EE was removing a sleeve from an oxygen bottle and felt something in his lower back. He worked up to August 26th before he started missing. I marked this as an occupational illness. (See attached medical report).

Employee was taking down a belt head and lay his hand on the rail and the belt roller fell on it and pinched his finger.

Employee was crossing the belt line and when he stepped on the belt rail his foot slipped and he fell to the mine floor injuring his back.

Employee was moving a fallen timber out of the walk way on 3A belt. When he picked the timber up he felt something pop in his lower back.

EE picked up a tool bag and felt pain in his left shoulder. He went to the doctor on the 15th and they gave him a full release. He went back to the doctor on 6/19/15 and they gave him a light duty slip but we didn't see it until 7/7/15 and he started missing work on 7/8/15.

Trying to put motor back on the track with the forklift. The forklift slipped out making the forklift bounce up and down hurting his lower back

Employee and three other was hanging miner cable in # 7 entry and as Employee lift the cable he felt pain in the lower back.

The elevators at 14 mile was down due to the software being bad. Brought all the men outside while the elevators was repaired it was ready to go at 7:00PM.

An unplanned roof fall occurred at approximately 3:15pm. The fall was at R-seals between #2 & 3 cross cuts in the 1-2A panel. the fall was about 9 foot high, 20 foot wide and 40 foot long. the fall was timber and danger off and will be clean up by a approve plan. The area was mine in 2008.

EE had been working on a ventilation curtain in #1 heading he completed task, he removed his safety glasses to wipe sweat of his eye and rubbed dirt in the left eye causing irritation. EE stated that he had mud on his hands from the ventilation curtain. EE started to miss work on3/31 thru 3/30 and returned to work on 3/31/15. EE worked tow day before going to the Dr.

Employee was walking down the slope belt and slipped on some ice as he went to fall he tried to catch himself and stained his right shoulder. Employee started missing work on 7-9-15 to have shoulder surgery.

EE was cutting a band on a bundle of bolts and something hit him in the eye. No one knew he had sought treatment for the incident until we received a bill from him on 6/22/15. He was given eye drops as a prescription and was told to follow up with the doctor on 1/28/15. I got the e-mail on 6/29/15. The first day of vacation.

An unplanned roof fall occurred at approximately 6:00 PM. The fall was at cross-cut # 32 on 6 South track. The fall was about 10 ft. high, 80 ft. long, and 20 ft. wide. The fall was timbered and dangered off and it will be cleaned up.

When employee went to change his drill steel out, a piece of draw rock fell hitting him on the right wrist. He had a simple fracture to the wrist.

Caught in, under or between a moving and a stationary object

employee saw that the scoop was possibly going to catch the lifeline, he approached the scoop on the operators side without notifying the operator. As he grab the lifeline the scoop started to tram in reverse, he slipped toward the scoop and his left foot went under the bucket. The operator could not see the employee and the scoop bucket came down on the employees' foot.

Employee was working on a 4 inch airline trying to unclogged a frozen airline when the pressure broke loose the airline struck him on the right side on is arm and leg. Causing hair line simple fracture to his right leg.

A unplanned roof fall occurred at this mine approximately 10:15 am. The roof fall occurred on the #2 South conveyor belt between cross-cut #40 and #41, 50ft in spad #9061. The non-injury roof fall measured 20ft long 19ft across by 3ft thick.

An unplanned roof fall occurred at this mine approximately 5:55 am. The roof fall occurred #3C belt conveyor cross-cut #26 and extending cross-cut #27 on the belt conveyor. The non-injury roof fall measured 95 ft. long and 18.5 ft. across by 9 ft. thick. All approaches to the roof fall were cribbed off and dangered off.

Employee was putting up a 12 foot cable bolt and as he went to put the bolt in the hole he felt pain in his left shoulder. The injury result in restricted work for 17 days, employee started missing work on 11/20/2014. (The mine was down for the week of Thanksgiving for vacation.)

Employee was carrying a V-Plow to install on 4D tail piece and he stepped into a hole causing him to twist his left knee. Employee work up to November 20, 2014 and started missing work. (The mine was down for the week of Thanksgiving for vacation.)

