Received a CWP diagnosis for employee on 3/27/2026.

Struck by falling object

Employee was finishing EE's inside bolt when EE went to drop EE's canopy. The canopy post collar dropped down striking EE's right pointer finger. This resulted in a small distal fracture of the right pointer finger.

Employee was attempting to slide the forks together on the forklift when EE lost EE's footing causing EE to mash EE's left index finger and middle finger resulting in a fracture and lacerations. 3 sutures in EE's index finger and 2 in EE's middle finger.

Employee was in the process of loosening a hydraulic hose on a low trac on the 004 MMU. While loosening the hose the pallet forks leaked off squeezing EE's head against the tilt arm of the low trac and the low trac frame. This resulted in 6 stitches.

Employee was sitting in the rear seat of the mantrip. During investigation it was determined EE re-adjusted EE's position in the seat and contacted a bolt installed in the mine roof.

Struck by falling object

Hogg Contractor bent over to pick up hand tools. After EE stood up, a piece of draw rock dislodged from the roof falling striking EE on EE's right foot. This resulted in a fracture to the #3 and #4 metatarsals.

Employee stated EE had unhooked a chain from the MRS on a LoBoy. When EE attempted to step off the LoBoy, EE's foot hit the end of the ramp causing EE to twist EE's left knee. Employee had surgery on 1/21/26 and started PT on 1/22/26. No lost days.

Employee stated they were carrying a box of foam sealant when they stepped on some broken pieces of block, lost footing and slipped resulting in pain in left knee and right hamstring. They continued to work until 11/13/25.

Employee loaded roof straps onto scoop. Once loaded EE took gloves off to steps them down. EE grabbed wire to tie down straps and EE's left hand slipped back causing EE to hit hand and pinky finger which resulted in a laceration to EE's pinky finger.

Employee was tramming roof bolter from the #2 heading to out by the LOCC. EE was walking backwards and holding drill cable slack pulling it back as EE walked and trammed the drill. EE stated EE stumbled over loose gob and catching EE's balance EE stepped on a ledge on corner of the rib at the intersection twisting EE's right knee.

Employee was attempting to remove the side guards off the run through and smashed EE's right middle finger between the guard and the frame of the run through. This resulted in a closed displaced fracture and stitches.

Employee stated EE was in the process of pushing a roof bolt into the hole. While pushing the bolt into the hole EE felt a pop in EE's right shoulder. EE continued to work until 7/16/25 when taken off work by a doctor.

Over-exertion in pulling or pushing objects

Employee was pulling/dragging an old cat idler away from the pan of the miner when EE felt a burning sensation in EE's right knee. Continued to work until 6/27/25 when taken off by a doctor.

Employee was holding cable over for drill in #5 while drill partner trammed drill into #5 behind drill bolting #5 to get drill out of miner's path of travel. Employee tramming drill not did see employee holding cable over and caught EE between the drill and coal rib. Left abdomen and left hand were the injured.

Employee alleged they were sitting on mantrip and raised up to adjust their belt. Through investigation, it was determined the employee was positioned on their knees and raised up to get their dinner bucket when they came into contact with the metal beam, resulting in a laceration requiring 4 stitches.

Employee stated they asked their bolt partner to hand them a slate bar to pull a piece of draw rock. During transition from one employee to another, the injured employee did not grasp the slate bar and it hit them in their front tooth, chipping it. Employee had the tooth temporarily repaired on 3/17/25.

Drill steel was bent, rotation was hit causing steel to break. It spun around and cut employee's right forearm. This resulted in a laceration requiring stitches.

Employee stated employee was pushing a bolt up. Employee had ahold of the pan and it caught the edge of a brow pushing the plate into employee's right hand. This resulted in a laceration to employee's right index finger requiring stitches.

Fall from machine

Employee was a passenger (driver's side rear) on a 4-man mantrip traveling outside at the end of shift. The mantrip hit a hole in the road, measuring 36"" wide x 36"" long and 2-3"" in depth, in the travel way and the employee shouted to stop. When the mantrip stopped the employee fell out of the ride. The employee complained of lower back pain, and numbness in left leg.

Employee was getting on forklift to check diesel fuel level when EE's foot slipped out from under EE causing EE to fall and twist right knee.

A non-injury roof fall that measured 25'Long x 18'Wide x 12'High occurred on Bear Branch Panel 4 in the #7entry at crosscut 26. The fall was isolated to the #7entry at the active pillar line. The release of supported roof was due to an overlying undetected slip above the anchorage zone of the primary supports installed.

