The rope gripper wear pads on Metz elevator wore down causing a pad wear microswitch to engage that causes the rope grippers to set, resulting in an entrapment of 2 miners approximately 350' down the shaft. The manual rope gripper release pump failed. The miners were removed from the elevator at 5:45 pm. No problems with equipment or compliance issues, training or PPE.

Struck by falling object

Injured was cutting down roof screen to unload ""bagged"" material at 3 block on 3 belt with a grinder and some loose rock that was in the screen fell out causing a laceration to the posterior side of EE's left forearm that required sutures to close. No problems with equipment or compliance issues, training or PPE.

While mining inby crosscut 23 of the #1 entry in 8 West injured was standing by rib bolting controls when a rock fell out from in between the roof straps and struck them in the back causing a closed fracture of spinous process of thoracic vertebra. No problems with equipment or compliance issues, training or PPE.

Struck by flying object

Using FCT to pull cable slack with loops of cable tied to a drill steel placed on the FCT while in the 8 West belt entry. Injured was holding drill steel to keep the rope from pulling off, but the rope pulled off anyways and the drill steel came back and struck them in upper left leg resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Inhalation of radiations, caustics, toxic and noxious substances

The mine received a Self-Insured Employer Decision on 3/16/26 from a medical report dated 12/18/25. The report identifies a 10% permanent partial disability possibly from occupational pneumoconiosis. Specific date and time of actual disease contraction is unknown.

Struck by flying object

Injured was striking a panline rack bar to get the panline to line up together on the 7W Longwall setup face when a piece of their pick hammer broke off and struck them in the chest, embedding itself in the soft tissue under the left pectoral. No problems with equipment or compliance issues, training or PPE.

Injured was using 28 center bolter in St Leo Mains 3-2 at 148 block. They had a drill steel laying on the tray above the drill controls and it was struck by the boom, flipping up and hitting them in chest resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable in the old 11 belt entry, which is now common with 9 South track, at 82 1/2 block. Fall was approx. 10' high, 16' wide and 30' long. The fall will not be cleaned up or re-supported. No problems with equipment or compliance issues, training or PPE.

The mine received a Self-Insured Employer Decision on 3/5/26 from a medical report dated 10/17/25. The report identifies a 12.55% permanent partial disability (hearing loss) possibly from occupational noise exposure. Employee has been retired since 4/16/25.

Struck against stationary object

Injured stopped EE's motor outby the 9 North 27 block switch underneath a 6"" aqualine (water discharge line). When EE stood up, EE struck EE's head on the airline causing pain in EE's lower neck (previously injured 4/28/25). No problems with equipment or compliance issues, training or PPE.

Injured was attempting to dismount Volvo rock truck with wet muddy boots and EE's foot slipped while climbing down steps causing an unexpected load on EE's left shoulder resulting in a sprain/strain type injury. Injured started missing work on 3/13/26 and was put off work pending an MRI. No problems with equipment or compliance issues, training or PPE.

Injured loading top belt structure into a belt tub in the Sugar Run Supply yard with the rest of belt crew. When EE placed the structure down the other employee pushed it over before injured's gloved finger was out of way and pinched it between structure and tub resulting in a chipped bone. No problems with equipment or compliance issues, training or PPE.

Injured holding up airline and cables at 97 blk 5 belt while another employee was using grinder to cut of belt structure nuts when the grinder caught and kicked striking EE's left index finger and causing a laceration that required 5 sutures. No problems with equipment or compliance issues, training or PPE.

Fall to the walkway or working surface

Injured was carrying tools past 2 parked mantrips in 8W belt spur. While walking between mantrips EE stepped on an uneven surface causing EE to fall. Injured started missing work on 2-18-26. No problems with equipment or compliance issues, training or PPE.

Fall onto or against objects

Injured was walking away after throwing switch at Sugar Run slope back switch when EE slipped on the rail, falling onto the point rail. Injured started missing work on 2-19-26. No problems with equipment or compliance issues, training or PPE.

Injured was rib pinning in sump at St Leo Mains at 129 block, while walking around the bolter, EE stepped in a hole and twisted EE's left knee. Injured started missing work on 1/27/2026. no problems with equipment or compliance issues, training or PPE.

