Injured employee was cleaning belt water sprays in a spray bar. They brushed against a fire tap at the location they were working, between the top & bottom belt. When they backed out of the belt and stood up, the fire tap valve suddenly blew off the 8"" waterline coupler and struck their left ankle, breaking it. Accident caused by a failure at the coupler tee due to corrosion.

When the scoop operator tightened up the winch rope, the hook slipped off the shield and struck employee's right leg above the knee. They suffered a contusion/bruising, pain & swelling. Employee failed to position themselves in a safe location.
Injured employee was helping hang the continuous miner cable. Lifted it to EE's shoulder and then up into a hanger. Felt pain in EE's shoulder and later after work, EE noticed a small knot and swelling in EE's shoulder. Reported the incident on 3-9-26 and sought treatment that day and began missing work. No rules or regulations were violated and EE Was wearing required PPE.
Employee was operating a roof bolter installing bolts on the 8W CM Section. EE stuck EE's left hand down on the bolter head to check suction and at that time, a drill steel fell out of the hole EE was drilling and smashed EE's left little finger against the drill head. Employee suffered 2 small fractures to the end of EE's little finger. Was wearing metacarpal gloves.
Injured Employee was walking beside a parked Shuttle Car and slipped on loose coal (sloughage) and fell against the rib. They stuck right forearm out against the rib to catch themselves. Forearm hit a spad that was in the rib causing a small puncture and a cut requiring 3 stitches to close. Employee has training and experience. Returned to work next scheduled shift.
Injured employee was walking and tripped on a crib block causing EE to stumble and roll EE's right ankle. Has been diagnosed with a torn ligament. No rules or regulations were violated and employee has training and experience.
Employee was using a grinder to cut a bolt when the grinder disk came apart and struck EE in the left hand resulting in a laceration. The injury resulted in 5 sutures.
Employee was operating a diesel locomotive on the 8W CM Section track. They parked the motor and stepped out of the deck onto a steel tunnel liner for hoses to pass under the track rails. They slipped on loose gravel that was on the steel cover, slipped and fell down, landing on their right upper leg. No rules or regulations were violated & employee has training & experience.
Employee was standing beside the continuous miner waiting to connect and hang the next ventilation tube. While they were waiting the top fell out between the roof straps and struck them knocking off their hard hat. A small piece of coal/rock followed and struck the top of their head causing a small cut. They received one staple to close the cut. Returned to work their next shift.
Injured was assisting pulling longwall hoses at the Longwall belt drive using a sheave wheel connected to a roof hanger. A nylon rope was run through the sheave and connected to the hoses. As the rope was being pulled, the hanger broke and the sheave struck employee's right knee. Employee was behind guarding but moved away from it during the process.
Employee was assisting installing J-screws into rib. When the bit on the drill became lodged and stuck, employee attempted to free it by pulling on the drill. Employee's hand slipped onto the bit, which spun and injured the employee's hand. Injury was not the result of a training or compliance issue.
Injured employee was dragging a ventilation tube to the right side of the CM for installation. As EE approached the coal pile behind the CM EE set the tube down and stepped across the pile. EE stepped on a loose piece of coal and rolled EE's left ankle. Diagnosed with a ankle sprain.
Injured employee was lifting a scoop battery lid to prop it up for venting while charging. As they were lifting the lid with left hand, it slipped and fell smashing the tip of right thumb. Employee suffered a fracture to the tip of the thumb. No rules or regulations were violated. Employee has training and experience.
Injured employee and a co-worker were loading 10-foot joints of 6-inch pipe (waterline) onto the 7W Longwall Belt. One person on each end of the pipe. As they were tossing a joint onto the belt, the injured lost grip and left arm became smashed between the pipe and the belt structure. They suffered a broken left ulna bone as a result. No regulations were violated.
Injured employee was using a rail hook to lift a track rail and felt a ""pop"" in their back. They then stood up and stumbled and fell on right hand smashing their index finger. Employee sought treatment the following day for back pain. No rules or regulations were violated.
Injured employee was roof bolting on the left side of the continuous miner. Employee was in the process of installing a rib bolt when a piece of coal/rock fell from the brow behind them and struck them on the left shoulder. Was diagnosed with a deep tissue contusion and given time off. No rules or regulations were violated.
Employee was stepping out of the diesel locomotive they had parked. They stepped off the locomotive as they reached for a handle for support but the handle wasn't on this locomotive. Lost balance and fell landing on right shoulder. Has muscle damage in the shoulder. Employee has training and experience. Was wearing required PPE.
Injured employee was lowering a scoop battery lid and as they removed the lid support arm with right hand, the lid slipped out of left hand and pinched the tip of right thumb against the frame. Diagnosed with a hematoma under the thumbnail. Missed one day of work due to this injury. Was wearing PPE (Metacarpal Gloves) at time.