Employee stepped off back of drill with left foot first when EE heard EE's ankle pop. Upon putting EE's right foot down, EE curled it. Resulted in a closed fifth metatarsal fracture in the left foot.

Employee was adjusting the width on the fork spacing to pick up a pallet of block with fork lift on lo-trac. When sliding fork in, EE caught EE's left ring finger between the fork and the frame of the fork lift on the lo-trac. This resulted in 3 stitches.

Employee was moving curtain from one rib to another when employee tripped and fell back on right wrist. This resulted in a fracture.

Employee was installing first bolt on the second row of bolts in the 4L punch through. EE had drilled the first piece of steel and connected the second piece of steel. While drilling the second piece of steel, a piece of draw rock fell and hit the drill pot then struck the employee in the mouth resulting in 3 stitches.

Employee stated EE was boosting a diesel scoop off. After getting it started, EE went to slide battery tray back in when EE stated EE felt a pain in EE's back. Lost time as of 8/30/24.

Employee was starting to drill center bolt on first row of cut. Employee started drilling hole and stated EE drilled approx. 1.5"" in roof. This caused a piece of rock measuring 48"" long x 20"" wide x 0-1.75"" thick to start spinning its way down drill steel striking EE in the side of the nose and below left eye.

Employee stated employee had pulled the drill in between #3 and #4 xcut when employee went to install the pogo. The one beside of it approx. 4' apart came loose and fell down striking employee on the bridge of the nose resulting in a fracture.

Employee had loaded roof glue into the tray of the roof bolter and stepped down into the bolt station when a piece of draw rock fell onto the canopy and part of the rock hit EE on the left calf. EE did not receive medical treatment at the ER other than cleaning and a Bandage. EE began days away from work on 6/24/24 due to a pocket of infection in EE's calf.

Employee stated while EE was removing the drill steel after drilling a rib bolt, EE's bolt partner applied pressure to the drill head causing the drill to shift, smashing EE's right middle finger between the drill steel coupling. This resulted in a fracture and stitches.

Employee was carrying 2 bundles of rope bolt plates from scoop to load on roof bolter. EE tripped on power supply cable for roof bolter causing EE to fall into the walkway of the roof bolter. The plates EE was carrying caught EE's middle finger on left hand between plates and floor of walkway. This resulted in a comminuted displaced fracture to EE's middle finger.

Contractor was riding in mantrip. EE got up on knees to adjust SCSR and to put coat on and grabbed side of mantrip. Mantrip was pulling out and smashed EE's hand between the mantrip and rib on the corner.

Employee was making up a bolt for a red hat. When EE went to hand the bolt to the red hat, EE placed hand on the top of the fan and a piece of draw rock fell striking EE on top of the left hand. EE continued to work until 4/26/24.

Employee was in the process of drilling a 16' hole for a cable bolt. Employee was attempting to line the steel up with the chuck. The employee bumped the rotation, at which time employee's glove got caught between the drill steel and chuck resulting in a dislocation to the middle finger and fracture to the ring finger on the left hand.

A non-injury roof fall that measured 20' long, 20' wide, and 12' high occurred on the 003 MMU, Ramsey Panel 11, #5 entry, LOCC. The fall was isolated to the intersection of the #5 entry at break #40. Entries to the fall area are dangered off and supported. The roof fall will be cleaned up.

Struck by falling object

Employee was operating the miner in the last left lift on the pillar section. A portion of the right rib dislodged and struck the employee on the back of the right leg below the knee resulting in a fracture.

A non-injury roof fall measuring 45' long, 25' wide, and 14' high occurred on the 003 MMU, Ramsey Panel 11, primary escape way. The fall was isolated to the intersection of the #3 entry at brk #25. Entries to the fall area are dangered off and supported. The primary escape way has been re-established in the #2 entry at brk #23. The roof fall will not be cleaned up at this time.

Employee states while pulling a cable bolt off drill to be installed, EE strained EE's back.

Struck by falling object

Employee was getting ready to start a cable bolt hole. They had drill steel pressed against the top and when they applied pressure a piece of draw rock fell from the top smashing their left middle finger between the drill pot and draw rock. This resulted in a fracture and 6 stitches.

Employee stuck a finger between a moving roller and a sensor mounted on structure. This resulted in a fracture to the left index finger and a laceration requiring 5 stitches.

Struck by powered moving object

Employee pulled 12-man mantrip up to intersection to hook up cable and didn't set park brake or turn the ride off. Employee got in front of ride with back toward the ride. The ride began to roll and contacted lower part of left leg resulting in a fracture.

