurred 1500' from the active face of the working section in the left return, entry #1, near spad # 10300. No equipment was involved. No injuries reported. Ventilation and passage of persons were not affected. Fall measured 25' long x 18' wide x 6' high. Fall will not be rehabilitated.

Member was walking down the steps of an overcast when he stepped on a broken piece of cinder block that was at the base of the steps, causing him to twist his right ankle.

Member had his left hand positioned on the belt rail and was reaching for the sheave rope with his right hand while pulling slack out of the take-up when the rope became hung between top structure and bottom roller. Bottom roller moved causing his middle finger to be pinched between roller bracket & top rollar causing laceration that was sutured.

An unplanned roof fall occured in the # 4 entry 100' inby spad 4275. 4000' from the active face fall measured 10' length x 16 ' width x 5' height. No injuries reported, no equipment involved. Ventilation was not affected. Passage of persons was affected. This area will be rehabilitated.

Member was standing on scoop batteries to hang bolter cable in the #2 entry at spad #4531. As he was climbing down off scoop, right knee gave out causing him to fall and twist left ankle. Member continued to work until 5/23/06 when due to swelling, he was told to remain off work to see specialist.

An unplanned roof fall occured on the #4A belt towards the #1 working section in the # 3 entry. Fall was 4500 ft.from the working section and measured 75ft long 20ft wide and 6 ft high. No injuries were reported and passage of persons and ventilation were not affected. Entrances to the fall have been breaken off & will rehabilated.

EE had just cleaned his knife blade in his utility knife, bent down to help electrian when his knife blade cut his wrist, knife was hanging off his belt by a clip and apparently the blade had slipped out. Sutured with eight stitches.

member & 2 0thers were unloading a piece of track rail approx 6'long off of a car, when as it was coming off it struck a longer rail causing it to come off the car and bounce mashing his left index finger in between the 2 rails. Member fractured tip of index finger and rec'd 7 stitches.

Member was removing backboard from face pan on longwall. Backboard slipped out of his hand and pinched his left middle finger against pan, causing laceration to middle finger which needed 2 stitches to close.

EE was in the process of drilling an 8 ft hole in the mine roof. As he was putting the 4 ft finisher and pushed together, the steel bowed as he raised the drill pot causing the steel to spring apart and strike the EE in his right thumb. X-rays revealsed he dislocated his thumb, but there were no fractures. Lacertaion was sutured closed with 3 stitches.

Member had just finished re-fueling the IT28 endloader and was in the process of climbing back in the operator's compartment. As he opened the door, the door swung back to close. He was reaching to grab the door to keep it from closing when the door shut catching his left thumb in the door causing a fracture.

EE was helping assist the foreman in hanging a D-Box cable whe EE grabbed the cable he came into contact with a bad place in the cable and was shocked. EE was checked by Ambulance personell and was deemed okay with no medical problems.

EE was operating Fairchild Scoop and had his L arm outside of the deck with his left hand resting on the stop block, when he turned to the scoop to the right, his middle index finger between the stop block and the frame fracturing his middle finger and a laceration.

Large volume of water inflow from pilot hole above Powellton seam. Source of water unknown.

Member was operating 35c scoop to clear roadway. Member started to feel shaky, heard a noise, raised up to see if something had fell off the back of scoop. He then passed out and fell out of scoop at which time the scoop rolled over left leg breaking his tibia. He is a diabetic & scoop was on an incline.

On 4/12/06 due to an occupational illness (degenerative bone disease) substantiated by medical records and reports, employee was diagnosed with ailments described in the medical reports. This was not due to one accident but repeated trauma and degenerative bone disease.

An unplanned roof fall occurred in the #5 entry approximately 2400' from the active face. The fall measured 22' long, 18' wide, 3.5' to 6' high. No persons were injured, passage of persons was not affected. Ventilation for return was affected.

MEMBER WAS PREPARING TO PUT HIS MANTRIP BACK ON TRACK THAT HAD JUST WRECKED. HE WAS WALKING FROM ONE END OF THE TRIP TO THE OTHER, WHEN HE TRIPPED AND FELL, PUTTING HIS RIGHT HAND DOWN TO CATCH HIMSELF, CAUSING A LACERATION FROM A TRACK TIE WHICH NEEDED STITCHES TO CLOSE.

