Employee stated was walking the belts and when getting done with C-Belt, employee stepped down, twisting left knee and fell on the ground. Employee worked until 7/24/2020 at which time employee had surgery and began to miss work.

Employee stated EE shut c belt off to remove a rock that was fouled in the structure as EE lifted the rock onto the belt EE felt a sharp pain in EE's groining area
Employee stated was attempting to pick up a roll of belt with the loader when the belt slipped off the forks. When the belt slipped off this caused the loader to teeter throwing EE into the canopy injuring neck. Employee sought medical treatment on 7/10/2019 and was placed off work under doctors care.

Employee stated EE was moving a 72"" belt structure out of a roadway, employee bent over to move structure and felt a pain in EE's lower stomach area.
Employee stated a bearing housing fell on left foot resulting in a fracture and restricted work days. Employee did not seek medical treatment until 3/11/2018 at which time incident became reportable.

Cable on counterweight 2 of 4 broke, dozer backed over cable, employee was guiding dozer operator off, counterweight dropped, main pull plate flew up and hit employee on right side, lifting ee off the ground and throwing back to the ground. Right leg, right abdomen, right ribs, right hip and back -- contusion, abrasion.

Employee was washing around the ""C"" belt braking resistors. He stated that he tripped on a wash down hose and his arm slid under the resistor and came in contact with power, receiving electrical burns to his left hand.