Injured employee was hanging a check curtain on the 7 North Mains CM Section. EE used an 18"" high work platform so EE could reach the mine roof. While standing on the platform, EE pulled up on the curtain, lost EE's balance and fell off it, landing on EE's right shoulder. Result was a dislocated shoulder. Employee has training and experience. No rules or regulations were violated.
Employee was installing a ram pin at 148 shield on the 7W Longwall face. EE was using a hammer to drive the pin in. EE switched hands to hit the pin squarely. As EE was striking the pin with the hammer EE missed and hit EE's right little finger, causing a laceration requiring 7 stitches to close. Employee has training & experience and was wearing required PPE.
Injured employee was assisting a co-worker hang a ventilation tube from the mine roof at the face beside the continuous miner. As they were hanging the tube in place, a rock fell from above the tube and struck employee on EE's right shoulder and upper leg / thigh area. Has swelling and bruising to EE's leg. No rules/regulations were violated and employee has training & experience.
Two employees were building a dry-stack block wall. As the wall was near completion and they were attempting to wedge it tight, it began to move toward them. As they attempted to get away from it, it fell striking injured on EE's right side and below EE's waist, knocking EE to the mine floor. Suffered contusions to EE's lower left leg & fracture to EE's left foot.
Employee was engaged in the mucking phase of shaft construction. A bucket of muck was sent out of the shaft when a rock fell striking them in the hardhat. This resulted in the employee having a small laceration which needed 2 staples and a mild concussion.
Injured employee was driving a scoop to the section charging station when EE ran over a crib block causing EE to bounce up off of the seat. When employee landed back in the seat EE felt pain in EE's back and tail bone. Employee has training and experience.
Injured employee picked up a shuttle car cable to move it out of the roadway. Felt a burning ache in EE's upper forearm. Employee worked EE's next 2 shifts but later sought medical treatment. Began missing work on 9-16-25 due to this injury. Employee has training and experience. No rules or regulations were violated.
Injured employee was operating a shuttle car when a small piece of coal fell from the rib and into the operator's cab and struck their left little finger. Employee suffered a small abrasion/contusion to the finger. Employee worked their next shift but sought treatment the following shift. Employee began missing work on 9-12-25 and missed 2 shifts of work.
Injured employee was walking past the Longwall Stageloader toward the face when a piece of coal fell from the roof and hit EE on the back causing EE to fall against the side of the Stageloader near the #1 shield. Received a cut above EE's left eyebrow requiring 5 sutures to close. Returned to work EE's next scheduled shift. No rules or regulations were violated.
Injured employee was walking along the shuttle car haul road on the 8W CM Section. EE heard a shuttle car approaching and hurried to get in a cross-cut. Stepped on a lump of coal and rolled EE's left ankle. Diagnosed with an ankle sprain. Did not violate any rules or regulations.
Employee was placing scoop batteries on charge. After plugging in the batteries, EE raised the lids to vent and was overcome by an unknown gas. Employee suffered watery eyes, runny nose, headache, and labored respirations. Employee was transported, given breathing treatments, examined & released. No regulations were violated and EE has training and experience.
Injured employee was operating longwall shields when EE stumbled on a relay bar. EE put out EE's right hand to catch self but EE's hand landed on a bolt plate (Jennmar Mini Monster Mat), causing a laceration. Employee sought medical attention and the laceration was glued closed and bandaged. No rules or regulations were violated and employee has training and experience.
Injured employee was roof bolting from the continuous miner at the face on 8W CM Section. After installing a bolt, employee walked back to the bolt tray to get another bolt and a piece of coal fell from the roof and struck EE on the right shoulder. Employee stated roof conditions were good and coal fell without warning. Has pain & soreness and possible Labrum damage.
Employee was operating a hammer drill and struck themselves in the mouth causing them to chip their right front tooth.
Employee was vulcanizing belt when EE attempted to pick up a winch and strained EE's lower back. The employee began missing work on 7/7/2025.
Injured employee was removing a flight from the longwall stage loader chain. As EE was removing the top section from the bottom bar, it moved unexpectedly and fell on EE's left index finger. Injured received 7 sutures and a fractured fingertip as a result. No rules or regulations were violated. Employee has training and experience. Returned to work EE's next scheduled shift.
Employee hung a ventilation skin on the back of the section fan, and as they turned to walk away, a 10'L x 4'H piece of the inby coal rib fell striking them on right side knocking them to the mine floor. They sustained a cut to the forehead, and contusions and abrasions to the right side of torso and lower body.
Employee was mixing B-Bond cement by hand to seal a block stopping. EE was wearing rubber gloves and using hands to mix the material but got material on their shirt and over the gloves. Material caused burns to right arm where it contacted their skin. Employee was wearing required PPE but failed to keep the B-Bond from contacting skin. Missed 2 days of work.
