Opening a set of air lock doors and strained right shoulder and neck.

A unplanned roof fall occurred at x-cut 42, in #4 heading at old 7 south. The fall was about 6 foot high, 20 foot wide, and 60 foot long. The fall timber and danger off.

Employee was releasing a chain ratchet from the track when the hook straight out and broke loose cause it to come back and hit him on top of the hand.

EE was removing a sleeve from an oxygen bottle and felt something in his lower back. He worked up to August 26th before he started missing. I marked this as an occupational illness. (See attached medical report).

Employee was taking down a belt head and lay his hand on the rail and the belt roller fell on it and pinched his finger.

Employee was crossing the belt line and when he stepped on the belt rail his foot slipped and he fell to the mine floor injuring his back.

Employee was diagnosed with pneumoconiosis by doctor. We received the e-mail on 7/8/15. He worked for Rockspring from Jan 2010 to March 2014 for a total of 3 years.

Employee was moving a fallen timber out of the walk way on 3A belt. When he picked the timber up he felt something pop in his lower back.

EE picked up a tool bag and felt pain in his left shoulder. He went to the doctor on the 15th and they gave him a full release. He went back to the doctor on 6/19/15 and they gave him a light duty slip but we didn't see it until 7/7/15 and he started missing work on 7/8/15.

Trying to put motor back on the track with the forklift. The forklift slipped out making the forklift bounce up and down hurting his lower back

Employee and three other was hanging miner cable in # 7 entry and as Employee lift the cable he felt pain in the lower back.

The elevators at 14 mile was down due to the software being bad. Brought all the men outside while the elevators was repaired it was ready to go at 7:00PM.

An unplanned roof fall occurred at approximately 3:15pm. The fall was at R-seals between #2 & 3 cross cuts in the 1-2A panel. the fall was about 9 foot high, 20 foot wide and 40 foot long. the fall was timber and danger off and will be clean up by a approve plan. The area was mine in 2008.

EE had been working on a ventilation curtain in #1 heading he completed task, he removed his safety glasses to wipe sweat of his eye and rubbed dirt in the left eye causing irritation. EE stated that he had mud on his hands from the ventilation curtain. EE started to miss work on3/31 thru 3/30 and returned to work on 3/31/15. EE worked tow day before going to the Dr.

Employee was walking down the slope belt and slipped on some ice as he went to fall he tried to catch himself and stained his right shoulder. Employee started missing work on 7-9-15 to have shoulder surgery.

EE was cutting a band on a bundle of bolts and something hit him in the eye. No one knew he had sought treatment for the incident until we received a bill from him on 6/22/15. He was given eye drops as a prescription and was told to follow up with the doctor on 1/28/15. I got the e-mail on 6/29/15. The first day of vacation.

An unplanned roof fall occurred at approximately 6:00 PM. The fall was at cross-cut # 32 on 6 South track. The fall was about 10 ft. high, 80 ft. long, and 20 ft. wide. The fall was timbered and dangered off and it will be cleaned up.

When employee went to change his drill steel out, a piece of draw rock fell hitting him on the right wrist. He had a simple fracture to the wrist.

employee saw that the scoop was possibly going to catch the lifeline, he approached the scoop on the operators side without notifying the operator. As he grab the lifeline the scoop started to tram in reverse, he slipped toward the scoop and his left foot went under the bucket. The operator could not see the employee and the scoop bucket came down on the employees' foot.

Employee was working on a 4 inch airline trying to unclogged a frozen airline when the pressure broke loose the airline struck him on the right side on is arm and leg. Causing hair line simple fracture to his right leg.

A unplanned roof fall occurred at this mine approximately 10:15 am. The roof fall occurred on the #2 South conveyor belt between cross-cut #40 and #41, 50ft in spad #9061. The non-injury roof fall measured 20ft long 19ft across by 3ft thick.

An unplanned roof fall occurred at this mine approximately 5:55 am. The roof fall occurred #3C belt conveyor cross-cut #26 and extending cross-cut #27 on the belt conveyor. The non-injury roof fall measured 95 ft. long and 18.5 ft. across by 9 ft. thick. All approaches to the roof fall were cribbed off and dangered off.

Employee was putting up a 12 foot cable bolt and as he went to put the bolt in the hole he felt pain in his left shoulder. The injury result in restricted work for 17 days, employee started missing work on 11/20/2014. (The mine was down for the week of Thanksgiving for vacation.)

