Struck by rolling or sliding object

Employee was placing a liner plate in the heavy media vessel. Employee was using a slate bar to slide the plate forward and the plate slid too far striking employee in the left ankle which resulted in a fractured ankle

Employee was prying on a screen to remove, felt a burning in lower back.

Employee was lifting a flume screen for replacement and felt a sharp pain in employee's lower back, employee continued to work employee's regular shift until 06/26/2017, plant was notified on 06/29/2017 that employee's Dr. had placed employee off work

Struck against stationary object

Tightening alternator belt on skid steer with wrench, employees hand slipped and came in contact with the hose clamp causing a laceration to the right hand. (Required 7 stitches).

Over-exertion in pulling or pushing objects

The employee was washing down the 3rd floor with a water hose. The water hose became stuck around a corner. The employee used more pulling force to free the hose, strained his back in doing so- which caused pain. On 12/15/2015 the employee had a follow up exam. On 12/18/2015 the operator was notified that the employee has received physical therapy. treatment.

Caught in, under or between a moving and a stationary object

Employee was using the handle of a shovel to bust ice off of the 360 belt. The employee's hand slipped and was caught between the handle and the shovel end causing a laceration to his right hand which required 6 stitches.

Over-exertion in pulling or pushing objects

On May 24, 2011 EE was pulling on a chain hoist to lift a crusher roll out of the crusher. He injured his shoulder when doing so and took himself to Summersville Memorial ER where he says they diagnosed him with an infection. On May 30, 2011 he went to his family doctor who referred him to a specialist.

Struck by falling object

Changing the segments on the crusher when a large guard fell on him, striking him in the neck and shoulder area. He was flown to CAMC where a CT Scan was negative. Guard was not secured as it should have been.

Struck against stationary object

On 2/2/11, EE was in the process of changing the clean coal screen on the 3rd floor, when the screen became lodged (stuck), resulting in EE striking his left knee cap against the sharp edge. EE left and drove himself to Summersville Regional Hospital for treatment. EE received 4 stitches and was released to return to work.

On 8/22/10, shift foreman was walking through the raw coal tunnel and stepped on a rock on the floor and twisted his knee and struck the belt structure. EE completed his shift and later sought medical treatment. EE had surgery on 12/19/10 and his 1st day away from week was 11/25/10.

The employee was working with chain falls and pry bars and apparently bumped or caught the back of his hand in a pinch point. Employee did not seek treatment until 7/16/10 and company was not made aware of injury until 7/20/10.

Over-exertion in lifting objects

EE was cleaning the area around the tail of Sugar Camp #2 Belt. EE was lifting a railroad tie onto the back of the belt truck and felt pain in his lower back and down his right leg. EE completed his shift and did seek medical treatment on the following day, however did work next shift. EE was diagnosed w/pulled muscles and part torn tendon by dr on 5/24/10, placed off work.

On 4/26/10, EE was walking thru the new shop into the basement of the plant. As he made the one step down, his right ankle and foot turned over w/ him. First aid was administered and EE was transported to hospital for treatment. EE was placed off work and referred to his doctor and a specialist.

Struck against a moving object

EE was pushing coal at loadout on #12 pile feeder & upon backing up the dozer dropped into a hole (appx 4') created by push & position changes of dozer. He was transported to hospital after feeling pain in his lower back from the jar. He was diagnosed with bulging discs, referred to a specialist for eval. on 12-23-09. Diagnosis: pulled muscles & strained ligaments. Therapy

Injury slipped on 3/3/09 coming down a ladder. He refused treatment. He worked til 4/15/09. He went off work on 4/15/09 after coming back from a 5 day suspension. He was suspended for insubordination and cursing a foreman. This accident is under protest waiting for brick street ins. I.M.E. to determine if accident is legit.

Over-exertion in pulling or pushing objects

On 7/8/08 EE was in the process of pulling a water hose from the deslime screend to the classifying sump area to clean off sump cover. During the process EE felt a burning in the lower part of his back, in the center of his back around the belt area, along with numbness sensation in hips/L leg area.

Struck against stationary object

Employee was trying to open door on IT 28 loader. He jammed his left ring & little fingers in doing so. On December 25, 2007, a specialist diagnosed employee with torn ligaments and he was put off work. Safety Dept. was not notified until Jan. 2 because of the holidays.

Fall from ladders

EE was working to abate an electrical violation on a garage door located in the shop. He set up a 12' extension ladder to climb up and remove ceiling tiles. As he was descending, the ladder began to slide. EE fell approx 6 ft to the ground. He was transported to the hospital where x-ray revealed a fractured left metatarsal.

On 2/15/07, EE was cleaning out the crusher chute. The crusher chute was plugged with rock and ice. EE was using a slate bar to dislodge the material so it would fall down the chute. The bar slipped and caught his finger between the bar and beam, smashing his right ring finger at the joint.

Employee was working welding handrail on raw coal screen walkway when another employee above him dropped a 3"" x 11"" piece of angle on him, grazing the side of his head.

Employee was trouble shooting a sampler belt in the loadout facility when he caught his arm in the belt. His arm was broken in several different places before he managed to free himself. He was transported to the hospital where he underwent surgery to set his arm.

Struck by flying object

EE was repairing a broken conveyor chain and pulled his glasses off because they were fogged up. He turned to speak with co-worker, but when he turned, co-worker struck a sprocket chain gear with his hammer. Something hit EE's eye. EE went to an eye Dr. on 4/7/2006 and metal was found in his eye.

