THE EMPLOYEE WAS RIDING ON A RUBBER TIRED PERSONNEL TRANSPORTER HE SAT UP TO TIE HIS BOOTLACE AND STRUCK HIS HEAD ON THE HEAD OF A CABLE BOLT. THIS CAUSED HIS HARD HAT TO STRIKE HIM ON THE BRIDGE OF HIS NOSE RESULTING IN A FRACTURE.

The injured and another person were installing conveyor belt structure. The two men lifted a bottom roller and were putting it in position when the injured's left knee buckled.

A lift was bein mined from a pillar on the 2-left off 2 south panel when a piece of rock fell on the head of the mining machine and fouled the machine. A retriever was hooked to the machine and the machine was free in approximately 10 min. A rock fell inby the left row of roof bolts and no belts were damaged. This report is submitted in protest to satisfy a citation.

The individual was cleaning the water filter at the end of the hard line. While working on the filter, he caught his finger between the water filter and the conveyor belt tail piece resulting in a laceration to his left middle finger.

A roo ffall occurred in the no. 4 belt entry of no. 1 north (2 breaks inby survey station no. 163. the fall measured approx. 45' long by 20' to 35' wide x 7' high. the area was mined in 1948 and is approx. 4 miles outby the altrp working section. the fall will be clean up. there were no injuries.

A ROOF FALL WAS DISCOVERED IN THE NO. 4 BELT ENTRY OF NO. 1 NORTH (1 BK INBY SURVEY STATION NO. 163). THE FALL MEASURED APPROXIMATELY 40' LONG X 20' WIDE X 8' HIGH. THIS AREA WAS MINED IN 1998 AND IS APPROXIMATELY 4 MILES OUTBY THE ACTIVE FACE. THE FALL CAUGHT THE BELT CONVEYOR FOR THE MINE AND WILL BE CLEANED UP. THERE WERE NO INJURIES INVOLVED.

THE EMPLOYEE WAS HANGING CABLE FOR THE ROOF BOLTING MACHINE WHEN HE FELT PAIN IN HIS RIGHT SHOULDER. HE CONTINUED TO WORK HIS NORMAL JOB BUT CONTINUED TO EXPERIENCE PAIN IN HIS SHOULDER. HE STARTED MISSING WORK ON 12-5-2005.

Employee was cleaning belt line when his boots stuck in the mud. he slipped and fell against belt then fell to the ground hitting a piece of structure. Employee broke a rib and fractured shoulder.

EE was hanging a check curtain when a piece of draw rock fell fromt he mine roof striking him on his legs.

EE WAS TRAMMING A RUBBER-TIRED UTILITY VEHICLE AND DROVE OVER A 4'-LONG METAL ROOF JACK. THE METAL ROOF JACK FLIPPED UP AND STRUCK THE EE ON THE HEAD, RESULTING IN A MILD CONCUSSION. An investigation revealed that the alleged injury was falsified by the employee.

A FALL WAS DISCOVERED IN THE #2 ENTRY OF 2 RIGHT OFF 2 MAINS WEST ABOUT 20' INBY SURVEY STATION 920. THE FALL MEASURED 60' LONG X 6' HIGH X 15' WIDE. THERE WERE NO INJURIES AND NO EQUIPMENT WAS INVOLVED. VENTILATION WAS NOT AFFECTED. THE FALL IS APPROX. 15,000 FEET OUTBY THE ACTIVE SECTION.

A ROOF FALL WAS DISCOVERED IN THE NO.2 ENTRY OF NO.1 NORTH OUTBY SURVEY STATION NO.1209. THE FALL MEASURED 55'LONG X 15'WIDE X 4'HIGH. THE FALL IS APPROX 3 MILES OUTBY THE ACTIVE SECTION AND WILL NOT BE CLEANED UP. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED.

Employee did not release the brake on the scoop he was operating causing the brake to get hot and smoke.

ROCK FELL BETWEEN RIB AND BOLTS HITTING EE ON BACK, KNOCKING HIM DOWN. HE FELL ON HIS SRU.