Injured employee was removing ""K-Wall"" panels and as EE was pushing forward on a panel, EE felt a ""pop"" in EE's right knee. Finished EE's shift but sought treatment the following day and began missing work. Initial diagnosis is torn ligament around kneecap.
Injured employee was pulling on a cable during a Longwall Power Car move and as EE pulled on the cable, EE hyperextended EE's thumb causing a fracture at the base of the thumb. Employee returned to work EE's next scheduled shift.
Injured employee was walking through the #5-6 cross-cut at 116 block to get a ventilation tube. Stated EE stepped over a trailing cable and lost EE's balance and fell. Put EE's right hand out to catch EE's fall and hit the coal rib. Has pain and swelling in EE's wrist. Stated EE did not trip or step on cable or obstruction.
Injured employee was using a small ""hook chisel"" to remove a staple from a hose fitting at #74 Shield on the 7W Longwall face. As EE was prying the staple out, it broke free and the chisel came up and struck EE's face causing a chipped tooth. Employee did not lose any workdays due to this accident but had the tooth repaired on 4-23-25. Has training and experience.
Injured employee was dismounting the continuous miner after climbing up onto it to examine the conveyor chain for a possible rock jam. A EE was getting down, EE fell and struck EEs leg against the edge of the machine's cable handler. This caused a deep laceration to EEs left lower leg. Received stitches and given time off work for wound to heal. Has training and 2 years experience.
Injured employee and co-worker were using a small crane mounted on a mantrip to lift and load a shuttle car reel on the end of the mantrip. As the reel was suspended it became stuck on the edge of the mantrip platform. The employees were attempting to free it up when the reel slipped off the hoist hook and fell on injured employee's right foot. Suffered a contusion and bone bruise.
Injured employee was riding on the outby end of a 7-man diesel mantrip. As the mantrip approached a track switch to get in, the mantrip operator failed to pay attention to the mantrip in front of them also entering the switch, and struck the end of that mantrip. The collision jarred the injured employee who complained of back pain which became worse during the shift.
Employee was operating a diesel locomotive on the outby end of a trip when they ran over a d-rail device causing them to d-rail and collide into the coal rib. Their right hand was caught between the locomotive body and rib. The result of the injury is amputation of the middle and ring fingers to the middle phalanges and amputation of the little finger to the distal phalange.
Injured employee was kneeling down installing a plug (cat head) on the Tailgate #2 CM Section feeder cable. When they stood up they felt a ""pop"" in right knee. Has been diagnosed with a torn meniscus. Employee has training and experience in job.
Employee was carrying a jack down the Slope Belt when EE stepped on loose coal, slipped and fell down. Hit EE's right arm on the floor and the jack EE was carrying struck EE's left side rib area. Employee missed 4 days of work for contusions, pain and soreness. No rules or regulations were violated. Employee has training and experience.
Injured employee was carrying a cinder block and stepped on a loose piece of coal/rock on the mine floor and rolled an ankle. Finished the shift but later noticed it was swollen. Sought treatment the following day and began missing work. No rules or regulations were violated, and employee has training and experience.
Employee turned a valve on at the #267 shield on the 7W Longwall face to pressurize a 3/4"" hydraulic line supplying pressure to a Z-drill outby the face in the tailgate entry. When EE turned the valve on, it broke at the stem where it entered the connection port and struck EE on the right side of EE's face. Suffered a small fracture, contusion to eye and 3/4"" cut requiring sutures.
Employee was getting into the operator's compartment of a diesel locomotive and struck left knee against the SCSR storage box. No rules or regulations were violated, Employee has training and experience. Has contusion, pain, swelling in the knee.
Injured employee was building a block stopping in 7W Longwall section. As EE was placing a cinder block on top of another EE smashed the right middle finger between the blocks causing a small laceration. Employee sought treatment and wound was closed using glue. Given 3 days off work. No regulations were violated.
Injured employee stepped out of the parked diesel locomotive employee was operating and stepped on loose coal causing employee to slip and twist left knee. Initially diagnosed with a strain/sprain and given time off. Employee has training and experience. No rules or regulations were violated.
Injured employee was supervising/assisting the loading of a scoop onto a scoop dolly. As the battery end of the scoop was being lowered onto the dolly, it pushed the dolly out from under the scoop and the dolly rolled back on the track rails and struck the employee's right ankle & foot, knocking employee down. Employee has been diagnosed with a strain/sprain, contusions, swelling.
Employee was operating the 6W Longwall Headgate and was pulling excess CO cable when EE tripped on belt structure that had been removed and stacked along the rib. EE fell back and extended left arm out to break the fall. EE landed with arm extended and jammed left shoulder. Was diagnosed with a strain/sprain and possible tear to the rotator cuff.
