A contractor drill caught fire in the pit and burnt up. The fire was not able to be extinguished within 30 mins. Due to the extent of the fire, the investigation into cause is still ongoing.

The employee was climbing up into the excavator and felt a sharp pop in the right calf. Employee immediately reported to the supervisor and the first aid process began.

A loader operator stopped the machine. When exiting the loader, turned to face the cab of the machine to begin climbing down the ladder not establishing 3 points of contact, losing its balance and falling to the ground. Management immediately executed the emergency protocols. No significant injuries were observed at the time of the fall.

Employee was lowering anvils into an anvil ring and EE caught left pinky finger between the ring and an anvil. The cut required stitches.

Employee was opening a door when it fell off the hinges and smashed right thumb between the door and the handrail of the man lift the employee was in.
Attempting to install bit into hydraulic hammer. EE was standing atop a rock pile when the rock the left foot was atop rolled causing EE to fall. The employee attempted to catch self causing injury to left arm/wrist.
EE was taking blow bars off impact crusher. One of the supporting shoes came loose as it was being removed and fell. EE attempted to stop the shoe from falling and EE's hand was caught between the shoe and the metal frame of the crusher.

While removing the shiv from a 300 hp motor that assist in running a Hazemag Crusher the shiv fell off of the shaft smashing employees fingers. Employee had to have his right hand index finger stitched and his second finger splinted.

Employee was exiting cab of locomotive and while stepping out on the two steps that stepped down to the cat walk he did not get solid contact with the 2nd step and twisted his leg. He grabbed the rail and stopped his fall but his leg hit the catwalk with alot of force...breaking the bone below the knee in 2 places.

WHILE HITTING A PLUGGED UP HOPPER WITH A #8 SLEDGE HAMMER, THE HAMMER BOUNCED BACK AND STRUCK EE IN THE FOREHEAD.
EE WAS CLEANING BESIDE A CONVEYOR BELT AND RT. FOOT MADE CONTACT WITH A PIECE OF METAL THAT WAS LEANING AGAINST THE WALL, THE METAL FELL & STRUCK THE RT. KNEE. EE MISSED NO WORK AND WAS PUT ON LIGHT DUTY UNTIL RELEASED FROM DR.

TRUCK DRIVER WAS BACKED AGAINST A RAMP THAT IS STATIONED ON THE WEST SIDE OF STRIPPING AREA, SO SAFE ACCESS CAN BE ACHIEVED TO SHOVEL DIRTY MATERIAL OUT OF THE CORNERS OF THE BED OF THE HAUL T RUCK. UPON LEAVING, THE DRIVER TOOK A SHORT CUT DOWN THE SIDE OF THE RAMP INSTEAD OF WALKING TO THE END OF THE RAMP AND AROUND TO THE CAB, AND FELL.
LOADER OPERATOR WAS PARKING FOR THE DAY AND NOTICED THE WINDSHIELD WIPER WAS NOT ATTACHED AS IT SHOULD BE. EE STEPPED FROM THE DECK OF MACHINE TO THE LEFT FRONT FENDER TO MAKE CORRECTIONS. UPO N ATTEMPTING TO STEP BACK TO DECK SLIPPED AND FELL TO THE GROUND.
BRAKEMAN RIDING ON RAILROAD CAR, CAME TO A SWITCH TO SWITCH TRACKS, THE CAR JUMPED OFF OF TRACK, THE BRAKEMAN CALLED FOR THE TRAIN TO STOP AND BEFORE THE ENGINEER COULD COMPLETELY STOP THE BRA KE, EE JUMPED CAUSING BRUISING TO LT. ELBOW AND RT. KNEE, WILL BE OFF ONE WEEK.
