An employee suffered fatal injuries after he fell down the hoist-well from the 3rd floor onto the basement floor of the preparation plant. The employee was on a ladder when it slipped or shifted causing the employee and ladder to fall over the handrails and down the hoist-well. Employee was transported to the ER by EMS.

Employee was installing a pin in the front spring of a tractor when the wrench/pry bar slipped and struck employees right wrist. Initial injury 01/02/13. On 01/07/13, employee picked up a charger and the discomfort in his wrist became worse, he continued to work until 06/18/13 employee underwent surgery and was excused from work.

Injured employee was using a utility knife to trim rubber from a screen panel when he accidentally cut his left Thumb. He was taken to the ER and received 2 sutures. He was released to return to work.
Injured employee was climbing into the coarse ref screen when he began to experience sharp pain in his back and neck. He continued to work but went to the ER later that day. The ER physician excused him from work.
Employee was assisting with the installation of a piece of pipe and apparently twisted suddenly, causing him to feel pain in his low back.
Employee was pulling on chainfall that came loose pinning his right hand between the chain fall and sump. Employee was wearing metacarpal gloves resulting in only bruise and abrasion, but a prescription medication was given.

Injured employee was replacing a piece of tin when the corner of the tin punctured his glove causing a small laceration to the top of his left hand. He received 3 sutures.
Injured employee was struck on the upper back and neck by a rock weighing approx. 6.3lbs that fell approx. 70 feet from above from the RC4 belt. Injured employee was treated by EMT's and EMS then airlifted to CAMC-General ED. He was admitted for observation and diagnosed with vertebrae fractures of the neck and tissue bruising.
Injured employee slipped and fell while connecting a water hose causing him to strike a belt roller laying on the ground resulting in a laceration to the right hand requiring 2 sutures. Injured returned to work the same day.

Employee was getting a 6"" pipe elbow out of a box outside of the plant. When he bent over to get the pipe elbow out of the box, his foot slipped on ice as he was lifting the elbow out of the box. In regaining his balance, he jerked suddenly and pulled a muscle in his left lower back.
Employee was using a torch to cut a piece of pipe. While handling the pipe, it slipped out of his hand and he moved suddenly to try to catch it and it hit his hand.
Employee was lifting a 5 gallon bucket with some concrete in it and felt something pull in his lower left side.
While using a jackhammer to break up a concrete floor, the jackhammer fell and when he tried to hold onto it, it jerked his left arm and shoulder. Caused a strain to his left shoulder.

Injured employee was taking slack out of the drive belts on Coarse Clean Coal when the wrench slipped causing him to fall approx. 6 ft. His foot became lodged between a bolt and the belly pan. He has been diagnosed with a 2, 3 & 4 Metatarsal Fracture. He has been placed on light duty.

Injured employee was using a utility knife to cut the belt from the #1 Slurry pump when the knife slipped and made contact with his left hand causing lacerations to his 4th & 5th fingers. He received 5 sutures in his 4th finger and 6 sutures in his 5th finger.
Packing buckets up stairs to seal proof. Buckets weighed 64lbs a piece. Had one in each hand, felt lower back pull.

Injured was changing the screen panel in the banana screen when the screen slipped and smashed his right pointer finger between the screen and frame causing a fracture to the distal joint of his right pointer finger. He completed his shift and later went to the ER where the diagnosis was made. He returned to work the next day on his regularly scheduled shift.
Injured was conducting an inspection on the 214A and 214B screens, when he tripped on a protective barrier, causing an injury to his right foot and ankle.

Member was using torch to remove lock covers off a tool box. He stated that he had completed the task, turned torch off & layed them down. As he turned around he felt his right side getting hot. Member received 2nd & 3rd degree burns on right side of abdomen.

Member was installing flex conduit and bent over to tighten nut when his head struck locking bar bracket, knocking his hard hat off of his head, causing a laceration on top of his head requiring 6 staples.
He stated that as he was taking up a chain hoist. While removing the refuse screen deck, it moved, bumping his right hip and he felt a pain in lower back. X-rays taken and he was removed from work on 9-6-2007.
Member was using a slate bar to unplug a chute; when the material freed up, it struck the bar, hitting his forehead, causing a laceration requiring stitches.
Crew was hoisting a box of parts in the machinery well in the plant. Injured employee was on floor helping guide the box thru the floor opening. Injured employee signaled to another employee on the floor above the raise the box. Injured employee his hand on the sheave cable, left hand got caught mashing the ring finger and small finger on the left hand.
While replacing a door glass in a Cat IT28, the door came open and when the employee closed the door, it hit his left knee. This caused the knee to be lacerated and bruised. The laceration required five stitches.
Was Twisting the shieve block to untangle cables on the hoist inside of plant A. Then someone on the floor above raised the hoist pinching his fingers on his right hand. Little finger recieved 2 stitches and ring finger recieved 3 stitches.
Employee was starting up the steps at the RC 4 Tower from the 2nd floor to the 3rd floor. As he reached the 3rd floor he slipped on ice causing him to fall onto his right arm. Wiley was then transported to Boone Memorial Hospital. X-rays were taken and were negative.

