While the injured was attempting to change the batteries on the # 8 scoop, the # 11 scoop was traveling outby and impacted the # 8 scoop causing it to move and pin the victim against the coal rib.

Sitting in the maintenance office and cleaning EE's ear with a pocket screwdriver. The chair had wheels and EE went to lean back and felt like EE started to fall; EE's reaction to balance self caused EE to poke self in the eye with the screwdriver.
The employee was using a chain ratchet (3-ton) to secure a tailpiece into the scoop bucket. As EE ratcheted it back the body of the ratchet twisted and the handle pinched EE's hand between it and the scoop bucket.
The employee was carrying a top roller to install for section advancement and stepped into a divot in the bottom. EE's ankle popped and twisted in the process.
Employee states that while cleaning up rock a piece of top measuring approximately 5"" wide, x 6"" long, x 5"" thick roof fell from between the bolts and struck EE on the leg.
Employee states as EE was walking down entry EE either stepped in a hole or on a rock and twisted EE's knee. EE also states EE felt EE's knee pop.
The employee stated that EE became dizzy and fell off the ladder while plastering a stopping. The ladder was 4', and total height in area was around 9 1/2', EE fell between 2' and 3' to the ground.
EE was walking to the back of the machine to put oil in and slipped while crossing the cable.
While bent over putting oil in bolter a piece of roof rock fell and hit them on their arm/shoulder. Because they were bent over in the walkway of the machine, the rock fell perpendicular and broke across the machine. The initial rock measured approximately 77"" long, 36"" wide, and 2 ½"" thick. Mining height in this area is 13'.

On January 29th, 2026, Mine management personnel received info about an injury that happened on December 3rd, 2025. Employee received a laceration behind EE's right ear from a roof bolt that required 3 stitches. The employee did not notify mine management of the incident, went home and went to the clinic on EE's own without notifying mine management of the injury.
The employee was installing bolts and went to lower the canopy when the seal of the canopy jack blew out and dropped down suddenly. The bottom of the canopy hit them on the top of their head and stoved them up.
A roof fall occurred near Spad # 34462 on Panel D1 (5A Belt Break #38) of the active Section No. 5 at 10:55AM. This fall was in the LOB where the miner was loading at the time. The fall is approximately 20ft X 35ft X 7 to 8 feet thick. The fall has covered the right continuous mining machine.
A roof fall was found near survey station 33820 at the 23 cross-cut on the B Submains on 4 section. The fall measured approximately 18', x 18', x 10' high.
A roof fall that was not previously considered reportable under the criteria is now being reported due to MMU equipment being installed in the area. At Spad # 30889 an unintentional roof fall occurred above the anchorage zone. The fall measures 20'x20', x 7-9' high.
EE was replacing the pin on the scoop lift jack pin. EE leveraged the jack with a chain and slate bar and was going to check alignment. EE used the left middle finger when the jack fell catching their finger.
The employee states that that while operating the Romac Water Truck it began to get stuck (Hung-Up) so EE looked backward to try and back up. While still looking backward EE shifted back into the forward gear which caused EE's face to lunge forward and hitting EE's face into the dash.

Power was lost off of the main grid. An employee was discovered to be a couple feet from the bottom on the elevator. Generator was started around 5:46 and they were lowered and exited the elevator at 5:49.
An unintentional roof fall was found by the weekly examiner near spad 30803 at 8:03. The area was cribbed and dangered off on all approaches. The fall was approximately 8' in height, and 20' long and wide.
A sudden PLC fault allowed slack to form and force the No. 4 hoist rope over into No. 1 hoist rope on the sheave wheel. The elevator was 30 feet from the mine floor and 8 miners inside. Shortly after the elevator was able to be manually lowered to get the miners off. It was placed back in service at 1:30PM.
A fire was discovered on the number 16 bolter while making a preshift exam in the number 9 entry of the number 4 section. Fire was extinguished by using fire extinguishers, rock dust, and water deluge.
Note: The original date of injury was 8/6. The employee continued to work until November 22 at which time the results of an MRI revealed that time off work would be needed. Description: Employee smacked the drill steel against another piece of steel while rib bolting to get it unstopped. EE felt a sharp pain radiate from EE's shoulder up into EE's neck.
Note: The original diagnoses was an infection. It was not diagnosed as a hernia until after seeing a specialist on 5/28/2024. Description: While dragging a timber down the offside of 3C head EE felt a pain in EE's groin area. The timber measured 6""x6""x10'.
When coming off the section to outside the mantrip hit a hole and caused the employee to bounce up into the canopy and back down.
While adjusting a wire mesh panel, a piece of top rock fell and broke through the previously installed and next panel of mesh and hit the employee's left hand/finger.
The cord to the elevator door switch was damaged. This caused the elevator to not work from 11:30PM until 1:35AM on the 20th. There were no employees when this occurred.
Employee was passenger on a rubber tire mantrip exiting the mine. The operator of mantrip steered closer to rib to avoid a hole in roadway at which time employee as passenger had their foot slip off the framing of the man trip causing EE's foot to get caught on a ledge and jerking EE's leg backwards.
The employee was holding up the curtain to allow the scoop to travel bucket first down the No. 4 entry. As the scoop trammed forward the deck of the scoop hit the injured employees left leg.
The employee was attempting to push in the bottom rib bolt. While at the same time EE was holding the wrench to seat the bolt head. EE then spun the wrench, and EE's glove was caught between the wrench and bolt head.