EE was helping miner move when EE saw a loose piece of rock. When EE went to scale it down, EE tripped over some loose coal & rock lying on the ground hurting EE's left knee. EE was given a knee brace. EE continued to work until 11/9/23 when EE had surgery.

A side lid came off in the cut getting wedged in between the miner and rub rail. Employee was removing the pin that holds the rub rail on. When EE got it free, the rub rail sprung out striking EE's leg causing it to twist. This resulted in a fracture.

Employee was moving a mantrip and as they exited the permissible ride they let go of the steering wheel. When they let go, the steering knob on the steering wheel swung around and struck their right index finger. This resulted in a fracture.

The release of supported roof was due to an overlying undetected separation above the anchorage of the primary support installed.

Employee was drilling the 2nd bolt in 2nd row of cut in #2 entry. EE finished drilling hole and brought steel down and it wouldn't separate at coupling. EE held starter in left hand and flipped pusher with right hand. The steel separated and pusher struck EE in the forehead above left eyebrow. This resulted in a laceration and 6 sutures.

Employee was using a 4 pound hammer trying to take link off the conveyor chain of the continuous miner when a small piece of metal broke loose from the link and penetrated EE's right leg approx. 2"" above the knee.

Contractor was in the process of hanging curtain with a pogo stick when an undetected piece of draw rock fell from the mine roof striking them resulting in a fracture to their back.

Contractor stated that they raised the lid on scoop batteries and when they went to let them down, they wouldn't close. Another employee raised the lid back up and when EE did EE stuck EE's hand in to check latch. The lid slipped out of the other employees hand and fell on EE's left hand resulting in fractures to EE's left index, middle and ring fingers.

A non-injury roof fall occurred on Northeast Mains, #3 entry, Break #26. The fall was isolated to the intersection of #3 entry intersection extending approx. 8' into the left xcut toward #2. The entries to the fall area are dangered off and supported. The lifeline has been re-established in the #4 entry and a cleanup plan has been developed to clean up the roof fall.

EE stated EE was spotting a roof bolt. The EE placed the new bolt in the hole. EE was holding the new bolt, plate and pan against the bottom of the damaged plate and pan to attempt to push them both to the roof with the new bolt. When the drill chuck contacted the new bolt, The EE's hand was pinched resulting in a laceration to EE's right hand and 4 sutures.

Employee was assisting bolting partner retrieve the steel out of a rib bolt hole when a piece of loose rib fell and struck EE's thumb against the drill mast. This resulted in 13 sutures.

Employee was in the process of exiting scoop, right foot was on the ground and when employee placed left foot on the ground employee felt pain in lower back.

Employee went to the back of drill and picked up approx. 20 pans. Employee turned to walk to the front of the drill and when they turned they felt a pain in left shoulder. Employee worked up until June 3, 2021 when taken off work.

Employee states: was bringing steel out of hole when employee got down to the last two pieces employee pushed up and jerked down when the steel came out and caught employee's right pinky finger between two pieces of steel. This resulted in a fracture and sutures.

Employee was drilling a hole to install a cable bolt in the last row of the cut. They reached to take the 3rd piece of steel out of the chuck. When they put their hand out to take the piece of steel from the drill chuck, the 2nd and 1st piece of steel became unstuck and fell from the hole striking them on the right 5th finger resulting in a laceration and fracture.

Employee was in the process of scaling draw rock in the intersection of the #4 entry when an undetected piece of draw rock dislodged from above a piece of wire mesh and fell striking the employee on the head, right hand and right leg. This resulted in stitches/staples to the hand and head and a fracture to the leg.

Employee had installed inside bolt of second row and was in the process of drilling the hole for EE's middle bolt. EE's steel had stopped up and EE removed the bottom piece to clear it out. EE re-installed EE's steel and was pushing it up to finish drilling when a rock struck EE on top of this left forearm resulting in a laceration and sutures.

Employee was installing rib bolt and had left hand resting on the bottom of the outer mast. EE dropped the mast down and left middle finger got pinched between the inner and outer mast at the bottom. This resulted in a fracture and sutures.

Employee was putting last piece of steel together and pushed up on the chuck. The steel became fouled against lip on chuck causing it to bow, dislodge and strike them on the web of left hand resulting in a fracture.

Employee bent over to pick up a pogo stick laying on the ground near the installed vent curtain. A piece of draw rock dislodged from the mine roof with height of 8'2"" hitting employee's right hand resulting in amputation to the right thumb.

The mine operator is submitting this 7000-1 for an alleged water inundation under duress due to citation No. 9210702 being issued for 50.10 on November 19, 2020 and in order to comply with MSHA regulation 50.20. On November 16, 2020 at approx. 6:05 pm the 004-0 MMU continuous mining unit intersected a 3"" core hole and water entered the mine.

