Fall from machine

Employee and another member were changing a bottom roller on the Cross Hollow belt at the # 130 mine. Injured was laying on the bottom belt changing the roller. The belt suddenly started and traveled approximately 49' before employee fell approximately 39' to the ground. EE passed away on 12/14/07 at 11:30pm.

Employee was assisting 2 co-workers slide track rail off the forks of a 930 G Loader onto a flat car. As he slid on of the rails off the forks onto the flat car one edge dropped onto his right foot, above the metatarsal, and caused a small fracture to the proximal phlange of his right big toe.

EE was changing a hydraulic jack on the A.L. Lee minitrac forklift. The forks were raised so he could disconnect the hoses, but no blocks were installed. EE disconnected the hoses to the jacks and the forks bled off and pinned him against the forklift.

The #3 S/C was going to right of miner and #2 S/C was coming to feeder from #1 miner. The #2 S/C hit #3 S/C 22"" from back of boom in the belt entry. EE's right side hit the armrest. EE was operating the #3 S/C.

Removing dirt that was in the crevis of the PTO on mine light battery, he was holding battery with his left hand and using utility knife with right hand to remove dirt. Knife slipped and caused a laceration between thumb and index finger of left hand.

EE WAS HAMMERS IN A SCOOP WITH A SLEDGE HAMMER. ON A DOWNWARD STROKE, THE HANDLE CAUGHT HIS LIGHT CORD. THIS CAUSED HIS FINGER TO BE CAUGHT WITH FORCE BETWEEN THE HAMMER HANDLE AND THE FRAME O F THE SCOOP.
