HE DID THIS ABOUT 3 OR 4 TIMES WHEN PRESSURE OR SOMETHING CAUSED THE PIPE TO MOVE POSSIBLY STRIKING HIM. THE PIPE RAISED UP AND LANDED ABOUT 80' UP THE HILL.

Employee was closing car doors on train cars using a pry bar when the pry bar slipped causing them to fall
Employee was cleaning and shoveling spillage on top of #1 and #2 raw coal silos while shoveling EE got a sharp pain in EE's lower left back. Injured employee work up till 3/27/2019.

The driver was hauling coal to the plant from spruce creek was starting down a hill the truck got out of gear and the engine quit on the truck he could not get it back into gear he went down the hill and turned over.

This report is for compliance, contractor feels the accident was not fault of driver or company. The truck hauled in a load of dirty coal, bed staged out and load did not dump, passenger side broke loose, driver's side stuck. Unbalanced truck flipped over, truck was level, but when load stuck, driver's side wheels sank, adding to turnover. Coal came from another mine site.
Employee was grinding some welds and the grinder kicked back and hit him in the face.

EE WAS HOISTING A SCREEN MECHANISM BEAM. IT SPUN AND PINNED HIM AGAINST A MOTOR SUPPORT BEAM CAUSING A MUSCLE STRAIN TO HIS LOWER ABDOMEN REGION
Employee was grinding - using grinder - & got a piece of metal in his right eye.
Employee sampling on auger platform, stepped on inspection hole cover (metal plate). Employee fell through hole along with metal plate cover. Fall was approximately 9.5 to 10 feet to the ground.
Employee was assisting three others in replacing accuator arm using chain hoists. One man let go of his end and accuator fell on employee's shoulder and neck causing muscle strain. This was not reported to office until 9/12/05 when employee stopped work due to pain from this injury.
Rocks falling off beltline struck subject in the head.
Employee was tightening a bolt & socket broke. His hand hit expanded metal guard cutting 3rd finger on right hand.
Employee was putting idler rollers on a belt & he was jerking on the belt roller to get it in place. Approx. 15 minutes later he states he felt pain in his back.

EE HAS A HISTORY OF BACK PROBLEMS. HE IS 53 YRS. OLD. HE CARRIED SOME BOTTOM BELT ROLLERS UP BELT LINE AND AFTERWARDS HIS BACK WAS HURTING HIM. THIS IS AN OCCUPATION ILLNESS. I TALKED TO SUPER VISOR IN DENVER ABOUT THIS. DIDN'T MISS WORK UNTIL 4-2-02.

EE WAS ON TOP OF A DRYER AND WAS USING A SLATE BAR TO PRY OPEN THE GATE. THE BAR SLIPPED AND HE FELL OFF DRYER ABOUT 5 FEET AND LANDED ON HIS FEET. THIS CAUSED A JAR AND TWIST TO HIS BACK. REA SON ACCIDENT REPORT IS LATE, I COULDN'T GET HIM ON THE PHONE TO EXPLAIN WHAT HAPPENED.

EE AND COWORKER WERE REMOVING A BROKEN SHEAR PIN FROM CC-4 CRUSHER USING A PORTA POWER PUSHING THE PIN AND A RATCHET AND SOCKET WITH A WELDED STUD PULLING THE FROZEN PIN. THE PIN BROKE FREE FOR ABOUT AN INCH AND EE SLIPPED STRIKING HIS HEAD INTO THE WEBBING OF THE CRUSHER CAUSING A CUT ON HIS FOREHEAD.