Member was cleaning off separator on 4th floor with a water hose. The separator had a small piece of metal sticking out that struck his right middle finger. Stitches were required, no days away from work.
He was losing a clamp on a pipe line to replace it when pipe slipped down mashing his little finger causing a fracture. No lost time occurred from this accident
While pushing refuse into the impoundment, the dozer operator went about 5 feet beyond the edge of the water, causing the embankment to fail resulting in the dozer being engulfed in the fine wet materials.
Employee, an excavator operator, disobeyed the instructions given to him by supervisor, by carrying a piece of pipe down a steep slope with an excavator. The excavator slid on the steep loose ground and caused the operator to lose control and overturn. (Post-accident drug testing showed that the operator was under the influence of marijuana - misc. mail 9/18/06 #5)
Was removing pipe, cut welds on one end through the other end was free. Pipe was hung, pushed again pipe broke free and smashed thumb on right hand between two pieces. Result was cut and possibly broken thumb.
EE was standing at the chain link fence of the R-8 belt drive. The drive chain broke hitting him on the right hand, amputating his right finger at the firsty joint.
Employee was using a sledge hammer to bend a piece of metal on the tail gate of his coal truck. He stated that something popped in his thumb causing pain.

Injured was installing a return trailing roller. While welding, bees came out of structure and injured swatted at them. His foot slipped off and he fell. He hit face first striking his left arm & neck & lower back. He had an MRI on 9-28 & was advised to stay away from work until further notice. His first day missed was 9-29.

Injured was exiting off the manlift on the 6th floor and reached to open the gate to step through when he slipped and fell. He was treated by FMTS and transported to Memorial Hosp. where x rays revealed a fracture to the right fibula. He visited a specialist on June 23rd where a soft cast was applied above the ankle on the right leg.
EMPLOYEE WAS PUSHING COAL WITH THE DION DOZER ON THE RAW COAL PILE. HE WAS IN REVERSE SECOND GEAR WHEN HE STRUCK THE CONCRETE TRUCK DUMP PAD. HE WAS TREATED BY EMS AND TRANSPORTED TO BOONE MEMORIAL WHERE XRAYS WERE NEGATIVE AND RELEASED BACK TO WORK. ON 5/23/05 HE WAS STILL EXPERIENCING BACK PAIN. HE RETURNED TO BOONE MEMORIAL AND SAW DR AND REFERED ON 5/25/05
Employee slipped and put his hand down to catch himself. His hand contacted a piece of metal, cutting his palm. He received stitches.
Employee was on the third floor stairway platform in A plant when he noticed the heavy media cyclone sump overflowing. When he started to descend the stairs his right foot slipped on the first step causing both feet to fly out. He landed on his lower back and buttocks about four steps below. At the hospital he was diagnosed with compression to L4 & L5 disks.
EE alleges that while operating D10 in the raw coal pile he backed the dozer over a windrow causing his neck to jerk when the dozer struck the ground. He states that this occurred at approx 10:00 pm, he completed the shift, notified his foreman at 12:15 am of the incident, filled an accident report and was taken to hospital by ambulance.
EE was putting the guard on. The guard slipped and caught his left hand between the sheave and guard causing a split in the finger nail. He received stitches.

Was pulling on a 12"" elbow across the floor with a choker, the elbow got caught on the corner of a platform and ee kept pulling on it. Strained the middle part of his back.

