Struck against a moving object

Employee was operating loader and skidsteer and felt pain in their back. Later was diagnosed with a shoulder strain/tendonitis.

Mechanics were performing maintenance on a Cat 375 excavator. The two mechanics set the part on the body of the excavator to adjust the rigging. While unhooking the rigging from the crane, the swivel began to roll off of the side of the excavator and caught the synthetic strap moving it towards the mechanic's hands. The strap caught the employee's finger, bending it backwards.

Haul truck operator runs over a pot hole and bruises ribs on arm rest of operators seat.

Contact with hot objects or substances

Employee was welding when a spark/slag became airborne and landed in EE's ear. No internal damage found during clinical observation. Employee was given antibiotics as a precautionary measure.

Subcontracted haul truck driver found unresponsive, crew, medics and emt's were undable to revive driver. Driver was a subcontract employee of another company so I dont have all the victim information available.

Caught in, under or between a moving and a stationary object

The injured employee was using a Dingo TX 525 to remove accumulated material beneath a structure. While operating the equipment in reverse, the bucket contacted a vertical steel member. The sudden motion crushed EE's left wrist between the Dingo controls and the diagonal structural steel member. The diagnosis was a strain and contusion of the left wrist; no broken bones.

Struck against stationary object

A Lead Hand and another miner were replacing a roller on a cross over belt for the screen plant. In order to remove the bolts from the back side of the roller, the Lead Hand knelt down on the under screen belt and instantly felt pain in his right knee. The Lead Hand was taken to the local ER, where he was diagnosed with knee pain and placed in a splint to immobilize his knee.

Employee was hanging the upper guard at the tail wheel section of the uphill conveyor. Another employee had one end of the guard and the IE had the other end. The guard was sitting on the top edge of the lower guard when the IE moved it to put a bold in it when it slipped off the top edge. His finger was in one of the holes and it was cut as the guards slid past each other.