Employee was carrying a V-Plow to install on 4D tail piece and he stepped into a hole causing him to twist his left knee. Employee work up to November 20, 2014 and started missing work. (The mine was down for the week of Thanksgiving for vacation.)

EE alleges he stepped on a shuttle car splice & received an electrical shock. He was taken by ambulance to the hospital, he was given full release by the ER doctor. He requested another day off which allowed him one day away from work. We feel this incident didn't occur or didn't keep him from returning to work on his next shift.

Employee was trying to stabilize the ATRS beam mount to flip it over. When flipping it over the mesh latch assembly came down and cut from the first knuckle off on his index finger on left hand.

Employee was moving miner cable over out of the road way and stepped in a hole and twisted right knee. (Employee started missing work on 1-9-2015 to have arthroscopy on the right knee.)

A roof fall occurred two crosscuts outby the elevator going south. The fall was 20 foot wide, 40 foot long, and 7 to 8 foot high. The fall was danger and crib off and will not be clean up.

EE was helping make a water line splice on the continuous miner, he had banded one end of the water line and was in the process of removing the excess band from the banding tool when the banding tool became fouled, he was pulling on the band when it released striking left hand palm area causing a laceration which required (7) stitches.

A unplanned roof fall occurred on the #4 section in #5 entry/intersection 70ft inby spad #400114. The effected area was danger off and timber off, The fall measured 6ft high by 19ft wide and 20ft long. A clean-up plan was submitted and approved on 8/25/14, of the effected area. The fall was cleaned up on 8/28/14.

Employee was diagnosed with pneumoconiosis by MD pneumoconiosis B Reader. I received the letter by e-mail on Aug. 12, 2014. HIS LAST DAY OF WORK WAS FEBRUARY 18, 2012.

An unplanned roof fall occurred along the 4B track way between crosscut # 38 & 39. The fall was danger and timber off. The fall was 8 to 10 foot long, 18 foot wide and 3 foot high.

EE was diagnosed with pneumoconiosis by Dr, board certified in internal medicine on 3/14/14. I received the letter my e-mail on July 22, 2014. EE has not work since August 9, 2012 because of a illness.

A unplanned roof fall occurred at X-cut #14 on the new way track. The fall was timber and danger off and will not be cleaned up. The fall was 20 foot by 20 foot and 6 to 7 foot high.

Employee was diagnosed with pneumoconiosis by the board certified in internal medicine on 3/26/14. I received the letter by e-mail on July 1, 2014. Employee is not working he been off since May 14, 2013.

An unplanned roof fall occurred at x-cut #21 on old 6F track. The fall was danger and timber off. The fall was 5 to 6 foot high, 19 foot wide and 20 foot long. The fall will not be cleaned up at this time.

Was advancing line curtain up when a piece of draw rock fell out hitting him on the back head. (He received 8 staples)

Moving miner cable out of the roadway tripped over the miner cable and jammed left little finger. (EE worked from 13th to the 16th before going to the Dr. to have his finger checked. He went on lite days on Monday the 19th and return to full duty on the 27th)

A non injury roof fall occurred above the anchorage zone of the 5 foot fully grouted torque tension bolt used for primary roof support in the 3-62 panel in the #7 return entry 3 - X-cut inby spad # 54682. The fall was 20 foot long, 19 foot wide, and 5.5 foot high. It was timber and danger off and will not be clean up.

Employee was in the process of pushing a flat car over to put back on the rail with a hydraulic jack. The jack kicked out and the bar hit him on the forehead. He still had the bar in his hand. He received 3 stitches.

AN UNPLANNED ROOF FALL OCCURRED AT X-CUT #28 IN THE #5 ENTRY ON THE NEW WAY TRACK. THE FALL WAS ABOUT 6 FOOT HIGH, 20 FOOT WIDE, AND 80 FOOT LONG. THE FALL WILL BE CLEANED UP AND REBOLTED BY THE PLAN.

EE was putting up a 12 foot rope bolt and some draw rock fell out around it and the plate came down and cut his wrist, he received 5 stitch.

Employee was making a belt splice. He was down on one knee and went to twisted and sprain his left knee. (He started to miss work on 4/2014 to have arthroscopy on the left knee.)