Caught in, under or between a moving and a stationary object

Employee was on crew changing rollers, and EE's hand was pinned between a roller and a locked out belt while replacing a roller in a cradle. Accident was not the result of a training or compliance issue.

Over-exertion in lifting objects

Injured picked up approximately 15-pound rock off of catwalk to the Sugar Run Metso Pump and felt pain in lower right abdomen. Initially treated for appendicitis, but no evidence of this was found at follow up appointment, so employee was diagnosed with an unspecified abdominal muscle strain. No problems with equipment or compliance issues, training or PPE.

Injured installing rib bolt while cutting turn 3-2 at 115 block 9 North when a piece of the rock brow fell out and struck EE's left shin resulting in a laceration that required 9 sutures to close. No problems with equipment or compliance issues, training or PPE.

Struck by flying object

Employee pulling a power car in 7 West 3 left 3 block intersection by hooking a chain up between the power car and a scoop. They had scoop back up to take slack out of the chain and it failed immediately, throwing a link that struck them in the forehead resulting in 3 lacerations that required 6 sutures total to close. No problems with equipment issues, training or PPE.

Accident type, without injuries

At approximately 8:03am the bracket holding the top counter weight wheel failed and a piece of the bracket fell and damaged a sensor on the bottom of the sugar run elevator causing 3 miners to become entrapped on the elevator. The miners were safely removed from the elevator at 9:33am. No problems with equipment or compliance issues, training or PPE.

Over-exertion in lifting objects

Injured employee was picking up a slope chain and placing it on the slope car when they felt a pain in their back. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Fall from ladders

Injured was hanging a run through curtain up in #2 entry of St Leo Mains 131-132 block, standing on the third rung of a 4' ladder leaning off the ladder to install a nail through the curtain when EE fell off the ladder and bruised EE's back when landing on the mine floor. No problems with equipment or compliance issues, training or PPE.

Caught in, under or between a moving and a stationary object

Injured picked up a lump of coal off the mine floor near 150 shield (6W) to throw into pan, but EE was closer to the pan than EE thought and struck EE's hand on the edge of the pan, pinching right ring finger between pan and coal, resulting in a fracture and open wound. No problems with equipment or compliance issues, training or PPE.

While guiding roof bolt sections (#6 bolter Sugar Run Rehab) together (with bolt head in chuck) the drill head rose slightly when the other drill removed up pressure causing their left ring finger to get pinched between bolt sections resulting in an open wound and fracture to distal phalange. No problems with equipment or compliance issues, training or PPE

While walking near the mantrip station in St Leo Mains the injured felt pain in EE's right knee (did not trip/slip/misstep, no walkway obstruction). Reported injury and started missing work on 12/4/25. No problems with equipment or compliance issues, training or PPE.

D35 traveling toward Metz with clearance and collided with D84 (traveling towards St Leo without clearance) at 0 block 9 South causing injured to be thrown into the cab of the mantrip passenger compartment, resulting in 3 fractured ribs and 2 fractured vertebrae. No problem with equipment issues, training or PPE.

Struck against a moving object

D35 traveling toward Metz with clearance and collided with D84 (traveling towards St Leo without clearance) at 0 block 9 South causing injured to be thrown into the cab of the mantrip passenger compartment, resulting in bruised ribs. No problem with equipment issues, training or PPE.

Caught in, under or between a moving and a stationary object

Injured was uncoupling a supply car incorrectly at the slope chute track and got EE's finger caught between the release lever and the coupler shank causing a crushing injury to EE's left middle finger that resulted in an open wound which required sutures to close.

At approximately 2:39 pm the governor rope for came off of the tail sheave and lodged in the governor wheel of the Sugar Run elevator causing one miner to become entrapped on the elevator approximately 20' from the bottom of the shaft. The miner was safely removed from the elevator at 4:12 pm. No problems with equipment or compliance issues, training or PPE

While removing fire suppression from the scoop charger during a power move in St Leo Mains (121 blk 2-3) the clamp slipped and when EE went to catch it EE ran EE's gloved hand down the edge of a roof strap resulting in a laceration that required sutures. No problems with equipment or compliance issues, training or PPE.

