Rock Truck bed safety latch came back and hit EE in the mouth.

Struck against stationary object

Employee was hosing the floor in the breaker building and as he started out the back side doorway that had a piece of curtain hung for a door he caught his left forearm on a jagged piece of metal hidden by the curtain causing a laceration which required 8 stitches to close.

Injured was cleaning up coal he had spilled. He was shoveling it back into the bed of his trailer. When letting his tailgate back down he had propped open he had a shovel in his left hand and the tailgate in the right hand straining a tendon in his right arm.

EE was working inside the #1 scalping screen, upon exiting the screen, he twisted his left knee while trying to make it from the discharge lip of the screen over to the crusher chute then onto the breaker building floor.

Injured was fixing lights using a utility knife, when the knife slipped, cutting his left finger. Injured went to the hospital and received 3 stitches. Management was not aware of the accident until 10-1-10.

Injured was cutting conveyor belt with a utility knife when he cut himself on the right thigh resulting in a laceration which required staples to close. Injury was to a contract laborer, operator was made aware that staples were administered on 4-2-09

EMPLOYEE WAS CLIMBING OUT OF RAW COAL SCREEN WHEN HE SLIPPED AND FELL BACKWARDS HITTING HIS LEFT SHOULDER.

Fall from ladders

Injured was fixing lights on a Benson trailer #132 standing on a ladder. He slipped and fell off ladder onto a concrete pad. Took to hospital by supervisor. Has a slight concussion to head and hurt right elbow.

Employee was washing a truck and slipped and fell on snow covered bank. Injured did not report injury until 1/29/07.

Struck against stationary object

Employee was installing raw coal screens. He was using a long handled sledge hammer to pound them into place. As he swung the hammer downward, the hammer handle came in contact with the screen deck causing his right middle finger to strike the screens causing a laceration.

Standing on a bucket of degreaser to reach maint. board. Foot went through the top of the bucket & splashed other leg causing 2nd degree burns.

Struck by flying object

Employee was pushing refuse with a cat D8 dozer. He stopped to spot the refuse truck and started to get a burning, watering sensation in his left eye. He finished his shift and went to hospital and had a metallic foreign object removed from his eye.

EE called supervisor 3-12-05 that he had pulled a muscle & could return to work on 3/15/05. He also told supervisor he had pulled the muscle lifting a tire.

EE went to supvervisor on 1-8-05 & gave him a dr's excuse to be off til 1-15-05. ee came back on 1-15-05 wihtout a release. he was told he could not come back to work w/o release. ee came backwith a light duty release. hwm does not offer light duty. he then for the first time said that he hurt himself lifting a tire. he has not returned to work.

EE WAS DOING MAINTENANCE WORK USING A PRY BAR. THE BAR SLIPPED CAUSING SUDDEN JERK ON ARM & SHOULDER.

MINER REACHED OUT TO GRAB THE HAND RAIL TO MOUNT A FRONT END LOADER AND SLIPPED ON A PATCH OF ICE. HE FELL TO THE GROUND AND FRACTURED THE MIDDLE FINGER ON HIS LEFT HAND.

MINER STRUCK SPROCKET SEGMENT ON DOZER TRACK WITH SLEDGE HAMMER. A SMALL PIECE OF METAL BROKE OFF, FLEW UP AND LODGED IN HIS CHIN. HE WAS TAKEN TO HOSPITAL WHERE THE METAL WAS REMOVED.

EE WAS ATTEMPTING TO DESCEND A FLIGHT OF STAIRS.HE SLIPPED ON AN OBJECT UNDER THE SNOW AND FELL DOWN THE STEPS RESULTING IN A SPRAINED SHOULDER.

MINER WAS WELDING ONT HE INSIDE OF SCALPING SCREEN. WHILE HOLDING HIMSELF IN POSITION, HIS LEFT FOOT SLIPPED OFF THE CENTER FRAME SUPPORT STRIKING HIS LEFT SHIN ON A SUPPORT BAR.

WHILE USING A SLATE BAR TO ROTATE A SCREEN MECHANISM TO REMOVE TIMING PINS, THE BAR SLIPPED CAUSING THE SHAFT TO SWING BACK. THE PIN SNAGGED THE VICTIM'S GLOVE AND PULLED HIS HAND IN BETWEEN T HE PIN AND MECHANISM RESULTING IN INJURY TO THE 2ND FINGER OF THE RIGHT HAND.

EE COAL TRUCK DRIVER WAS EXITING HIS MACK CH 612 COAL TRUCK IN THE PARKING LOT AT THE TRUCK SHOP LOCATED AT THE PREPARATION PLANG AS HE PLACED HIS LEFT FOOT ON THE GROUND HE SLIPPED ON SOME IC E AND TURNED HIS LEFT ANKLE FRACTURING IT.