Employee thought that had lost suction to the drill head and EE placed hand on the drill chuck to check for suction. When EE placed hand over the chuck, EE's drill steel that was left hanging in the hole broke free and fell hitting EE's finger against the drill chuck. This resulted in a displaced fracture to EE's left ring finger.

Employee was standing on EE's platform of the drill, swung out to put up EE's middle bolt. The platform hit a rock and pushed EE up into EE's canopy, causing pain in EE's upper back.

Employee was loading bag dust from a pallet when EE alleged that EE felt pain in lower back.

Struck by falling object

EE was operating a mini trac lifting the belt up to prep for a move. The suspended belt dropped approx. 16"" when the machine pushed on a rope attached to the belt. The dropping belt made contact with the EE's head in a sitting position on the lo trac and pushed EE forward suddenly which resulted in a hyperextension and displacement and fracture of #4 vertebra.

Employee stated EE was in the process of lifting the forks up on the AL Lee mini trac for the purpose of adjusting them to make a wide base for moving materials. As EE lifted the fork, EE moved it too far up pinching EE's left index finger in the slide and top of the fork base resulting in a laceration requiring 4 stitches.

A non-injury roof fall that measured 50' long 19' wide and 10' high occurred on Smith Ridge Panel 8, in the #6 Entry at break 29. The fall was isolated to the intersection of break 29 in between the block. Minimal damage other than the impedance of travel in the single entry in the immediate area. The area was dangered off and standing cribs were installed.

Employee was loading top roller on trailer and stepped in a hole rolling EE's ankle causing EE to fall. Became LT on 7/10/20.

Employee was in the process of changing batteries and was connecting the battery bell to the charger when they felt heat in their glove from an arc that resulted in a minor burn to fingers. Employee worked until 4/30/2020.

Employee was in the process of pulling the curtain out of the #3 heading and thought all pogo sticks had been removed out of the curtain. When employee pulled the curtain towards self, a loose pogo stick hit employee in the mouth which resulted in a cut to the upper lip requiring stitches.

Employee was in the process of turning a left hand x-cut on the second lift when a piece of rock slid down out from the flush face, hitting the ground and flipping over striking the employee and knocking EE to the ground. The employee landed on material on the mine floor and sustained a non-displaced fracture in the upper part of the right fibula.

Employee stated as EE was in the process of moving a piece of draw rock that EE had just pulled approx. 4' on the mine floor EE felt pain in EE's lower left abdomen. EE continued to work the remainder of the shift. The rock was 39"" long x 14"" wide by 6"" thick. Employee continued to work until 2/24/20.

Employee alleged to have an on the job injury while EE was in the process of stacking block on a pallet and felt pain in the left groin area.

EE stated EE and 2 other employees were sitting an SCSR box on a shelf. The box became stuck and the EE was pulling the box while the other 2 employees were pushing. When the box freed up, it caught EE's right hand between the box and shelf. It caught EE's right middle, ring and pinky fingers. This resulted in a laceration/avulsion to the pinky finger that required 14 stitches.

Employee was operating right side of face bolter #24. EE had head rolled over to install rib bolt. Employee then rested left hand on the drill while EE retrieved steel from hole sliding the drill mast back pinching the left middle finger. This resulted in a fracture and 5 stitches.

Employee had finished drilling hole for a rope bolt. As EE reached to get rope bolt it became entangled. EE jerked on the bolt to get it loose resulting in pain in EE's lower back. EE was taken off work on 12/9/2019.

Employee was injured while trying to jack over a remote boom on an idle belt line while using a 6-ton bottle jack to apply pressure. The jack dislodged and came back striking employee in the mouth. The impact lacerated employee's upper lip requiring stitches and moved the front two teeth backwards.

Two employees went to drop the lid on a tool box, employee tried to catch a tube of grease that was falling from the box when a finger was caught between the lid and the side of the tool box. After the initial treatment for a laceration, the ER Physician referred them to be evaluated by an orthopedic surgeon. The first joint of the right index finger was removed by the surgeon.

Contractor stated was riding out of the mine at the end of the shift. When the man trip turned a corner on the travel way it must have hit a hole in the roadway raising the contractor up and the contractor moved forward hitting head of the latch for the man trip stored SCSR cache. This resulted in a laceration to the forehead that required stitches.

Employee states that while traveling the 2A belt on EE's exam EE was looking for draw rock when EE's mantrip bounced EE and EE felt pain in EE's lower back. Management traveled the same area right after the incident and found no deformities that would cause EE to bounce.

