Struck by falling object

Employee was on-shifting the 9N#3 entry from the mouth to the 25 break t-car. The employee had gone through the first check curtain located just inby #4 break and was headed towards the 2nd check curtain when a rock, approximately 3' x 3' x 1' thick, fell from the top and struck the employee on the hard hat driving ee to the ground.

Employee was pulling a 1 1/2"" water line in the pit area of the slope belt when they felt a pain in the lower back.

Employee was attempting to lift a box filled with bolts when they felt a pain in the lower back.

Employee was mounting #10 Jeep when employee slipped and struck right knee on the tram lever of the jeep. The employee was not removed from work until 09/14/2018 and began to miss work on 09/14/2018.

Employee was using torches to cut the studs off of a scoop tire when a piece of slag blew out and went underneath the employees safety glasses and came into contact with EE's right eye.

A roof fall has occurred at #4 crosscut from the #2 entry to the #3 entry located in the 9E Bleeder area. No injuries, entrapment, nor was any equipment damaged in a result of the roof fall. The roof fall will not be cleaned up. The size and height of the roof fall is unknown.

Employee was struck in the right hand by a piece of belt structure while replacing another piece of damaged belt structure along the 8 Haulage #1 Conveyor Belt. The employee was removed from work on 06/18/2018 by EE's doctor.

Employee was installing permanent roof support when a rock measuring 12"" x 12"" x 3.75"" fell from the mine roof and struck the employee in the top back of EE's hard hat.

Ee was installing front drive line axle on 988 machine. Used a wobble socket to tighten bolts on the yoke, socket slipped and struck ee in mouth, causing laceration to upper lip and fractured 2 front teeth.

The main service elevator encountered an over travel fault. The sensor was repaired and the elevator began normal operations at 3:06am on 04/16/2018.

Employee was using a pole ax to loosen track ties from the rail at the end of 9M track. The employee started to feel a pain in lower back while bending over and striking the track ties with the pole ax.

Employee was placing the trolley pole back onto the trolley wire when a rock fell from the mine roof and struck the employee in the right side of head and back.

Employee was walking inby in the #2 entry on the 9O CM Unit when the continuous miner cable and waterline that was hanging across the intersection was pulled down and struck the employee in the right side of the neck and back.

Employee received injuries to the left hand when their hand was caught between the tailpiece controls and the tailpiece.

Employee was assisting another miner in moving a piece of metal from the offside of the stageloader to the walkway side of the stageloader when employee felt a pain in employee's right shoulder.

The main service hoist located at the 8 Haulage shaft had a trip lever in the governor broken. The part was replaced and the 8 Haulage hoist was back in operation at 10:08pm 06/25/2017.

Employees were spraying lime on ponds to settle materials and stopped to adjust linkage on motor used for spraying materials. Employee complained of the fumes from the exhaust and walked away. Another employee saw employee lying on ground and went to check on employee.

Employee was cleaning belt line using a rake when the rake came into contact with a moving belt and caused the rake to strike the employee in the right side of the employee's face. Initially the CT Scan showed no fractures and employee returned to work on employee's next regular shift & worked until On 2/22/2016 another CT Scan showed a fracture in employee's right jaw.

Employee was riding on top the #31 mantrip coming off of the 9L section at the end of the shift. The trolley pole came off of the wire, struck the mine roof, then broke into. One of the jagged edges of the broken pole struck the employee on the right hand between the middle and ring fingers causing a laceration that required 5 stitches.

Employee was shoveling loose material on the 8 West #2 belt at #37 crosscut. As he raised the shovel up to dump the material, he felt a pain in his lower back.

The 8 Haulage elevator was inoperable in excess of 30 minutes due to one of the wedges catching. No one was on the elevator during this time.

Switch on the bottom elevator doors was misaligned and would not operate. Switch was re-aligned and tighten down.

Injured was installing 6' torque tension bolts in #4 entry on 9M. When he pushed away and up on the roof bolt he felt a pop in his right shoulder. He was transported to Welch EMG where he was treated for pain in right shoulder.

Injured was tramming scope when he hit a hole in the bottom throwing him into the canopy of the scoop. Causing pain in his neck. He finished the shift and saw the PA on site.

The weekly examiner found an unintentional roof fall inby the 4 Right EP. This area is not travelled or examined. The fall did not effect any other travel or ventilation.

An unplanned roof fall occurred on the 9L section at the #28 break, 2x1 crosscut. The fall was approximately 95' long by 18' wide by 9' high. The fall was above anchorage but did not affect ventilation or passage.

Injured was pulling a rock 3.5""X15""x6"". As he was pulling the rock with a slate bar, another rock 1'x4'x2' on top of it fell sliding down the slate bar striking injured on top of right wrist. He was transported outside. He was treated on site by PA and received 5 stitches.

Injured was walking around the belt drive when he slipped in mud and water reaching out he caught himself against the motor shroud pushing it into the fan catching his hand. He was transported to Welch Emergency Hospital where he was referred to a specialist.

Injured was making a cable splice on the roof bolter when a rock fell from the 15' roof 8""x6""x2"" to 0"" striking him on the right shoulder driving the knife into of his right leg. He was transported to the hospital where he received 5 stitches in his right leg and contusions to right shoulder area.