Injured employee and co-worker were carrying and placing sections of bedrail along the ribline for installation/belt advancement. As IE was setting the end IE was carrying down, the IE's co-worker dropped the other end. This caused IE's right middle finger to get pinched between 2 pieces of bedrail. Suffered a cut to the tip of the middle finger. Has training & experience.
Injured employee was exiting the vehicle after arriving for work at the Prep. Plant. As EE stepped out of the truck, EE slipped on ice/snow and fell down. Employee suffered a fracture to their upper left arm. Employee had not yet reported for work and was not on the job or performing any work activity. Employee has training and experience.
Employee doesn't recall an incident but believes somehow got dust behind contact lens causing irritation. Employee sought treatment the following day and was given medicated eye drops. No rules or regulations were violated and employee has training and experience.
Injured employee was walking up the #3 entry toward the continuous miner when a piece of coal 4-6"" thick fell from between roof straps and struck employee on the back of head/neck/shoulders. Has pain and soreness and is off work awaiting an MRI. X-rays were negative. Normal roof conditions at accident location.
Received notification on 11/11/24 that this retired employee has been awarded 10.53% PPD award for occupational hearing loss. Employee was enrolled in a Hearing Conservation Program & was trained in the use and the areas where hearing protection is required. Employee failed to properly use hearing protection throughout their mining career. Employee retired on 9/14/20.
Employee was walking across the 6W Longwall face between the #3-5 shields when EE tripped on loose rock/coal and fell down. EE extended EE's right arm out to break EE's fall. Has a strained right shoulder. No rules or regulations were violated and employee has training and experience.
Employee was bolting off the right side of the miner when EE was pushing a bolt up and the plate shifted. EE then reached up to adjust the plate but was still pushing the bolt up causing EE to pin EE's hand between the plate and the strap. EE had skin abrasions on EE's hand.
An Electrical Foreman was helping an employee lift a spool of leaky feeder cable and felt a ""pop"" in EE's upper right arm. EE was diagnosed with a torn bicep muscle. EE was given a sling to wear for support as needed and released. EE returned to work EE's next shift. No rules or regulations were violated and employee has training and experience.
Employee was using an angle grinder to cut a piece of metal angle. The grinder slipped while cutting and contacted their left upper leg causing a cut. Employee received 4 stitches to close the wound. Returned to work their next shift. Employee failed to maintain control of the grinder. No rules or regulations were violated and employee has training and experience.
Injured employee was unloading crib blocks off a mine car and stacking them in a scoop bucket. As EE was placing a crib block in the bucket, EE smashed the tip of left middle finger. A suture was put in the fingernail to keep it in place but later removed and the fingernail also removed. Employee is off work to allow wound to heal. No rules or regulations were violated.
Employee was working on the 7 North #1 belt when they slipped and fell on a fire tap causing it to break and the water knocked them back injuring their left knee and lower back.
Employee stepped under a loaded muck bucket to clean the bottom when it dropped suddenly striking employee in the top of the hard hat.
Injured employee was using a hand-held grinder to cut a 4"" long x 1/4"" diameter steel pin. As employee was cutting on the pin, a piece the griding wheel broke and the grinder kicked/jerked suddenly. The wheel contacted employee's left wrist causing a cut requiring surgery and stitches. Employee failed to properly control the tool during use.
Employee & co-worker were removing ventilation tubes from the mine roof to transport to another entry. As they were preparing to get a tube, they inadvertently knocked another tube out of its support chain causing it to fall and strike employee's right forearm causing a contusion and pain. Employee missed 1 shift of work due to this injury. No rules or regulations were violated.
Injured Employee was changing a cutter shaft on the Longwall tail drum. As EE was removing the shaft retaining ring, hot hydraulic fluid squirted out onto EE's hands causing burns & blistering. Employee failed to properly relieve system hydraulic pressure. Employee missed 1 shift of work due to this injury.
Employee was changing rollers on the 7 North #1 Belt. Employee was carrying a roller on shoulder. They tossed it off their shoulder to the mine floor but on its way down it hit the belt structure and bounced off and landed on their right foot. No rules or regulations were violated. Missed 1 shift of work due to a contusion on the foot.
Injured employee was dragging a ventilation tube and stepped on loose coal/rock causing EE to slip and fall backward, twisting their right knee. Has been initially diagnosed with a knee sprain. No rules or regulations were violated.
Injured employee was hanging trailing cables on the boom of the loading machine to be advanced up the entry. They extended their right hand to hook the pull rope on the boom at the same time the boom was re-positioned. Employees hand was caught between the boom and coal rib. Employee suffered a broken little finger on their right hand.
Employee was cleaning concrete forms and turned to move striking their left shoulder into the jack bolt on the form directly behind him.
