SECURITY GUARD WALKING BACK TO GO INSIDE GUARD SHACK AND TRIPPED ON CEMENT EDGE OF PORCH THEREFORE FALLING AND INJURING WITH A KNOT ON HEAD AND 2 BROKEN ARMS PER ARMS SUPERVISOR STATED EMPLOYEE HAD TOLD SUPERVISOR AFTER TREATMENT

TRESSPASSER ON PROPERTY. GUARD WAS STANDING AT BACK OF THE VEHICLE WAITING FOR TRESSPASSER TO STOP. THE TRESSPASSER HIT THE GUARD PINNING EE BETWEEN VEHICLES CAUSING GUARD TO HAVE HIP PAIN.

Employee was at the end of catwalk coming off of raw coal silo concrete pad when foot slipped off edge of concrete pad twisting left knee

Employee was closing car doors on train cars using a pry bar when the pry bar slipped causing them to fall

Employee was cleaning and shoveling spillage on top of #1 and #2 raw coal silos while shoveling EE got a sharp pain in EE's lower left back. Injured employee work up till 3/27/2019.

The injured employee was cleaning up some metal and got EE's left index finger caught in between to pieces metal and received stitches.

Employee was cleaning a tank vessel. As he exited there was a sharp edge around the entry/exit point. Employee lacerated right middle finger - 5 sutures.

The driver was beginning to dump in the top truckdump at the Consol No. 1 plant and while dumping the truck turned over.

Bitten by stray dog on mine property, bite to right forearm. Employee was getting lunch out of vehicle when dog approached.

The driver was hauling coal to the plant from spruce creek was starting down a hill the truck got out of gear and the engine quit on the truck he could not get it back into gear he went down the hill and turned over.

Employee was exiting Co. Vehicle when middle finger on right hand came in contact with a sharp object on the headrest. Employee sustained laceration to middle finger requiring 10 sutures.

This report is for compliance, contractor feels the accident was not fault of driver or company. The truck hauled in a load of dirty coal, bed staged out and load did not dump, passenger side broke loose, driver's side stuck. Unbalanced truck flipped over, truck was level, but when load stuck, driver's side wheels sank, adding to turnover. Coal came from another mine site.

Employee was grinding some welds and the grinder kicked back and hit him in the face.

While employee was changing a mop head, one side of the head stuck & slipped across his left hand, cutting his index finger on his left hand, on the knuckle.

He was changing tires on a 10 wheeler and twisted his back.

Employee was changing out a 72"" top belt roller on raw coal belt & strained his lower back.

Lifting a spring he hurt his back.

EMPLOYEE LIFTED A 12""SCH 80 PIPE & SUFFERED A LOWER BACK STRAIN. THIS INJURY WAS NOT REPORTED UNTIL 3/23/06 WHEN MEDICAL ATTN. WAS SOUGHT.

Employee was working on the elevator in the tipple and it was stuck between floors. When it came loose it cut his finger.

EE WAS HOISTING A SCREEN MECHANISM BEAM. IT SPUN AND PINNED HIM AGAINST A MOTOR SUPPORT BEAM CAUSING A MUSCLE STRAIN TO HIS LOWER ABDOMEN REGION

Employee was grinding - using grinder - & got a piece of metal in his right eye.

Employee sampling on auger platform, stepped on inspection hole cover (metal plate). Employee fell through hole along with metal plate cover. Fall was approximately 9.5 to 10 feet to the ground.

Employee was assisting three others in replacing accuator arm using chain hoists. One man let go of his end and accuator fell on employee's shoulder and neck causing muscle strain. This was not reported to office until 9/12/05 when employee stopped work due to pain from this injury.

Rocks falling off beltline struck subject in the head.

Employee was tightening a bolt & socket broke. His hand hit expanded metal guard cutting 3rd finger on right hand.

Employee was putting idler rollers on a belt & he was jerking on the belt roller to get it in place. Approx. 15 minutes later he states he felt pain in his back.

EMPLOYEE STEPPED ON LOOSE ROCK AND TWISTED LEFT ANKLE WHEN HE SLIPPED

While dismounting dozer, he slipped on bottom step and twisted his back. He hurt his back at home and missed several months of work and this is a recurrence of his accident at home.

EE WAS WALKING DOWN THE STEPS OF THE OFFICE TRAILER AND TRIPPED ON A CINDER BLOCK AND FELL TO THE GROUND. INJURY TO HER BACK, RIGHT ANKLE AND WRIST.

WHILE PULLING A SPEED REDUCER INTO THE WAREHOUSE, EE FELT PAIN IN NECK AND BACK.

WHILE LIFTING A HYDRAULIC JACK, HE FELT PAIN IN HIS BACK. DIDN'T START MISSING UNTIL 4/14/03.

EE WAS GRINDING A METAL WITH A GRINDER, AND A PIECE OF METAL FLEW INTO HIS LEFT EYE.