Employee was in the 4 left break loading out shot up rock and trimming rib line when a piece of unknown debris was thrown from the cutter head of the miner striking EE in the face causing a laceration below the left side lower lip requiring stitches.

Employee had just completed installing rib bolts when EE traveled back to the area of the drill inch control tram levers. EE stated EE went to bend down under a piece of wire mesh that was bolted to the top. The top of the wire mesh was hanging down approx. 4"" in the area & caught the top portion of EE's right ear lobe resulting in a laceration. The cut was cleaned & closed.

Contractor was removing steel from the coal rib. While EE was bringing the steel out, EE pulled the mast back towards EE to reconnect the steel and felt sharp pain in the middle finger on the left hand. The bottom of the mast hit EE's finger lacerating it against the back of the frame of the slide. This resulted in 10 stitches to close the laceration.

Employee was in the process of squaring up a rock brow in order to drill for a shot after the coal had been undercut. As EE started to shear down in the rock, a piece of metal (carbon) projected back down the entry towards EE. It struck EE approx. 2"" above the left eye above EE's eyebrow resulting in a laceration requiring 3 stitches.

Employee was retrieving steel out of a cable bolt hole when the steel hung up in the hole. While trying to retrieve the hung steel from the hole, the employee came in contact with the drill steel connection and became entangled in the steel while it was rotating which resulted in the amputation of left arm below the elbow.

Minimal damage other than the loss of approx. 60' of belt, belt structure and control line in the immediate area. The area was cleaned up and the belt restored.

Employee had stepped on top of a covered duct near the fan turbine shaft near an overhead metal grating. EE reached up to hold the edge of the metal grating to step down slipping backwards catching wedding band on a 1/8"" tip of metal that was on the grating. The ring due to the downward force created an avulsion type wound to left ring finger. This resulted in stitches.

Operator of man trip was following a diesel scoop when the lights in the rearward position on the scoop blinded the operator of the man trip causing EE to steer to the right striking the rib. Employee was loading structure. As EE loaded the first piece of top structure EE felt a burning pain in EE's back. EE stopped and waited a minute then loaded 3 more top structure and then went back to the #3 section to try and walk around to get some relief but the pain got worse.

Employee was in the process of changing the hydraulic pump on a shuttle car in the shop. While pulling hydraulic hoses to be able to access the pump, EE felt a strain in EE's right shoulder down to EE's elbow. EE continues to work and started physical therapy on 3/7/2018.

A non-injury ignition occurred at this mine in the face of the #7 heading. After investigation, it was determined that the machine had 16 dull or worn bits out of the 71 available. Due to the worn bits coming into contact with the sandstone intrusion in the face of the #7 heading, it is believed to have resulted in an ignition.

The injured miner did not lose consciousness and was able to call for help, but was treated for Compartment syndrome of the left lower extremity, and a Left hairline fracture to the fibula.

Employee started to step down from a ladder The ladder shifted and pulled rivets from the inside of the ladder that did not appear to have been previous damage. The employee fell from the second step down to the mine floor and injured the right ankle. Employee worked the remainder of the shift with no issues but was later taken off work due to swelling.

Employee stated that EE was in the process of moving a tool box that had a weight of approximately 45 lbs. to set it on a tool sled when EE felt pain in EE's lower back. This was a non-witnessed event that occurred off of the section. The employee has had a medical history with EE's back in this region and had been treated in the past for similar episodes.

A pillar fall occurred on the active 002 Pillar Section that entrapped a Mobile Roof Support. Two MRS units were located in the No. 2 Entry and were blocked when rock fell from the left pillar lift and some of the material slid out in front of the MRS units. One of the MRS Units (# 3 MRS) was retrieved and the other (#4 MRS) was entrapped when the roof in the pillar line fell in.

Operator proceeded to flip extension to break it loose from finisher. Didn't realize it had already come apart, ee pulled the extension toward ee and struck self in the mouth, chipping two front teeth.

Employee had pulled walk thru roof bolter in #6 entry to bolt and set the T-bar on the roof bolter. The employee had grabbed a couple of bolts to lay in the bolt tray. EE placed the bolts down in the tray when a piece of undetected draw rock fell striking EE on the right ring finger resulting in a fracture to the end of the finger and required stitches.

Employee was helping pull the head shaft out of the feeder. The right side slid and came out sooner than the left side causing the right side to drop suddenly and the left side to rise up. The employee pulled the spanner wrench toward EE to loosen because of a metal guard that was on the back of the power center. When the coupling broke loose the end of the spanner wrench came back and struck EE in the mouth knocking EE's right front tooth out.