Employee was replacing the longwall headgate drive winch. As EE was removing bolts from the winch, a piece of coal/rock fell out from between the roof channel straps and struck the back of EE's leg forcing EE against the headgate drive. EE has pain and swelling in the right knee and ankle. No rules or regulations were violated.
Injured employee was roof bolting on the right side of the continuous miner. EE was drilling a hole for bolt installation and was in the process of adding an additional section of drill steel when the existing steel in the roof fell out, striking the top of EE's left hand. EE suffered a broken ring finger and a cut requiring stitches.
While spraying shotcrete employee received some material in EE's right eye.
Employee was roof bolting on the right side of the CM and was advancing /adjusting the ventilation tubing when a rock fell from the roof and struck EE's right lower leg breaking the tibia & fibula bones above the ankle. No rules or regulations were violated and employee has training and experience.
Injured employee was roof bolting on the left side of the CM. As EE was installing a bolt and roof screen, a piece of rock fell and struck employee's left shoulder causing contusions/abrasions and pain. No rules or regulations were violated and employee has training and experience.
Injured employee was standing near the right side roof bolter station of the CM. CM was being serviced and injured employee handed a bucket of bit inserts to another employee and a small piece of coal/rock fell and struck the top of EE's right hand causing a laceration. Employee received 11 stitches to close the cut. Returned to work the next shift.
Notification was received on 07/1/2024 that this retired employee has received a 15% award for Occupational Pneumoconiosis. This incident was caused by the employee's failure to wear appropriate respiratory protection during employee's mining career. Employee did not violate any rules, regulations, or policies and has proper training. Employee's date of last exposure was 1/9/2020.
Injured employee was installing roof bolts from a center bolter. Placed right hand on the drill guide and grabbed a drill steel with EE's left. While reaching for the drill steel a rock fell and struck employee's hand on the drill guide. EE was diagnosed with a fractured middle finger and 14 stitches.
Employee was removing an old solenoid cable from a longwall shield. The cable was hanging down and employee jerked on it forcefully, expecting it to break free but it remained stuck and the reaction force caused an injury to right ring finger. Employee continued to work and first sought medical attention on 8/16/24 and was diagnosed with a fracture & possible tendon damage.
Employee was getting out of a mantrip on the portal bottom at the end of the shift and twisted knee. Employee did not slip/trip/fall. Employee has adequate training and experience.
Two employees were loading 10 ft. joints of 6"" metal waterline onto a skid. Positioned on each end of the pipe, they were tossing the pipe onto the skid. The end of the pipe bounced back and struck the injured employee on the left side of face/head. The 2 employees failed to use proper communication to make their intentions clear. Injured has training & experience.
When the work platform was landed on bottom it placed slight tension on the entire makeup of the rebar boot. EE unscrewed the clevis and did not hold the cable it was attached to as instructed by EE's supervisor. As a result, the clevis once unattached sprung and swung striking EE on the cheek. There was no bleeding, swelling or visible injury seen.
Employee was walking alongside the longwall shear observing the cut-out process. EE stepped on a piece of coal, stumbled and began to fall. EE reached out with right hand and grabbed the handrail to catch self and at that moment the shear cable pulled on top of EE's hand. Employee received a laceration to the top of the hand and 2 broken metacarpal bones.
Employee was utilizing a straw blower to spread mulch on exposed ground. Employee stood on top of the mulcher guard which caused the guard to collapse. Employee fell onto the chute and employee's feet were struck by rotating chains. Another employee was nearby to assist the fallen employee off of the mulcher. The employee sustained fractures on both feet.
Employee was walking through a cross-cut in an outby recovery area of the mine when they stepped in a small depression in the mine floor and ""jammed"" lower back. Has pain and soreness. No rules or regulations were violated and employee has training and experience.
Employee was using a small sledge hammer to remove a block from a stopping at a regulator for the purpose of installing a de-gas pipe through the stopping. EE was holding the hammer with left hand and as EE was striking the block wall, EE smashed their hand against the metal frame of the regulator and the hammer handle causing a cut and fracture to the tip of EE's left thumb.
Employee was installing roof bolts on the left side of the CM. A piece of coal/rock fell out from the roof and struck employee on the back & neck. Employee finished installing the bolt & stepped away from the bolter station toward the rear of the CM. Another piece of coal fell from the roof, hit the rock deflector & then hit employee on the back as they were walking away.
Employee was walking on gravel/ballast along the CM section mantrip station and twisted knee in the loose gravel. Has pain and soreness in the knee. Was diagnosed with a left knee sprain. No rules or regulations were violated, and employee has training and experience.
Injured employee was unloading wood crib blocks from a scoop bucket and stacking them in a storage area during longwall recovery. As EE picked up a crib block, EE felt a sharp pain in the left wrist. Was later diagnosed with a sprained wrist and missed 2 days of work. No rules or regulations were violated and employee has training and experience.
