Struck against a moving object

While working a steep backfill slope doing reclaimation work, the dozer operator was unable to maintain control, resulting in rollover of dozer approx 4-5 times. Although wearing seatbelt, a broken back and swelling of C-spine resulted, causing loss of sensation and paralysis.

On Wednesday, 11/11/2015 the mine operator was notified that the employee had been diagnosed with 16% hearing loss as a result of repeated exposure to noise over his 40 year career. Medical documents support this claim. The employee has been off of work since 8/14/2015 for an unrelated medical issue.

Fall to the walkway or working surface

Employee exited his tire truck and stepped onto packed snow and ice covered ground located at the old truck line. Upon stepping on the ground, he slipped and fell landing on his hip and attempted to catch himself with his hand. This caused a sprain to the right wrist which required a splint.

EE was unhooking 1.5 cables from crane hook while holding the safety latch the back cable came off and hit him in the face busting top and bottom lip and chipping his front tooth

Accident type, without injuries

Mantrip had taken men to their equipment and parked up at about 6:15 am. EE was in his hoe and noticed steam or smoke about 6:30 from under the van and went to check on it. It was on fire so he called for help on the radio. Several other operators responded with fire extinguishers. Van was a total loss. No one was injured as a result of the fire.

While trying to remove a 6' water line from a Gorman Rupp pump, ee started to fall he let go of the hose. another ee who was helping ee also let go of the hose and it fell on ee left leg pinning it to the ground. ee received a broken fibula from the accident.

Employee was traveling down the 5 block on a decline of approximately 9 percent grade when he lost control of his truck ran off the road to the left up on the embankment and over turned it on its right side.

On 1/9/11 EE had just coupled the Add-Car Cathead and was placing into the Add-Car tray, when it snagged on his glove, causing it to land on his left hand fingers. EE completed his shift and then sought medical treatment for a broken middle finger, left hand.

EE was removing brake pack from Komatsu loader using a 1"" drive impact wrench with a made up socket keeper (made from welding rod) while operating wrench the keeper caught his glove and cut his left hand index finger.

On 9/21/10 ee was in the process of installing a hydraulic bake steering pump in the CAT 992D loader and while winching into position the pump lodged and when it came free it struck and mashed ee right hand against the transmission mount. First aid was administered and ee transported to Summersville Regional Hsp for treatment. EE was treated and released to RTW.

EE was setting up tooling to press out the king pin on the Rock Truck and while hooking up the chocker cable to re-position the 100 ton ram sitting on the spindle the ram fell over and the cable hooked on his left thumb. First aid was adm. and EE was transported to hospital for treatment. EE was referred to a orthopedist by the ER Dr.

EE was walking to the launch and while going around the pan loader he rested his arm on the back upright. When he pulled away a sharp burr of metal cut his right forearm. Seven stitches were used to close the laceration.

EE was in the process of removing a sprocket with another mechanic. EE stepped back to observe the sprocket while the other EE struck it with a 8 lb sledgehammer. A piece of metal came off and struck EE in his right forearm. First aid was given and EE was transported to ER. Dr. referred EE to a surgeon.

An unintentional highwall fall occurred on 12/8/2009. The fall occurred at Robinson North / Blackberry. No injuries occurred from the fall. The fall severely damaged the #1 HWM Launch and 3 mechanic trucks.

On 10/22/09 EE was in the process of unloading D-11 tracks from the boom truck. EE was lifting with the boom one track when it came caught on the other track. While trying to swing it free the chain broke and EE jumped from the truck where he landed wrong and tried to catch himself and fell. EE retuned the truck and drove himself to Summersville Memorial Hospital for treatment.

On 10-21-09 EE was operating the maintenance truck boom/winch by remote while in the process of pulling belt through the highwall miner add-car frame. Co-worker was watching for hangups at the belt roll end. The steel belt roller that was used for the cable upward pull dislodged & struck him in the face. First Aid was administered and employee was transported to CAMC for treatment.

EE was in the process of manually pinning the blast shield and while co-worker was working the handle to align the holes, injured EE was pushing the pin and cut his left hand ring finger. 1st aid was administered and EE was transported to Summersville Memorial Hospital by the safety department for treatment.

EE was tramming around the sediment control ditch, when a loose rock released from the slope above the safety bench. The rock slid down the bank and hit the safety bench and continued down the slopes, striking the front end of the excavator's cab. 1st aid was administered at the scene and EE was transportd by Health Net to Charleston Area Medical Center.