A CONT MINING MACHINE WAS BEING USED TO MINE PILLARS ON THE MMU 001, A SOUTH PANEL. THE SECOND PILLAR CUT OUT OF THE NO.5 ENTRY. A PILLAR FELL AND CAUGHT THE MACHINE. THE FALL MEASURED APPROX 30' LONG BY 20' WIDE BY 44' THICK. A RETRIEVER WAS USED TO RECOVER THE MINING MACHINE.

DURING THE WEEKLY EXAMINATION A ROOF FALL WAS DISCOVERED IN THE NO. 1 NORTH RETURN, 1 BREAK INBY SURVEY STATION NO. 669. THE FALL MEASURED APPROXIMATELY 20' LONG BY 20' WIDE BY 4-1/2' HIGH AND IS LOCATED 2-1/2 MILES OUTBY THE WORKING SECTION. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED. VENTILATION WAS NOT AFFECTED.

A ROOF FALL WAS DISCOVERED IN THE NO.6 ENTRY 30' OUTBY SURVEY STATION NO. 2884 OF 2 RIGHT OFF A NORTH ABOUT 500 FEET OUTBY THE MMU 001-0. THE FALL MEASURED 5' HIGH BY 18' WIDE BY 12' LONG. VEN TILATION WAS NOT AFFECTED. THERE WERE NO INJURIES AND THERE WAS NO EQUIPMENT INVOLVED.

EMPLOYEE WAS TAKING COVER OFF CONTROL PANEL. COVER FELL AGAINST SCOOP CAUSING INJURY TO FINGER.

A FALL WAS DISCOVERED DURING THE WEEKLY EXAMINATION OF THE 1 SOUTH PANEL AT SURVEY STATION 2500.THE FALL WAS NEAR THE TOP END OF THE PANEL IN THE #7 ENTRY. ABOUT 4,000' FROM THE ACTIVE SECTION . THE FALL MEASURED APPROX. 150'L X 20'W X 6'H. THERE WERE NO INJURIES OF EQUIPMENT INVOLVED.

A ROOF FALL WAS DISCOVERED DURING A PRE-SHIFT EXAMINATION OFF THE MMU 001-0 A SOUTH PANEL. THE FALL WAS IN THE NO.3 HEADING (S.S.2574) ABOUT 200' OUTBY THE FACE. THE FALL MEASURED APPROX. 70'L X 20'W X 8-10'H. THERE WERE NO INJURIES OR EQUIPMENT DAMAGED.

AN UNINTENTIONAL ROOF FALL WAS 30' INBY STATION SPAD 253L #1 ENTRY ON A SOUTH SECTION. FALL WAS 80' LONG, 20' WIDE &6' HIGH. IT WILL NOT BE CLEANED UP. IT WAS TIMBERED AND DANGERED OFF.

AN UNINTENTIONAL ROOF FALL WAS AT STATION SPAD (25 FT INBY SPAD 2497) #4 ENTRY. FALL WAS 20' LONG, 18' WIDE, 10-12' HIGH. THE AREA WAS BOLTED WITH 6' SUPER BOLTS WITH ADDITIONAL SUPPORTS INSTA LLED.

A UNINTENTIONAL ROOF FALL WAS AT STATION AND 1943 ABC MAIN WEST INTERSECTION ON BELT LINE. FALLW AS 20 FT LONG 18 FT WIDE 10 TO 12 FT HIGH. THE AREA HAD BEEN BOLTED WITH 6 FT SUPER BOLTS WITH ADDITIONAL SUPPORTS INSTALLED. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED.

EE WAS MAKING A SPLICE WHEN THE KNIFE SLIPPED CUTTING HIS L. THUMB.

EE HAD HAND ON DRILL HEAD, STARTED TO DRILL TOP.A ROCK FELL STRIKING HIS LEFT LITTLE FINGER.