Injured and witness were standing at the ch4 monitors, when the miner quit tramming. A slicken sided rock 8'x6'x3' fell on the boom and slid down the operators side breaking and striking injured on the top of his right foot. He was transported to the hospital. Treated and referred to a specialist.

Employee was working in an area that had cement mixture floating in the water. The employee had the water soak into his rain suit and rubber boots and the chemical reaction caused chemical burns to the skin.

At approximately 2:10am the weekly examiner found an unintentional roof fall in the 8 South #1 entry 1 crosscut out by the 8 South #1 seal.

Injured operating continuous miner when started cutting rock he felt sharp pain in left bicep and arm. He had a puncture wound to his left bicep. He was transported to the emergency room where he was referred to another doctor to have a piece of carbide removed from his left bicep.

Employee was traveling through a mandoor from the #2 to the #3 entry at the #35 crosscut on the 9L section. As he passed through the door, the door shut on his right hand causing a laceration that required stitches to his index finger.

Employee was operating continuous miner. While cutting coal and rock a piece of carbon broke off of the tip of one of the miner bits, flew back and struck the employee in the left forearm. The carbon became embedded in the employee's arm. The employee was standing behind the boom of the miner.

Injured was helping to hang a #6 AWG scoop charger cable from roof bolt plates at 9K Longwall # 46 break, #2 to #1 Break through in 7.5' high top when he felt pain in his left shoulder. He is not working but no communication on whether he has been to a doctor for a diagnosis.

Employee was operating continuous miner when a rock fell out of the top and struck him on the right hand.

Employee was dragging hose for application of rock dust to the area when the hose came loose from the coupling and the employee lost his balance falling onto the ground injuring his left shoulder.

Injured employee was a passenger on #25 bus and spurred into 9G to allow supply motors to pass, when the supply motor hit the 9G switch, the motor veered into 9G, striking the #25 bus and injuring a miner.

Employee was installing belt support in #1 entry when a rock fell out of the top and struck him on the right arm.

Employee was removing a miner bit from a continuous miner and got his right finger caught in-between the miner bit he was extracting and the tool he was using to perform the work.

Continuous miner cut into a degas hole that was in the top instead of in the coal seam. The sparks from the miner bits cutting the rock caused an ignition of a pocket of gas in the degas hole.

Employee was conducting a pre-shift examination and he went to lift a piece of metal from the top of the roof bolter, the metal fell onto his left foot.

Employee was moving miner cable when he stepped on the cable and twisted his right ankle as he fell to the ground.

An unintentional rock fall occurred in the 8M Bleeders at #53 Crosscut. The fall was above anchorage, and additional floor to roof support has been installed. A request to the MSHA District 12 office has been made to move the Evaluation Points.

Employee was adjusting the forks on a diesel forklift when he caught his fingers in between the forks.

We had an unintentional roof fall above anchorage at the #72 crosscut on the 8 Haulage #2 beltline. The fall was approximately 50'L x 18'W x 8'H. The fall occurred in an intersection where there appeared to be transitional roof at or near the 8' height. The beltline in the #72 crosscut was covered up by the fall.

EE, along with another EE, were pulling slack on cable and completed task and moved on to putting wrench on 1"" pipe when pump broke free from pipe and spool struck EE on back shoulder & elbow on right side.

While attempting to open an airlock door, the door abruptly shut on his left finger. The employee received three stitches to his left middle finger and returned to work the next day. The employee was wearing mandatory gloves.

We reported a roof fall on the White Oak Loaded side of the mines near the Weir. After further investigation we concluded that the fall was not above the anchorage, it is not in a traveled area, and does not interrupt any ventilation for the mines. This was an extenuation of the rock fall that occurred on 22 June 2014. The rock measured 18' X 20' X 5.

he 8 haulage elevator, our secondary escapeway, had one of the four auxiliary breaking systems lock into place, causing the elevator to stop movement. The elevator was operational at 5:00pm

EE was helping to prepare the work site to begin daily grout work. The crew had to pull tow 500' joints of polyline three crosscut, appx. 525'. EE stated he felt a pull in his lower back while pulling the first joint. There were 5 men spread out on the poly line, EE being the third in line. The size of the poly line was 1 1/4"". EE did not report this accident till 3/14/2014

Employees were removing a shaft from a Scoop gear box, and using a hammer to assist in relieving pressure due to the shaft being in a bind. While striking the shaft with a hammer, a piece of spline struck the injured employee in the left forearm.

EE was closing gate and caught his left hand in locking part of gate. Doctor's applied 3 stitches.

Employee was on his knees, working on the track, when the employee behind the injured employee was using a slate bar and the slate bar slipped and struck the injured employee in the back of the head. The employee reported to work as regularly scheduled on 10/8/2013. On 10/14/2013 the employee was placed on restricted duty and was released to full duty on 10/23/2013.

While installing ventilation tubing on the section, the tubing slipped out of his grasp and struck him in the head and neck.

Employee was traveling as a passenger in track ride facing the direction of travel on West Mains track. At #142 break the employee struck a roof bolt causing the employee's head and neck to recoil. The employee was transported to the local hospital.

A weekly examiner identified a roof fall on 8 South, #3 entry at #13 crosscut. This roof fall measured approximately 10' X 10' with a distance of 16,189' from the nearest working face of the mines. This area is not normally traveled by our employees and does not interfere with air or travel ways.