On Feb 12,2006 EE ceased the work of removing vessel flights when he experianced pain in his lower back then radiated down into his left thigh area. EE had a kidney removed in 2005 and has experianced difficulties with his back and surrounding areas since his surgery.

EMPLOYEE WAS DOWN ON HIS KNEES LIFTING A RETURN BELT ROLLER INTO PLACE. HE WAS TRYING TO DO THIS FROM THE CATWALK INSTEAD OF CLIMBING DOWN ON THE STRUCTURE WHICH IS APPROXIMATELY 30 TO 35 FEET OFF THE GROUND. HE BEGAN TO GET A SHARP PAIN IN HIS BACK.

EE was helping pull cars to move to the next hole. He was throwing a loop of cable, he then slipped and caught his anked between drifting car pin and pusher jack.

Inhalation of radiations, caustics, toxic and noxious substances

Ee was attemping to switch ammonia from liquid to vapor. the forman asked him to turn the liquid valve off and open the vapor make. Shortly he radiold my that he had turned the valve and a vapor was coming out. I told him to turn it back off and i would be thru. We immedicately put oxygen on him and ambulance took him s'ville hosital.

Driver had finished filling tank. While returning hose to his truck, he slipped on the mud and struck right leg against a pipe flange bolt. He received five stitches to close the wound.

INJURED WAS WALKING DOWN INTO THE TUNNEL TO GREASE MAG PUMP WHEN HIS RIGHT FOOT SLIPPED, CAUSING HIM TO TURN HIS RIGHT KNEE. HE WAS SEEN BY A PHYSICIAN ON 5/10/04. AN MRI WAS PERFORMED ON 5/12/04 AND CAME BACK POSITIVE. SURGERY WILL BE PERFORMED DURING HIS REGULAR VACATION SHUTDOWN TIME.

INJURED STEPPED ON A LUMP OF COAL CAUSING HIM TO TWIST HIS LEFT KNEE. BUT DID NOT SEE A PHYSICIAN UNTIL SOME TIME IN DECEMBER. AT WHICH TIME A MRI WAS PREFORMED AND CAME BACK POSTIVE. SURGERY WAS PERFORMED DURING LAID WORK DAYS.

INJURED WAS TAKING SAMPLE BUCKET AND LIDS TO THE THIRD FLOOR DOWN A FLIGHT OF STEPS WHEN HE TRIPPED AND FELL STRIKING HIS HEAD ONT HE TOEBARD AT THE BOTTOM.

MEMBER WAS HOLDING THE LID OF THE RAW COAL DISTRIBUTOR WITH ONE HAND (HOLDING IN RAISED POSITION) AND WAS TURNING THE DISTRIBUTOR WITH HIS OTHER HAND WHEN HE STRAINED HIS BACK. HE COMPLETED HI S SHIFT ON 7/17/02 AND WORKED 7/18/02, 7/19/02 AND 7/20/02. HE BEGAN MISSING WORK 7/22/02.

MEMBER WAS PUSHING COAL BACK AT NO. 9 STACKER. AS HE WAS BACKING UP THE COAL UNDER HIS RIGHT TRACK BROKE AWAY ALLOWING THE DOZER TO SLIDE OVER THE RETAINING WALL. HE CONTINUED TO WORK THROUGH AUGUST 9, 2002. AFTER HIS SHIFT HE WAS EXAMINED BY A DOCTOR AND BEGAN MISSING WORK.

ON 5-10-2002 EE KNEELED DOWN TO PICK UP A FLIGHT. WHEN HE RAISED UP WITH THE FLIGHT HE HAD A SHARP PAIN IN HIS BACK. 6-24-2002 EE STATES HE AGGRAVATED HIS BACK USING COMEALONG WHEN HE TRIED T O HELP METAL OVER HANDRAIL. 6-27-2002 EE BEGAN MISSING WORK.

EMPLOYEE WAS UNPLUGGING A CLEANUP PUMP, WHICH HAHAD BEEN HOSED DOWN DUE TO BEING HOT; HE STOOD BEHIND THE PUMP & BEGAN TO LOOSEN THE TOP UNTIL IT WAS REMOVED. WHEN CAP WAS SEPARATED FROM THE WATER IN DISCHARGE LINE BLEW DOWN, SPLASHING ON-TO HIS ARM, RESULTING IN A BURN INJURY. P, THE WATER IN THE DISCHARGE LINE BLOWED DOWN, SPLASHING ONTO THE ARM, RESULTING IN A BURN INJURY.

DRIVER UNFAMILIAR WITH ROAD TOOK WRONG ROAD. HE MADE A TURN DOWN AN UNAUTHORIZED ROAD TOO STEEP FOR LOAD TRUCK TO TRAVEL. GOT TRUCK OUT OF GEAR,PUMPED AIR OUT BUT COULD NOT GET SLOWED DOWN FOR TURN.

Struck by flying object

A CHOCKER WAS BEING USED TO MOVE A PIECE OF BELT IN CONJUCTION WITH A SHACKLE. THE SHACKLE HAD TENSION ON IT. WHEN THE EE TOOK THE PIN OUT OUT THE SHACKLE TO REMOVE THE SLING, THE SHACKLE FLEW UP AND HIT HIM IN THE HEAD.
