Changing cutting blades on W-900 loader when the bolt started to spin EE put EE's hand on the bolt to push it back up in the socket while the bolt was turning and this resulted in a cut to EE's palm.

Over-exertion in lifting objects

Lifting and carrying cardboard boxes of misc. items during cleaning of an office area. EE did not immediately feel pain to EE's lower back during the shift on Thursday. EE felt discomfort on Friday, next day but didn't report to TA supervisor until the following Monday. EE went to the doctor on October 27th and reported on Monday the 30th EE is assigned modified duty.

Mechanic moved a D-11 dozer to a loading ramp for relocation into the mine work area. While dismounting the dozer, using foot cleats and grab handles on the way down, and on the final step onto the ground, the mechanic felt EE's ankle roll and made a ""pop"" sound. The ground was uneven below vegetation where the mechanic stepped off the dozer.

Fall from machine

Heavy hauler operator fell from Caterpillar 777 cab access ladder while descending from the machine. Operator missed second step of ladder when operator let go of the hand railing and fell to the ground.

Struck by falling object

Mechanic was installing belly pan on a D-10 dozer. While installing a bolt using an impact wrench, resistance was felt before bolt was seated so the bolt was backed out. When mechanic lowered wrench the bolt dropped down and hit EE's lip causing a split on the inside. One stitch was required as a result of the incident.

While cleaning the head of the dredge with a pry bar EE felt a pulling and burning sensation in EE's lower back

Scrap contractor was cutting scrap at mine upper laydown area during normal day shift hours. Dry vegetation, scrap wood and the ends of stockpiled power poles were ignited and reported after the end of the shift by a Coalvew contractor employee working on Pond 3C fine coal recovery operations.

Employee was walking from his service truck to a haul truck to disconnect fuel hose. Employee felt knee go out from under him.

Employee was removing a bushing from a shaft and the bushing quit moving. He then used a screwdriver to pry on the bushing when it suddenly broke free and slid down the shaft and pinched his finger between the bushing and the manlift he was standing in causing cut to his right index finger. He was taken by private vehicle to the emergency room for evaluation and received 1 stich.

Employee was dumping a trash bin into a dumpster. When he reached for the chain which attaches the trash bin to the forklift a bee that was nesting under angle iron by the chain stung him on the right hand. Employee was transported to local doctor by supervisor and was treated and released. No lost time or restricted duty.

Assumed Bird strike into power line. Bird caught on fire and dropped to the ground. Resultant fire burned approximately 2.5 acres. No injury or Illness resulted from fighting fire.

Employee completed chaining down a D11 dozer to the lowboy. Employee stepped across the 30"" wide by 4' deep space between the lowboy and the loading ramp. Employee's right foot slipped and as he fell his left foot stayed on the loading ramp. Employee suffered tore ligaments, meniscus, and ACL. Employee is on light duty following accident.

Employee was installing a lifting hook onto a crane hook fitted with a shackle. He did not lower the crane hook as far as he should which resulted in manualy lifting the hook hook higher than he should. As a result he placed his left hand in a pinchpoint on the shackle when he dropped the hook into place.

EE was using a high torque on the D11 performing final torque. He was doing final torque on segment bolts doing 120 deg turn when reaction arm pinched the tip of his right index finger between reaction arm & hub. This pinched & severed the end tip of his right hand index finger.

A chunk of material came off of the shovel bucket and landed on the headache rack of the haul truck. Driver later complained of neck pain. Seen in the ER and was diagnosed with a muscle strain, placed on restricted duty.

Employee received a laceration which required stitches to his right index finger, from the lid on a can of soup while attempting to open it with a pocket knife.

Employee was welding the floor plate on Komatsu truck box. While he was chipping slag off of a welded area with a chipper, he felt something go into his right eye underneath his glasses and face shield. Seen by MD and had a small piece of metal removed from eye.

Employee was greasing the tail pulley bearing on conveyor in processing plant. A small amount of fine coal dust from conveyor got into eyes. Imbedded piece of fine coal removed from eye at hospital.

Grader drove over a large rock at the edge of a cut. Operator complained of neck and back pain. Transported to hospital for evaluation of back and neck strain, began missing time on 10/03/06.

Employee felt pain in low back when lifting hood of lube service truck for a maintenance check. Began missing time the next day due to lumbar pain.