Employee was removing wire cable from ripper on dozer, when sharp edge caused a laceration of the left hand.

On 4/2/2009 Employee was in the process of hooking up a 4' retrieval hook link and it slipped out of his hand and fell on his right hand middle finger mashing it against the launch pad. First Aid was administered and he was transported to Summersville hospital for treatment.

Employee was rolling out lead line. When he crossed over a berm, he twisted his ankle and fell. A rock then rolled onto the ankle. Employee went to local ER, diagnosed with contusion and abrasion.

On 2-27-09, employee had been performing work on an Add Car which included grinding a piece of brass. He felt something in his left eye a short time after completing the grinding. First aid was administered and flushing his eye was unsuccessful. He was transported to Summersville Memorial Hospital where he was treated (foreign material removed from left eye) and released.

On 2/23/09, EE was in the process of dismounting the D10R Dozer he was operating, when his foot slipped off the step, causing him to put strain on his right arm and shoulder. EE continued to work and did not seek medical treatment until 3/3/09 and again on 4/13/09. The first day EE missed was for pre-surgery testing on 7/27/09.

EE was attempting to break free a frozen highwall miner catherd on the add car by striking the cathead against the add car. In the process he smashed his right index finger, between the cathead and the add car. First aid was adminstered and he was transported to summersville memorial hospital for treatment.

Ee was removing a 1"" steel cable from the path of the d11 Dozer and did not see that a couple of strands on the steel cable were broken. The weight of the cable caused the broken strands to penetrate his glove and cut the palm of his right hand. First aid was adminstered and he transported to hospital for treatment.

On 1/7/09 Ee was in the process of placing the last right rear jack pad on the drill deck to bring out. He had placed the jack pad on his leg and when he began to raise the jack pad up onto the drill deck it fell and caught his left hand ring finger between the jack pad and the handrail. First aid was adminstered and he was transported to hospital for treatment.

On September 28, 2008, employee was on top of the miner checking the center gear case when the grease hose started to reel up, catching the blast shield lever. This resulted in the blast shield lowering down on employee's head causing fractures.

On 8/22/08 EE was pulling on the water line that was falling out of the car when the car dropped off the pad. His left foot was positioned under the car when the car dropped off the pad, crushing his left foot.

On August 15, 2008, Injury was dismounting the 992 G Loader to re-check the oil while the tire man was finishing up changing a front tire. He was walking down the steps when he twisted his ankle and felt a sharp burning pain in his ankle. He had 3 points of contact and did not fall down.

EE cut a tree. It fell on a tree that was previously cut when it had been logged. A stick from the tree on the ground flew up and punctured a hole in his shield on his hard hat and went into his eye.

Employee was using sledge hammer to drive trailroller on conveyor chain. When hammer struck chain, piece of metal flew off hitting employee in lower lip.

On March 31, EE was pushing in shot and when he turned on air conditioner, particles came out and went into his eye. He was inside cab of dozer and didn't have eye protection on.

On 12/22/07, EE was cleaning mud in the highwall miner pit when right front tire went in hole, causing operator to bounce back and forth. Note: 1/8/08, Nicholas Energy Safety Dept. requested official info from doctor, a neurologist in Charleston, WV, regarding injury. Info was recened on 1/21/08 and has been deemed as lost time accident, will reflect on 2007 NFDL Rates.

Employee was welding on D-11 cutting edge. The boom malfunctioned and raised cutting edge. A bolt fell out striking employee in the head.

On the above date, employee & co-worker were working on a door latch. When he was checking the latch, co-worker shut the door on employee's finger. He was taken to S.M.H. where x-rays on finger were negative. Doctor cleaned finger tip and applied a splint.

EE was replacing engine encloser. He was using a grinder and lost his grip on the grinder. When he grabbed for it, he started grinder back up. When it hit the floor, it bounced back striking his left forearm.

EE was putting a connection link in a conveyor chain on a Joy 1415 continuous miner head on a high wall miner. He struck the connection with a hammer. A piece of metal sheared off, cutting his left bicep on the inside.

Smoke was found coming off of the Stockton 450 pile. The smoke was discovered at approximately 11:00 am.

EE was getting a ride back to the parking area from the tire yard from Rt 772 coal truck. He was climbing up the right side ladder up to the passengers side of the truck when on the second step he slipped and fell approximately 6 ft to the ground landing on his left side.