EE was scooping on roadway & ran over something in the roadway & complained of pain in his back. He worked for the next two days & we were off the next week for the holidays. He started missing on Dec 30th 2013. We are filing this as an occupational Illness due to the Dr. report I will send. ** Dr. notes indicate no acute injuries only degenerative changes & osteophytosis **

Employee was washing the feeder belly pan out and a piece of draw rock fell and hit on the right middle finger cutting it. Employee received 6 stitches.

The employee thought he put the scoop bucket on the ground, but it was setting on a big piece of coal. While he was loading the block in the scoop bucket the piece of coal burst out and scoop bucket hit the side of his right foot.

The employee was breaking a bolt loose on the planetary of the miner with a cheater bar. The bar slipped and something pulled in his right shoulder. Employee started missing work on 11/4/2013.

EE was diagnosed with pneumoconiosis by Doc on 9/8/2013 by a chest x-ray. I received the letter by email on Oct 16, 2013. EE is not working he been off since Dec 21, 2013 he retired Jan 2013.

Employee was hanging a curtain in #9 entry when ladder he was climbing on collapsed causing him to fall to the ground hurting his back. ""Bruised only"" NOTE: Suspected fraud, under investigation. No lost time, worked 17 shifts before claiming he was hurting. On 11/4/2013, his wife called supervisor stating that EE fell at home over the weekend and would be off work a couple of days.

Was loading roof bolter & the shuttle car was trammed toward him knocking him to the ground pinning his left arm above his head. NOTE: Suspect fraud, see attached statements from men that worked with him. When mine started laying off, he alleged injury on 7/30/13, but did not miss until 9/19/13, almost 2 months after the alleged injury! Mine laid off contractors 12/4/13. Job ended.

Employee was walking from the power box to the feeder when a piece of small draw rock fell out of the top hitting him on the top of the right hand. He received a cut that took 4 stitches.

An unplanned roof fall occurred in the 4D area in the #4 entry at spad #58944. The fall was timber and danger off and will not be cleaned up.

Moving cable & stepped backwards tripping over a rock & falling on his side. He continued to work until 5/14/13 at which time he started missing work. The reason this is late being submitted is we were under the impression he was off due to a medical condition rather that an incident at work. **Subsequent findings indicate it was due to Degenerative Disease of the cervical spine

Employee was hanging a curtain, tripped on the curtain and fell causing pain in his left knee. Employee continued to work until May 30th, 2013 at which time he began missing work and was scheduled for arthroscopic surgery on Monday, June 3, 2013

Employee was lifting a timber to build crib he had an incident with his wrist which he was released from two doctors and two hospitals to return back to work. The reason this is late being submitted we were under the impression he was off due to a medical condition rather then a incident at work.

Employee was diagnosed with pneumoconiosis by Dr. on December 6, 2012 by chest x-ray. We received the letter March 18, 2013. Employee is still working he has not missed any work yet.

Loading boards on roof bolter and hit his left arm again a stack of pizza pan and cut his arm above the elbow.

EE was hooking up a sling duster to the scoop bucket as he went to tighten up the chain the duster fell off on his right foot causing a simple fracture to it.

Unplanned roof fall occurred at X-cut 12 in the 3A area in the #6 entry 1 X-cut from spad #57321. The fall was 20 foot long, 18 foot wide, and 8 foot high. The fall was crib and danger off and it will not be cleaned up.

Hydraulic line burst, spraying oil on exhaust pipe and ignited. Driver exited truck no injuries. Fire Dept called and extinguished.

EE was changing out a top roller on a 60"" belt and he was standing closer than he was aware of and hit his little finger between the roller and the belt rail.

An unplanned roof fall occurred at x-cut #2 on 4C belt. The fall was 18 foot wide, 20 foot long, and 6 to 7 foot high. The fall was cleaned up and the area was rebolted.

Roof bolter was getting ready to tram out, waiting on the fly pad to be moved, ee was rolling up the fly pad when the bolter operator sat down and bumped the reel control, causing the cable to jerk, flipping ee onto his head and rt shoulder.

Employee began missing work on February 27, 2013 after he started to have trouble out of his right shoulder that he injured on Dec 18, 2012 while hanging a miner cable. (Employee had a pre-existing non mining related injury and had surgery to the right shoulder).

