Employee was on-shifting the 9N#3 entry from the mouth to the 25 break t-car. The employee had gone through the first check curtain located just inby #4 break and was headed towards the 2nd check curtain when a rock, approximately 3' x 3' x 1' thick, fell from the top and struck the employee on the hard hat driving ee to the ground.

Employee was pulling a 1 1/2"" water line in the pit area of the slope belt when they felt a pain in the lower back.

Employee was attempting to lift a box filled with bolts when they felt a pain in the lower back.

Employee was walking in the #2 Entry at #22 Break on the 9O CM Unit Panel when ee slipped in a mud hole injuring right knee and hip. The employee began missing work on 09/14/2018.

Employee was mounting #10 Jeep when employee slipped and struck right knee on the tram lever of the jeep. The employee was not removed from work until 09/14/2018 and began to miss work on 09/14/2018.

Employee was knelt down speaking with the preceding shift crew members on the 9O CM Unit, as the employee attempted to stand up from a kneeling position foot slipped and the employee twisted right knee.

A small roof fall has occurred between 9M and 9N recovery entries.

Employee had just finished installing a rib bolt when a rock fell from the mine roof and struck the employee in the head causing a laceration just above the left eye.

Employee tripped over top of a cinder block and felt a pain in lower back.

Employee was installing roof support when a rock fell from the mine roof and struck the employee in left bicep causing a laceration.

Employee was using torches to cut the studs off of a scoop tire when a piece of slag blew out and went underneath the employees safety glasses and came into contact with EE's right eye.

A roof fall has occurred at #4 crosscut from the #2 entry to the #3 entry located in the 9E Bleeder area. No injuries, entrapment, nor was any equipment damaged in a result of the roof fall. The roof fall will not be cleaned up. The size and height of the roof fall is unknown.

Employee was struck in the right hand by a piece of belt structure while replacing another piece of damaged belt structure along the 8 Haulage #1 Conveyor Belt. The employee was removed from work on 06/18/2018 by EE's doctor.

Employee was installing permanent roof support when a rock measuring 12"" x 12"" x 3.75"" fell from the mine roof and struck the employee in the top back of EE's hard hat.

Employee was attempting to raise up from a kneeling position when foot slipped causing employee to fall forward. Employee tried to catch self with left arm and felt a pain in left arm and shoulder.

Ee was installing front drive line axle on 988 machine. Used a wobble socket to tighten bolts on the yoke, socket slipped and struck ee in mouth, causing laceration to upper lip and fractured 2 front teeth.

The main service elevator encountered an over travel fault. The sensor was repaired and the elevator began normal operations at 3:06am on 04/16/2018.

Employee was using a pole ax to loosen track ties from the rail at the end of 9M track. The employee started to feel a pain in lower back while bending over and striking the track ties with the pole ax.

Employee was placing the trolley pole back onto the trolley wire when a rock fell from the mine roof and struck the employee in the right side of head and back.

A rock measuring 10"" x 8"" x 1"" to 1/2"" fell from the mine roof and struck the employee in the right cheek and upper lip causing lacerations to both the cheek and lip.

A roof fall has occurred at break number 33 in the 9N Headgate Entry.

Employee was walking inby in the #2 entry on the 9O CM Unit when the continuous miner cable and waterline that was hanging across the intersection was pulled down and struck the employee in the right side of the neck and back.

Employee received injuries to the left hand when their hand was caught between the tailpiece controls and the tailpiece.

Employee was assisting another miner in moving a piece of metal from the offside of the stageloader to the walkway side of the stageloader when employee felt a pain in employee's right shoulder.

Employee was walking around a 585 Scoop when ee tripped and fell, fracturing ee left ankle.

Employee was walking through a set of airlock doors when EE tripped and fell, striking EE's left knee on the mine floor on 10/16/2017. The employee returned to work on EE's next regularly scheduled shift and continued to work until EE was placed off of work due to the injury from the fall on 11/01/2017.

Employee was walking along side of a power center when they tripped on some cables and stumbled, when the employee stepped forward with the right foot they felt a sharp pain in the foot.

Employee was removing a rock from the top of a continuous miner using a slate bar when employee's hand was caught between the slate bar and the continuous miner causing a laceration to employee's right little finger.

Employee was cleaning on the slope belt when struck in the left hand by a rock causing a laceration to the left ring finger.