Member was reaching to pick up a crib block, when another member who was helping him, threw a crib block in the same direction, which struck member in the left hand causing a laceration which needed 6 stitches to close.

Member was in the process of using a utility knife. to cut a piece of 48"" belt, when the knife slipped and caused a laceration to his left calf area, which needed 4 stitches to close.

Member was on the surface of an underground mines, in the process of uncoupling a supply can from on motor, when as the pin was moved the car shifted pinching his finger. 3 stitches were need to close the wound.

Member is on electricing, who was in the process of serving, pinning him to the ground. Rock measured 6'x3'x2' and fractured his l, thru l5 vertabral.

An unplanned roof fall occupped in 12-13-05 at 6 am at spad 12301 in the #5 & #6 entries at the #1 section of the old white night mines measuring 50' x 20' x 6', no persons were injured, travel was not affected, entilation was not affected. 8,000' tranactive section.

AN UNPLANNED ROOF FALL OCCURRED IN THE #4 ENTRY. 300' INBY SPAD #114 ON 5 BELT. APPROXIMATELY 20,000' FROM ACTIVE FACE. THE FALL MEASURED 70' LENGTH, 20' WIDTH AND 5' HEIGHT. NO INJURIES WERE REPORTED. VENTILATION WAS NOT AFFECTED.

EMPLOYEE WAS THROWING TRACK SWITCH WHEN HE TORE A BICEP TENDON.

Member was attempting to hang his buggy anchor, he was located on the left side of the feeder on the #2 section, as he was hooking the chain over teh anchor bolt with his other hand, he felt a sharp pain in his lower back. X-rays were taken and were negative but he was advised to stay off work until 10/17/05.

MEMBER WAS FINISHING UP ON THE STOPPING HE WAS BUILDING WHEN THE STOPPING FELL OVER TOWARDS HIM AND STRUCK HIS RIGHT HAND CAUSING A LACERATION TO HIS PINKY FINGER, WHICH TOOK 3 STITCHES TO CLOSE.

MEMBER WAS LOADING ROCK OUT WHEN A PIECE OF ROCK FELL 12 FT HITTING HIM ON RIGHT HAND CAUSING LACERATION TO HIS SMALL FINGER THAT TOOK 9 STITCHES TO CLOSE

An unplanned roof fall occurred int he #5 entry about 2 breaks inby spad #122998. The fall measured 30' x 20' x 7'. All entrances to the fall were breakered off and area will be rehabilited. There were no injuries reported but passage of person was affected.

EE AND COWORKER WERE IN THE PROCESS OF MAKING A MECHANICAL SPLICE IN THE BELT AT #4A PONY HEAD ON #1 SECTION. AN UNPLANNED ROOF FALL OCCURRED AT THE LOGAN'S FORK MINE 15,000 FT FROM THE ACTIVE FACE OUTBY AT CROSSCUT 2.3 SPAD #243. THE FALL WAS 19'W AND 4-5 ' HIGH. NO INJURIES WERE REPORTED AND VENTILATION WAS NOT AFFECTED.

Employee was on his knees on the ground over top of a piece of belt. He was cutting with his utility knife, when the knife slipped causing a laceration in his right leg, which needed stitches to close.

EE was in the process of building a stopping when he stepped on a piece of cinder block. Twisting his ankle.

EE was assisting the continuous miner operator in hanging cable when he felt pain in his left shoulder. He continued to work until 4-4-05.

Employee was walking thru the 3 to 4 crosscut when he slipped in mud and twisted his left knee.

Member was carring his cap light in hand and tripped over his cap light cord falling off of tailpiece he was welding on, landing on his shouler.

Member was standing on shuttle car cable. Another c0-worker started up shuttle car, and doing so the shuttle car took up cable slack causing him to loose balance and fall on his cap light battery breaking one rib.

While attempting to re-rail a supply car, one jack creeled out and jack bar struck member in jaw.

EMPLOYEE WAS WALKING WHEN HIS LT. FOOT SLIPPED & TWISTED HIS KNEE. HE WORKED UNTIL 10/11/04 WHEN HE WENT TO THE HOSPITAL & WAS DIAGNOSED WITH A SPRAINED KNEE, REFERRED TO A SPECIALIST ON 10/12/04. HE WAS TOLD TO REMAIN OFF OF WORK UNTIL RE-EVALUATED ON 10/19/04.

member had just dropped pull rope off of continunous miner boom cmo was unaware of his position and started tramming miner toward face, boom of miner struck member on pelvis and pinned him in the rib.