Contact with electrical current

EE alleges he stepped on a shuttle car splice & received an electrical shock. He was taken by ambulance to the hospital, he was given full release by the ER doctor. He requested another day off which allowed him one day away from work. We feel this incident didn't occur or didn't keep him from returning to work on his next shift.

Employee was trying to stabilize the ATRS beam mount to flip it over. When flipping it over the mesh latch assembly came down and cut from the first knuckle off on his index finger on left hand.

EE was helping make a water line splice on the continuous miner, he had banded one end of the water line and was in the process of removing the excess band from the banding tool when the banding tool became fouled, he was pulling on the band when it released striking left hand palm area causing a laceration which required (7) stitches.

A unplanned roof fall occurred on the #4 section in #5 entry/intersection 70ft inby spad #400114. The effected area was danger off and timber off, The fall measured 6ft high by 19ft wide and 20ft long. A clean-up plan was submitted and approved on 8/25/14, of the effected area. The fall was cleaned up on 8/28/14.

Moving miner cable out of the roadway tripped over the miner cable and jammed left little finger. (EE worked from 13th to the 16th before going to the Dr. to have his finger checked. He went on lite days on Monday the 19th and return to full duty on the 27th)

A non injury roof fall occurred above the anchorage zone of the 5 foot fully grouted torque tension bolt used for primary roof support in the 3-62 panel in the #7 return entry 3 - X-cut inby spad # 54682. The fall was 20 foot long, 19 foot wide, and 5.5 foot high. It was timber and danger off and will not be clean up.

Employee was in the process of pushing a flat car over to put back on the rail with a hydraulic jack. The jack kicked out and the bar hit him on the forehead. He still had the bar in his hand. He received 3 stitches.

AN UNPLANNED ROOF FALL OCCURRED AT X-CUT #28 IN THE #5 ENTRY ON THE NEW WAY TRACK. THE FALL WAS ABOUT 6 FOOT HIGH, 20 FOOT WIDE, AND 80 FOOT LONG. THE FALL WILL BE CLEANED UP AND REBOLTED BY THE PLAN.

EE was putting up a 12 foot rope bolt and some draw rock fell out around it and the plate came down and cut his wrist, he received 5 stitch.

EE was scooping on roadway & ran over something in the roadway & complained of pain in his back. He worked for the next two days & we were off the next week for the holidays. He started missing on Dec 30th 2013. We are filing this as an occupational Illness due to the Dr. report I will send. ** Dr. notes indicate no acute injuries only degenerative changes & osteophytosis **

Employee was washing the feeder belly pan out and a piece of draw rock fell and hit on the right middle finger cutting it. Employee received 6 stitches.

The employee thought he put the scoop bucket on the ground, but it was setting on a big piece of coal. While he was loading the block in the scoop bucket the piece of coal burst out and scoop bucket hit the side of his right foot.

EE was diagnosed with pneumoconiosis by Doc on 9/8/2013 by a chest x-ray. I received the letter by email on Oct 16, 2013. EE is not working he been off since Dec 21, 2013 he retired Jan 2013.

Fall from ladders

Employee was hanging a curtain in #9 entry when ladder he was climbing on collapsed causing him to fall to the ground hurting his back. ""Bruised only"" NOTE: Suspected fraud, under investigation. No lost time, worked 17 shifts before claiming he was hurting. On 11/4/2013, his wife called supervisor stating that EE fell at home over the weekend and would be off work a couple of days.

Caught in, under or between a moving and a stationary object

Was loading roof bolter & the shuttle car was trammed toward him knocking him to the ground pinning his left arm above his head. NOTE: Suspect fraud, see attached statements from men that worked with him. When mine started laying off, he alleged injury on 7/30/13, but did not miss until 9/19/13, almost 2 months after the alleged injury! Mine laid off contractors 12/4/13. Job ended.

Employee was walking from the power box to the feeder when a piece of small draw rock fell out of the top hitting him on the top of the right hand. He received a cut that took 4 stitches.