EE was tramming a shuttle car toward the miner, hit hole in the roadway and felt sharp pain in his back.

EE was walking by the section power center and stepped on a miner cable causing him to twist his right knee. The EE began missing work on 12/16/12.

EE was putting a 6' roof bolt in the drilled hole when he went to straightening the bolt out it slipped and came back and hit him in the hand cutting his finger. EE received 6 stitches. He was wearing knucklehead X10 gloves.

EE was taking down a buggie cable when he unhooked the hanger the hook came around and hit him in the nose and cut it he received 7 stitches.

An unplanned roof fall occurred at x-cut # 14, 1 brake inby spad #36142 on the 12 south track. The was about 18 foot wide, 24 foot long, and 6 to 7 foot high. The fall was timber and danger off and will be cleaned up at a later date. These area was mined around 11/11/2005.

EE was getting a piece of draw rock off the miner and it slid across his hand cutting it. The cut took 7 stitches.

Employee was making a belt splice and cut his leg with a utility knife. It took four stitches.

AN UNPLANNED ROOF FALL OCCURRED AT X-CUT # 8 ON THE 1 SOUTH BELT. THE FALL WAS ABOUT 20 FOOT LONG, 15 FOOT WIDE, AND 4 TO 6 FOOT HIGH. THE FALL WAS TIMBER AND DANGERED OFF. A PLAN WAS APPROVED AND THE FALL WAS CLEANED UP AND A CANOPY PUT IN IT.

AN UNPLANNED ROOF FALL OCCURRED AT X-CUT # 33 ON THE OLD WAY TRACK. THE FALL WAS ABOUT 50 FOOT LONG, 20 FOOT WIDE, AND 6 TO 8 FOOT HIGH. THE FALL WAS TIMBER AND DANGERED OFF AND WILL BE CLEANED UP AT A LATER DATE.

Employee was handing a block to his buddy and tripped and fell with the block in his hand and cut his finger it took 2 stitches.

An unplanned roof fall occurred at x-cut # 37 on 6 south belt. The fall was about 20 foot long, 20 foot wide, and 10 to 12 foot high. The fall will be cleaned up and rebolted.

He was putting up his outside bolt and bent it. So he set over to drill a new hole and drilled into the bent bolt caused him to bend the drill steel. While he was trying to get the steel out of the hole it broke and came around hitting him in the face.

An unplanned roof fall occurred one x cut inby spad # 59453 in between #7 and 8 heading Outby the 35 section. The fall was about 25' long,18' wide , and 7-8 feet high. The fall will not be cleaned up.

Employee was operating a continuous miner when a piece of draw rock fell out between the bolts. The rock hit him on the left arm and he received a laceration that took 10 stitches.

An unplanned roof fall occurred at X-cut #27 on the new way track. The fall was about 20 feet long, 18 foot wide and 5 to 6 foot high. The fall will be cleaned up and rebolted.

The EE was moving the miner over to his next cut and the corner of the rib rolled off and hit him in the back of the lower leg. EE was not able to return to his regular job.

An unplanned roof fall occurred at X-cut #13 in the #4 entry in the intersection on the new way track. The area was bolted with 6 foot torque tension bolts. The fall was about 20'W X 20'L X 6' High. The fall will not be cleaned up and it has been cribbed and dangered off.

An unplanned roof fall occurred in #3 entry about 1200' from #3 unit, across from spad # 58833. The area was bolted with 6' torque tension. The fall was cribbed and has been dangered off. The fall was about 18' W X 19' L X 8' high.

This is an old injury that happened on April 4, 2012. EE had been going back to the doctor several times since the injury. EE started missing work on October 26, 2015 for surgery on his left shoulder.

Employee was stepping off the platform of the bolter and he laid his left hand on the supply tray and a piece of draw rock fell hitting him on the hand.

Employee bent down to pick up a 48"" top belt roller and when he started to raise up he felt something pull in his lower back. The employee continued working until March 12th when his doctor took him off work.

While loading shuttle car a rock slid off the car hitting him on the left ankle.

Employee began missing work on June 1, 2012 due to degenerative change in his cervical spine. Phillip ""bumped"" his head on a roof bolt, walking through a crosscut to the feeder.