Employee sustained an injury to left knee while working on the mine track on 09/18/2017. The employee did not begin to miss work until 04/16/2018 due to the injury sustained during the incident.

EE was walking down the slope belt when they slipped on some loose material on the mine floor and twisted left knee. The employee finished shift and returned to work at next regularly scheduled shift. Then on September 18, 2017 the employee was walking when the knee popped and they went to the ER and was placed off of work starting September 19, 2017.

Employee was standing beside a scoop bucket in an intersection loading belt onto a flat car when the employee slipped and fell. As the employee fell, hand went between the scoop bucket and the ram. The employee suffered lacerations on right hand after the ram smashed hand against the scoop bucket.

Employee was carrying rock dust along the belt line when employee slipped on a wet piece of belt laying on the mine floor, slipped and fell injuring right hip and lower back.

Employee was using a 1 1/2"" wash down hose, employee bent over to wash out from underneath the slope belt, as employee stood upright employee felt a pain in employee's lower left back.

Employee was riding in track mounted mantrip when EE's foot was caught between the mantrip and a scoop axle laying beside the mine track.

Employee was cleaning a trash pump, as employee lifted up on the pump felt a sharp pain in groin area.

Employee was struck by a rock (12""x6.5""x2"") in the top of hard hat while operating a rail mounted jeep along 8 Haulage Bypass causing an abrasion and contusion to the head.

Employee struck their left index finger with a sledge hammer while holding a chisel.

An unplanned roof fall below anchorage has occurred in #4 crosscut between the belt entry and the #1 Monkey Butt between 9M and 9N recovery faces.

Employee received a laceration on ee right index finger when it was smashed while lifting a heavy object on the 9L Plow Head Gate.

Employee was picking up trash by hand and throwing into dumpster when employee struck employee's left elbow on the fork lift frame assembly.

Employee slipped on mine track and fell twisting back and feeling a sharp pain in mid to lower back.

Employee felt a pain in EE's lower back as EE attempted to twist and throw an airbag onto a rail mounted motor.

Employee was walking along the 8 Hlg #1 belt and slipped and twisted right knee.

A roof fall has occurred at #16 Break, #3 Entry, 9M CM Unit

A continuation of a roof fall was discovered adjacent to the #2 light side track.

Employee was operating a electric (walk behind) pallet jack to unload a truck when employee ran over their right foot.

Employee was struck in the back by a rock measuring 2' wide x 2' Long x 1 1/2"" thick, causing a contusion to the employee's lower right back and hip.

Employee was struck in the left hand by a piece of metal that broke off of the pick hammer employee was using while installing a 6"" hugger onto a main water line.

Employee felt a pain in the back as EE moved a bottom roller out of the roadway.

Employee was lowering ventilation tubing from the mine roof when EE felt a pain in left shoulder.

Employee was performing preventive maintenance on section feeder when employee stepped into a hole in the mine floor and felt a pop in employee's right knee.

A roof fall has occurred on the 9L Longwall panel overtop of the crusher and stage loader area. No injuries have been reported as a result of the roof fall.

Employee was crawling along the panline on the 9L Longwall panel when a rock rolled out and struck the employee in the left arm.

Employee was using a concrete block to knock a hole in a ventilation stopping and smashed left ring finger.

Employee was struck in the arm with plow chain while assisting in putting the plow chain back together.

Employee was advancing a longwall shield when a rock struck the employee in the middle of the back.

A roof fall has occurred inby the longwall face in the bleeder area.

Employee was crawling along the longwall when a rock struck the employee in the right foot.

A roof fall has occurred on the 9N CM Section inby the loading point at break #32 in the #3 entry.

Employee was attempting to get a bolt wrench out of the offside drill pot and back spun the wrench when it came out of the drill pot and struck the employee in the right side of head causing a laceration.

The tracking system for the mine was down do to a software issue. The system was rebooted and the tracking system was inspected and found in working order.

Employee felt a pain in left shoulder after bending a roof bolt while trying to install it into the mine roof.

Employee was unloading a bundle of belt stands when the belt stands fell off of the flat car and struck the employee in the right ankle.

Employee was using a slate bar to pry when the slate bar slipped and caused the employee to step backwards onto rock and gob causing injury to left foot.

