EE STATED HE WAS EXTRACTING A BEARING RACE FROM PLANETARY, WHEN A PIECE OF METAL CHIP FROM RACE FLEW OFF AND HIT HIM ON LEFT SIDE OF HIS NECK.

OPERATING FORK LIFT TO PULL JACK PINS OUT OF MOBILE TAIL PIECE. THE WELD ON A NUT BROKE. THE NUT AND CHAIN CAME BACK STRIKING HIM ON THE HEAD 3""ABOVE LEFT EAR.