One strand of the No.6 traction cable broke and unwound causing the strand to bunch up on the drive motor sheave wheels. This caused the rope gripper brake and the car safety wedge brakes to engage. There were 4 individuals on the elevator when this occurred. No injuries were incurred.
The employee was changing the bearings in the #24 tow motor. The machine was blocked, and a crib block placed under the hub and a bottle jack placed under the trunnion. After changing the top bearing, the employee raised the trunnion with the bottle jack to remove the crib. The trunnion slipped off the jack and caught EE's right forearm between the hub and the crib block.
Employee was installing the fork back onto the forklift. While trying to get it into place EE's left-middle finger was caught between the back-clip of the fork and the cleat of the fork-frame. EE received 2 stitches in the left middle finger.
As the EE was loading the roof bolter they stepped off the back of the machine to get more supplies and sprained an ankle.
The employee was carrying two full buckets of miner bits and tripped on a rock. They fell into a rib and felt pain in their back, legs, and pelvic region.
Employee was in the process of positioning the drill station to drill a corner bolt on the outside of the entry when a rock/kettle bottom fell from the roof, sliding down the roof screen and struck EE in EE's back. The strike to EE's back pushed EE forward causing EE to hit mouth against the canopy.
While bolting, the employee was placing a piece of mesh on top of the ATRS and adjusting it with EE's hands. A rock fell and caught the inby corner of the mesh and caused the mesh to spin around and strike EE in the mouth area and knock out 2 teeth.
While loading a top roller into a scoop insert, EE stated felt a pop in lower back.
Two employees were hanging bottom roller on structure during a belt move. The roller was being lifted simultaneously by each employee with the bottom belt laying loose over top. The injured employee felt a pop in shoulder. Note: The employee worked until 6/12 when employee was then required to have surgery to repair damage sustained during this action.

While traveling up 3A belt conducting an on-shift the EE's ride broke down. When EE stepped off and began to stand up EE lost balance and fell back into the ride. EE's left foot was caught between the frame of the ride and the belt frame causing a fracture to the ankle and fibula.
The employees were pulling in/out 1000' of belt. While the injured employee was pulling the redwing (AKA Duck-Bill) over by hand to keep the belt straight the redwing slipped off while under pressure and struck the employee in the forearm.
The employee was removing the lower ATRS jack with a slate bar. The jack was partially stuck and when EE was prying up, EE felt a sharp pain in the right bicep.
The employee was walking from 3 towards 2L at the Paint Creek Shaft site when EE slipped/fell on a ledge and tried to catch self with a chain-fall hoist chain that was hanging from the roof. In the attempt to catch self from falling a burr on the chain cut EE's palm.
While in the process of changing scoop batteries when they were lowering one side of the lid their fingers were caught between the battery lid lock and battery tray.
The employee and crew had stretched out an old piece of high-voltage cable for repair. When the EE stepped out of the forklift, EE's ankle rolled and fractured two bones in their foot on an old high-voltage cable plug.
While installing a roof bolt, the employee raised the drill pot to insert the roof bolt into the wrench. The employee's glove was caught between the wrench and the bolt and when the rotation was bumped it twisted their hand around the wrench fracturing wrist.
Two employees were re-stacking a stopping that had been being used as the diesel regulator. There were 2-3 rows of block left attached to mine roof that became dislodged as the witness was straightening blocks already stacked in place. A block fell causing a fracture to the injured EE's left pinky finger.
Employee was tramming the #2 scoop thru a set of fly pads in the #7 entry when EE ran over a rock and it bounced EE into the canopy causing EE to hurt back. After an MRI was done it was determined it was found that EE has degenerative disk disorder.