EE WHILE LIFTING 55# BAG OF FLOG GOT PAIN IN HIS NAVEL AREA.

EE WAS WALKING DOWN 2TH FLOOR STEPS AND SLIPPED AND FELL BACKWARD AND HIT HIS HEAD ON HAND RAIL, HURTING HEAD AND BACK.

EE WAS INSTALLING BATTERY INTO 992 C END LOADER AND GOT PAIN IN BACK.

EE WHILE DISMOUNTING FROM D8N DOZER, HIS FOOT SLI-PPED AND HE FELL TO GROUND, HITTING HIS BACK ON BACKHOE AND HURT RIGHT KNEE.

EE WAS WALKING AT TAILPIECE AT CC7 AND STEPPED IN LOOSE COAL AND SLIPPLED AND FELL, LANDING ON BUTTOCK, HURTING NECK. EE STARTED MISSING WORK ON 12-05-02.

WHILE PICKING UP A 50 LB BAG OF FLOC HE FELT PAIN IN HIS BAG.

EE WAS PUTTING 8"" PIPE INTO PIPE STAND AND PIPE STAND CRILLED OVER. PIPE HIT HIM ON LEFT KNEE, CAUSING A CUT TO THE KNEE REQUIRING 8 STITCHES.

EE HAS A HISTORY OF BACK PROBLEMS. HE IS 53 YRS. OLD. HE CARRIED SOME BOTTOM BELT ROLLERS UP BELT LINE AND AFTERWARDS HIS BACK WAS HURTING HIM. THIS IS AN OCCUPATION ILLNESS. I TALKED TO SUPER VISOR IN DENVER ABOUT THIS. DIDN'T MISS WORK UNTIL 4-2-02.

EMPLOYEE WAS CLIMBING DOWN STEPS ON END LOADER AND GOT FOOT HUNG IN STEPS AND FELL TO GROUND TWISTING RIGHT KNEE. EMPLOYEE STARTED MISSING WORK ON 2-22-02.

STARTED MISSING WORK ON 9-24-01 DUE TO CARPAL TUNNEL SYSDROME.

EMPLOYEE WAS CLEANING OUT AROUND TAIL ROLLER. HE FELT PAIN IN HIS BACK. EE HAS A HISTORY OF BACKPROBLEMS. THIS IS A RECURRENCE OF OLD BACK INJURY. (REPEAT TRAUMA) LET HIM ON LIGHT WORK DUTY.

EE HURT RIGHT KNEE WHEN HE JERKED IT MOUNTING MANLIFT ON 4TH FLOOR.

EE WAS HELPING TEAR DOWN OLD OFFICE BUILDING AND FELL OFF BUILDING INJURING HIS BACK & HIP. FRACTURE TO BACK AND HIP.

EE WAS ON TOP OF A DRYER AND WAS USING A SLATE BAR TO PRY OPEN THE GATE. THE BAR SLIPPED AND HE FELL OFF DRYER ABOUT 5 FEET AND LANDED ON HIS FEET. THIS CAUSED A JAR AND TWIST TO HIS BACK. REA SON ACCIDENT REPORT IS LATE, I COULDN'T GET HIM ON THE PHONE TO EXPLAIN WHAT HAPPENED.

EE WAS WORKING ON D & R R/C SCREEN AND JUMPED DOWN FROM SCREEN FRAME ONTO CONCRETE FLOOR AND HURT LOWER BACK.

EMPLOYEE WAS REPLACING MUFFLER ON D8 DOZER AND HIT HIS LEFT THUMB WITH HAMMER RESULTING IN CUT REQUIRING 6 STITCHES.

EMPLOYEE WAS SHOVELING UP LOOSE COAL ON 2ND FLOOR OF PREP PLANT AND STRAINING HIS BACK. EMPLO9YEE STARTED WORKING ON RESTRICTED DUTY ON 9-11.

WHILE MOUNTING DOZER HE SLIPPED AND FELL ON DOZER TRACKS HURTING BACK, NECK, AND BOTH KNEES.

EASTERN MINGO COAL STOCK PILE WAS ON FIRE

CLEANING FROZEN COAL FROM RAW COAL FEEDER IN SLID WITH SLATE BAR AND EE AND FELT PAIN AND NUMBNESS IN LOWER BACK. EE STARTED MISSING WORK ON 6-12-00.

EE AND COWORKER WERE REMOVING A BROKEN SHEAR PIN FROM CC-4 CRUSHER USING A PORTA POWER PUSHING THE PIN AND A RATCHET AND SOCKET WITH A WELDED STUD PULLING THE FROZEN PIN. THE PIN BROKE FREE FOR ABOUT AN INCH AND EE SLIPPED STRIKING HIS HEAD INTO THE WEBBING OF THE CRUSHER CAUSING A CUT ON HIS FOREHEAD.

EASTERN MINGO COAL STOCKPILE WAS ON FIRE.