Over-exertion in pulling or pushing objects

Injured was rolling a shuttle car tire from the scoop bucket to the side of the shuttle car to install the tire and felt pain in EE's right shoulder (7 West 1 left entry, 0 to 1 block). No problems with equipment or compliance issues, training or PPE.

Struck by falling object

While drilling hole for cable bolt in St Leo #3 126-128 block the steel got stuck in top, miscounted how many steels were in roof (removed 2 of 3 steels) and last steel fell out striking back of left hand causing open wound that required 11 sutures to close. No problems with equipment or compliance issues, training or PPE.

Injured was changing winch rope on duckbill scoop in 7W (75 blk 1-2 crosscut). They had their left hand resting on the mast of the scoop and threw the winch hook onto their own left thumb, causing an avulsion to the thumb nail. No problems with equipment or compliance issues, training or PPE.

Injured removed 70"" roller and set it along the rib near the St Leo mantrip station, when the roller fell over and struck their right middle finger pinching it between the roller and a flat car resulting in an open wound that required 5 sutures. No problems with equipment or compliance issues, training or PPE.

DC drive failed on the Metz elevator which interfered with its use for more than 30 minutes. No miners were on the elevator at the time of the fault. Drive replaced and elevator returned to service by 12:15 pm. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable on 3 belt just outby the 3 block intersection at approximately 6:46 am on 10/31/2025. Fall occurred in an adjacent brow left from a previous roof fall that occurred 8/2/2020. Fall was approximately 8' long, 16' wide, and 6' high. Area was mined near December of 1988. No problems with equipment or compliance issues, training or PPE.

10 miners entrapped on Metz Elevator until 8:38 pm when a VFD failed on the elevator. The miners were exiting the mine and were still approximately 700' down the shaft. No problems with equipment or compliance issues, training or PPE.

Lowering empty trash car down Sugar Run Slope that de-railed around the 1492 mark, allowing rope slack to build up and coil behind the brake car. When tension was re-applied the rope caught on itself resulting in a kink in the rope approx. 700' away from the brake car. Hoist taken out of service until rope replaced. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable at 21 block 3 belt. Fall approximately 14' wide, 14' long and 5' high. 21 block overcast was damaged and compromised from the roof fall. No problems with equipment or compliance issues, training or ppe.

Caught in, under or between a moving and a stationary object

Injured operating fletcher center bolter in motor barn rehab area between #3 and #4 seal installing a bolt in a bacon strap and had left hand resting on drill mast. As mast went up it pinched the tip of EE's middle finger in the mast resulting in a crushing injury that required 6 sutures to close the wound. No problems with equipment or compliance issues, training or PPE.

While applying a chain around a recently coupled supply car to a motor when the motor on the other side of the trip tried to uncouple, but the couplers got stuck and ended up pulling the supply cars and motor a short distance causing a strain to their shoulder and upper back. Started missing work 9/20/25. No problems with equipment or compliance issues, training or PPE.

The Miracle Run elevator faulted around 3:40 pm, faults reset and then tested when it faulted again with slightly different results. Through investigation, in cooperation with Schindler elevator service it was eventually discovered that the bottom safety shoe had turned and resulted in broken pins. Injured standing in first crosscut outby 8 west equipment doors near the end of loose take up rails while a scoop was unloading a double stack of take up sections on top of the loose rails. Duckbill drug across a loose rail, kicking it up when it got to the end of the rail striking injured, knocking EE down and landing on EE resulting in a fractured vertebrate.

Injured was bolting off side of miner in St Leo 113 blk 3-2 with new section of roof mesh, not bolted to top yet, on the miner when a piece of coal (3'x3'x5"") dislodged, slid down the screen, bounced off the rib and struck EE, resulting in a fracture to the L1 vertebrae. No problems with equipment or compliance issues, training or PPE.

Module on the back of the touch screen/display went bad which blew a control fuse and caused rope grippers to engage, entrapping 1 miner on the Sugar Run Elevator until 9:25 pm. No problems with compliance issues, training or PPE.