EE was removing a differential housing from a Caterpillar 992G end loader. The housing fell, EE twisted his rt. ankle avoiding the falling housing.

EE was replacing sheet metal on D10 Dozer after servicing it. EE finished putting guards on left side & was going to right side of dozer. He stepped off dozer with rt foot & rolled ankle on a rock. EE heard a pop and felt shart pain in ankle.

Tank truck top loads from a fixed platform with a hinged ramp to the top of truck. Upon completion of loading, employee pulled the ramp up and gloved hand slipped causing him to fall back 2 feet striking the safety rail brusing low back.

The EE was assisting the driller with the removal of rods (drill steel) from the borehole. There was an ignition of methane (gas). The EE received 1st degree burns to his left arm, face and neck. The EE also received a 2nd degree burn on his left arm.

Contracting company M & W were drilling a hole into an abandoned mine for slurry ejection. While removing the drill steel from the hole, the drillers encountered methane and an ignition occurred. One driller sustained miner burns to his left arm.

On April 8, 2007, employee was getting tools out of tool box. Lid shut while he had his hand on top of tool box causing injury to his right ring finger.

Victim was cutting a tree approx 10"" in diameter. The tip of the chainsaw bar hit a smaller tree behind the tree being cut, causing the saw to kick back, cutting the victim's leg.

Employee was checking antifreeze in truck. He started taking cup off of radiator and realized it was hot. He started putting cup back on when it blew off covering him with hot antifreeze.

EE was trying to change a table bushing on drill because it would not go down to allow the breakout wrench to clear it. EE began to pry up on the table bushing with a pry bar when he felt a sharp pain in his lower back.

On 9/1/06, EE was using a sledge hammer to drive a center pin on an equalizer bar on #621 D11R. A small sliver of the pin broke off and entered EE's right arm.

EE was trying to remove drain plug to drain engine oil. plug was stuck, when it broke loose his carried him back where he hit his head on the bumper of truck.

Removing rock off of miner had hand placed on edge of pan, rock slide off of boom smashing finger.

Employee was getting off bus when his feet slipped. He was holding on with his arm and shoulder popped out of socket. Note: history of shoulder problems prior to working in mining industry. Employee has had multiple shoulder dislocations.

EE was helping load blast on drill bench. As he was walking he tripped over a rock causing him to fall to ground. Treated for cut at local ER received three stitches.

Employee was guiding a truck liner into a new 930E truck bed. The liner was being moved with a boom truck and employee was holding the plate while it was swinging. His hand caught under the plate and in between the truck bed. His left middle finger was pinched.

On April 25, 2006, EE was pulling into the shovel pit area and the truck shifted because of a dip in the terrain. The shift caused EE to twist his right wrist.

EE WAS WORKING ON A ROLLER MACHINE AND WAS CHECKING FUEL LINES WHEN THE HOOD FELL, STRIKING HIM ON THE HEAD. EE HAD A LACERATION ABOUT 2"" LONG ON THE TOP OF HIS HEAD (4 STAPLES).

EMPLOYEE WAS INSTALLING CUTTING EDGES, WHILE CUTTING SPACER PLATE OUT OF MOLD BOARD TO CLEAN DIRT OUT FROM BEHIND IT. THE SPACER PLATE HAD PRESSURE ON IT WHEN HE CUT IT LOOSE IT SPRUNG OUT & HIT HIM ON HIS FOREHEAD.

EE was hauling rock down Fork Road in a Volvo 830C when the truck kicked out of gear. He couldn't get the jake brake to work and regular brakes wouldn't hold the truck on the steep grade. 3rd trip down the hill and everything was operable on the truck til the time of the accident.

Employee was raising the hook of a utility hoist with his left hand. The hook caught a lip on the side of the tool bed, smashing his finger and pulling his fingernail off.

Employee was cutting a zip-tie with his pocket knife when it slipped cutting his left hand. The cut required 4 stitches and he returned to work the next day. He was instructed to never use his pocket knife while cutting zip-ties and to use issued cutting pliers.

After completing his pre shift. He left the parking area heading for the robinson north coal pit. He noticed the driver side door was not shut. He open the doors and attempted to re-shut it. The truck came into contant with a rough patch in the road causing the door to slam shut on his right hand.

Injured was tramming endloader to the parking lot when he encountered a rough place in the haul road. The bucket of the loader struck the ground causing pain in his lower back. Injured continued to work until 5/3/05.