While using a utility knife to make a splice on a shuttle car, the utility knife slipped off of the cable and struck the employees left index finger causing a laceration.

Employee was struck in the left hand by a rock when it fell between two longwall shields.

Employee was attempting to turn a water valve on and felt a pain in neck / arm (left). Employee did not report incident until 8/12/2016. Was released to full duty. Then on 8/18/2016 received Dr. slip from family doctor to be off until 8/25/2016 and then released to light duty on 8/25/2016 while awaiting MRI and diagnosis.

A roof fall has occurred at #26 Break in the #3 Entry on the 9M section outby the dumping point.

A roof fall occurred at #42 - #43 crosscut on 8 Haulage Extension/#1 Belt

The main service hoist located at the 8 Haulage shaft had a trip lever in the governor broken. The part was replaced and the 8 Haulage hoist was back in operation at 10:08pm 06/25/2017.

Employee was walking down the #2 entry when ee stepped on a broken 3x3 ventilation timber and twisted right ankle and knee.

EMPLOYEE WAS UNLOADING FIBER CRIBS WHEN ONE OF THE FIBER CRIBS SLID AND SMASHED THE EMPLOYEE'S LEFT RING FINGER.

Employee was dismounting a 930 Loader when EE fell from the bottom step and landed on EE's back.

Employee was building a stopping & attempted to raise a cinder block above head and felt a pain in left shoulder.

Employee lifted a 5 gallon can of hydraulic oil off of the roof bolter to place on the mine floor and felt a pain in the lower right side of back.

Employee was struck on right hand by a piece of drill steel while lapping belt onto a flat car. Employee went to Dr. and no fractures were found and EE was released to work full duty. Employee went back to Dr. on 5/5/16 and a fracture was found and employee placed on restricted duty.

Employee was struck in the shoulder by a rock, this caused the employee to fall. The rock came to rest on the employee's left foot

Employee was laying out a belt move when EE stepped into a hole causing the employee to fall and strike EE's knee on a rock ledge. Employee was placed off of work on 3/21/2016

Employee was standing by tray of roof bolter when a rock fell out of the mine roof, struck the ventilation tubing, and then struck the employee. Employee was released and returned to work on 3/18/2016. Employee sought additional medical attention on 3/21/2016.

Employee was standing on crib block to reach top of double doors to plaster when the crib employee was standing on rolled out causing the employee to fall and scrape right knee.

EMPLOYEE WAS DRAGGING 100' OF TROLLEY WIRE WITH THE HELP OF THREE OTHER EMPLOYEES WHEN THE TROLLEY WIRE GOT HUNG CAUSING THE EMPLOYEE TO BE PULLED BACKWARDS AND FELT PAIN AND POP IN RIGHT KNEE.

Employees were spraying lime on ponds to settle materials and stopped to adjust linkage on motor used for spraying materials. Employee complained of the fumes from the exhaust and walked away. Another employee saw employee lying on ground and went to check on employee.

Employee was cleaning belt line using a rake when the rake came into contact with a moving belt and caused the rake to strike the employee in the right side of the employee's face. Initially the CT Scan showed no fractures and employee returned to work on employee's next regular shift & worked until On 2/22/2016 another CT Scan showed a fracture in employee's right jaw.

Employee was walking when EE stepped into a hole and felt pain in EE's right foot.

Employee was riding on top the #31 mantrip coming off of the 9L section at the end of the shift. The trolley pole came off of the wire, struck the mine roof, then broke into. One of the jagged edges of the broken pole struck the employee on the right hand between the middle and ring fingers causing a laceration that required 5 stitches.

Employee was shoveling loose material on the 8 West #2 belt at #37 crosscut. As he raised the shovel up to dump the material, he felt a pain in his lower back.

The 8 Haulage elevator was inoperable in excess of 30 minutes due to one of the wedges catching. No one was on the elevator during this time.

Switch on the bottom elevator doors was misaligned and would not operate. Switch was re-aligned and tighten down.

Injured was installing 6' torque tension bolts in #4 entry on 9M. When he pushed away and up on the roof bolt he felt a pop in his right shoulder. He was transported to Welch EMG where he was treated for pain in right shoulder.

Injured was tramming scope when he hit a hole in the bottom throwing him into the canopy of the scoop. Causing pain in his neck. He finished the shift and saw the PA on site.