Injured was cutting a zip tie off an oil soaked hydraulic hose in the Metz Motor Barn washdown bay when EE's left hand holding the hose slipped off and EE struck EE's left index finger on the razor knife blade resulting in a laceration that required 4 sutures to close. No problems with equipment or compliance issues, training or PPE.

Injured bent over to pick up a powder bag with 4 16 oz. bottles of water in it and felt pain in EE's lower back from a muscle strain/sprain. No problems with equipment or compliance issues, training or PPE.

Injured operating 15 man personnel carrier down 9 South haulage between 90 and 120 block. Foot came off deadman switch, causing vehicle to go into neutral, injured looked down to switch vehicle back into direction and struck EE's hard hat off the mine roof. Unplanned roof fall discovered reportable on 4 belt at 40 block at approximately 12:30 am on 08/24/25. Fall primarily occurred on the offside walkway at the intersection. Fall was approximately 20' long, 6' wide and 4 1/2' high. Area was mined near December of 1967. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable on 8 belt at the 26 block intersection. Fall approximately 35' long, 30' wide and 9' high, area mined near June 1978. No problems with equipment or compliance issues, training or PPE.

Fall from scaffolds, walkways, platforms

Having trouble fixing an air tugger, EE was on the platform and coworker just looked at them and had eye contact. Coworker turned to keep working with third person and they heard a noise and EE was on the ground. No one knows exactly what happened. The assume that the safety catch for the drill steel came out.

Unplanned roof fall discovered reportable between #3 and #4 seal of 1 South Seal set (approx. 20' long, 16' wide, 6' high). No injuries, entrapments, and ventilation not disrupted. All seals along seal set can be examined. Area mined January of 1990. No problems with equipment or compliance issues, training or PPE.

Injured was picking up a rock that was caught in the grizzly bars of the loading machine at 42 block for the Harvey rehab project when EE felt pain in EE's left arm/shoulder and neck. Started missing work 08/20/25. No problems with equipment or compliance issues, training or PPE. Previous history with neck/shoulder/arm pain.

Injured was roof bolting on the 36 1/2 block main west roof fall with an air drill when some head coal around an adjacent roof bolt fell out and allowed a bowling ball sized rock to slide out and strike them in the upper back, resulting in a possible muscle strain/sprain (no bruising, x-ray and CT scan negative). No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable on Main West Haulage at 36 1/2 block at approximately 8:45 pm. Fall approximately 20' long, 16' wide and 10' high. Area mined near July of 1967. No problems with equipment or compliance issues, training or PPE.

Building block wall between track and return entries between #3 and #4 seals of motor barn seal set and felt pain in upper chest area while lifting a block above EE's head and twisting. No problems with equipment or compliance issues, training or PPE.

While trying to loosen a stuck dust hose fitting with a pair of channel locks on the twin boom bolter in St Leo Mains at 105 block they felt pain in lower back. No problems with equipment or compliance issues, training or PPE.

Injured was de-trashing the 7W section in the #2 entry at 56 block and while attempting to pull a piece of curtain out that was stuck in the mud EE felt pain in EE's lower back. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable on 1 North Haulage at 14 block at approximately 5:37 am. Fall primarily occurred in an adjacent crosscut and came out into the track haulage entry, continuing inby. Fall was approximately 25' long, 16' wide and 5' high. Unplanned roof fall discovered reportable at 72 block intersection of 10 belt. Fall occurred primarily across the southern portion of a 6 way intersection. Fall was approximately 16' long, 12' wide and 5' high. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable midway in between #1 and #2 seal of the Motor Barn Seal Set air course. Fall approximately 50' long, 16' wide, and 6' high. Both sides of the fall can be examined, and all seals along set are travelable. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable on 3 belt at 34 block. Fall primarily occurred in an adjacent crosscut and came over to the off side of the belt walkway. Fall was approximately 22' long, 8' wide, and 8' high. No problems with equipment or compliance issues, training or PPE.

Loading bags of rock dust onto FCT dust rack between 54 and 55 block #1 entry and felt pain in EE's lower back and burning in left side of lower back. Was not scheduled for work 7/22/25 due to roof fall on 3 belt. Diagnosed with muscle strain. No problems with equipment or compliance issues, training or PPE.