The washdown hose on the miner was coiled up on the rear of the machine with the valve end drooping over from the top of the machine to the floor. The employee was in the process of cutting coal when the frame of the machine caught the ball valve and broke it in two. The sudden release of water pressure caused the broken shut-off valve to strike the employee in chin.
Note: The employee did not begin missing work until 11/2/2021. Description: After drilling the inside bolt hole the employee inserted the glue and as EE was inserting the bolt into the hole EE felt a pop in EE's left shoulder.
While the employee was drilling to install a roof bolt, approximately 3' into the hole rock broke out around hole and fell hitting the drill pot and reflected into EE's right leg. The rock was approximately 30"" x 12"" and 3"" - 1"" thick.
While adjusting the forks on the # 21 Forklift the fork came off and struck EE's lower right leg.
Employee had removed the bottom plug from drill pot to work on suction. Employee placed right index finger inside the pot to check for debris and while doing so they inadvertently contacted the joy stick causing the drill pot to rotate catching the finger between the cleaner pins at the bottom of the drill pot.
Employee was installing the bottom rib bolt when a piece of rock fell from the roof striking EE on the left leg right above the knee causing a laceration.
While the employee went to add their second piece of steel together to finish drilling the hole, a piece of loose roof fell and hit EE on the arm.
The employee had just completed pre-op and was handing it to foreman when a piece of rock measuring 3'x4' and from 10' to 2 ½"" thick fell from in-between a row of roof bolts. This rock struck the EE on the head pushing EE downward into the walkway of the roof bolt machine. EE's right hand was caught between the rock and the frame of the machine.
Employee was in the process of moving the continuous miner from #9 entry to #6 right face and in between the #8 and #9 crosscut employee was pinned between the conveyor boom of the continuous miner and the coal rib.
Employee had bent over to pick up gloves and when they stood up they hit left ear on a piece of damaged roof mesh resulting in a laceration.
While the employee was tightening up a ratchet strap the hook slipped off on the lip of the flat-car. EE's end recoiled up and caused EE to fall backward and jar their shoulder. EE continued to work until 3/31 at which time was placed off from work by the doctor. The incident was under investigation and determined to be reportable on 4/12.
While roof bolting, the employee placed the roof bolt on the push plate of the drill pot in order to push the bolt up far enough to place the bolt head in the drill chuck. The up pressure and the fast feed feature before the bolt was in the hole. This caused the bolt to bend and pinch right middle finger between the bolt and the drill mast.
EE had removed a knot out of the miner cable and waterline. Afterwards, when EE reached down to pick up the slack rope to hook it to the CM EE felt a sharp pain in their back. EE continued to work until the date of August 9th when EE was put off of work from a doctor.

The employee had EE's hand on the canopy post and inadvertently lowered the canopy catching EE's right middle finger between the collars.
An unplanned roof fall occurred in a non-traveled intake air course outby the section loading point. The fall was approximately 60' long, 18' wide and 7-8' high. It began 40' outby survey station 6593 and continued to survey station 25631.
As EE was lowering the drill pot, the pot pulled off of the wrench and the wrench sprang back towards EE and struck EE's chin.
Employee was dragging a piece of curtain up #9 entry to ventilate the face while they bolted it when a rib measuring 11"" thick X 29"" wide X 27"" high rolled out and struck the employee on the right ankle which resulted in a fracture to the right ankle.

Employee was adjusting forks on forklift and pinched left ring finger between the forks and the slide bar.
Employee was handling the forks of a forklift and smashed finger between the fork and the frame
Employee was operating a roof bolter and as employee pressurized the drill steel against the mine roof to begin drilling a hole a piece of rock fell and hit the employee on the left hand resulting in a laceration and fracture to the left ring finger and an amputation to the left middle finger at the first joint.
Employee was mining the 2nd cut out of 3 lt. crosscut when a piece of rock bounced out of the shuttle car EE was loading and struck EE in the mouth.
Employee was loading a stab jack for a roof bolter into a Wallace diesel mantrip with the help of another employee. The stab jack slipped out the employee's hand and caught left index finger between the stab jack and the side of the mantrip. This resulted in a fracture to the tip of the left index finger.
Employee was attempting to load a 20"" X 6"" I Beam. While doing so the beam rolled and caught employee's left index finger between it and another beam resulting in the amputation of the finger just behind the finger nail.
An unplanned roof fall occurred in the secondary escapeway for the No.3 section. Located near survey station 22958, and was approximately 18'x 18' in area and estimated to be 10-12' in height. Agencies were notified promptly and employees withdrew from affected area.
2018 · 11 incidents
Employee opened the air lock door in the roadway on 3 D Belt for the grader. It has to be secured with a hanger. The rope hanger on the plate of the rib bolt came loose and the door struck and knocked them down. EE said they felt something pop in left knee. Due to events surrounding this injury it has been under investigation and deemed reportable.
Explanation: ODI was 7/31; ee did not begin to miss work until 8/6. Description: Employee was operating a scoop and was attempting to follow the Left Center SC across the section. When the center shuttle car went by the #3 entry, the other SC pulled out and struck the bucket of the scoop as ee was coming by. The jolt caused the scoop operator to strain back.
Incident occurred on 7/28/18. Employee was knocked down attempting to keep the bolter cable off of the rib standing on it. The cable pulled tight and knocked EE down on EE's right shoulder. Employee began missing work on 11/8/18 due to conditions related to this injury. After an investigation was completed by AIG it is considered work related and reportable under the rule.
While passing another mantrip the employee had foot on the side rail of the machine. EE foot became fouled between a timber that was set against the rib and the rail of the machine.
When the vertical elevator was attempting to bring employees to the surface it went down and would not operate. It was manually lowered down at 5:53 to remove the employees; then replaced resistor and resumed normal operation at 6:50.
While EE was re-entering the scoop, for balance, EE placed right hand /arm on the operator deck side board. EE's arm slipped, which caused EE to lose balance and strike mouth against the machine's bell resulting in a chipped tooth
Injured and a foreman were installing a steel beam. They had a chain on the end of the beam that the injured was holding. The foreman was trying to install the chain on the other end, when the beam slipped. The injured tried to hold the beam, but the weight pushed them to the ground. Injured stated they did not think they were hurt.
A face ignition occurred while the miner was attempting to square up the face
Explanation: Due to events surrounding this incident an investigation was initiated. After careful consideration it was concluded it was to be deemed reportable under the rules on 3/28/2018. Description: The employee was pulling a piece of belt off of the beltline and miss stepped which triggered pain in right knee.
Employee had positioned side of the machine to install the next row of bolts. EE then extended the slide of the canopy out and attempted to raise the canopy higher. EE unknowingly left finger on the top side of the canopy which pressurized against a rock brow causing a fracture to right pinky finger.
While standing beside EE's ride, a piece of rock fell and hit EE on the neck/shoulder. Employee worked until 10/22/19 when EE began missing work. This was recently diagnosed as a degenerative disk disease.