An unplanned roof fall occurred on the #3 unit in the #6 entry in the intersection 2 x-cut inby spad #58426. The fall was about 20 foot wide, 30 foot long, and 10 to 12 foot high. The area was bolted with 6 foot torque tension bolts. The fall will be cleaned up and rebolted by the clean up plan.

Unhooked chain to let vehicle leave property. Chain caught on vehicle and pulled it through hand and cut finger.

Employee holding a piece of curtain up for shuttle car to pass thru when a lump of coal came out from under tire hitting his Rt. knee. He went to ER on Dec 13,and was given off 1 week. He followed up with specialist on 12-19-11 and released to return to full duty. He started his scheduled vacation on Dec.16th and not scheduled to return until(12-27-11)from vacation

AN UNPLANNED ROOF FALL OCCURRED ON THE #8 SOUTH BELT AT X-CUT #30. THE AREA WAS MINED AROUND OCTOBER 2007. THE FALL WAS ABOUT 6 TO 7 FOOT HIGH, 20 FOOT WIDE, AND 100 FOOT LONG. THE FALL WENT FROM #4 ENTRY TO #5 ENTRY ON THE BELT LINE IN THE INTERSECTION. THE INTERSECTION WILL BE CLEAN UP AND REBOLTED. ALL OTHER APPROACHES WILL BE CRIBBED OFF.

EE began missing work on 10/7/2011 due to progressive massive fibrosis.

AN UNPLANNED ROOF FALL OCCURRED AT X-CUT 82 ON THE OLD 6B TRACK AREA. THIS AREA WAS MINED AROUND MARCH 2001. THE FALL WAS ABOUT 60 FOOT LONG, 19 FOOT WIDE, AND 6 TO 8 FOOT HIGH. THE FALL WILL BE CLEAN ACCORDING TO THE APPROVED CLEAN UP PLAN.

EE was standing beside of his shuttle car and a piece of draw rock fell hitting him on the right knee. EE received 4 stitches.

The hoist was down for more than 30 minutes due to the extreme amount of heat. The temperature was in the high 90 and the fans were down in the hoist house. They installed 4 new fans.

A CONTINUATION OF A ROOF FALL OCCURRED ON THE 1 SOUTH TRACK AT X-CUT #12. THIS AREA WAS MINED AROUND MAY 1996. THE FALL WAS ABOUT 8 FOOT HIGH, 20 FOOT LONG AND 20 FOOT WIDE. THE AREA WAS BOLTED WITH 4 FOOT BOLTS. THE FALL WILL BE CLEANED UP BY THE CLEAN UP PLAN.

The elevator at the camp creek portal was down for more than 30 minute de to a door switch being bad. The elevator was repaired and put back in service. Person could travel via the slope by slope car or walk while the elevator was down.

An unplanned roof fall occurred on 1 North belt 22 x-cuts from the tailpieces. This area was mined around Feb 1980. The fall was about 5 foot high, 40 foot long, and 19 to 20 foot wide. The area was bolted with 8 foot point anchor. The fall was cleaned up and rebolted and it was cleaned up by the clean up plan.

The elevator was down for more than 30 minutes due to a coil being down that caused the brakes to lock up. The coil was changed out and repaired and the elevator was put back into service. People could travel via the slope by slope car or walk up the slope while the elevator was down.

The elevator at the camp creek portal was down for more than 30 minute to to the brake being lock up. The elevator was repaired and put back in servile. Person could travel VIZ the slope by slope car or walk while the elevator was down.

An unplanned roof fall occurred on the 1 South belt at x-cut #9. The fall was about 6 to 8 ft high, 20 ft wide, and 20 ft long. The area was bolted with 5 ft resin bolts. The fall will be cleaned up and rebolted, by the clean up plan.

EE injured his right knee when stepping off a supply car. The EE began missing work 12/27/11 after electing to have his right knee repaired.

EE began missing on May 2, 2011 due to sickness. The Employer was notified on December 20, 2011 that the EE has been diagnosed with CWP.

EE was installing a bolt when a piece of draw rock fell from the top and hit him on the left arm. He received 9 stitches.

An unplanned roof fall occurred at spad #46815 off of the 18th X-cut from the track going the new way. The area was bolted with 5' torque tension bolts. The fall was approx. 6' H. X 19' W. X 180' L. The fall will not be cleaned up it was timbered and dangered off.