Moving cable & stepped backwards tripping over a rock & falling on his side. He continued to work until 5/14/13 at which time he started missing work. The reason this is late being submitted is we were under the impression he was off due to a medical condition rather that an incident at work. **Subsequent findings indicate it was due to Degenerative Disease of the cervical spine

Employee was hanging a curtain, tripped on the curtain and fell causing pain in his left knee. Employee continued to work until May 30th, 2013 at which time he began missing work and was scheduled for arthroscopic surgery on Monday, June 3, 2013

Employee was lifting a timber to build crib he had an incident with his wrist which he was released from two doctors and two hospitals to return back to work. The reason this is late being submitted we were under the impression he was off due to a medical condition rather then a incident at work.

EE was hooking up a sling duster to the scoop bucket as he went to tighten up the chain the duster fell off on his right foot causing a simple fracture to it.

EE was changing out a top roller on a 60"" belt and he was standing closer than he was aware of and hit his little finger between the roller and the belt rail.

Fall to the walkway or working surface

Roof bolter was getting ready to tram out, waiting on the fly pad to be moved, ee was rolling up the fly pad when the bolter operator sat down and bumped the reel control, causing the cable to jerk, flipping ee onto his head and rt shoulder.

Employee began missing work on February 27, 2013 after he started to have trouble out of his right shoulder that he injured on Dec 18, 2012 while hanging a miner cable. (Employee had a pre-existing non mining related injury and had surgery to the right shoulder).

Struck against a moving object

EE was tramming a shuttle car toward the miner, hit hole in the roadway and felt sharp pain in his back.

EE was walking by the section power center and stepped on a miner cable causing him to twist his right knee. The EE began missing work on 12/16/12.

EE was putting a 6' roof bolt in the drilled hole when he went to straightening the bolt out it slipped and came back and hit him in the hand cutting his finger. EE received 6 stitches. He was wearing knucklehead X10 gloves.

EE was getting a piece of draw rock off the miner and it slid across his hand cutting it. The cut took 7 stitches.

AN UNPLANNED ROOF FALL OCCURRED AT X-CUT # 8 ON THE 1 SOUTH BELT. THE FALL WAS ABOUT 20 FOOT LONG, 15 FOOT WIDE, AND 4 TO 6 FOOT HIGH. THE FALL WAS TIMBER AND DANGERED OFF. A PLAN WAS APPROVED AND THE FALL WAS CLEANED UP AND A CANOPY PUT IN IT.

AN UNPLANNED ROOF FALL OCCURRED AT X-CUT # 33 ON THE OLD WAY TRACK. THE FALL WAS ABOUT 50 FOOT LONG, 20 FOOT WIDE, AND 6 TO 8 FOOT HIGH. THE FALL WAS TIMBER AND DANGERED OFF AND WILL BE CLEANED UP AT A LATER DATE.

Employee was handing a block to his buddy and tripped and fell with the block in his hand and cut his finger it took 2 stitches.

An unplanned roof fall occurred at x-cut # 37 on 6 south belt. The fall was about 20 foot long, 20 foot wide, and 10 to 12 foot high. The fall will be cleaned up and rebolted.

He was putting up his outside bolt and bent it. So he set over to drill a new hole and drilled into the bent bolt caused him to bend the drill steel. While he was trying to get the steel out of the hole it broke and came around hitting him in the face.

An unplanned roof fall occurred one x cut inby spad # 59453 in between #7 and 8 heading Outby the 35 section. The fall was about 25' long,18' wide , and 7-8 feet high. The fall will not be cleaned up.

Employee was operating a continuous miner when a piece of draw rock fell out between the bolts. The rock hit him on the left arm and he received a laceration that took 10 stitches.

The EE was moving the miner over to his next cut and the corner of the rib rolled off and hit him in the back of the lower leg. EE was not able to return to his regular job.

An unplanned roof fall occurred at X-cut #13 in the #4 entry in the intersection on the new way track. The area was bolted with 6 foot torque tension bolts. The fall was about 20'W X 20'L X 6' High. The fall will not be cleaned up and it has been cribbed and dangered off.