EE drilled the starter hole and then continued to put the pusher and finisher in the hole to complete the hole depth. An attempt was made to fast feed the steel into the hole. In doing so the steel bowed at the same time they hit the rotation. This caused the steel to strike EE in the palm of the hand.
Employee was sliding an 6"" I-beam into place for a belt head. Another I-beam tipped over and caught employee's finger between itself and the other beam.
A face ignition occurred while the roof bolter was drilling through a metal strap against the roof.
The incident referenced herein, of an original date 9/20/2017 has been under investigation. It was determined on 10/4/2017 that this will be deemed reportable under the rules. Employee was unloading a drill pot from a 14 man mantrip. Employee stood the drill pot up on its end and it fell over pinching left pinky finger between the drill pot and the metal seat of the mantrip.
The above shows original date of injury. The employees first missed work day was 8/2/2017. Description: The employee was climbing in and out of the miner conveyor chain making repairs. Employee felt a pop in employee's knee.
While traveling on the vertical elevator to the mine bottom, the elevator stopped. The employee remained in the elevator (in excess of 30 minutes) until the part was replaced.
NOTE: This incident was not classified as reportable until the date of July 22, at which that time employee began to miss work. Description: Employee was tramming shuttle car towards the miner and the tire ran up and over a corner ledge which caused employee to bounce up into the canopy.
Explanation: The original date of injury was 6-16-2017, this employee continued to work until the date of 1-31-2018 at which time ee began missing work due to this injury. Description: While walking to the mantrip at the end of their shift, ee stepped into a hole and twisted knee.
A contractor employee was unloading high voltage guarding from a man trip. As employee was walking backwards stepped into a hole causing employee to sprain left ankle. Employee continued to work until 5/15/2017.
The CM operator backed the machine out of the face due to the machine's conveyor not working. The electricians had CM operator try to start the conveyor to determine the problem. A piece of rib of what would become the No. 2 face fell out onto the CM operator. The rib measured 53"" long, 36"" wide, and 20"" thick.
The original date of injury reflects 4/6/17; The employee has continued normal duties. On 5-31-2017, the employee went to a doctor and was diagnosed with a hernia. Description: The employee was lifting a timber onto the ride and felt a pulling/stinging feeling in right groin area.
Employee bent over to flip over a loop of CM cable and felt a pop in shoulder.
INFO: This incident was not reportable until EE had surgery and began missing work on 8-16-2017. Description: The employee was walking around belt head drive and inadvertently stepped into a hole and twisted knee.
An employee of our operation was diagnosed with pneumoconiosis and has exercised their part 90 rights to transfer to a less dusty area of the mine where the concentration of dust is less than .5 milligram per cubic meter.
While the employee was descending off of a piece of machinery, struck knee against the body of the machine. UPDATE: On January 17th the employee had a follow up visit to an orthopedic surgeon. It was determined employee had damaged MCL, and has been put off of work.