While walking around a mantrip at the mantrip station of St. Leo Mains EE struck EE's lower right leg on the coupler, resulting in a fracture to the tibia. No problems with equipment or compliance issues, training or PPE.

On Miracle Run bottom (3 track) EE was attempting to adjust brakes on a battery motor and felt pain in the left side of EE's chest while attempting to loosen a bolt with channel locks, resulting in an acute chest wall strain. No problems with equipment or compliance issues, training or PPE.

Caught in, under or between a moving and a stationary object

Disassembling equipment doors at 73 blk 2-3 6W that were laying on scoop bucket. While removing a support leg the long beam shifted and pinched their left ring fingertip between bucket and beam resulting in a fracture to the distal phalange and small open wound that required 2 sutures to close. No problems with equipment or compliance issues, training or PPE.

During an examination of the Metz Elevator it was found that the bottom 60 feet of the counterweight rails had deformed from heat expansion. The elevator was immediately taken out of service until the counterweight rails could be replaced, before any damage was done to the counterweight or the ropes. While trying to uncouple an empty flat car that was off track from motor, they had the other motor pull on the car but it did not uncouple and shifted causing the coupler to swing and pinch left calf between the coupler and the motor resulting in a contusion. 64 block 9 north track. No problem with equipment issues, training or PPE.

At 37 1/4 block 4 belt an unplanned rib roll/brow fall occurred approximately 12' long, 5' wide and 10' high that damaged some belt structure, preventing the belt from running for more than 1/2 an hour. The material that fell was the rib/brow left from a previous roof cavity. No problems with equipment or compliance issues, training, or PPE.

While cleaning rock from the walkway at 3 drive EE was lifting an approximately 50 pound rock onto the belt, struck their shoulder on the waterline and felt pain in their groin from a muscle strain. Sought medical attention and started missing work on 7/7/25. No problems with equipment or compliance issues, training, or PPE.

Fall to the walkway or working surface

While making preshift examination on 6 West belt, just outby the middrive, the examiner miss stepped on left foot, stepping on its side and rolling the ankle, resulting in a fracture to the left fibula. While falling they struck left elbow on belt structure resulting in a chipped bone. No problems with equipment or compliance issues, training, or PPE.

In St Leo Mains walking from loader to miner after getting a water bottle at #1-#0 crosscut 109 block the rib ""popped"" dislodging a softball sized projectile that struck them in the left side of the face resulting in contusions to left eyebrow and nose. No problems with equipment or compliance issues, training, or PPE.

While using a pry bar installing new track ties near 106 block 9 South they felt a tear/pop/pain in the soft tissue of upper left thigh, possible muscle strain. X-rays negative, referred to specialist for possible MRI. No problems with equipment or compliance issues, training or PPE.

Passing T channel across miner in 7 West (face of #3) and slipped off step on back of miner hitting left arm on rib strap resulting in a laceration that took several sutures to close. No problems with equipment or compliance issues, training or PPE.

Injured was assisting panline bolter operator near headgate of 5W longwall recovery face, attempting to install a bolt wrench in the bolter head when their right foot slipped and went under bolter head while it was lowering, pinching their foot between bolter head and mine floor, resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

While installing a roof bolt off of the miner in St Leo Mains (3-2 107 block) the injured miner cut EE's left thumb (distal joint, anterior side) picking up a short T channel roof strap. EE was wearing gloves at the time. Received 4 sutures, given days off work because cut was on joint. No problems with equipment or compliance issues, training or PPE.

Over-exertion in pulling or pushing objects

Injured taking a K wall panel out of an O-Belly (trash car) located at 100 block St Leo, that was stuck so they pulled harder and felt pain in shoulder. Continued to work until 9/25/25 when they started missing work pending surgical repair of a tear. No problems with equipment or compliance issues, training or PPE.

Injured was walking through personnel door at 83 block St Leo (track to belt) and shut EE's left hand in the door resulting in an open fracture of the distal phalange to EE's left middle finger and minor lacerations to index. Was wearing gloves. No problems with equipment or compliance issues, training, or PPE.

