A driver rolled ankle while making a delivery to the Bellingham plant. The rolled ankle resulted in a fracture.

Fall protection was not being utilized correctly resulting in a fall from height. 2 broken ankles and a broken leg.

Cement got in water. Water became alkaline. Water got into gloves and on coveralls. 2nd degree burns on right forearm, ribs and legs.
