Employee was pouring concrete on 5/9/11 and had hole in boot. Got concrete in boot causing dermatitis. Sought medical care on 5/12/11, but company was not aware of accident and treatment until 5/26/11 by insurance carrier.

Employee was descending scaffold when a loose piece of 2x10 lumber struck the individual on the foot. Amended 4/8/11 - Employee was carrying steel angle. He set the angle on the ground, pinching his fingers.
Worker was moving rebar with a fork lift. The load shifted while it was in transit. When the worker unstrapped the bars from the forks, the bars came off the forks. The load was approximately waist height. The worker was struck by the load and landed on a piece of angle stored nearby.
Power line installers working at, but not for, mine. Crew was installing guy anchors for power poles when employee set an anchor attachment onto his foot. Only sprain and bruising, no significant damage or injury. Called information into 800 number as soon as we were notified.

While performing decking work, employee stepped on a piece of unfitted unwelded decking, falling 17 feet to the deck below. He had a facial cut; no other visible injuries. He was asked recall questions, said he was ok and refused care. He worked rest of the day; gait, balance and other activity were normal. Early 12/22 at local ER was diagnosed with lung/rib injuries.
Crane operator stepped on outrigger beam on 75-ton hydraulic crane and his foot slipped and he fell onto the ground (approx 3 feet). He caught his fall with his hands and cut one of his palms on a rock on the ground. Required stitches. Was wearing gloves. No other injuries.
