While roofbolting, a piece of roof rock hung up on the drill steel and as the steel continued to rotate, the rock broke off striking the injured on the left shoulder and head. This resulted in contusions to the head and shoulder.

The employee was bolting the crosscut 7 right when the drill steel became stuck in the drill chuck. When the employee worked it loose, it struck him in the nose.
While picking up a bottom belt roller on the #3 belt it slipped out of his hand and he mashed his right middle finger between the roller and a header board.
While roof bolting in on the 1 Left section 2 left crosscut, the injured had installed the outside bolt and was swinging the drill head in to install the inside bolt and had his right hand resting on the drill head when a piece of shale rock fell stiking his rt. hand and index finger.
A roof fall was found outby spad #400 and inby #0393. for a distance of 60' in length x 19-20' wide and 4-7' high the date and time of fall is not known. the area was supruted with 48"" resin bolts and supplemented with cable bolts 12' a horseback was present.
A roof fall above the ankorage zone of the 48' resin bolts occured on the 001-0 NMU section in #3 entry 2nd open x-cut 90' inby spad #415 and 30' outby spad #415 the length was approx. 120' long X 19-20'winex 4-7' high the intersection and supplemental 12' cable bolts installed.
While operating a Joy 21 S.C., the injured had his right hand resting on the bottom of the operator deck & he caught his rt. middle finger between the deck & the pavement.
While cutting a piece of conveyor belt with a utility knife it slipped and cut the top of his right thigh 11 sutures.

EMPLOYEE WAS CARRYING TIMBERS & MADE HIS BACK SORE BY STRAINING MUSCLES.
EMPLOYEE WAS PUTTING UP INSULATION BOARD & FELL OFF A LADDER APPROX. 10'. EE WAS TOLD BY HIS IMMEDIATE SUPERVISOR TO NOT GET ON THE LADDER. STATEMENTS AVAILABLE UPON REQUEST.
EMPLOYEE SUSTAINED AN ALLEGED EYE INJURY. TRIED REACHING EE ON SEVERAL OCCASIONS STARTING ON 9/26/05, WHEN STATE MINE INSPECTOR INFORMED 1A MINE (AFTER EE CALLED HIM ON 9/25/05). SPOKE TO HIS WIFE ON TWO OCCASIONS BUT INJURED EE HAS NOT RETURNED ANY OF THE CALLS.