Injured installing rib bolt with 3' metal rib strap off right side of miner 41 block 1-2 crosscut 7W when the strap rotated from vertical towards horizontal and while trying to catch it, it struck EE's right wrist resulting in a laceration. Bolt was not rotating at time. No problem with equipment or compliance issues, training or PPE.

Gas inundation occurred while cutting through SC-6 CBM well on 7 West section #2 entry near 41 block. Horizontal well bore was larger than anticipated and the mine had to acquire larger packers to plug the well. Methane levels were over 1.5% for more than 15 minutes after ventilation was established. Unplanned roof fall discovered reportable 23 block intersection Main West Haulage. Fall approximately 8' high, 15' long and 16'wide. Separation appears to be mostly along clay intrusion lines, area mined in 1967. No problems with equipment or compliance issues, training or PPE.

Employee strained left hip installing a top roller into structure on a 7W belt advance on 5/5/25, did not report injury until 5/8/25 when they went to seek medical attention and missed one day of work for a muscle strain. No problems with equipment or compliance issues, training or PPE.

Injured was hooking a chain onto a roll of belt inby 10 belt take up to be moved by the belt winder when another employee accidentally hit the lever of the belt winder causing the chain to tighten and catch the injured right pinky between the chain resulting in a distal tuft fracture. No problems with equipment, compliance issues, training or PPE.

Major wind storm in area caused a temporary power interruption while the Sugar Run Elevator was in motion causing an unintended motion fault which engaged the rope grippers and drum brakes, entrapping a miner until the faults could be reset. IE lifted the boom of a microtraxx machine. While IE reached in with right arm to check the boom pin, IE leaned on the remote box with chest, engaged the boom down function and lowered the boom onto IE's right arm. IE was trained as a microtraxx operator, but this was a compliance issue, as the machine was not de-energized, locked, tagged, or blocked against motion.

While operating golf cart going into 60# off 1 North haulage the injured struck hard hat off a wooden roof plank, causing pain in neck, upper back and shoulders. No problems with equipment or compliance issues, training or PPE.

Injured passenger in mantrip that collided with parked diesel dusters at 47 block St Leo. Some material fell out of the top and got into the mantrip operators eyes and when EE got their vision clear, EE was too close to dusters to stop in time. Injured broke right knee during collision. Injured felt pain in right shoulder while sliding slider tube forward (face of #2 in 6W) and then felt pain in upper back around shoulder when lifting new tube up into place. While installing a bacon strap, Harvey Run Rehab area at 42 block, to secure screen, and pushing strap into place when the strap got caught in the screen stopping the strap, causing EE's hand to slide along the edge, resulting in a laceration to EE's right index finger requiring 2 sutures. The mine lost power to the elevator while it was going into the mine empty, which caused the rope grippers to set. The rope grippers would not reset due to a sensor fault that was caused by the grippers dropping too far. They were manually pumped off and then a shim was installed to keep them from dropping too far to clear the fault. Returned to service 3:30 pm on 9/21/24.

Miner starting turn into 54 block 1-2 crosscut and had mined enough to install first roof strap. Injured climbed up on the rear bumper and reached over the conveyor tail boom to retrieve roof strap and got pinched between the tail swing jack and the roof bolt tray on the frame of the miner, resulting in two fractures to their pelvis.

Roof fall above anchorage discovered reportable during weekly examination of Motor Barn Seals. Fall located between #3 and #4 Seal and was 16' wide, 16' long and 7' high. Ventilation was not affected to the seals, but travel way is affected. Area will be supported and provided with a travelway.

While driving mantrip down Main West Haulage a small amount of roof material dislodged, with some of the material getting behind EE's glasses into EE's left eye. While walking in the Miracle Run Loaded Track injured tripped and fell, striking left side of ribs against mine rib, causing a contusion. Injured employee stepped towards a stationary 650 scoop to unhook the winch hook and hit lower left leg on the scoop bucket causing a laceration, that later got infected and started missing work on 7/30/24 to treat the infection. The EE was helping unload double stacked belt sleds from a duck bill scoop. The winch hook became fouled, and wouldn't allow the sleds to slide of off the bill. The injured struck the hook with a sledge hammer, as EE did this the sled dropped. The sleds pinched the EE's left foot between the bill and sleds. Began missing 5-16-24.

