EE injured hand while removing guard.

Fall from machine

Employee operating a loader parked on uneven surface. While dismounting the piece of equipment, the employee stumbled and fell. In an attempt to brace the fall, the employee injured EE's left wrist.

Over-exertion in lifting objects

EE was lifting changing screens and heard a pop in right elbow. Was issued a brace on 3/7/16 making it reportable.

Employee was prying a 8 to 10 inch rock from a plugged chute. As the rock broke free, the bar moved and pinched left ring finger between bar and motor mount. No stiches were required but occupational therapy will be required to sustain movement and feeling in finger.

Beginning on or about 9/11/07 we had a job loading 20,000 tons of a 2 x 8 inch rock. EE said the motion of his loader while loading that material made his upper back begin to hurt. He spoke of a ""whip"" as the bucket came back against the stops and having to stretch to operate the scale

Employee was operating a wheel loader loading pit trucks on 3-15-06, when he felt a small pain in his back on the left side. He continued to work and didn't report anything until the morning of 3-22-06.

EMPLOYEES NOTICED PLANT MANAGER WAS VOMITING. THEY MONITORED HIS CONDITION. HE ASSURED THEM HE WAS FINE & DIDN'T HAVE ANY OF THE ""CLASSIC"" SYMPTOMS OF A HEART ATTACK. HE WAS LATER OBSERVED SLUMPED OVER HIS DESK & AN EE CAUGHT HIM & LOWERED HIM TO THE FLOOR. EMERGENCY RESPONDERS WERE CALLED & HIS CONDITION WAS MONITORED UNTIL THEY ARRIVED. DEATH OCCURRED APPROXIMATELY 3:45 PM