The weekly examiner found an unintentional roof fall inby the 4 Right EP. This area is not travelled or examined. The fall did not effect any other travel or ventilation.

An unplanned roof fall occurred on the 9L section at the #28 break, 2x1 crosscut. The fall was approximately 95' long by 18' wide by 9' high. The fall was above anchorage but did not affect ventilation or passage.

Injured was pulling a rock 3.5""X15""x6"". As he was pulling the rock with a slate bar, another rock 1'x4'x2' on top of it fell sliding down the slate bar striking injured on top of right wrist. He was transported outside. He was treated on site by PA and received 5 stitches.

Injured was walking around the belt drive when he slipped in mud and water reaching out he caught himself against the motor shroud pushing it into the fan catching his hand. He was transported to Welch Emergency Hospital where he was referred to a specialist.

Injured was making a cable splice on the roof bolter when a rock fell from the 15' roof 8""x6""x2"" to 0"" striking him on the right shoulder driving the knife into of his right leg. He was transported to the hospital where he received 5 stitches in his right leg and contusions to right shoulder area.

Injured and witness were standing at the ch4 monitors, when the miner quit tramming. A slicken sided rock 8'x6'x3' fell on the boom and slid down the operators side breaking and striking injured on the top of his right foot. He was transported to the hospital. Treated and referred to a specialist.

Employee was working in an area that had cement mixture floating in the water. The employee had the water soak into his rain suit and rubber boots and the chemical reaction caused chemical burns to the skin.

At approximately 2:10am the weekly examiner found an unintentional roof fall in the 8 South #1 entry 1 crosscut out by the 8 South #1 seal.

Injured operating continuous miner when started cutting rock he felt sharp pain in left bicep and arm. He had a puncture wound to his left bicep. He was transported to the emergency room where he was referred to another doctor to have a piece of carbide removed from his left bicep.

The power supply failed on the 8 Haulage elevator causing the elevator to be inoperable for over 30 minutes.

Employee was traveling through a mandoor from the #2 to the #3 entry at the #35 crosscut on the 9L section. As he passed through the door, the door shut on his right hand causing a laceration that required stitches to his index finger.

Employee was operating continuous miner. While cutting coal and rock a piece of carbon broke off of the tip of one of the miner bits, flew back and struck the employee in the left forearm. The carbon became embedded in the employee's arm. The employee was standing behind the boom of the miner.

Injured was helping to hang a #6 AWG scoop charger cable from roof bolt plates at 9K Longwall # 46 break, #2 to #1 Break through in 7.5' high top when he felt pain in his left shoulder. He is not working but no communication on whether he has been to a doctor for a diagnosis.

Employee was operating continuous miner when a rock fell out of the top and struck him on the right hand.

Employee was dragging hose for application of rock dust to the area when the hose came loose from the coupling and the employee lost his balance falling onto the ground injuring his left shoulder.

Injured employee was a passenger on #25 bus and spurred into 9G to allow supply motors to pass, when the supply motor hit the 9G switch, the motor veered into 9G, striking the #25 bus and injuring a miner.

Employee was installing belt support in #1 entry when a rock fell out of the top and struck him on the right arm.

Employee was removing a miner bit from a continuous miner and got his right finger caught in-between the miner bit he was extracting and the tool he was using to perform the work.

Continuous miner cut into a degas hole that was in the top instead of in the coal seam. The sparks from the miner bits cutting the rock caused an ignition of a pocket of gas in the degas hole.

Employee was conducting a pre-shift examination and he went to lift a piece of metal from the top of the roof bolter, the metal fell onto his left foot.

While sitting on the roof bolter, eating his lunch, a piece of slate fell out of the top and struck him on the back of the neck.

Employee slipped on ice in parking lot while coming into work. Employee had not clocked into work.

Employee was scaling to top when the rock he was scaling struck the slate bar he was using, and the slate bar struck the employee in the right thigh and knee.

Miner was walking in #1 entry when he tripped over a Side Push Jack on the Longwall Tail Piece and felt pain in his ankle.

Employee was helping transport 5 gallons of oil by hand, when he felt a ""pop"" in his lower back, then informed his foreman of excessive discomfort.