Injured employee & a co-worker were unloading a track ""frog"" from a mine flat car. As they lifted it, EE felt a sharp pain in EE's upper right arm and down into EE's forearm. Employee is off work awaiting orthopedic evaluation. Employee didn't violate any rules or regulations & has training & experience.
Employee was replacing a top belt cradle on the 7 North #1 Belt. While removing the old cradle, the employee smashed left middle finger between the cradle and the bedrail causing a laceration requiring 7 sutures and a fracture to the tip of the finger.
Employee was using a knife to open a box of belt rivets. While cutting, the knife slipped and cut employee's right hand below the thumb, palm side. Received 4 stitches. Employee failed to maintain control of the knife and was cutting toward self. Employee was wearing protective gloves. Returned to work the next shift.
Injured employee was lifting/pulling on the continuous miner trailing cable to place it out of the roadway when employee felt pain in lower back. Employee missed 2 days of work with a low back strain. No rules or regulations were violated.
Injured Employee was installing roof bolts from the left side of the CM. Stated knelt down on left knee. As EE was kneeling, a piece of coal fell from the roof and hit EE's right knee causing a cut & contusion. Employee received 3 sutures to close the cut. No rules or regulations were violated & employee has training & experience.
Employee was operating the Roof Bolter installing roof bolts on the 6W Longwall Development face. As employee was installing a bolt, a large piece of rock/coal fell out from the roof onto the ATRS and broke up. A piece of rock struck employee in employee's back, knocking EE forward causing EE to twist as EE fell down. Employee has pain in lower back and left knee.

As EE was doing this, EE was walking backward and stepped on a lump of coal and rolled right ankle and fell down. Employee failed to pay attention to where EE was stepping. No rules or regulations were violated and employee has proper training.

This incident was caused by the employee's failure to wear appropriate respiratory protection during their mining career. Injured employee was attempting to open the 4W Track switch to park the mantrip. The switch lever was stiff to operate so employee was using their right foot to push on the handle to get it to open. When the handle ""broke over"", employee fell and twisted right knee. No rules or regulations were violated & employee has training/experience.
Injured employee was removing the continuous miner trailing cable from a cable hanger. As EE was removing the cable, the hanger struck EE in the mouth causing a small chip in EE's right front tooth. No rules or regulations were violated & employee has training/experience.
Injured employee positioned a longwall leg jack onto the bedplate extension so the jack could be removed from the face. EE failed to secure the jack against movement. Employee briefly looked away from the jack & at that time the movement of the shearer caused the jack to roll off the bedplate onto EE's left leg resulting in a fracture to the fibula.
Injured employee was lifting a longwall jumper cable to place into a cable hanger when the employee felt a ""pop"" in right shoulder that became painful later during the shift. Employee began missing work on 10-23-23 due to this incident. No rules or regulations were violated.

Injured employee was using a hammer and chisel to remove a plug in a longwall ram jack that needed replaced. While hitting the plug with the hammer & chisel, a piece of it broke off and flew into employee's left eye. Received a cut to the globe part of EE's eyeball that required sutures. Injured employee was traveling on a mantrip with a co-worker to go make a belt splice. Injured employee dismounted the mantrip at a track switch for the mantrip to enter and as the mantrip passed by employee, a splice board was sticking out and struck upper left leg. Employee has pain and a contusion. The employees failed to follow Company safety policy, rules & regulations.
Employee was walking down the Prep. Plant Office hallway toward the exit door when EE tripped and stumbled, twisting EE's right knee. Has pain and swelling in EE's knee and is missing work due to EE's injury. No rules or regulations were violated and EE has training and experience.
Employee was using a grinder to cut on a belt cradle on the 4 West #2 Belt. When the grinding wheel made contact with the cradle, it ""kicked"" back & contacted employees left leg causing a laceration above EE's knee. The laceration required 4 sutures to close. Injured employee was lifting up on a track rail when EE felt pain in EE's lower back. Employee was helping others place track rails along rib line for future installation. No rules or regulations were violated and employee has training and experience.
Upon arriving at work, injured employee was getting out of vehicle and as EE stepped out, EE's foot slipped on the gravel causing left knee to twist. Employee felt pain/soreness in the knee but finished the shift. Employee worked until 6-23-23 when EE began missing work due to this injury.

As they cut the zip-tie away, the knife contacted left hand cutting the top of middle finger. Received 3 stitches to close the cut. Accident caused by employee's failure to use a knife in a safe manner.
Injured employee & co-worker were putting a Loading Machine conveyor chain together. Injured employee was holding the connecting link in place while co-worker was driving link in place with a hammer. Co-worker missed with hammer and struck injured employee's right little finger causing a laceration requiring 6 sutures to close. Employee has training & experience.