Employee had gathered tools & belongings onto ride. As employee backed up the tire rode onto a cinder block & caused the tire to lock up. Consequently, this action caused employee to hit shoulder on the steering wheel. Update: The employee did not seek medical advisory until 12/16/2016. The doctor determined during examination that employee needed time off of work.
Note: The original date of injury was 11/29/2016, employee continued to work under normal duties. Consequently, EE followed up with EE's physician on 12/27/2016 and was referred to be taken off of work. Description: The employee was dragging a water line from the continuous miner to help assist in watering the roadways. EE states EE felt a sharp pain in EE's lower back.
The employee states that after swinging in from installing outside bolt, ee set stab jack to begin drilling the inside bolt. A piece of rock measuring; 54"" long, by 24"" wide, and 1 1/2"", to 4 1/2"" thick fell and struck leg while on knees drilling.
Employee was carrying a timber to be set on the belt line due to wide bolts. As employee lifted and began to drag the timber employee felt a pain in lower stomach/groin area. The employee did not receive medical treatment until 12/2/2016, in which employee was diagnosed with an Inguinal Hernia.
An unplanned roof fall was found and called out by the weekly examiner while traveling an airway with an MSHA inspector. This fall was found to be 80' long, 8' high, and 20' wide. This was in the examiner's normal route of travel.
Employee injured EE's right thumb on Sep 29 while replacing a ripper-head jack on a continuous miner. The jack slipped pinching EE's thumb between the frame of the CM & the jack. The employee did not report this injury until 10/11 at which point it was investigated and later determined it meet reportable criteria.
After the employee drilled part of the hole, EE put the two pieces of steel together to insert back in the hole to complete the hole depth. When EE reached out to push the steel into the hole a piece of rock measuring 23"" long, 10"" to 1"" in wide, and 1 1/2"" thick fell and hit ankle.
Injured was carrying top structure to place in scoop bucket. While walking EE fell into the bucket, catching finger between two pieces of structure.
The employee was attempting to pick up roof mats to put onto the scoop. Employee bent over to pull, and lift mats. The employee states they then felt a pop in back.
The employee was taking a motor down to unload into the core bin with the maintenance scoop. When employee rolled the motor off into the bin it bounced, and pushed ankle into the side wall of the bin.
An unplanned roof fall occurred on the number 3 section. It began 20 foot outby SS# 17795, and continued approximately 35 foot. It was roughly 18 foot wide, and 0"" to 3 foot thick. All appropriate agencies were notified, and employees withdrew from affected area.
The employee had drilled through one end of a roof strap while against the mine roof. As EE backed the steel out of the hole, it caused the strap to fall, swing around, and hit EE in the side of the face.
The brakes of the Braking Car were stuck in the closed position. Employee initially tried to repair by replacing a solenoid which did not correct the condition. After trouble shooting, the cause of the problem was a loose wire on the terminal strip. After tightening the wire the condition was corrected.
The employee was walking up the entry as a SC made a turn, the cable took up, undercut EE's feet, which caused the employee to fall to the ground.
The cable rope on the slope hoist drum became tangled. This prevented the brake car to be used.
While the employee was traveling back toward the elevator employee encountered an unintentional roof fall at or near survey station 14234. The fall extended away from the travel area, and toward survey station 14232 approximately 20 foot. The fall measured 20' long, 0 to 10' thick, and approximately 18' wide. An MSHA inspector was present after the fall and went and looked at it.

EE was installing belt hangers. He stated that the chuck was coming out of the wrench and while holding the wrench while pushing upward his finger was caught between wrench and chuck.
The employee was going to pick up one pallet to load into the trash car. When he attempted to lift one pallet, two others were attached also. The employee states that he felt a pain in his abdomen. He went to a family doctor who referred him to a specialist on August 31. This specialist determined that it was in fact a hernia.
The fire-boss was making a pre-shift examination across the faces of the No. 2 section. He encountered an unintentional roof fall between the 6 and 7 entry in the LOB. The fall was approximately 2 breaks inby SS# 15944. The fall measured to be around 52' in length, 18' in width, and 1' to 8' in height. All appropriate agencies were notified of the condition.
While the employee was walking across the section he hit his head on a flyboard, stubbing his neck. This report is being filed to terminate citation #8149175.
The processor that controls the elevator malfunctioned.
A contractor from Frontier Kemper was conducting an annual pull through test on the elevator. During the test one side of the brakes caught faster than the other and bent the linkage.
The employee was loading the roof bolt machine. EE was setting a handful of bolts on the machine as it moved forward to the next row. The tire ran on top of his foot.
As the weekly examiner was making his airway he encountered an unplanned roof fall. The fall was located near survey station 4972 in the number one section left return. The fall was approximately twenty foot in length, sixteen foot in width, and two to three foot in thickness. The section was pulled as well as MSHA and state agencies notified.
Employee was injured picking up 6 inch aluminum water line. When he lifted water pipe, he felt a pop and pain in his right arm from elbow to wrist.