The 8 haulage elevator, our secondary escape way, hover travel switch lock into place, causing the elevator to stop movement. The elevator was operational at 8:24pm

Employee was installing rib bolt, when a piece of his arm guard, required PPE for our operators, became entangled in the Roof Bolt Wrench and injured his right arm.

Employee was moving rock dust from the track to the belt entry, when she tripped over a cable in the entry and struck her right shin on the floor of the mine.

Employee was assisting in moving belt structure, when he slipped on uneven surface and felt pain in his right ankle.

The 8 Haulage elevator, our secondary escapeway, had a 1st floor door switch malfunction. The elevator was operational at 7:45 a.m.

Employee was moving miner cable when he stepped on the cable and twisted his right ankle as he fell to the ground.

While walking up an entry on the section, the employee slipped and twisted his right knee.

An unintentional rock fall occurred in the 8M Bleeders at #53 Crosscut. The fall was above anchorage, and additional floor to roof support has been installed. A request to the MSHA District 12 office has been made to move the Evaluation Points.

Employee was adjusting the forks on a diesel forklift when he caught his fingers in between the forks.

We had an unintentional roof fall above anchorage at the #72 crosscut on the 8 Haulage #2 beltline. The fall was approximately 50'L x 18'W x 8'H. The fall occurred in an intersection where there appeared to be transitional roof at or near the 8' height. The beltline in the #72 crosscut was covered up by the fall.

EE, along with another EE, were pulling slack on cable and completed task and moved on to putting wrench on 1"" pipe when pump broke free from pipe and spool struck EE on back shoulder & elbow on right side.

While attempting to open an airlock door, the door abruptly shut on his left finger. The employee received three stitches to his left middle finger and returned to work the next day. The employee was wearing mandatory gloves.

A mechanic felt pain in his back as he was maneuvering a tool box onto a mantrip. His first day of missed work was 8 September, 2014.

Employee was walking up the entry to retrieve a forklift to unload supplies, when he slipped on the steep grade and fell onto his back.

Employee was assisting other miners carry a 14' beam, when he felt a pain in his abdomen.

Employee tripped over belt material on the mine floor and fell, striking his right rib on a tool handle he was carrying on his miners belt.

Employee was adjusting belt rollers on a conveyor system, when he felt a 'pop' in his lower back.

Employee was removing a 5 horsepower pump from the mantrip, when he felt a pain in his right shoulder.

As the employee was exiting from the scoop's operators deck, he felt his left knee twist and buckle under him.

While lifting a belt splice cooker over a piece of elevated pipe, the employee felt a pain in his lower back and legs.

Employee was shoveling the rib, when he felt a pain in his lower back.

Employee was unloading crib blocks off a flat car when he slipped and fell, and felt pain in his lower back.

Employee was carrying rock dust bag when he struck his head on a crib block, causing pain in his neck.

We reported a roof fall on the White Oak Loaded side of the mines near the Weir. After further investigation we concluded that the fall was not above the anchorage, it is not in a traveled area, and does not interrupt any ventilation for the mines. This was an extenuation of the rock fall that occurred on 22 June 2014. The rock measured 18' X 20' X 5.

Employee was walking to feeder on the section, when he felt a pop in his left knee.

While attempting to tighten a fire valve, the employee incidentally activated the cut off valve causing a surge in pressure and the fire struck him in the right hand.

While attempting to place a jack under a de-railed mantrip, the employee heard a pop in his right shoulder.

Employee was moving belt structure at the section feeder, when he said he felt a strain in the middle of his back. The employee did not fill out an incident report with his supervisor before he left the property.

Employee was operating a scoop with an interlocking double door. The safety pin was not installed correctly into the hasp and the doors opened while the scoop was in motion and struck the employee in the lower back.

Employee was transporting supplies to section on a motor trip, when he struck his head in low top and felt pain in his neck.

Employee was cleaning rib to advance the line curtain and injured his back while attempted to move a large rock 50' long X 2' wide X 10' thick.

Employee was shoveling rib after Pinner trammed out and the rib rolled out onto his left leg.

The injured employee and a helper were making a belt splice when the helper struck the injured employees index finger on the right hand with a hammer.

Employee was walking when he slipped and struck his left knee on the ground.

Employee was lifting the lid on a mechanics tool box, when he felt a pain in his lower back.

Employee was moving belt structure, when he felt a pain in his right arm around his elbow.