Employee was carrying belt rollers from the Longwall belt entry through the L.O.C & placing them in a scoop bucket. While carrying a roller, employee's foot caught a piece of string that was attached to the roller EE was carrying & caused EE to fall. Extended left arm to break fall but injured shoulder when EE landed. Employee has training and experience in task.
Employee was cleaning along the 4 West #2 belt with a shovel and felt a sharp pain in back. Complained of soreness and stiffness in lower back. Finished shift but sought treatment 2 days later and began missing work. No rules or regulations were violated and employee has training and experience.

Employee was removing a bolt that was helping secure a scoop bucket to an equipment dolly. As employee was removing the bolt the scoop bucket shifted pinching employee's right index finger between the bolt and bucket. Employee received an avulsion to the finger. Employee was performing routine maintenance on a diesel mantrip in the surface diesel shop. EE stepped out of the operator's compartment down onto the shop floor and felt a ""pop"" in the left knee. Employee was a passenger on a mantrip that was struck by another mantrip as it entered the 7 North Main Line Haulage from 4W section haulage at the 4W section switch. When the collision occurred, the employee's head struck the window frame of the mantrip causing a laceration. EE received 5 staples to close the cut. Injured employee didn't violate any rules or regulations.
During mining on the L/W face an uncharted gas well was exposed. Methane gas in excess of 1.5% inundated the area and caused monitors on the shearer and L/W Tailgate to de-energize the section power. Curtain was installed to ventilate the area to reduce the CH4 level. The exposed well was later plugged and minded through. There were no injuries or damages as a result of the event.
Employee was lifting a piece of track rail with a hook when they felt pain in lower back. Diagnosed with a lower back strain. Employee didn't violate any rules or regulations and has training & experience in task.
Received notification that a belt take-up roller had caught fire due to a bearing failure. The internal bearing failed and frictional heat ignited the bearing grease. Mine employees arrived at the scene and extinguished the grease fire using fire extinguishers and rockdust. There were no injuries associated with this incident. Employee was using a pry bar to remove a rock off the top of the continuous miner. As employee was prying on the rock the pry bar slipped out from under the rock causing employee's left elbow to strike the mine roof. Has pain & soreness in elbow. Employee was working at the longwall belt booster drive. A piece of PVC pipe that was cut out of the longwall face, traveled on the belt to the transfer where it jammed and came off the belt hitting employee in the mouth. Employee had to have a tooth removed as a result. No rules or regulations were violated. Employee has training & experience.
Employee was removing a top roller cradle from the 3 West Belt during belt maintenance. As EE was pulling the cradle out it bumped into something causing it to stop briefly. As EE was removing the roller, a piece of the broken roller can contacted EE's right forearm causing a laceration. Employee later received 4 sutures. EE was carrying a crib block to 96 block when they tripped on the track rail and fell, pinning right hand between the crib block and the track rail. Employee was using a ratchet & socket to loosen a bolt on the infrared shield initiation sensor mounted on the longwall shearer so EE could adjust it's position. Injured was exiting loading machine to hang a trailing cable and struck head on the loader canopy, resulting in a neck strain. Occurred in 5W #1 entry turning right crosscut #2-3. Employee was wearing required PPE and has training and experience in job.
The injured employee was trouble shooting an oil pump. When EE turned the air supply on the pump, plunger piston moved and contacted EE's right thumb resulting in a laceration requiring 7 sutures.
While removing belt structure on 3W belt conveyor, employee swung a hammer to hit a bedrail pin, missed, and hit their thumb. This resulted in a bruise to the left thumb. Employee was wearing required PPE and didn't violate any rules or regulations.
Injured employee was operating a CM as part of the ""mucking"" process at the Raised Bore Intake Shaft drilling site. EE was standing at the rear of the CM when a rock fell from the shaft and ricocheted off of something and struck EE on the right forearm. EE's right radius bone was broken as a result. Employee was wearing required PPE & didn't violate any rules or regulations.
While operating a diesel locomotive the injured employee stopped to open a set of haulage doors. When IE stood up to get out of the locomotive IE hit their head on guarded high voltage cable. IE again struck their head on a roof strap later in the shift while standing up to get off the same locomotive. IE complained of neck/back pain, nausea and dizziness.

EE was holding a drill steel in place with EE's right hand and was attempting to press the lever to dump EE's dust collection box but inadvertently pressed the tilt lever causing EE's finger to be smashed between the drill steel and strap jack.

Employee was parking EE's mantrip on the 7 North Portal Bottom, 11 blk. spur. As EE entered the spur track, EE pulled on EE's brake lever and locked up EE's brakes and slid into a parked mantrip. Employee fell from EE's seat and landed on EE's back inside the mantrip.