The employee was hit in arm by a piece of rock while drilling the hole for a roof bolt.
An unplanned roof fall was encountered when traveling toward the number one section. It was located at survey station 5043. The fall was approximately twenty- five foot long, sixteen to eighteen foot wide, and five foot high.
The hoist man informed the evening shift mine foreman that he was unable to set the brakes up on the brake car. He thought it may be a sensor. He then called both the state and federal agencies to inform them of the issue. After maintenance was called in the issue was corrected and both agencies was called back and informed of the status.
An unplanned roof fall occurred on the number 3 section. It was in the number two entry approximately inby 80' survey station 14233.
Employee states while drilling the foot jack slipped off of a rock and caused the canopy to hit his head.
The employee was removing a rub rail and it slipped off, and fell on his left foot. This report is being filed to terminate citation # 8149176.
At 7:50am an unplanned roof fall was encounterd in the secondary escapeway for the number one section. It began approxiamately 50' inby survey station 5261 and extended 5' into the intersection of survey station 5278. The fall was 25-30' long, 16' wide and appeared to be 5-6' in height. All aproaches were dangered and breakered off. MSHA was notified at 8:00am.
Employee was driving a tow motor to 1 section. Near spad 5784 he encountered 2 rope hangers tied together hanging from the roof. His neck became caught by these hangers. He was treated and released. Returned to the doctor on 3-15 and was released. On 3-17 he went to his family doctor which referred him to a specialist who put him off of work. His first missed day was 3-18.
Employee was bolting the face of the #2 entry on the #3 section and when he put his drill steel against the roof a piece of rock came loose and hit the ATRS which deflected the rock toward him striking him on his right arm above the elbow resulting in a laceration.
An unplanned roof fall was encountered in the First Right Panel off of the South West Mains Section of the mine. The fall occurred near survey station 1675 and extended outby a distance approximately fifty foot. The size varied from five to six foot in thickness. All approached were previously dangered and breakered off. The fall has been marked on the mine map as required.
An employee of our operation was diagnosed with pneumoconiosis and has exercised his part 90 right to transfer to a less dusty area of the mine where the concentration of dust is less than 1 milligram per cubic meter.

To begin with this accident was not reported, nor was anyone aware of it occurring until 11/14/13. At which point we put it under investigation to determine its validity. The employee was changing a breaker in a power center and the breaker panel slipped and fell which caused a laceration to his right ring finger.
The employee was grading bottom with the CM. As he was trimming the bottom the roof ""bumped"" and drew his attention to the roof, the CM cutter drum was still cutting the bottom and while looking up a piece of foreign material went underneath his glasses.
The employee was placing the drill steel into the drill pot. He was unaware that his glove had become pinched in-between the wrench and the steel. He initially thought his hand was clear, he started to rotate the steel which pulled his hand/glove around the steel.
The injured was working with another employee to find out why the # 23 ride would not tram. The employee knelt down into the drivers seat with his legs extending out of the ride. They were working on the tram/linkage cable, the ride took off unexpectedly which had caused his left to get between the ride and the rib.
The employee was bolting roof. The roof bolt nut had become stuck in the drill chuck. He attempted to remove it by placing a piece of drill steel against the mine floor and up into the bottom of the pot. He exerted downward pressure with the joystick and inadavertently maneuvered the joystick into the rotation phase and the bolt spun and hit him in the ear/face area.
The miner operator was hit by a shuttle car when he was behind the miner checking his center line.
The employee was dropping a an impact roller out of a tailpiece which had become fouled. He broke it loose by using a slate bar, when it fell approximately 5"" his hand was placed on the frame of the tail piece which was struck by the roller assembly.
The hoist operator noticed the temperature was higher than normal and called for maintenance. After maintenance arrived and evaluated the situation they downed the hoist for trouble-shooting purposes.