The 9I Longwall face had a floor bleeder liberating methane in excess of 5%. The operator removed all miners with the exception of those addressing the liberation issue.

Injured was carrying a back of rockdust when he stepped off the mantrip when he felt a pop in his left knee. He reported to supervisor of no pain and finished the shift.

he 8 haulage elevator, our secondary escapeway, had one of the four auxiliary breaking systems lock into place, causing the elevator to stop movement. The elevator was operational at 5:00pm

EE was helping to prepare the work site to begin daily grout work. The crew had to pull tow 500' joints of polyline three crosscut, appx. 525'. EE stated he felt a pull in his lower back while pulling the first joint. There were 5 men spread out on the poly line, EE being the third in line. The size of the poly line was 1 1/4"". EE did not report this accident till 3/14/2014

The 8 haulage elevator, our secondary escapeway, had the rectifier for the brake coil short out. The elevator was operational at 11:17pm

Employee was leaning forward to perform work on a pipe, when he lost his footing and felt pain in his right knee.

Employee was shoveling gravel from a flat car, and felt pain in his right shoulder.

Employee tripped over banding material at 8 A fan and lost her balance. As she attempted to catch herself during the fall, she felt pain in her right shoulder as she extended it to help break the fall.

Employee was loading 12' track rails onto a flat car for transportation when he caught his right hands, middle finger between the rail he was loading and the flat car frame.

Employee was crossing an overcast and lost his footing and fell on his left ankle.

Employee was using a wheelbarrow to spread rock dust, and felt pain in his ankle while walking in entry.

While installing an outside bolt on a section, a rock fell out of the top and struck the employee on the left arm.

Employee was unloading supplies when his right hand was caught between the frame of the scoop and the blade.

Employee was re-attaching the fueling handle on the fuel pump, when his left hand was caught between the pump and the refueling handle.

Employees were removing a shaft from a Scoop gear box, and using a hammer to assist in relieving pressure due to the shaft being in a bind. While striking the shaft with a hammer, a piece of spline struck the injured employee in the left forearm.

A roof fall occurred on White oak empty side track at breaks #121 to #122. This fall is 30,035 feet from our nearest working face and does not interfere with our air course.

EE was closing gate and caught his left hand in locking part of gate. Doctor's applied 3 stitches.

Employee was on his knees, working on the track, when the employee behind the injured employee was using a slate bar and the slate bar slipped and struck the injured employee in the back of the head. The employee reported to work as regularly scheduled on 10/8/2013. On 10/14/2013 the employee was placed on restricted duty and was released to full duty on 10/23/2013.

While installing ventilation tubing on the section, the tubing slipped out of his grasp and struck him in the head and neck.

Walking down stairs, mine was spraying water, slipped and fell on stairs.

Employee was traveling as a passenger in track ride facing the direction of travel on West Mains track. At #142 break the employee struck a roof bolt causing the employee's head and neck to recoil. The employee was transported to the local hospital.

Employee was filling a pumpable roof support crib while standing on the first step of a 3 foot step ladder. When the employee finished filling the pumpable roof support crib he stepped from the first step of the ladder to the ground and hyper extended his left knee.

While installing rub rail on a CM, the employee felt a pain in his lower back.

Employee was loading equipment onto a man trip and felt a pain in his lower back. Employee had initially had an issue with his back on 23 June, 2013 but missed work on 9 July due to apparent back issue.

Employee fell from same level and felt pain in his lower back.

A weekly examiner identified a roof fall on 8 South, #3 entry at #13 crosscut. This roof fall measured approximately 10' X 10' with a distance of 16,189' from the nearest working face of the mines. This area is not normally traveled by our employees and does not interfere with air or travel ways.

Employee was installing 12' cable bolts when he felt a pain in his right shoulder.

The door switch for the 8 Haulage elevator shaft, our secondary escapeway, malfunctioned.

Employee was placing the trolley pole back onto the trolley wire and received an electrical burn. The effected employee is back to work.

The drive card for the motor for the 8 Haulage elevator, our secondary escape way, shorted out and was replaced.

The leveling switch on our 8 Haulage elevator shaft, which is our secondary escape way, malfunctioned.

The 8 Haulage elevator, our secondary escapeway, had a leveling switch malfunction.

Employee was on top of mantrip and was struck in the side of the head with the trolley pole.