Employee was installing a piece of rubber belt in the throat of the miner. The employee was using a utility knife to cut the rubber and the knife slipped out and cut his right leg. (He received 2 stitches).

Employee was walking from #6 to #7 entry to get the scoop when a piece of draw rock fell between the rib and roof bolt striking him. It knocked him down and landed on his foot.

On 3/1/11 EE was walk across the section and tripped and fell causing him to dislocate his shoulder. EE continued to work until 9/13/11 when he elected to have surgery on his shoulder.

Section Foreman was sitting on the rear bumper of the Roof Bolting machine when a piece of draw rock fell striking him in the top of his head. The rock measured 12 inches by 16 inches by 1 inch thick, mining height in this location measured 7 1/2 ft. The accident occurred in #3 entry last open cross-cut on the #5 working section.

EE was bolting in left X-cut as he went to pull up for the next roll of bolts a piece of draw rock fell out between the rolls of bolts hitting him on the right side of the head, shoulder and arm. He received a cut to the right arm that took 20 stitches.

Employee was walking back to the roof bolter in 2 left X-cut and a small piece of draw rock fell from top and hit him on top of the left hand cutting his hand.

EE was making a water line splice on the miner. He was down on his knee and a piece of draw rock fell from the top hitting him on the back of the head and cutting him on the head. EE received 5 staples in the back of his head.

EE states he was greasing equipment when a piece of rock fell and struck him on his head and right shoulder.

EE was cleaning out the scrubber on the miner and the scrubber door fell and hit him on the left index finger. EE received 5 stitches.

He was cutting a move box cable, his knife slipped, cutting his left middle finger. The EE received 4 stitches.

An unplanned roof fall occurred 1 X-cut outby spad #46558. The fall was about 16' wide, 20' long and 5-6' high. The area was bolted with 5' torque tension bolts. The fall will be cleaned up and rebolted.

The elevator at the camp creek portal was down for more than 30 minutes due to a stop switch at the top level was bad. The switch was repaired and elevator put back in service. Person could travel via the slope be slope car or walk.

Employee had been roof bolting, stopped and walked beside the miner; while talking to the miner operator, a piece of rock struck him on the head.

Employee was loading supplies on the tray of the bolter. As he was reaching across some roof straps to get the glue, the palm of his right hand hit the end of the strap cutting it. EE received 4 stitches.

While hanging a line curtain, he slipped to fall and cut his thumb on a roof pan.

While lowering middle belly pan on dozer, sling slipped and pan fell against right arm causing laceration.

As he was tramming the miner, he was standing in the red zone and was struck by the boom.

EE was unloading 5 gallon oil cans off a flat car. He stumbled and fell to the ground, hurting himself in the groin area. He started to miss work on 10/8/10.

An unplanned roof fall occurred at spad #20079 off the #1 section. The fall was about 80 feet long, 20 feet wide, and 10 feet high. The area was bolted with 5 foot torque tension bolts. The fall will be timbered and dangered off and will not be cleaned up.

An unplanned roof fall occurred on the 011-0 MMU on section #2 in the #2 entry about 480 feet from the face. The fall was about 640 ft long, 18 to 20 ft wide, and 10 to 12 ft high. The area was timbered and dangered off. This area was bolted with 5 and 6 ft torque tension bolts. The section moved to new set-up.

The elevator was down at the Camp Creek Portal for more than 30 minutes due to a bad relay on door. The relay was repaired and the elevator was put back into service.

An unplanned roof fall occured at the 4-way at the bottom of the slope. The fall was about 20' W X 80' L X 8-10' H. the area will be cleaned up and rebolted and additional support where needed.

A roof fall occurred in the belt entry 5 X cut inby #1 north drive. The fall was 20' W X 20' L X 8' H. The area will be cleaned up and rebolted and cribbed on the side.

A piece of draw rock fell out between roof bolts, hitting him on the top of the right hand cutting it. EE received 3 stitches.

A roof fall occurred off the #4 section area in the #11 entry at spad #52311. The fall was 20 feet wide, 20 feet long, and about 6 feet high. The area will not be cleaned up. It was cribbed and dangered off.

