While lifting pumps onto a pickup, employee felt a burning sensation in his groin area. Employee was examined on 01/26/2015 and was diagnosed with a hernia, and received a full release to return to work.

Employee was removing charger cables from bobcat battery, when his right little finger contacted the fan blades causing a dislocation. A splint was applied.
Employee was removing a piece of brattice cloth from the heavy media vessel when he felt a sharp pain down his right leg and in his back.

Moving a welding machine using a forklift. He stopped the forklift, set the parking brake, and turned it off after he lowered the welding machine onto the ground. As he was unhooking a stabilizer chain the welder shifted on the ground pulling the chain and the tip of his index finger was mashed between the chain and the forklift mast. Was wearing leather gloves.
Employee stepped up onto a ladder and his foot slipped off the rung, causing him to twist his ankle, resulting in a mild sprain.
Employee was welding and leaned to get another welding rod. When he did, his neck and arm contact hot metal causing a burn to the right side of his neck. He continued to work until 9/28/10 at which time he sought medical treatment for an infection to the burn.
Employee was stepping over to the side while a fork truck was being used to lift some angle and channel off the ground. He stepped on a lifting lug that was sticking up out of the ground with his left foot, causing him to twist his ankle.
Employee was moving a piece of grated flooring material and it brushed against his forearm, causing a laceration.
Employee was shoveling and noticed what appeared to be an insect bite near his right eye.
Employee was removing a guard from a pump in the basement area when it shifted and the corner hit his left arm causing a gash requiring stitches.

EE was using a sleever bar to line up a bolt hole for a brace in a truss that was set on the ground. While attempting to put a bolt in the other end, standing on a clip about 26"" off the ground, his foot slipped causing him to fall against the sleever bar.

EE was steam cleaning a 980 end loader, pulled the hose from off the steam cleaner, and bumped his right elbow on the end loader counterweight. he sought medical attention and thing broken. he continued to work until july 27th when he began misisng work. he was admitted to the hospital due to welling having to have his elbow drained.

EMPLOYEE WAS HELPING TO CHANGE OUT BELT ROLLER. AS HE WAS DOING SO, A ROLLER SLIPPED AND CAUGHT HIS RIGHT HAND BETWEEN THIS ROLLER AND THE BELT STRUCTURE.

EE WAS LOADING A TRUCK. SAID HE HIT A BUMP AND FELT PAIN IN HIS BACK. HE SAT DOWN, ATE LUNCH, AND ERNT TO 6TH FLOOR TO HELP CHANGE A CYCLONE PIPE. WHILE WALKING ACCROSS THE FLOOR HIS RIGHT LEG & FOOT GAVE OUT.
EE WAS ATTEMPTING TO TRIM A TORN MUD FLAP WITH A POCKET KNIFE. THE KNIFE SLIPPED & CUT AN INCISION ACROSS ALL FOUR FINGERS ON THE LEFT HAND. ALL FOUR FINGERS REQUIRED STITCHES.
EE WAS FITTING 14""PIPE, 20""LONG, WHEN HE SLID THE PIPE BACK TO CUT THE PIPE, THE PIPE STARTED TO FALL AND HE MOVED TO STEADY IT. AS HE DID SO, HE EXPERIENCED BACK PAIN, BUT CONTINUED TO WORK UNTIL 7-31-02. (HIS LAST WORKDAY WAS 7-30-02).

EE WAS DESCENDING FROM A CRANE OPERATING CAB. HIS LEFT HAND SLIPPED OFF THE RAIL AND HE LOST HIS FOOTING. HE CAUGHT HIMSELF WITH HIS RIGHT HAND THUS HANGING BY HIS RIGHT ARM NOT TOUCHING THE G ROUND. DISCOMFORT WAS NOT A PROBLEM INITIALLY, BUT AS TIME PROGRESSED HE EXPERIENCED GREAT DISCOMFORT. A DR. VISIT PLACED HIM ON RESTRICTED WORK DUTY.
EMPLOYEE HAD BEEN AWARDED A COMPENSATION BENEFIT OF 13.2% FOR A PARTIAL HEARING LOSS CLAIM.
EMPLOYEE WAS CLEANING ROCK OFF THE TOP OF THE GRIDS THAT COVER THE CLEAN COAL FEEDER. HE STEPPED INTO ONE OF THE OPENINGS CAUSING HIM TO TWISTHIS RIGHT KNEE. HE CONTINUED TO WORK UNTIL 7/10 AT WHICH TIME HE VISITED THE DOCTOR AND WAS TOLDTO REMAIN OFF WORK UNTIL HE COULD SCHEDULE AN MRI.