Struck against stationary object

The injured was bolting on a dual boom bolter putting in 16' cable bolts in 9 North Mains. The EE was on the last bolt, when the glue broke in the hole. The EE pulled it out and threw it towards the rib. The injured struck arm on the ATRS, causing a fracture. Began missing work 5/2/24.

Fall from ladders

The injured was hanging belt structure using a step ladder in 7W Belt Trench. A scoop in an adjacent entry caught a cable running through the trench. The cable pulled contacting the ladder knocking the EE to the ground. EE began missing 5/1/24.

The injured and another EE were loading straps on the miner. The injured put the inby side of the strap up on the miner to be slid forward. As EE turned, two straps slid off striking EE in the right leg. The EE finished shift and began missing work 2-29-24.

The EE was setting a post on the tight side of St. Leo Haulage at 12 blk. An approaching ride thought the injured was in clear. The mantrip coasted past the area, and the injured was contacted by the mantrip. The injured finished the shift, but began missing 2/21/24.

An unplanned roof fall was discovered at 1 North Left Return at 25 block. The area is approximately 36' long x 16' wide x 6' high. No one was injured during the fall, travel way is obstructed.

Struck by flying object

The EE was assisting the changing of inserts in the CM head. When one of the inserts became stuck, the EE struck it with a hammer. A piece of the insert broke off and struck EE's left bicep. EE worked until 1/2/24, when EE was put off.

Struck by falling object

The EE was bolting on the miner in the 9 North Mains Section. As EE was inserting glue in the hole for a bolt, a piece of roof material fell striking EE in the lower back and right lower leg. The injured finished the shift, but began missing work the next shift 12/12/23.

Fall onto or against objects

The EE was climbing down off of D-7 dozer. The injured slipped on the step at the blade arm and fell. EE's left ear struck the dozer cat, causing a laceration. EE received 4 stitches, and did not miss any scheduled shifts.

The EE was rib bolting in 9 North Mains on the operator's side. A piece of roof material fell out and rolled off the bolter rock deflector and contacted the injured EE's left foot and leg. EE sustained hairline fractures in EE's foot, and began missing work 11/9/23.

Employee was in the x-cut 2-3 at 42blk. when the rib popped. Employee turned to leave the x-cut and air line came down striking the employee in the nose. The employee sustained fracture to nose and began missing work on the 11-7-2023.

The injured was carrying monorail outby on 4W belt line. As EE sat down a piece, EE's left middle finger was between the monorail and bed rail of the belt; smashing EE's finger. EE received 10 stitches and fractured the end of their finger.

The injured EE was operating the tail motor of a supply trip heading back to Sugar Run. The EE had head down in a lower, and did not hear partner telling EE they were going to stop. The injured couldn't stop in time, and contacted an empty flat. The EE sustained a chipped tooth.

The injured EE was tubing on the 5W Section. The injured was standing back by the FCT, when a piece of head coal and rock fell from the roof and struck the EE on top of the head. The piece was approximately 2'x2'x5"" thick. The EE briefly lost consciousness. EE was treated and released from the hospital on 9/11/23. Began missing 9/12/23.

Contact with hot objects or substances

The EE was operating the thermal dryer from 12 AM to 12 PM WW shift. The EE lost the large fan in the drying chamber and went to turn on the sprays. As the EE entered the Roll Feed Room, flames from an access door contacted EE. EE sustained burns on 50% of their body.

The EE was in 9 North Mains Section, bolting in the ladders on the left side. The injured was installing 16' cable bolts. The hole was drilled, and the starter steel became stuck. The EE reached for another steel, and when EE turned around the starter steel fell from the hole and struck EE in the head. EE received 3 staples in the head, but didn't miss any work.

At approximately 3:50 PM the Sugar Run Elevator went down. The elevator was being sent down to the mine bottom when this occurred. Shortly after elevator was returned to the surface. After an examination it was determined that a lower limit switch was the cause. No person were trapped and elevator returned to service at 6:08 PM.

