While replacing guarding after replacing tail roller @ tail of plant feed Belt. The guarding fell catching employees left Middle finger between the guarding and structure: Employee received medical attention where he had Debridement left middle finger, local tissue re-arrangement.
EE stumbled and fell while working in the plant.Medical att.was immediately.Drs exam indicated neg xrays but did validate advanced degenerative disc disease as noted in the attached drs notes on pg9 (handwritten pg#s). Employee would have returned to work but was unable to due to complications being caused by the advanced(DDD)which was discovered by the med.staff during the exam
Employee has been diagnosed with Occupational Pneumoconiosis. Marfork Safety Department received notification of his diagnosis on March 27, 2015. Employee retired in June 2014.

Employee was changing belt roller and felt pain in back.
Employee pulled groin while shoveling material from floor. Employee is scheduled for hernia repair surgery on November 11, 2014. The Safety Department will update MSHA with days lost as soon as its available.
Employee was helping to remove a sieve box from the 6th floor using chain falls and caught his finger in a pinch point.
Employee was welding sleds for top belt rollers with another Employee, when he received flash burn in his right eye. Employee was treated at Raleigh General Hospital and also followed up with Dr.
Employee was installing fence guarding under the plant feed belt. Employee kneeled down and reached out to pick up one end of a fence panel and felt a pain in his back. Employee was treated at CAMC Memorial Division where he received injections. Employee began missing work 3/17/14 and did not contact Management or Safety to inform us of his medical treatment.

Employee was lifting sheeting for the belt line and began feeling pain in his low back.
Employee has been diagnosed with Occupational Hearing Loss.
While checking rollers, as injured was walking down #3 ref belt when checking #240 top roller, swung crescent wrench at the roller, the roller grabbed the wrench and pulled injured's left arm and shoulder between the belt and roller.
Employee exercised his rights as a part 90 miner due to occupational Pneumoconiosis.
Employee was cutting a protected lid from a new 5 gallon oil can, to fill speed reducer on #5 refuge belt drive. While cutting the lid off the utility knife slipped and cut the top of his left hand, between the middle finger and ring finger.
Employee exercised his rights due to occupational hearing loss.

Employee was painting a feeder and while the employee was exiting the feeder the employee lost his balance causing the employee to fall on his right knee.
Employee was changing fuel filters on a D10R dozer and while putting the side door back on the employee felt a stinging sensation in his neck. Note: Employee did not miss first day of work until 8/11/12
He was using the pry bar end of a spud wrench to adjust a clip on a screen. The bar slipped and hit employee on bottom front tooth.
EE was standing next to #2 Deslime screen when he said he got hit on the head with a crib block. The plant was idle. A crib block 32"" was found nearby but investigators could not determine an origin. He was hospitalized for a concussion and released. He remains off work.
EE was standing on top of an enclosure that was five feet off the floor. He was removing a 1.5 t. chain fall from its anchor. He slipped and landed on his feet. The impact shattered his right heel bones. He is off work for surgery.

EE was changing springs on a vessel clean coal screen. When he reached to pull the spring out, the Harrington 3T chain hoist gave way, allowing the screen to fall down mashing his four fingers between the screen pedestal and the spring. He received a contusion only. He is off work until further notice for therapy.
Rebuild shop fabricator was removing shims from the rear differential of a 35C Fairchild scoop. A floor jack was being used to support the differential while bolts were being removed. He failed to check the jack, which was not tightened against the differential. He removed the last bolts and the differential mashed his finger to the jack. He received 24 sutures and was released.
Prep plant mechanic was stepping into the screen deck to perform routine maintenance when he scraped his right thigh on a guarding bracket. It created a puncture that required three sutures. He returned to work following the procedure.
EE was moving some 30""x6""x6"" crib blocks on the lower storage yard when he felt a sudden pain in his lower right abdomen/groin area. The weight of the crib block was not an issue. EE finished his shift, due to increased pain he sought medical attention and was diag. with a hernia. He underwent surgery on 4/6/11.
Rebuild shop fabricator was removing shrink wrap from a pallet of parts using a utility knife. He was cutting with his right hand and pulling wrap with his left. The knife slipped and cut his left thumb. He was treated at CAMC and taken off work for a few days.
Employee was getting out of the #1 deslime screen when he tripped and slipped causing him to fall. In trying to catch himself, he landed on his left ring finger causing a fracture. He did not miss work and returned on his next scheduled shift.