EMPLOYEE PULLED PINNER INTO PLACE TO START BOLTING, WHEN HE PLACED THE T-BAR THE FIRST TIME HE NOTICED LOOSE ROCK. HE LET THE T-BAR BACK OFF, RELOCATED THE PINNER, THEN PUT THE T-BAR UP AGAINS T LOOSE ROCK CAUSING ROCK FROM RIB TO ROLL OFF AND HIT EMPLOYEES FOOT.

EE WAS DRILLING TOP WHEN SOMETHING HIT HIM IN THE LEFT EYE.

EE WAS SUPERVISING MINER WHEN ROCK ROLLED OFF OFRIB AND STRUCK HIM ON THE BACK OF HIS LEGS CAUSING HIM TO FALL INTO THE MINER AND BREAK HIS NOSE

EE WAS PULLING SHUTTLE CAR OFF FEEDER. THE SCOOPMOVED WHEN HE WAS HOOKING UP CHAIN & MASHED HIS LEFT INDEX FINGER.

EE WAS CUTTING A BAND OFF OF A BUNDLE OF BOLTS. THE BAND STRUCK HIS RIGHT HAND CAUSING A LACERATION. HAD TO HAVE SUTURES.

#1 ENTRY PILLAR WORK STARTING CUT OUT OF SOLID RIB HAD AROUND 4 FT OF COAL A CRACK CAME IN ROOFCAUSING A SLIP TO FALL OUT ACROSS MINE. 25'LONG 12FT WIDE, 5FT H

A BUNDLE OF ROOF BOLT PLATES FELL OFF ROOF DRILL. EE TRIED TO CATCH THE BUNDLE AND THE SHARP EDGE OF THE PLATES, CUT LEFT WRIST.

ROOF FALL ON NO2 SOUTHWEST BELT LINE. #4 ENTRY FALL WAS 120'L, 20'W & 6-8' HIGH. FALL WAS 140';OUTBY SPAD 1276 AND 30' INBY SPAD #1276. FALL WAS ABOVE ANCHORAGE AREA WAS BOLTED WITH 4' GHERE BOLTS

HE WAS DOING REPAIR WORK ON MAC 8 MANTRIP, USING AIR WRENCH; WRENCH SLIPPED, CATCHING HIS RIGHT THUMB BETWEEN WRENCH AND FRAME OF MAC. REQUIRED STITCHES.

THE INJURED PARKED #1 S/C IN THE #3 INTERSECTION TO HELP MOVE THE MINER CABLE OUT OF THE HAULAGE ROAD; AS HE WAS RETURNING TO HIS S/C, THE #2 S/C WAS APPROACHING HIM FROM THE X-CUT (BETWEEN #1 -3 ENTRIES). OPERATOR OF #2 CAR DIDN'T SEE THE VICTIM & RAN INTO THE #1 CAR, CATCHING THE VICTIM'S LOWER LEGS BETWEEN THE TWO SHUTTLE CARS.

FIRE BOSS DISCOVERED FALL 6-5-2000 AROUND 7:15 AM ON LEFT BLEEDER. FALL WAS 30-40' LONG, 18' WIDE AND 4 1/2-5' HIGH. ABOVE ANCHORAGE. WILL NOT BE CLEANED UP. FALL WAS AT SPAD 1179.

DISCOVERED ROOF FALL AROUND 2:30 AM ON 5/6/00. FALL WAS ABOVE ANCHORAGE LOCATED ON 2 SOUTHWEST, 20 FT LEFT OF SPED 1345 BETWEEN #4 ENTRY AND 3 ENTRY. FALL 10' HIGH 18' WIDE 35' LONG. WILL N OT BE CLEANED UP. CRIBS AND 8 FT BOLTS PUT IN.

FIRE BOSS LOCATED FALL AROUND 04:30 AM. LOCATED 16' INBY SPAD 1331 #6 HEADIN GON #2 SOUTHWEST. FALL WAS 16'LX4 1/2'T2"" THICK X 12'W.

INJURED WAS MOVING MINER, STEPPED ON CABLE AND TWISTED LEFT KNEE. EMPLOYEE DIDN'T START MISSING WORK UNTIL MARCH 1ST, 2000.
