EE was working to replace a high voltage starter for the slope belt and was in the process of adjusting the mechanical linkage connected to the switch when EE came in contact with electricity. EE was transported via ambulance to CAMC Memorial where EE passed away from the injuries sustained from the incident.

Employee was lifting short pieces of beam to load them in the back of a truck. On the last beam they twisted after they picked it up, causing them to strain their lower back. Employee went to doctor the next day and was told to rest their back every 40 minutes. Employee has been placed on modified duty.

The employee was climbing a ladder to begin welding. The ladder was erected on a sloped surface with uneven legs. Once EE reached the 4th step, the ladder fell sideways causing EE to fall to the concrete below. The employee sustained a few abrasions and contusions to lower back and rib area. EE missed zero days of work but is still on physical restrictions.

Employee was in the process of changing an O-ring in the tilt jack of a D8R Dozer, EE had gathered tools and was climbing between the blade and the machine when EE slipped which twisted EE's left knee. On 12/1/2021 the employee received the results of an MRI which revealed a tear of the PCL. No surgery is required. There were NO Lost or Restricted Work Days.
Putting side clamp on, cable broke releasing slack which in return caught hand in between side clamps
Employee fell approximately 3' from a 4' ladder and landed on EE's left side. Employee had a can of starter fluid in their jacket pocket. Employee sought medical treatment on January 13th, 2020 and was diagnosed with a fractured rib and which time this became a reportable incident. No lost workdays.

Employee slipped and fell in an area with a slight incline which resulted in a dislocated right elbow. Employee received medical treatment and returned to work on EE's next scheduled shift without restrictions.
Employee was assisting crew with the installation of a section of 18"" polyurethane pipe using two chain falls when the load shifted and hit the employee who was standing adjacent to the pipe on the hard hat, jamming the employee's low back.

The employee was operating a lizard truck on the refuge haul road when the truck struck a hole causing the employee to bump their elbow against the armrest. The employee sought medical treatment and was released back to work. The employee later sought medical treatment on 8/28/2018 and was taken off work.
The employee stated was going down the outside steps at the plate and frame building when foot slipped causing the employee to fall on posterior and sliding down the other 3 remaining steps.
The employee stated ee was stepping up into a lizard truck when ee said ee right knee gave out. Employee sought medical attention on 4/24/2018 and was taken off work. This incident is being reported at Lost Time Accident.

Employee reported fall on 10/30/17. Stated stood up to walk out and got foot hung in chair wheels. States as falling hard hat fell off and employee landed on back and head bounced on floor. Employee was terminated on 10/30/17 but not due to occurrence was caught sleeping on 10/28 and 10/29/17
Employee was handling a piece of steel plate lying on the floor and felt something pull in EE's back.
Employee was lifting and using a chipping hammer throughout the day and began feeling discomfort in right elbow.
The employee stated was dismounting a D-7 Dozer, and slipped off the push arm of the Dozer. The employee stated as the employee fell the employee landed on right shoulder. Employee sought medical attention and was diagnosed with a fractured shoulder. Injury being reported as Lost Time Injury.

Employee was in the process of taking a load up to refuse area to dump. In that process employee stated, hit a rut/hole in the road measuring 4'long x 3' wide x 2' in depth, causing jarring to right shoulder on the arm rest. Employee later sought medical attention and was placed on restricted duty. This incident is being reported as restricted duty.
The employee was in the process of cutting trees and brush when employee tripped and struck the top of left foreman against the saw blade causing lacerations that required sutures.
The employee stated EE was cleaning a de-icer spray bar when the pressure was released from the spray bar causing the de-icer to contact the employee's face and left eye. The employee was taken to the hospital where EE's left eye was flushed, and the employee was prescribed eye drops making this incident medical-reportable.

The employee stated that he was walking through the entrance door to the plant when he struck his elbow on the door handle causing a contusion. On 12/6/2015 the employee sought medical attention and was prescribed physical therapy due to the 11/11/2015 incident. This is being reported as a medical injury.
EE stated that his feet got tangled up in a water hose as he walking toward the #1 press to wash it. Fell to the floor straining his left hand and wrist. 10/9/15 incident became a reportable injury because ee sought medical attention and was diagnosed with a possible hairline fracture and prescribed a wrist brace. He returned to work at his normal job his next scheduled shift.
The employee stated that while operating the 740 truck on the refuse dump he drove over a hole and strained his back. He sought medical attention and placed on restricted duty for two days.
The employee stated that on 4/20 he was operating the D6 Dozer in the right grain ditch when he ran over a rock and strained his back. The incident became reportable on 5/11/2015 after the employee sought medical attention and removed from work for a back strain.
The employee stated that he and another employee were lifting the clean-up pump guard from the pump and did not realize the guard contained mud. As they lifted it, the additional weight caught him off guard causing a strain to his back. The employee was diagnosed with a strain and did not return to work on his next shift.
The employee stated he had just cut a piece of metal for an overflow box. As he turned the piece of metal over, he slipped and dropped the metal onto the top of his right foot. He continued to work and sought medical attention on 1/12/15, at which time he was diagnosed with a fracture and place on modified duty. This incident is being report as restricted duty.

Employee was helping move a 10"" section of pipe that had gotten caught on an adjacent pipe and sump, when the pipe shifted and pinched his finger between the pipe and sump. The employee was wearing metacarpal gloves at the time.
The employee stated he was attempting to empty a belt magnet when he caught his ring finger between the magnet and the chute. The employee began missing work on 12/29/14 due to the injury.
The employee stated that the crusher had become clogged requiring the chute access door to be opened. While observing the chute the access door swung closed striking the back of his right hand. The employee continued working, but sought medical treatment on 10/18/14 due to continued swelling and was diagnosed with a fracture. The incident became medically reportable on 10/18/14.
On June 3, 2014 at 11:22 a.m. we were notified that this individual that had previously been employed at this location was diagnosed with hearing loss. This is being reported at an illness.
On May 27, 2014 at 4:26 p.m. we were notified that the individual that had previously been employed at this location was diagnosed with pneumoconiosis. This is being reported as an illness.

The employee stated that he was operating an articulating truck of the refuse area when he drove into a hole in the roadway and caused him to strain his shoulder. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.
The employee was changing a bad bearing on a roller for the #1 belt press when the roller popped out of place causing his left ring finger to be caught between the roller and belly pan resulted in a laceration that required sutures to close.
Employee walking down o/s steps at plant, steps were snow covered, he slipped & as he fell, hit head on rail cut rt. ear.
The EE stated he ran into a hole in the road at the #1 switch back curve. The EE stated he felt pain in his lower back. He sought medical attention that day & returned to work under modified duty until the 25th at which time he was placed off work. EE missing work on 1-25-13. *On 3/19/14 the employee was awarded 5% Permanent Partial Disability as a result of the injury.

HE HAD WALKED THE CUT OF CARS & WAS 5 CARS FROM THE LOCOMOTIVE. HE WAS ON THE CAR & HIS FOOT SLIPPED OFF, HE HELD ONTO THE HANDHOLD TO CATCH HIMSELF, STRAINING HIS BACK.