Employee was using a torch to cut a guard that was on the ground. He turned around and slipped, and reached out with his right hand to catch himself, and injured his right wrist.
EE was walking to B Tube when he stepped onto uneven grating which caused him to stumble and fall onto his right knee. His knee became bruised. He did not start missing work until 11/1/10 when he said it began to hurt. He is off seeking treatment. This was reportable as a file only until 11/1/10.
Employee was carrying a small dolly down the plant steps and slipped, causing him to fall down 3 or 4 steps of stairs on his right hip.
While splicing #7 refused belt, EE was using a utility knife to prepare belt. Knife slipped and cut the top of his right hand. He drove to the doctor and received four stitches. He returned to work on his next scheduled shift.

Employee was placing a nylon choker onto the locking pins of a fork lift when the operator was instructed by injured party to push the pin outwards. This caused injured's right ring and index finger to be caught in a pinch point.
Employee was helping raise a 30' long beam up a hoist opening by use of hoist. While manuvering the beam through the 2nd floor, his right hand was caught by the pinch point between the steel beam and adjacent 12"" metal pipe.

Member was removing a rubber belt guard off a continuous miner and he felt a sharp pain in his groin area. Member was diagnosed with a hernia and is scheduled for surgery to repair hernia.
Using a chisel to spread clamp to remove pin. Hit pin with hammer and chisel flew out, hitting EE on the forehead.
Employee received an annual hearing test as part of the conservation program in use at out mine site locations, at which time it was noted that he has had a recordable change in the form of moderate hearing loss for high pitched sounds in both ears. He was re-tested within the 30 day MSHA allowable period and moderate loss was confirmed.

Member was pushing refuse material with the dozer at the impoundment and he had pushed material from the belt to an area where the refuse material was being loaded out from ealier in the day. The area he was pushing to had a drop off of about 13' where he had pushed too far and the dozer went nose first, causing his face to make contact with radio, causing 2 teeth to dislodge.
EE was operating dozer on evening shift. It was a cool night so he decided to turn on the heat. EE backed dozer off pile, exited dozer, lifted engine compartment reached inside and shut off one valve then as he reached for 2nd valve he struck his right hand into the belts& pulley system causing multiple fractures 2 severed tendons & 1 severed artery on his index finger.
EE was in the process of cuting 155 ft piece of 10' schedule 80 pipe. The pipe was being held up off ground with a 980 loader while cutting the pipe in two the pipe flexed and pinned him between the pipe and the frame. EE received fractured ribs due to this accident. *Amended - ee did not fracture ribs - contusion only.*

EMPLOYEE WAS USING 1/2 IMPACT WRENCH, TAKING OFF A NUT. WHEN THE NUT WAS FULLY UNTHREADED, THE AGITATOR TO THE DIFUSER FELL OFF STRIKING HIS RT. HAND CAUSING A FRACTURE.
Employee was helping a co-worker make a splice on the 5C Overland belt. He reached to move speed loader and was struck on his right index finger by a Flexco hammer, breaking his right index finger.
EE WAS USING A TORCH TO CUT CONNECTING LINK ON CONVEYOR CHAIN SO THE CHAIN COULD BE PULLED OF S/C. THE LINK FLOPPED UP EE USED RIGHT HAND TO FLPO IT BACK DOWN AND BECAUSE OF THE WEIGHT OF THE CHAIN, IT MOVED AND CAUGHT RIGHT HAND BETWEEN THE DECK AND CHAIN FLIGHT
Employee bent down at the knees to pick up a 13"" length of 12"" pipe weighing about 100 pounds to place it on a table for grinding. As he stood up, he felt a burning sensation with the knee and tingling down into the leg which worsened over time. Employee did not seek medical treatment until 2/2/05 when the pain didn't subside.

Due to employee work history he has developed carpal tunnel disease determined by a physician. employee is scheduled for corrective surgery
EE has worked in the coal mining industry for a period of 30 years. He has been diagnosed with a lower lumbar disc herniation and deterioration.
EE DEVELOPED SEVERE CARPAL TUNNEL WORKING IN AND AROUND COAL MINES FOR 16 YEARS. THE MEMBER STARTED MISSING WORK ON 3/24/04 BUT WAS NOT DIAGNOSED OR COMPENSATION APPROVED UNTIL 4/22/04.
While installing belly pan on D10N, chain slipped off causing pan to strike EE on left knee.

