Employee was performing maintenance activities inside the Barge Jaw Crusher. EE was standing on a piece of plywood that shifted, causing EE to lose footing and fall into the jaw area. EE was wearing fall protection and SRL (Self-retracting lanyard) that stopped EE from falling farther into the jaw crusher.