EE WAS IN SHUTTLE CAR IN AN INTERSECTION GETTING READY TO LOAD UNDER THE MINER WHEN ANOTHER SHUTTLE CAR STARTED UP TOWARD THE MINER. THE 2ND SHUTTLE CAR OPERATOR DID NOT SEE EE AND STRUCK THE DECK AREA OF THE SHUTTLE CAR, PINCHING EE'S ARM. RESULTED IN A LACERATION.

MEMBER WAS EXITING AN S&S RAIL ROVER ON CHARGE. AS HE WAS CLIMBING OUT OF THE DECK HE SLIPPED AND FELL BACKWARD LANDING ON HIS LEFT SHOULDER. HE DID NOT START MISSING WORK UNTIL 8/26/04.

Member was walking across section when he stepped in a hole and twisted his ankle. diagnosed June 28 2004 as a stress fracture, right tibia.

MEMBER WAS LOADING BOLTS ONTO BOLTER WHEN HE FELT A SHARP PAIN IN BETWEEN HIS BACK & SHOULDER BLADES.

Member was crossing old belt line. He had one foot on belt rail and one foot on belt. He lost his balance and tried to catch himself against the top when his left hand contacted a roof disc.

EE AND ANOTHER SHUTTLE CAR OPERATOR WERE LIFTING THE MINER WATER LINE WHICH WAS STRETCHED TIGHT WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. HE DID NOT CEASE WORK AT TIME OF ACCIDENT, BUT DID SEEK CARE AT A LATER DATE.

An unplanned roof fall occurred at spad 10538, 2900' from active face, developed 4-16-04, all entrances breakered off. The fall measured 90' length, 20' wide, 10' high. No accidents reported.

THE MEMBER WAS DRILLING HIS INSIDE HOLE ON HIS 3RD ROW OF BOLTS IN 6 RIGHT. WHEN HE HAD THE HOLE DRILLED HE USED THE FAST FEED TO LOWER THE DRILL PAD. THE DRILL STEEL STUCK IN THE HOLE, WHEN HE WENT TO REACH FOR IT, THE DRILL STEEL FELL & HIS HIS MIDDLE FINGER ON RIGHT HAND AGAINST THE DRILL PAD.

THE MINER OPERAOR WAS PUTTING AN INSECT IN THE MINER, WHEN A PIECE OF METAL HIT HIM IN LEFT ELBOW.

THE MEMBER WAS PULLING ON A 3/4 DRIVE RATCHET, WHEN HE FELT PAIN IN HIS LOWER BACK.

EE WAS IN THE PROCESS OF INSTALLING A ROOF BOLT WHEN A PIECE OF DRAW ROCK WEIGHING APPROX. 3 TO 4 LBS. FELL FROM THE MINE ROOF STRIKING HIM ON THE NOSE.

EE WAS LOADING CONVEYOR BELT WITH A SIDE CLAMP & STEEL CABLE. WHILE THE SCOOP WAS PULLING A SECTION OF THE BELT INTO THE BELT SLED, THE SIDE CLAMP CAME OFF AND STRUCK EE ON THE LEFT ELBOW.

WHILE PUTTING UP AN INSIDE BOLT A PIECE OF ROCK FELL HITTING THE CANOPY THEN STRIKING EMPLOYEE IN THE FACE. THE ROCK FELL APPROX 12"" HITTING THE CANOPY THEN CAUSED A CUT ON EMPLOYEE'S NOSE.

EE WAS WALKING TOWARDS THE SHUTTLE CAR WHEN A PIECE OF ROCK FELL FROM THE TOP CUTTING HIS RIGHT CHEEK. THE ROCK MEASURED 4' LONG X 7"" WIDE AND 1.5"" THICK.

AN UNPLANNED ROOF FALL OCCURRED IN THE #1 SECTION, ENTRY #6, SPAD #10302. THE FALL MEASURED 70'L X 19'W X 8'H. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERED OFF. THERE WAS NO INJURIES REPORTED .

