Over-exertion in pulling or pushing objects

Employee stated employee and another employee were removing a bearing from the top of the load out. As employee pulled on the bearing employee felt pain in employee's middle back. This resulted in a back strain and lost work days. The employee began missing work on 7/11/17.

Struck against a moving object

Employee driving down the hill and wrecked vehicle, went over the hill. EE told the supervisor that steering wheel locked up and gas petal stuck. EE told supervisor ee had cut knuckle and a concussion and could return to work on 2/16/17

A fire occurred in the coal storage silo feeder area, above the Unit 410 Belt Feeder. No persons were injured during this event.

Over-exertion in lifting objects

Employee stated that he and another employee were lifting a piece of floor grating into place on top of the clean coal silos. As he lifted, he felt pain in his left shoulder. The employee was diagnosed with a SLAP (superior labrum from anterior to posterior) Lesion and began missing work on 12/9/2014 due to the injury.

Fall onto or against objects

Employee stated that he and another employee were carrying a 34"" x 34"" piece of sheet metal up the belt catwalk. The employees foot slipped causing him to fall against the hand rail resulting in a back sprain.

Employee stated that he was walking the catwalk and as he made the 45deg turn to another catwalk he felt pain in his lower back.

Employee stated that he was getting out of the mantrip when he stepped on ice/snow and slipped causing him to fall to the ground and twist his back. This resulted in a back strain. Employee continues to work, but was placed on restricted duty and began receiving medical treatment on 5/6/14.

The operator was provided with medical documentation diagnosing the employee with Carpal Tunnel Syndrome on 7/8/2014.

Employee stated he employee was helping remove a piece of stainless steel expansion metal from the float platform of the pond pump. The metal popped off the bolt and caught employee's right wrist just past the right glove. The cut was approx. 2 inches long. He was treated with first-aid and transported to Boone Memorial Hospital, where he received 8 sutures to close the laceration.

EE was at work when he felt pain in his back. He was seen by doctor and diagnosed with degenerative disc disease at multiple levels.

EE was positioning a stainless steel chute over the top of the heavy media cyclone sump. The chute was hanging by 2 electric hoists & being lowered into the sump. The area was tight causing it to hang up & it had to be shaken or jarred loose. In an attempt to free it, EE put his hand on the hoist block & pushed. The block then twisted & caused a laceration.

Employee was removing broken belt from the A-side static thickner drive when his hand was pulled against the drive pulley. This resulted in a laceration that required 9 stitches to close.

EE stated he stepped om a slick spot on the floor and fell, he struck his head suring the fall, causing a laceration to the back of his head. EE received 8 staples to close the wound and returned to work the nest shift.

Fall down stairs

Employee state that after he cleaned out the chute at the head of the plant feed belt, he stepped down from the grating and his foot slipped causing him to fall. He caught himself with his right hand causing injury. He began missing work on 3/13/08.

Employee stated after refueling his 777C truck he was climbing the entrance ladder in order to enter the operators compartment. In the process of going up the ladder his foot became caught on the third step up and caused him to slip. He stated that he was able to hold onto to the handrail but this caused him to twist his low back. He continue to work until 9/28/07.

Employee stated that he was checking gravity reading on B-vessel headbox on the 5th floor of plant. When he stepped off of the platform his right knee twisted and he fell to the floor. He said he felt pain in his right knee and right elbow.

Upon an investigation was noted: On 1/8/07 at 4PM, EE was told to clean up 276 belt and 275 belt. EE who had been pulling on the water hose, was seen laying on the ground. An assessment was started immediatley and no paulse could be detected. The AED and CPR were administrated until an ambulance arrived at the scene. He was taken to ER and treated for a heart attack.

EE stated he was pulling water hose onto dozer. The hose got hung up on blade causing his right little finger to bend back. The ee continued to work his regular duties with intermittent trauma causing discomfort & swelling. He has been diagnosed with inflamation in his wrist & little finger. X-rays show no fractures or dislocations. He was taken off work 2/5/07.