EE WAS INSTALLING A HARD BAR BEARING. WHILE POUNDING THE BEARING WITH A #5 HAMMER EE MISSED, HITTING HIS RIGHT THUMB. HE WAS TRANSPORTED TO BMH WHERE HE RECEIVED PROPER MEDICAL TREATMENT (10 S TITCHES).
EE GOT ONTO MANLIFT. HIS CO-WORKER PUT A BELT HOIST ONTO MANLIFT. INJURED WAS ON 4TH FLOOR. THE HOIST CAUGHT ON THE 5TH FLOOR, FELL DOWN TO THE 4TH FLOOR STRIKING EE ON THE HEAD.
EE SQUATTED DOWN TO PICKUP A 20-25 LB. PIECE OF CHANNEL ABOUT 2' LONG & STRAINED HIS BACK. THIS WAS ORGINALLY TREATED AS A REOCCURRENCE OF A PREVIOUS INJURY, BUT LATER WAS DETERMINED TO BE A N EW INJURY BY THE TREATING PHYSICIAN BECAUSE THE STRAIN OCCURRED IN THE THORACIC REGION OF THE SPINE VERSUS THE LUMBAR FOR WHICH HE AS RECEIVING CHIROPRATIC TREATMENT AT THE TIME.
EE WAS MOVING CABLE. WHEN HE LIFTED IT IT SLIPPED AND CAUGHT THE CORNER OF THE I BEAM CAUSING A LACERATION.
EE WAS HAULING POND SEDIMENT FROM ORE MICKNER POND TO THE TOP OF ELISA FORK IMPONDMENT. AFTER COMING OUT OF TURN #4 THE TRUCK ENDED UP DROPPING INTO THE DITCH. WHEN HE TURNED THE WHEEL TO GET IT OUT OF THE DITCH, IT LAYED OVER ON IT'S SIDE. TRAVELING UPHILL.
THE EE INDICATED THAT OVER A PERIOD OF TIME WHILE ENGAGING IN BOLTING UP AND OTHER CONSTRUCTION ACTIVITIES, THAT HE BEGAN TO FEEL PAIN IN HIS LOW BACK THAT GOT WORSE OVER TIME. WHEN ASKED HE COULD NOT IDENTIFY ANY PARTICULAR TIME OR EVENT WHEN THE PAIN STARTED.
HE WAS WORKING ON CLEANING GOB OUT FROM UNDER BELT TO CHANGE BEARING ON SNUB ROLLER, WHEN A ROCK FELL FROM CATWALK STRIKING HIM IN BACK OF HEAD.
EMPLOYEE WAS SHOVELING THE OVERLOADED SILO BELT WHEN HE TWISTED HIS BACK GETTING OFF OF THE BELT, EE WALKED TO THE TAIL WHERE HE FELL DOWN TO THE GROUND.
SETTING COOKERS. SLIPPED & FELL & HURT BACK.
EE LEFT B PLANT TO ASSIST CO-WORKER WITH A PUMP THAT WAS ""ON AIR"" AND HOT. THE PUMP HAD BEEN TURNED OFF AND EE WAS ATTEMPTING TO PULL THE SUCTION LINE FROM THE SEDIMENT WHEN IT WAS BLOWN APART FROM THE PUMPHOUSE CAUSING TWO-DEGREE BURNS TO BOTH ARMS BELOW THE ELBOWS.COLD WATER WAS APPLIED TO THE AFFECTED AREAS AND WAS TAKEN TO THE HOSPITAL.

THE EE GRABBED THE 3RD COWORKER SO HE WOULD NOT GET HIT WITH THE ACID & THIS MOTION CAUSED THE BOAT TO SHIFT AGAIN PUTTING THE EE DIRECTLY UNDER THE LEAKING LINE

EE WAS HAULING REFUSE MATERAIL TO STACKER BELT ON IN POUNDMENT. HE WAS DUMPING MATERIAL, IT STUCK IN BED AS IT WAS RAISED. THE CMB OF TRUCK WAS LIFTED UP AND IT SWIVLED ONTO ITS SIDE. HE ALLEG ED IT STRAINED HIS BACK. BED OF TRUCK DID NOT TURN OVER.
EE WAS BACKING THE D10 REFUSE DOZER WHEN THE LIGHTS ON THE STACKER MADE IT HARD TO SEE THE REFUSE PILE. EE BACKED INTO THE PILE & STATED HE FELTPAIN IN HIS NECK. RECOVERY TIME IS UNKNOWN

INJURED HAD TAKEN ALL BOLTS OUT AND WAS REMOVINGSIEVE WHEN HOOK BENT AND SLIPPED OFF. HAND SLIPPED UP SIEVE CUTTING SKIN OFF OF RIGHT LITTLE FINGER DOWN TO BONE AT THE KNUCKLE.