EE WAS IN THE BASEMENT UNHOOKING DECK PANELS FROM THE CENTER WELL HOIST THAT WERE BEING LOWERED DOWN FROM THE 3RD FLOOR. EE WAS POSTIONING ONE DECK PANEL OVER ANOTHER WHEN HE FELT PAIN IN HIS LOWER BACK. HE BEGIN MISSING WORK ON 11/29/02 DEGREE 3 CLASS 9.
WAS STRUCK IN THE LOWER RIGHT LEG BY A PIECE OF 3' BY 20' METAL FLOOR GRATING THAT WAS SUSPENDED BY A CENTER WELL HOIST. WHILE USING A 1 TON RATCHET CHAIN TO BRING THE GRATING ONTO THE SECOND FLOOR. THE GRATING BECAME FOULED ON A HAND RAIL THE HAND RAIL BROKE SUDDENLY, ALLOWING THE GRATING TO STRIKE THE INJURED.
EE WAS REMOVING NUTS AND BOLTS FROM A HARD BAR ON A CATERPILLAR D10 N DOZER, WHEN THE 1"" AIR IMPACT WRENCH HE WAS USING SLIPPED OFF OF THE NUT CAUSING THE WRENCH TO SPIN AND FALL STRIKING EE O N THE LEFT SIDE OF HIS FOREHEAD NEAR HIS EYE BROW. DEGREE 6 CLASS 10.
EE WAS IN THE REAPRI SHOP TRYING TO REMOVE A BUSHING FROM A JOY 14CM 15 CUTTER HEAD. AFTER SPLITTING THE BUSHING WITH A TORCH, HE USED A 3202. HAMMER AND A CHISEL TO REMOVE THE BUSHING. WHILE STRIKING THE CHISEL A PIECE OF METAL SLIPPED OFFOF THE BUSHING AND STRUCK EE IN THE LEFT LEG.
EMPLOYEE WAS WALKING DOWN METAL GRATING STAIRS FROM THE SECOND FLOOR OF THE STOKER PLANT. HIS FEET SLIPPED ON THE WET GRATING AND HE FELL BACKWARDS. THE FORCE OF THE IMPACT CAUSED A SHARP PAIN IN HIS RIGHT SHOULDER. EE CONTINUED TO WORK WITHOUT RESTRICTIONS UNTIL OCT 7, 2002, WHICH WAS HIS LAST DAY OFF.
EE WAS ASSISTING IN CUTTING OUT A SUMP WHEN A COWORKER BUMPED HIM ON THE LEFT ELBOW WITH A SLATE BAR OR CHAIN FALL HOOK. EE CONTINUED TO WORK WITHOUT RESTRICTION OR LOST WORK DAYS UNTIL 9/9/02 WITH INITIAL DIAGNOSIS OF CONTUSION.
EE WAS CUTTING OUT A PIECE OF METAL ON THE SUMP ON THE FIRST FLOOR. AFTER CUTTING THE METAL, CORNER BECAME CAUGHT. WHILE ATTEMPTING TO KNOCK IT FREE WITH HIS FOOT, IT SUDDENLY FELL FREE CAUSIN G HIM TO LOSE HIS BALANCE. HE FELL CATCHING HIS LEFT ARM ON THE MESH LINING INT THE FRESHLY CUT METAL, CAUSING A SEVERE LACERATION. THE EE WAS WORKING AT A HEIGHT OF 5'6"" BUT WAS USING FALL.
EMPLOYEE WAS SUPERVISING HIS CREW WHILE THEY TRIED TO REMOVE THE RIGHT END CUTTER DRUM FROM A CONT MINER IN THE SHOP AREA, THEY WERE USING A 4"" BY 2"" BAR WELDED TO THE DRUM AND PUSHING IT WITH A PORTAPOWER WHEN THE BAR BROKE STRUCK EE IN THEUPPER CHEST.
EMPLOYEE WAS WALKING DOWN STAIRS AT PLANT CONTROL ROOM, WHEN HE REACHED THE THIRD STEP FROM THE BOTTOM, HIS LEFT ANKLE TWISTED CAUSING HIM TO FALL. EE RETURNED TO WORK ON HIS NEXT SHIFT WHICH WAS 2-14-02.

INJURED WAS BEATING ON FROTH CELL PEDDLES TO SEPERATE SHAFT FROM COUPLING, WHEN HE MISSED WITH 4 LB HAMMER. AS A RESULT, THE INJURED HIT HIS LEFT THUMB WITH THE HAMMER MASHING IT BETWEEN THE H AMMER AND METAL HIS THUMB WAS RESTING AGAINST (THIS IS DEGREE 6 CLASS 10.
THE EMPLOYEE WAS STANDING IN A SCREEN REMOVING SPRAY NOZZLES FROM SPRAY BARS USING A PIPE WRENCH. THE PIPE WRENCH SLIPPED AND THE EMPLOYEE FELL BACK AND STRUCK HIS ELBOW.

INJURED WAS CHANGING FUEL FILTER ON A DOZER. HIS FOOT WAS POSITIONED ON THE TRACKS AND HIS RT. FOOT SLIPPED CAUSING HIM TO FALL, CAUSING A MUSCLE STRAIN & STRAINED LIGAMENTS TO RT. LEG. HE RET URNED TO WORK UNTIL 5-7 AND RETURNED TO DR ON 5-10 TO BEGIN THERAPY.