The injured was cleaning rock off the from the top of the miner. He and another employee was attempting to push the rock into the conveyor boom. As the rock started to shift into the conveyor it pivoted upward and caught his finger in- between the rock and the roof.
The injured was setting some cribs for supplemental support. As he was beating a wedge over top of the crib the hammer caught some lifeline and caused him to miss the wedge and hit some fingers.
The injured had installed his outside bolt and began swinging the boom toward the inside to install the center bolt. A piece of rock measuring 8"" long x 8"" wide x 4"" thick at the center down to a sharp edge fell and hit him on the arm causing a laceration.
The Original Date Of Injury was 9/3. The employee continued his normal work dutied while seeing a chiropractor. It was not until 09/27/12 that the employee began missing work from this injury.(Injury description)The employee was walking from a 2 man ride to the # 1A belt drive for fire bossing purposes, while walking he hit his head into a roof bolt and stubbed his neck.
The injured and another employee were attempting to move a screen deck with a forklift and nylon strap. The forklift operator went to pick one end of the deck up, the strap broke and the deck fell and smashed the other employee's foot.
At the above mentioned time an un-planned roof fall occurred in the secondary escapeway for the #2, and #3 section. The fall began 15' outby spad 26, and 90' outby spad 25. The fall continued approximately 75' inby toward spad 38. All approaches were timbered and dangered off.
The injured was attempting to install a roof bolt and plate with a 4' metal strap on top for supplemental support. While inserting the glue before putting the bolt in the hole a small unknown size piece of rock fell, hit the strap, which caused it to hit his arm and cut it.
While installing cable bolts, the injured drilled the hole to the required depth. He then went to put a wrench in the drill pot to install the cable bolt. A piece of rock fell from the roof and struck the employee on his right thumb while he was putting the wrench in the chuck.
The employee was riding in a mantrip at the end of his shift. The driver of the mantrip stopped and swerved suddenly to avoid holes in roadway. A plastic bit bucket with a nylon tether inside of it came off the top of the machine and hit the employee in the shoulder. This incident has been under investigation and is now ruled compensable.
An unplanned roof fall was discovered in-between survey station 1533 and 1515 in the intake air course that travels toward the number 2 and 3 section. The fall was approximately 60' long, and 16' wide and 4' high. The area was dangered and breakered off, as well as plotted on the mine map.
The injured was stepping out of the deck of a roof bolter, it was on top of a rut. When he stepped out of the bolter, it slid over against his foot/ankle. He did not start missing work until 5/29/2012 due to this condition.
The injured was doing some spot bolting outby the face 1 break in the # 6 entry. The operator of the machine dropped the ATRS and moved up for the injured to spot another bolt. After the ATRS was set the injured was walking beside of the machine and a thin, sharp piece of draw rock fell from the roof and struck the victim on the ear.
This incident originally occurred on 4/15/2012, he did not start missing work until 4/19/2012. The injured was helping to move a 8"" I-beam that was 11' long, to position it for a belt drive. He strained his back in the process.
The injured was helping to load bulk bags of rock dust with a forklift into the hauler duster. He held the loop up that is located on top of the rock dust bag to let the forklift lift the empty rock dust bag out of the hopper. He motioned for the operator to pull forward, the left front tire of the forklift trammed into his right leg.
The injured was preparing a piece belt to be vulcanized. While cutting a strip of belt off, the belt knife came out and cut his leg.

While the injured was installing a fly board with a conventional chuck bolt the hole was not drilled to the proper dimension. He was forcing the bolt threw the pre-drilled hole and once it popped thru the board sprung back and caused a curtain hook to cut his palm.
As the injured was installing roof bolts a piece of draw rock fell and struck him on the foot.
The injured and another employee was putting a ram jack for a scoop onto a Johnson ride, as they were setting it down it slipped and caught his fingers in-between the jack and the ride.
As the injured was cleaning the # 3 left cross cut at or near survey station 6781 a piece of rib rock rolled out and struck him in the left shoulder.
The employee bent over to pick up and get out the way a section of miner water line. When he bent over he stated that he felt a pop in his back.
The injured and three others were going to recover some high voltage cable from old works, they had knocked an old stopping to gain access to the area, after they traveled thru the old stopping with a scoop they were attempting to hang a piece of curtain up and some block that was left on the roof had fell and struck the employee in the back of the head.
During a belt move the injured was trying to seperate two pieces of waterline. While pushing/ pulling he cut his hand on the edge of the waterline.
The conveyor chain had broke on the continuous miner. The injured and another employee was working to get the chain back together with a chain hoist. While the witness was putting tension on the conveyor chain and the hook slipped out and struck the victim in the face.
A non-injury roof fall occurred at 0530 am on our # 1 belt between survey stations 223 & 253 on our # 1 belt.
After the injured had drilled his hole to insert a roof bolt he was putting his glue into the hole and a piece of rock fell and struck the victim on his right arm.
The injured was attempting to square off the end of some conveyor belt to make a splice, he was pulling the belt knife toward himself and the knife slipped out of the belt and cut him on the right knee.
The injured was bolting in the 7 right crosscut. After he stacked his pusher and finishing piece of steel together and was drilling to his required depth, he went to get a bit out of the tray to change on his starter steel and a piece of rock fell on top of his canopy, and continued to fall on his left hand.
The injured was loading roof bolt plates onto the scoop and they cut through his glove causing a laceration.
The injured was operating the bolter to install a fly board for the face of #1. Curtain was hung across the intersection to keep air in the face. After he drilled his hole and was pulling the steel down he used his hand to help bring it out and the air blew the curtain into the driller station catching the steel and wrapping his arm into the steel.
The original date of injury was 4/26/2011, this incident was not classified a reportable incident until 5/17/2011 after a follow up visit to the physician that revealed a fractured bone in his foot and he was put off from work. The injured set the miner up in the # 7 entry and went to get the slack rope off of the machine and a rib rolled on to his foot.
While loading a shuttle car it shoved the boom of the miner into the top and sheared a roof bolt off which came down and struck the injured on the corner of his eye.
Injured stated that after lifting block over his head to build a stopping his back started to hurt and continued to get worse until the point he could not continue to work.
A piece of rock struck the employee while bolting. His hand was setting on the tray of the machine when the rock fell and hit his hand.
A roof fall occurred at the above mentioned time outby the # 3 section loading point in the # 3 entry in a neutral air course (not in a escapeway).Inby survey station 5561. The fall was approximately 40' long, 10' wide, and fell about 3' high. All approaches have had breaker timbers set and been dangered off.
The injured was trying to situate the cat pads into place under the miner to change them out and strained his back in the process.
The injured was using a belt knife to cut a piece of hose to fit for a bantam duster, while doing so, his thumb was cut by the knife.
The injured was helping to move the section tool boxes up closer to the section. When he was pulling the tool boxes up into the scoop bucket, he strained his back.
Received PPD of 6.22% from a hearing loss claim. Awarded after retirement date of 9/1/2010.
As the employee was installing roof bolts a piece of rock fell and hit his head.
The injured was pushing his drill steel against the mine roof to start a hole, when a piece of loose rock fell and hit on the arm.
The injured was swinging a sledge hammer to knock block out of a stopping, when he swung he hit his hand on the block.

