Struck by falling object

Employee was assisting in the removal of a steel beam. While preparing to cut a beam another employee in the man basket struck a piece of steel causing it to fall on the employees leg. EE has a small fracture on EE's patella as well as a laceration above EE's knee. EE is on modified duty.

Fall from scaffolds, walkways, platforms

Worker was walking on 2nd floor of transfer. EE stepped on unsecured decking. The decking slipped and EE fell to the floor below. EE hurt EE's knee in the initial fall and then EE's chest when EE fell over after landing. EE was wearing fall protection but was not tied off.

While welding arms for a return roller training bracket, a piece of welding slag went into miner's ear. Employee has continued to work with no lost days. Employee had outpatient surgery on 01/08/2021. Accident is medical reportable.

Employee was assisting with the removal of a screen side board from the basement of the plant when it slid down a post it was leaning against and caught the employee's right foot. The employee was wearing metatarsal boots at the time.

Employee was climbing on side of truck dump getting ready to weld and piece of existing 8"" channel broke loose and pinched 3 fingers. Employee was wearing metacarpal gloves at the time.

Employee was driving on Rt. 60 toward Montgomery area when the employee slowed to turn right into prep plant, employee got hit by the back / side area.

Struck by flying object

Employee was operating an air impact wrench. The Snap lock ring broke on the wrench causing the socket to come off. The socket struck the employee in the mouth causing a laceration. The laceration required 6 sutures.

Employee was cleaning out a belt chute with a slate bar. A rock broke loose pulling slate bar away from employee's hand and pinning it against the chute. This resulted in a laceration on the left thumb that required 5 sutures.

Employee was mounting an 850 John Deere dozer, lost footing due to muddy conditions and fell against dozer tracks striking rib cage.

Employee was cleaning out a belt chute with a slate bar. A rock broke loose striking the employees hand and pinning it against the metal wall and the slate bar. This resulted in a laceration that required 7 sutures.

When support column was loosened it fell and hit employee's right foot/heel causing a cut to employee's right heel.

Accident type, without injuries

A fire at the Mammoth Magnetite Plant storage belt stacker. The plant was being deconstructed by Powell Construction. At approx 6:20pm a pedestrian traveling Rt 60 noticed smoke and stopped by Mammoth Guard Shack and notified the guard on duty. The time of the fire and duration are unknown, however minimal damage occurred.

Employee was installing grizzly bar on the down river crusher. EE picked the bar up with an electrical hoist. When putting the bar into place the EE pinched right thumb between the grizzly bar and the frame of the crusher. This caused the thumb to burst open and caused a laceration that required 8 sutures.

Employee was attached a 4' section of 4"" threaded pipe to a 90 degree pipe el and it slipped when the threads did not catch and part of the threaded portion struck employee on the left wrist.

Over-exertion in pulling or pushing objects

Employee was helping 3 other employees drag metal grating across the plant floor. Employee felt a sharp pain in lower stomach. Employee continued to work, and was diagnosed with a hernia on 10/27/2016. The employee has been released to work regular duties until 1-13-2017 when employee will go before a surgeon to have the hernia repaired.

The operator was notified on 5/24/2016 that the former employee (Last exposure date worked 10/2/2015) has been diagnosed with Occupational Pneumoconiosis.

Over-exertion in lifting objects

Employee bent down to pick up a piece of 8"" channel, he felt something pull in his lower abdomen. He continued to work his shift. On 10/9/2014 the employee decided to go to the doctor nothing was found. The employee continued to work until he returned to the doctor a second time on 11/7/2014 where a hernia was found. His first day missed work was 11/10/2014.

Employee was changing out the dryer basket in #2 dryer when his hand slipped, and rammed the basket. The basket hit his hand cutting through his glove, and causing a laceration, resulting in the employee receiving 5 sutures.

EE was removing bolt out of taper lock bushing with an air impact wrench. As he held the taper lock the bushing shattered, cutting the palm and thumb of left hand. Was wearing leather gloves.

Employee was removing snap ring off track take up on D8 dozer. The snap ring slipped and cut his index finger on left hand.

Employee was helping remove falk fluid drive coupler when it fell off shaft unexpectedly and pinched the thumb of his right hand between the coupling and frame. His thumb was smashed partially dislodging his thumb nail. He received five sutures to attach his thumb nail. He returned to his regular job at full duty the same day.

Employee was mounting D10R dozer and slipped and fell to ground on his back.

Employee was shoveling out the down river feeder. When he pushed the shovel forward his arm caught a piece of metal on the feeder box causing a small laceration.

Member was working to unstop a drain pipe that ran from second floor of plant to the basement. He entered a restricted area and was walking across a metal sub floor to reach the pipe. A section of floor broke and he fell 15' 9"", striking a Catflock tank and sliding down it to the basement floor.
