Struck against a moving object

Torque Contractor was coming off dump hill and hit jake brake. Acted like it went from 3rd stage to 1st stage and jerked EE's neck. First day missed was 12/10/2024.

Employee was climbing down off dozer. The ground was further down than EE thought. EE slipped down while still holding onto the push horn with right hand. This resulted in a nondisplaced fracture of coronoid process of right ulna.

Struck by rolling or sliding object

Employee was welding on excavator bucket. The bucket had been removed from the machine. While welding the shank onto the bucket, the bucket rolled toward EE pinning both feet. This resulted in a fracture of the 5th metatarsal on EE's left foot.

Emp. working on excavator wiring issue. Stepped on roller frame, placed knee on track, knee popped. felt slight pain in knee rest of day. Next AM EE's knee was sore, weight sensitive. Called supervisor, Work Partner said put ice on knee and take ibuprofen. Injury did not result in restricted work days or days away from work. Carter was notified 10/2/23 of upcoming surgery.

Technician was working block time for customer, and was installing transmission in a 993k loader. Technician was exiting transmission area on the loader when EE's left foot slipped and caused EE's body weight to transfer to left arm. Tech felt pain in the bicep.

Struck by flying object

Employee was welding retaining tabs on a drill head output shaft and drill sub. Employee tacked a 2"" x 4"" x 1/4"" piece of metal onto the output shaft. Employee began hammering the metal to bend it around the shaft. The tack broke allowing the metal to fly, striking employee in the face knocking out teeth.

Technician was using a 30 ton hydraulic ram to break the inside tire bead on the RRI of a 785 rigid frame haul truck. When the pressure was released from the ram it fell to the ground then bouncing crushing technician's thumb on left hand against the hydraulic pump.

Employee was removing shims in a wheel unit off of a 785 haul truck. While removing the shims, one slipped falling across EE's fingers resulting in lacerations and stitches.

Operator was carrying a large rock out of pit. The loader turned down grade and rear came off the ground causing rock to come out of bucket which resulted in loader bouncing and bouncing the operator in the cab causing excessive strain on EE's left shoulder.

During meeting the phone rang and EE stepped out to take the call. As EE was walking down the steps, EE put EE's weight on EE's left leg and felt pain in EE's lower back and tingling in EE's left leg from EE's knee down.

Field technician was cutting a bushing from a 993K frame using a torch. While cutting, a piece of hot metal dropped onto the test port hose for the brake accumulator. The hot metal burned the hose which discharged the oil. The oil sprayed onto the cutting torch which immediately flamed up, causing a burn to the side of the technician's left face and ear

Drill steel fell out of rack and hit employee on the left side pushing his right side into a piece of pipe. He was tying his boom to the steel to secure it from falling to be welded. This resulted in 1 fractured rib.

Mechanic was doing a spot check of steering components on a 785 Caterpillar hauler. The mechanic was making his exit from underneath the hauler when the driver turned the steering and caught the mechanic in a pinch point between the hauler tire and the fuel tank of the hauler. Injury resulted in a multiple clavicle fracture with both lungs partially collapsed.

Performing 1000 hours service on 345 excavator. EE attempted to climb on to the machine and due to rainy & muddy conditions, his foot slipped off the step causing him to slip and twist his left shoulder while holding onto the handrail. Pain in shoulder got much worse on 3/2 and ee sought treatment.

Employee was cutting a tree that had a grapevine in the top, when the tree started falling the grapevine redirected it toward the cutter, striking him. EE sustained a dislocated shoulder and a small cut above the eye.

Fall from machine

Injured climbing up the ladder of the loader, when his right hand slipped off the ladder causing him to swing ground and fall from the loader hitting on his butt and then backwards causing his head to strike the bottom edge of the hyralic tank.