The continuous miner was hung up on a rock ledge. They cribbed up the ripper head so they could bow it down and raise the frame of the miner to get some wood under the cats of the miner, while the injured was pushing wood under the cats of the miner the cribs under the ripper heads crushed/rolled out and caught the EE's right thumb between the cat and the crib.
During a weekley exam of the intake split of air(not the primary escapeway) used to ventilate the # 3 working section a roof fall was encountered. It was 50' long and 18' wide. All approaches were cribbed and dangered off.
The employee had laid a bolt with a pie pan against the rib. The bolt tipped over and hit his face which caused a cut.
While trouble shooting the ram on a hauler duster, the injured was looking up under the ram, his foot was down in a cleaning hole in the false bottom of the hauler, he had the operator pull the ram back which closed in on his foot.
While the elevator was being reprogrammed from indians (online) it malfunctioned with an fault. MSHA was called @ 10:30 am and the state @10:42. Nobody was trapped or injured during the incident and both agencies were notified when it was back operational.
As the weekly examiner was traveling an air course he encountered a roof fall in the return air course. The fall is located at spad # 2455 and extends outby 100', it is 18' wide and 5' high. The area was dangered and breakered off.
Injured was sawing a timber when the blade became stuck. He forcefully tried to pull it back towards him. The blade came out of the timber unexpectedly and struck him on the left forearm, causing a laceration that required 10 stitches to close.
An unplanned roof fall was encountered outby the dumping point of the number 3 section out by survey station number 4317 in the intake split of air used to ventilate the setion. MSHA was notified at 08:20 am on 09/20/2010. An AR of MSHA came to investigate on 09/23/2010.
On Aug. 29th at 01:15 am the dispatcher informed personel that the elevator was down with a crew of men inside it. A member of that crew had leaned into and pushed in the e-stop. MHSA and the WVOMHST was notified that the elevator was running at 02:10 am.
A piece of metal broke and caught the bottom door hanger. The elevator was pulled to the surface via a manual assist. The door was repaired and put back into service.
On the night shift, the electricians had moved the feeder toward the power entry to replace the conveyor chain. The injured was moving the old flights out of the way for the new. He was going to toss an old flight over the bed of the machine when it slipped and he tried to catch it, the flight smashed his left index finger against the bed plate.
While the midnight mine foreman was exiting the mine he encountered an unplanned roof fall. It was located in the secondary escapeway for the # 3 section. Spad # 3101. All approaches were cribbed and dangered off, also the lifeline was re-routed so to travel the escapeway in it's entirety. MSHA was called at 06:25 a.m the same day. An inspector looked at it on the Monday the 26th.
Injured was operating a scoop. The scoop went over a ledge and jarred the operator. He informed his foreman of incident and worked the rest of the shift. Called off work the next day complaining of pain in his lower back. Placed off work by a doctor.
Elevator would not operate. Mechanic adjusted brake switches.
Coal Hauler approached continuous miner to load. Hauler struck operator in leg and operator sustained fracture to lower right leg.
Injured was dropping miner cable when a hook on the rope hanger caught his hand between his fingers and cut his hand. Required 5 stitches to close wound.
The employee hit a bump in the roadway and struck his head on the canopy of the machine. The employee subsequently resigned on 3/15/2011.
Injured was moving from the front of the bolter to the rear. He cut his arm on a metal strap extending over the edge of the bolter. Required stitches to close wound.