An unplanned inundation of methane occurred when the 5W SC2 West Leg was intersected in the face of #1. No more than 3.0% was found during a legal gas check. After interviews, some miners stated their detector went over range. No injuries occurred as a result of this incident.

EE did not finish the shift and went to seek treatment. The injured was prescribed eye drops. The EE didn't miss any time.

Struck by falling object

The injured was standing behind the miner as it backed out of the face on the 5W Section. The rock binder fell out from the rib, and struck the EE in the left arm and wrist. The EE finished their shift, but began missing work on 7/24/23.

The injured EE was using a grinder to cut a piece of pipe on the 9th floor of the plant. The wheel got bound and swung back and struck them in the hand. The EE was wearing gloves, and the grinder had a guard. The EE did not miss any scheduled shifts.

The injured EE was bolting on the miner in the 5W Section. A piece of the mine roof fell between the rock deflector and rib door striking the EE in the lower back. EE sustained only bruising, but began missing work 7/18/2023.

The EE was using a razor knife to remove tape from a drill steel. The injured did not close the blade, and their left pinky and middle finger contacted the knife. The lacerations required 6 stitches. The EE was wearing all appropriate PPE at the time of the accident, and did not miss any work.

Injured was sheaving an old motor from the crosscut near Sugar Run Bottom when the sheave wheel broke and a piece of the sheave wheel struck the injured in the left forearm causing a contusion. No problems with equipment or compliance issues, training or PPE.

Walking down 8 Belt and tripped, landing on left shoulder resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Employee was exiting mantrip at end of shift and twisted/sprained right knee. Employee was not able to put weight on right knee.

Injured was installing a roof bolt in #2 entry of the 5W CM section when a chunk of rock binder fell out and hit EE's left foot causing a contusion. No problems with equipment or compliance issues, training or PPE.

The EE was making a cable splice on the FCT, when the blade slipped causing EE to cut left knee with the utility knife. The EE was wearing all PPE. Began missing work 5-18-23, Returned 5-22-23

The EE was carrying Cement blocks, when EE tripped over the plastic banding that secures the bundle of blocks. The EE caught self before falling to the ground and felt a pain in lower back. The EE was wearing PPE. Started missing work 5-18-23, Returned 5-23-23.

The EE was in the process of securing a belt scrapper with a yellow rope hanger. The hanger caught something on the EE pulling their right arm into the belt roller. The EE was wearing the appropriate PPE. The EE began missing on 5/16/23.

The EE was operating the miner on the 5W section, and was retrieving a bolt from the bolt tray located on the miner when a piece of slate/rock fell from the mine roof striking the EE right index finger. The EE was wearing gloves at the time of the incident.

Injured was replacing rear end of a scoop on the 5W miner section. When EE went to stand up from being prone EE felt a pop and pain in lower back. No problems with equipment or compliance issues, training, or PPE.

Caught in, under or between a moving and a stationary object

While pulling rope slack back from a motor their glove got caught in the rope and pulled their hand partially into a sheeve wheel causing a contusion and abrasion to their left ring finger. No problems with equipment or compliance issues, training or PPE.

Struck by falling object

The EE was walking around the curtain of #6 seal of the 3 North Rehab, when a rock fell from the mine roof and struck the EE's left shoulder. The injured finished the shift, and began missing work 5/3/23.

Struck against stationary object

The EE was walking down 3 ML Belt at 3 block. EE stepped on a loose rock felt a pop in their knee. EE finished the shift, and began missing work 4/27/23.

Over-exertion in lifting objects

EE was picking up an air bolter from 27 block Main West Haulage. The injured lifted the drill and twisted. Felt discomfort in lower back. EE began missing worked on 4/25/23.

Struck by falling object

The EE was helping load supplies on the miner. While on the right side of the machine, laid their hand on the rib door. A piece of material fell from the mine roof and struck EE's right middle finger causing a fracture and laceration. The EE was wearing the appropriate PPE. EE began missing on 4/20/23. Also received 5 stitches.