On 09/20/06 worker was bit by unkown type of insect under the jaw causing redness, swelling and pain at site. Employee was taken to MD and treated for localized reaction. On 09/25/06 followed up with MD due to increased swelling, redness, drainage at site. Treatment included wound debridement, antibiotics and light duty.

Employee was tightening a head bolt on a drill using an allen wrench and hammer. Hammer bounced off of steel rack hitting employee's knuckle of his left index finger causing a laceration that required stitches.

Terex truck driver attempted to raise bed of truck to dump load. Truck bed would only raise slightly, before dropping back down. Attempted several times to raise bed before calling for assistance, which caused jarring of the truck. Later in the shift employee began to complain of back pain. Seen in ER and diagnosed with a back strain, placed on restricted duty.

EE was driving haul truck, hit a bad spot (hole) in haul road. Right hand operators cab window broke & shattered. The ee was covered with broken glass particles w/some particles entering right eye. EE was taken to hospital ER, eye was cleaned of glass & released. He received additional medical treatment in proceeding days; prescribed antibiotics, pain meds, restricted duty.

EE was operating mine haul truck when he felt his ankle & foot swell. No spider was observed @ that time.EE was seen @ local hospital ER for treatment EE was given prescription for spider bite @ antihistamines, ibuprofen, & antibotic. EE was released for full duty.

Employee was removing the dozer push arm from D8N dozer. He lifted a 1"" air impact wrench at the same time he was twisting his body. Employee felt some lower back discomfort but continued to work on dozer arm. At start of shift next day, he reported heavy lower back pain. Employee seen at local hospital, treated, needs light duty.

Employee was hooking up a pipe at pond 3E pumping station, stepped over a pipe onto a rock causing left ankle to twist. Seen MD on 08/07/06 and was diagnosed with a left ankle fracture and placed on restricted duty for 3 weeks.

When exiting cab of 777 haul truck at wash rack employee thinks boot lace caught on something causing employee to fall from truck. Treated at hospital for a concussion, right eye abrasion with hematoma, small laceration to back of head and a right elbow contusion.

Employee was crossing a dump location at approximately 2 - 3mph in haul truck. When driving over a rough spot in road, he felt a sudden onset of neck pain and later developed numbness in first 3 fingers of right hand and pain down his right leg. Employee was transported to hospital for evaluation.

Employee was moving a man-lift down haul road. Man-lift struck a pot hole causing bucket of man-lift to jar employee, who then fell striking knee on deadman switch cover, causing a laceration to right knee which required stitches. Employee was placed on restricted duty.

Employee was attemting to repair a headlight problem on unit 3056. While in the process of cutting the boot off the back of the headlight, his pocket knife slipped and cut the side of his left thumb. Laceration required stitches.

Employee was operating a scraper going down grade in cut area. Cushion hitch of scraper was turned off as scraper entered cut down grade. Floor of cut area was slippery as scraper entered cut; machine slid into rough area. Employee strained lower back when traveling along rough area. Local ER @ hospital did evaluation & 2 days restricted duty then full duty.

Employee descended boarding ladder on the front end loader and pivoted to the left. Felt a sudden onset of pain in the right knee. This became reportable on October 28, 2005 when employee had laproscopic surgery for a torn meniscus; began missing time.

EMPLOYEE WAS STRUCK IN THE ARM WITH A SLIVER OF METAL WHICH ORIGINATED FROM THE REMOVAL OF A BEARING RACE WHICH WAS BEING WORKED ON APPROX. 8' AWAY USING A DRIFT PUNCH. EE WAS SEEN BY MD AND THE SLIVER OF METAL WAS REMOVED FROM HIS FOREARM.

Employee was getting into bucket of bucket truck. When he attempted to step into the bucket, he alleges twisting his left knee. This became reportable when employee underwent orthoscopic surgery to repair meniscus on 4/8/05.

A 10' HQ core barrel full of core was being lowered onto the core horse when the spearhead broke. The barrel fell approx 4' contacting an empty 10' core barrel. The end of the full core barrel raised up and struck EE on her right side of bottom of rib cage. Tenderness at site, EE placed on restricted duty.

Employee has had a history of spinal sterosis w/degenerative disc disease of his neck & underwent a multi-level cervical fusion in 1999. EE has had ongoing problems with pain & numbness, both arms. EE started taking sick leave on 11/04/04. On 12/20/04 we were notified by the state the EE alleges time off was from an incident which occured on 10/04/04 from pulling on a wrench.