AN UNPLANNED ROOF FALL WAS DISCOVERED DURING A WEEKLY EXAM ON THE OLD WORKS BIG DADDY MAINS, CROSSCUT 26, SPAD #753. SIZE OF FALL WAS 320' LONG, 18' WIDE & 6.5' HIGH WITH NO INJURIES REPORTED. ALL ENTRANCES TO FALL HAVE BEEN BREAKERED OFF.

WHILE REPAIRING LEVELING CHAIN ON A BOLTER, EMPLOYEE WAS STRUCK IN THE STOMACH AREA BY A PIECE OF THE HAMMER. HE WAS USING A HAMMER AND PUNCH AT THE TIME OF THE ACCIDENT. EMPLOYEE DID NOT MISS ANY SCHEDULED WORK.

WHILE DRILLING A HOLE TO INSTALL ROOF BOLT IN #8 ENTRY, THE DRILL STEEL STUCK IN THE HOLE AS HE DROPPED THE HEAD ON THE BOLT MACHINE. WHEN HE REACHED FOR THE DRILL STEEL IT FELL, SMASHING HIS LITTLE LEFT HAND FINGER BETWEEN THE DRILL STEEL AND DRILL POT. NO WORK TIME WAS MISSED.

WHILE BEATING A LOOSE MINER BIT OUT OF CONVEYER BOOM, A PIECE OF METAL CAME OFF OF BIT AND STRUCK VICTIM IN THE RIGHT HAND INDEX FINGER. HAVING THE PIECE OF METAL REMOVED INVOLVED STITCHES.

EE WAS OPERATING A JOY 14-15 CM IN THE #1 LEFT ROOM WHEN A PIECE OF DRAW ROCK MEASURING 38""L X 28""W X 10"" THICK FELL FROM BETWEEN BOLTS STRIKING EE ONT HE HEAD AND RIGHT SHOULDER, KNOCKING HIM TO THE MINE FLOOR. DEGREE 3 CLASS 7.

AN UNINTENTIONA ROOF FALL OCCURRED NEAR EP 16 IN THE 1,2,& 3 ENTRIES. APPROX. 20000 FT FROM THE ACTIVE FACE, THE FALL MEASURED 350'L X 18' W X 7 1/2' H. THE FALL ARE HAS BEEN DANGERED OFF AND WILL NOT BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL OCCURRED 10 BRKS INBY EP #2 IN THE #2 ENTRY APPROX. 20000 FT FROM THE ACTIVE FACE THE FALL MEASURED 150'L X 18;W X 7'H. THE AREA AROUND THE FALL HAS BEEN DANGERED OF F. THE FALL WILL NOT BE REHABILITATED.

TWO EMPLOYEES HAD COMPLETED BOLTING THE #1 ENTRY WITH A FLETCHR ROOF RANGER II BOLTER. ONE WALKED BACK TO THE OFFSIDE FRONT TIRE TO CHANGE A DRILL BIT. AFTER LOWERING THE ATRS, HIS CO-WORKER T RAMMED THE MACHINE BACK ONE TO TWO FEET AND TRAMMED THE MACHINE OVER THE OTHER'S FOOT.

AN UNPLANNED ROOF FALL OCCURRED AT #19 BREAK ON #5 BELT, APPROXIMATELY 15,000 FT FROM THE ACTIVE FACE. THE FALL MEASURED 30'L X 20'W X 6'H. THE FALL AREA WILL BE REHABILITATED.

EE WAS LOCATED 5 BREAK INBY 3A HEAD TRIMMING CLIPS FROM A FLEXCO SPLICE IN THE SECTION CONVEYOR BELT, USING A UTILITY (RAZOR) KNIFE, WHEN THE KNIFE SLIPPED STRIKING HIS RIGHT HAND BETWEEN THE THUMB AND INDEX FINGER RESTULTING SIN A LACERATION REQUIRING 3 SUTURES . DEGREE 3 CLASS 10

EMPLOYEE AND A CO-WORKER WERE SETTING TIMBERS IN THE #5 ENTRY. WHEN HE WALKED BACK TO THE SCOOP TO DISENGAGE THE PTA AND MOVE THE SCOOP OVER, A RIB ROLL 81""L X 16""W & UP TO 6"" THICK FELL STRIK ING HIS LOWER LEFT LEG AS HE WAS GETTING INTO THE OPERATOR'S DECK.

