Employee was performing maintenance activities inside the Barge Jaw Crusher. EE was standing on a piece of plywood that shifted, causing EE to lose footing and fall into the jaw area. EE was wearing fall protection and SRL (Self-retracting lanyard) that stopped EE from falling farther into the jaw crusher.

Employee and coworker were positioning a chute guard, weighing approx. 50 pounds. As they lifted the guard, one of the two employes handling the guard felt a pop in his left shoulder.
Employee was walking from the shop to the crusher when he slipped and fell on snow covered ice causing his right knee to twist.
Employee was replacing bearing and stub shaft assembly. Employee put thumb between upper stub shaft and screw to pull out gasket. The movement caused the screw and stub shaft to come together pinching the employee's finger.
while holding adjustment ram out of the way of adjustment ring on 1144 cone crusher, employees finger became pinched between end of ram and adjustment ring.
EE WAS STRUCK BY A PIECE OF CONCRETE WHICH FELL APPROX 15' FROM FEEDER HOPPER. EE WAS CLEANING DEBRIS FROM THE TOP OF A PULLEY GUARD WITH HIS HAND. CONCRETE STRUCK HIS LEFT HAND SMASHING EE RING AND LITTLE FINGER
WHILE POURING A 5-GALLON CAN OF OIL INTO SWINGER BOX, EE STOOD-UP AND HIT THE LEFT SIDE OF HIS HEAD ON LIGHT GUARD.