Employee was setting a timber and a piece of draw rock fell from the top knocking of his hard hat. A second piece fell hitting him on the head cutting it. The size of the pieces of rock was about 8 inch x 10 inch and 1/4 to 1 inch thick. He received 10 stitches

The elevator at the camp creelportal was down for more than 30 minutes due to a ground in the power leads coming from the substation to the main building. Power was fixed going to the building. Persons could travel via the slope by slope car or walk.

While walking up to the power box, a piece of rock fell and hit right shoulder and both hands.

EE was roof bolting in 8 right entry and was putting the last outside bolt in. He just had struck the steel in the pot and a rock fell striking his right hand.

A roof fall occured on the 8 south belt at spad # 43453 between x-cut # 22 & 23. This area was mined around 9/25/2007. The fall was 40 foot long, 19 foot wide and 7 foot high. The are was bolted with 6 foot tention bolts. The fall will be cleaned up and re-bolted.

Employee was putting a new knife blade in his knife and it slipped and cut his index finger. He received 4 stitches.

While he was walking to hang a piece of curtain a piece draw rock fell out of the top sticking him on the hard hat and right leg. He received 7 sutures.

While he was setting timber on the main line a piece of draw rock fell out and hit him in the head and left arm. He received 6 sutures to his head.

A roof fall occurred on the 2 south track one x-cut inby spad #9182. The fall was about 2 to 3 foot high, 10 foot wide and 15 foot long. The area will be cleaned up and rebolted.

EE was walking around the edge of the feeder and slipped and fall and he went to catch himself with his left hand and arm. He strained his left shoulder. He had surgery on his left shoulder on 7/15/10 and he started missing work on the date.

EE was cutting a zip tie off the end of a spotter probe. He received 6 sutures.

A small brow fell on the outby end of an overcast, at #4 X-cut on old 9A track. It was about 8' long, 12' wide, and 3-4' high. It was cleaned up and re-bolted.

EE alleged that he was injured while driving a scoop to the battery station. EE said he hit a hole causing him to bump his head on the canopy of the scoop. EE worked the entire 9 hour shift.

Employee was dropping a cross under guard loose from a belt line. The chain that was holding the guard in place was wrapped around a 4 inch water line. He dropped the guard and the chain unrolled very fast hitting him in the face. Employee received 3 sutures to the nose.

This report is being filed to abate citation #8128500. Employee was in the process of cutting off a piece of metal at 3B tailpiece when a small piece of rock fell from the discharge roller and struck him over top of the eye resulting in restricted duty work.

Employee was walking through an open crosscut and stumbled and fell. He tried to catch himself by extending his arm and felt pain in his right arm and shoulder. The employee began missing work on December 27, 2010 after electing to have surgery on his shoulder.

EE was pulling a big rock off top of a fall with a chain. The chain broke and the scoop went down to the ground bouncing his head and neck into the canopy.

EE was cutting a cut of coal in 5' x-cut and a piece of bit broke off and came back hitting him in the left side of the neck that resulted in laceration (4sutures). (The mine was down from 11/19/09 till 11/30/09 fro the holiday).

While Injured was spot bolting on new #1 section setup, a piece of rock fell from the top, hitting him on the forehead and nose. He recieved 14 stitches.

While Injured was drilling a bolt hole, a piece of draw rock fell from the top, hitting him on the right arm. He received 11 stitches.

While loading supply on bolter, Injury turn to pick up a stack of pizza pans and felt pain in his back.

A roof fall occurred on the #5 section outby the feeder in the #4 intersection at spad #49614. The fall was 8-10 feet high, 20' by 20'. The area was bolted with 6' torque tension bolts. and rope bolts. The fall will be cleaned up and a Jennmar Corp. 3 piece radius arch set system put in place.

EE was jacking up a take-up at a new belt setup. He was on his knee pushing down on a jack bar and it slipped out, strikin ghim under the left eye and caused a laceration. He received 6 stitches.

A roof fall occured on #5 section inbetween #4 & #5 entry spad # 49721. The fall measured about 30'l x 19'w x 6-8' h. The area was bolted with 6' torque tension bolts and 12' rope bolts.The fall will not be cleaned up. The area will be cribbed and danger off.

EE was cutting a piece of rubber tape off of a buggy cable anchor and cut his index finger on his right hand. EE received 10 stitches.

Employee was tramming the roof bolter forward to put up his third row of bolts. A piece of draw rock fell from the top and hit him in the left shoulder knocking him to the ground. He started missing work on 10/15/2009.