EE WAS OPERATING A JOY 14-15CM CONTINUOUS MINING MACHINE WHEN A ROCK BROW FELL, STRIKING HIM IN THE BACK RESULTING IN A HIARLINE FRACTURE OF THE L-2 VERTABRAE. THE ROCK BROW MEASURED 96""L X 12 "" THICK AND FELL FROM THE UPPER PORT OF THE SEAM.

EMPLOYEE WAS UNLOADING RETURN BELT CONVEYOR ROLLERS FROM A MANTRIP AND STACKING THEM ON A PALLET WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK, RESULTING IN A MUSCLE STRAIN.

EE WAS RIDING IN A RAIL MANTRIP. AS IT WAS TRAVELING UP THE SLOPE, THE MANTRIP BECAME FOULED ON THE SLOPE FLOOR. THE PASSENGERS GOT OUT TO DETERMINE WHAT CAUSED THE TRIP TO FOUL. HE WAS STANDI NG BY THE MANTRIP WHEN THE OPERATOR OF THE TRIP WAS INFORMED THAT HE NEEDED TO BACK THE MANTRIP UP. WHEN IT BACKED UP, HIS FOOT WAS CAUGHT BETWEEN THE SLOPE FLOOR & THE BOTTOM OF THE MANTRIP.

EMPLOYEE WAS IN THE #1-2 CROSSCUT NEAR SPAD #744 LAYING ON THE MINE FLOOR CHECKING ELECTRICAL READINGS IN A CONT MINER PANEL WHEN A PIECE OF ROCK MEASURING 6"" BY 1"" BY 10"" FELL STRIKING HIM IN THE FACE RESULTING IN A LACERATION TO HIS RIGHTEYE.

EMPLOYEE WAS UNLOADING A 60"" BOTTOM BELT ROLLER IN THE TRACK ENTRY ON THE #5 BELT AT THE #5 TAILPIECE WHEN HE DROPPED THE ROLLER ON HIS RIGHT MIDDLE FINGER RESULTING IN A FRACTURE.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE CASTLE WHITE KNIGHT PORTAL SECTION NEAR SPAD #12609 #4 ENTRY, 6000'FROM THE ACTIVE FACE. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERED OFF AND THE AREA WILL NOT BE REHABILITATED.

EE WAS USING A 24"" ADJUSTABLE WRENCH TO TIGHTEN TRACK BOLTS WHILE INSTALLING 80 LB TRACK RAIL ON THE #2 SECTION. THE WRENCH SLIPPED OFF A TRACK BOLT HE WAS TIGHTENING & STRUCK HIS FACE.

INUNDATION OCCURRED ON THE 4WEST PANEL ON THE MMU-010-0 AND 011-0 MMU SECTION. IN THE #1 ENTRY AT SPAD #12988. THERE WERE NO INJURIES AND NO EQUIPMENT COMPROMISED.

EMPLOYEE WAS OPERATING A CONTINUOUS MINER ON THE LOGAN'S FORK MMU 002-0 SECTION WHEN A PIECE OF ROCK FELL FROM BETWEEN BOLTS. THE ROCK MEASURED 9"" X 12"" X 4"" THICK. THE ROCK STRUCK HIM ON THE RIGHT WRIST RESULTING IN A LACERATION REQUIRING 8 SUTURES. MANAGEMENT WAS NOT AWARE UNTIL 11/19.

EE WAS OPERATING A SCOOP IN THE #1 ENTRY WHEN HIS SCOOP CANOPY CONTACTED THE RIB AND THE RIB BROKE UP INTO SEVERAL PIECES, WHICH FELL INTO THE SCOOP DECK STRIKING HIM ON THE BACK, ARM & LEG RE SULTING MAINLY IN A CONTUSION TO HIS BACK.

EE WAS IN THE FACE OF THE #3 ENTRY OPERATING THE CONTINUOUS MINER WHEN A ROCK MEASURING 6' LONG X 3' WIDE X 0-8"" THICK FELL FROM THE MINE ROOF, STRIKING HIS RIGHT KNEE RESULTING IN A CONTUSION AND SWELLING.