When employee stepped over the subframe employee's right foot landed on a bag of sack crete and employee rolled right ankle. Employee was operating a continuous miner removing material from the bottom of a previously drilled shaft. As the material was being removed, an unexpected release of water caused the material to wash out knocking the employee down & pushing EE down the entry. Injured Employee was supervising repair of the longwall shearer. As the shearer was being repositioned, a cutting bit contacted a shield and a small piece of metal broke free & traveled approx. 30' & struck the employee on the inside of EE's left forearm and became embedded under the skin. The piece of metal was later removed and employee received 3 stitches.
Employee was taking a bag of trash out of the bath house facility to put in a dumpster. After throwing the bag of trash in the dumpster, EE stepped in a small hole in the gravel parking lot and twisted left ankle. Employee failed to pay attention to where was stepping. Employee was standing in the last open cross-cut near the headgate area of the 3W Longwall, waiting for the face to advance before EE entered the area. As EE was standing there, a piece of coal fell from the roof and struck EE on the hard hat and between EE's shoulder blades. Employee began missing work on 7-21-21 due to this injury. EE didn't violate any rules or regulations.
Employee was crossing over an idle longwall conveyor belt. Employee stepped off the belt onto the mine floor and twisted right knee. Employee experienced pain & swelling in the knee by the end of the shift. Employee didn't violate any rules or regulations and was wearing required PPE. Employee has training and experience.
Injured employee was stepping through a man door left foot first. As they were bringing right foot through boot caught the door frame knocking off balance & shifting all weight onto left knee causing it to twist & they fell to the floor. Has pain & swelling in knee.
Injured employee was getting out of the operator's compartment of a parked scoop when EE's right foot slipped on the floor causing EE to fall out of the scoop doorway. EE twisted and strained muscles in EE's right hip. Employee began missing work on 6-11-21 due to this injury. No rules or regulations were violated and employee was wearing required PPE.

Injured employee was walking through a door to the shop when their coat got caught on the door latch. Employee reached behind them with right hand and grabbed the coat and began pulling on it to free it. As they pulled on the coat, the door closed on left little finger that had been placed near the door hinge. The little finger was fractured and cut.
Employee was roof bolting on the right side of the CM when a large piece of ""head coal"" fell out from 2 roof channel straps and struck EE on the head, neck, shoulder. Employee was briefly knocked unconscious. Was transported to the hospital for evaluation and released later that shift. Employee was wearing required PPE and has training & experience.
Employee was walking alongside the left side of the CM when EE tripped on loose coal/rock and fell. EE fell into the side of the CM and hit EE's right wrist against a step on the CM. The mantrip operator failed to slow down for a kink in the rail at 83 1/2 blk. on the 3W section track haulage causing the mantrip to de-rail. Injured employee fell sideways striking left shoulder against the side of the mantrip.
Injured employee opened the 3W Section track switch for a mantrip. EE then closed the switch and started to walk toward the mantrip. As EE walked away EE stepped on the switch tie/bracing causing EE's ankle to ""roll"". Employee diagnosed with a sprained left ankle. Employee followed rules & regulations but failed to pay attention to where EE was stepping.
Injured employee was installing a set of pinch rollers at 3W belt drive. Had roller tower secured with scoop bucket and cribs. Employee was preparing to climb onto the top belt to assist another employee with structure alignment. The movement on top of the roller tower caused it to pivot & fall forward in the scoop bucket, striking the employee's right lower leg.
Three employees were hanging mono-rail cables in cable hangers for a future Longwall section. As they were hanging the last cable, the weight of the cables overcame the hangers causing them to fail and fall in succession. The injured employee tried to hold them back but they fell on right shoulder, knocking them down. Employee sustained a strain to right wrist and shoulder.
Injured employee was working on the 3rd floor of the Prep. Plant changing a de-slime screen. EE lifted the metal framework for the screen and felt pain in left shoulder. Employee continued to work and sought treatment the following day. Employee overexerted shoulder while lifting. Was wearing required PPE and has experience in task.
Employee was installing Tensar roof screen at the face of the 4W CM section during mining. As employee was raising roof jack against the screen, right pinky finger got pinched by the screen causing a small avulsion/laceration to the tip of the finger. Employee failed to pay attention to hand placement.
Employee was traveling an intake aircourse with an MSHA Inspector when EE tripped and fell to the mine floor. EE has pain and soreness in right & left shoulder, neck, an abrasion on right elbow, and pain in right knee. The area was examined for tripping hazard/obstructions but none were found. No rules or regulations were violated & EE was wearing required PPE.
Notified on 2/25/21 that this employee has been awarded 2.2% PPD award for occupational hearing loss. Employee was enrolled in a Hearing Conservation Program & was properly trained in the use & areas where hearing protection is required. The incident occurred because the employee failed to properly use hearing protection throughout mining career. Employee retired on 4/26/20.