AN UNINTENTIONAL FIRE WAS DISCOVERED ON THE STOCKTON COAL STOCKPILE AT 10:00 AM. THIS FIRE MEASURED 6' X 8' AND WAS BURNING AT OR NEAR THE SURFACE OF THE PILE.

Injured was air-arching a hitch pin block on the back of a parts truck when a piece of metal flew behind his safety glasses striking him in the eye.

Operator exited cab to clear offside mirror. He slipped on a bolt laying on the deck. He tried to catch his balance and his left wrist struck the breather on the truck.

EE was cutting off bolts when acetylene hose punctured at back of torch and catching fire from where he was cutting. the flame set his clothes on fire burning his right side above waist in toward center of chest.

EE AND COWORKER WERE CHANGING RIGHT REAR TIRE ON SERVICE TRUCK. TIRE WAS MOUNTED AND EE WAS CHECKING THE GAUGE WHEN THE SIDEWALL EXPLODED KNOCKING HIM TO THE GROUND.

EE AND COWORKER WERE CHANGING RIGHT REAR TIRE ON SERVICE TRUCK. TIRE WAS MOUNTED AND EE WAS IN PROCESS OF AIRING. EE LEANED OVER TO CHECK PRESSURE AND THE SIDE WALL OF THE TIRE EXPLODED KNOCKING EE OFF BALANCE.

Employee was prying on a rock with a slate bar. As the rock rolled outward it hit the bottom safety cable, the rock broke and flipped up striking employee.

EMPLOYEE WAS IN THE PROCESS OF DISMOUNTING THE 403 IR DRILL WHEN HE SLIPPED CAUSING HIM TO CATCH HIS WEDDING BAND ON THE GRILL OF THE RADIATOR. DUE TO THE LOSS OF CIRCULATION, HIS FINGER WAS AMPUTATED.

While operating a dozer ee ran up on a rock causing him to be jarred around in the cab. EE has suffered pain in his upper back and neck. On Aug 3rd he was seen by a E.R. physician and referred to a specialist. He was seen by the specialist on Aug 25th. He is still waiting on the specialist report.

MECHANIC WAS MAKING REPIARS ON ENGINE. WHILE WALKING FROM THE TOOL BOX TO THE TRUCK, HE STEPPED INTO A HOLE AND TURNED HIS ANKLE. ER DOCTOR TREATED WITH SUPPORT BRACE AND RX.

CHANGING HOIST CYLINDER. EE WAS INSTALLING THE REAR RETAINING PLATE. THE RETAINING PLATE SLIPPED OUT OF HIS HAND AND STRUCK HIM ON HIS MOUTH.

FUELER FAILED TO PROPERLY PRE-TRIP UNIT AND HAD INADEQUATE BRAKES ON A GRADE. HE EASED THE UNIT INTO A MUDDY DITCH TO STOP. VEHICLE IMMEDIATELY INSPECTED AND FOUND TO HAVE NEW BRAKES THAT WERE OUT OF ADJUSTMENT. THE EMPLOYEE CONTINUED TO WORK BUT SOUGHT MEDICAL TREATMENT FOR ALLEGED INJURIES AFTER LEARNING OF PENDING DISCIPLINARY ACTION. THE COMPANY HAS BANNED HIM FROM THE PROPERTY

INJURED WAS PULLING ON A RACHET TRYING TO LOOSEN A DRAIN PLUG. HE REMAINED WORKING UNTIL 1-28-04 WHEN HE SAW A PHYSICIAN WHO INFORMED HIM THAT SURGERY WOULD BE NEEDED TO REPAIR A TORN TENDON. THIS WILL BE AN OFFICE SURGERY AND WILL BE PERFORMED ON HIS DAY OFF. HE WILL RETURN TO HIS REGULAR JOB ON HIS NEXT SCHEDULED SHIFT.

EE AND OTHER COWORKERS WERE IN PROCESS OF GETTING OUT OF THEIR EQUIPMENT & SETTING UP IN ORDER TO WELD HOOKS ON THE CAB PROTECTOR TO SECURE THE LADDER. INJ EE CLIMBED THE LADDER TO GAIN ACCESS TO THE TOP OF THE CAB PROTECTOR & THE UNSECURED LADDER SLID OUT FROM UNDER HIM CAUSING HIM TO FALL.
