Installing track on D11R and tried to fold link down with prybar. Bar slipped out of bolt hold and struck ee under his chin, chipping 3 teeth.
Cutting a piece of plastic air line, knife slipped and cut left thumb, 5 stitches.
Employee stated that he stepped in a drill hole that was hid by a small amount of dirt on top of the hole. Employee reported accident on 4-12-10.

Loader man was putting 2nd bucket on truck. Rock slipped out of bucket hitting off side of truck bed, jarring driver.
Employee was boring saddle bore on a D9 Dozer and was removing the tooling after finishing the repair. He was attempting to inch the shaft in one direction only 1/4 of a turn in order to access the screws. During one turn, he missed the stop button and the shaft spun around further than 1/4 turn and struck him in the chin, causing laceration to chin.
Employee stated that while climbing on the powder truck he fell/slipped and hurt his back. Reported to Management 4-3-07. Went to Doctor 4-6-07.
Employee stated that while pushing rock in a shot his foot slipped off throttle causing him to jerk his neck. Reported to foreman at 3:30 am.
Employee informed supervisor on 10-19-06 that he hit his head on the door support while getting out of grader. He did not miss work or complain about this injury until 1-4-07 at which time he informed his doctor about this injury.

While trying to brake loose steel using a hydraulic wrench, employee mashed his middle finger. ""Two steels in the ground at the time of the accident"".
Employee was getting off the drill when he twisted his right ankle.
EE was operating a D-11 dozer. Dozer slipped off a rock and he jarred his back.
Employee was removing wear liners from dozer blade when the middle liner that was resting on the blade and ground slipped and slid forward pinning the technician's right ankle/leg to the ground.
EE WAS OPERATING A 785 ROCK TRUCK, WHEN A DOZER BACKED INTO THE ROCK TRUCK. EE STATED THAT THIS JARRED THE TRUCK AND CAUSED HIS BACK TO HURT.

Dry Lenses that were in the engine compartment were ignited by a from spark thus burning fuid line into which burnt the rust of the machine.
Employee was walking underneath large mining shovel and was attempting to step over and around a muddy area when he lost his footing. When he tried to correct and regain his footing, he rose and struck his head on part of the equipment.
While operating a 992G loader, the employee was bounced in the seat, the seat belt pulled against his stomach.
Employee had completed pre-shift inspection. As he was getting on the loader, he slipped and caught himself with his left arm. This strained his shoulder.
Employee was under a machine securing the wheels and when he was raising up from a squatting position, he forgot that the safety cable was in place and ran into the cable, hitting and cutting the bridge of his nose.

EE OPENED THE PARTS TRAILER AND WAS SURPRISED BY A HOUSE CAT WHICH HAD BECOME TRAPPED INSIDE. WHEN STARTLED, THE EE TRIPPED DOWN THE STAIRS AND FELL ON HIS LEFT ARM.
WHILE UNLOADING A CRIB BLOCK FROM HIS TRUCK, HE STEPPED ON A SMALL ROCK AND TWISTED HIS RIGHT ANKLE.

THE INJURED, A SECURITY GUARD, WAS MAKING HIS ROUNDS AND CHECKING PROPERTY WHEN HE OBSERVED 3 4 WHEELERS (ATV) PARKED NEAR A DOZER. GOT OUT OF VEHICLE, CHECKED DOZER, FOUND NOTHING NEAR A DO ZER. GOT OUT OF VEHICLE WHEN HE WAS WALKING BACKTOWARDS VEHICLE WHEN HE WAS STRUCK IN BACK OF HEAD AND KNOCKED UNCONSCIOUS FOR 30-40 MIN.
WHILE LIFTING A REAR SPRING ON A SERVICE TRUCK, THE PERSON FELT PAIN IN BACK AND GROIN AREA.

EMPLOYEE STATED THAT HE HAD COMLETED GREASING THE BUCKET ON A 988 LOADER WHILE STANDING ON THE LOWER LEVEL OF THE LOADER HE ATTEMPTED TO STEP DOWN AND WHEN DOING SO, HE APPARENTLY TWISTED HIS KNEE CAUSING IT TO GIVE OUT. THE EE HAS HAD PREVIOUS KNEE INJURIES AND APPARENTLY RE-INJURED THE KNEE.

EMMPLOYEE WAS OPERATING LOADER, LOADING MATERIAL INTO 785 ROCK TRUCK. THE REAR OUTSIDE TIRE ON THE TRUCK BLEW OUT AND SENT A 2""X 2"" ROCK THRU THE SIDE GLASS ON THE LOADER STRIKING HIM IN THE M OUTH. 5 STICHES IN LOWER LIP, 3 TEETH EXTRACTED.

EMPLOYEE WAS KNOCKING A TOOTH OFF HTE LOADER BUCKET. HE STRUCK IT WITH A HAMMER AND A PIECE CHIPPED OFF STRIKING HIM THE ABDOMEN. DR'S REMOVED THE IMBEDDED METAL AND APPLIED TWO STITCHES TO CLOSE THE WOUND.
OPERATOR WAS USING THE LOADER TO PUSH A ROCK OUTOF THE PIT AREA. THE ROCK SLIPPED CAUSING THE LOADER TO BOUNCE VIOLENTLY. THE OPERATOR WAS THROWN INTO THE TOP OF THE CAB WHICH RESULTED IN HIS NECK AND BACK BEING STOVE UP. HE STATES HE WAS WEARING THE SEAT BELT.
EMPLOYEE SAYS HE SLIPPED AND FELL ON THE STEPS TO THE FUEL BARRELS TWISTING HIS KNEE.

EMPLOYEE WAS WORKING UNDER D11 DOZER. HE CLIMBEDOUT AND HAD TO CROSS THE TILT CYLINDER. HE TRIPPED OVER THE CYLINDER AND FELL, CATCHING HIS FALLWITH HIS RIGHT HAND.
