THE EMPLOYEE WAS RIDING ON A RUBBER TIRED PERSONNEL TRANSPORTER HE SAT UP TO TIE HIS BOOTLACE AND STRUCK HIS HEAD ON THE HEAD OF A CABLE BOLT. THIS CAUSED HIS HARD HAT TO STRIKE HIM ON THE BRIDGE OF HIS NOSE RESULTING IN A FRACTURE.

The injured and another person were installing conveyor belt structure. The two men lifted a bottom roller and were putting it in position when the injured's left knee buckled.

A lift was bein mined from a pillar on the 2-left off 2 south panel when a piece of rock fell on the head of the mining machine and fouled the machine. A retriever was hooked to the machine and the machine was free in approximately 10 min. A rock fell inby the left row of roof bolts and no belts were damaged. This report is submitted in protest to satisfy a citation.

The individual was cleaning the water filter at the end of the hard line. While working on the filter, he caught his finger between the water filter and the conveyor belt tail piece resulting in a laceration to his left middle finger.

A roo ffall occurred in the no. 4 belt entry of no. 1 north (2 breaks inby survey station no. 163. the fall measured approx. 45' long by 20' to 35' wide x 7' high. the area was mined in 1948 and is approx. 4 miles outby the altrp working section. the fall will be clean up. there were no injuries.

A ROOF FALL WAS DISCOVERED IN THE NO. 4 BELT ENTRY OF NO. 1 NORTH (1 BK INBY SURVEY STATION NO. 163). THE FALL MEASURED APPROXIMATELY 40' LONG X 20' WIDE X 8' HIGH. THIS AREA WAS MINED IN 1998 AND IS APPROXIMATELY 4 MILES OUTBY THE ACTIVE FACE. THE FALL CAUGHT THE BELT CONVEYOR FOR THE MINE AND WILL BE CLEANED UP. THERE WERE NO INJURIES INVOLVED.

THE EMPLOYEE WAS HANGING CABLE FOR THE ROOF BOLTING MACHINE WHEN HE FELT PAIN IN HIS RIGHT SHOULDER. HE CONTINUED TO WORK HIS NORMAL JOB BUT CONTINUED TO EXPERIENCE PAIN IN HIS SHOULDER. HE STARTED MISSING WORK ON 12-5-2005.

Employee was cleaning belt line when his boots stuck in the mud. he slipped and fell against belt then fell to the ground hitting a piece of structure. Employee broke a rib and fractured shoulder.

EE was hanging a check curtain when a piece of draw rock fell fromt he mine roof striking him on his legs.

EE WAS TRAMMING A RUBBER-TIRED UTILITY VEHICLE AND DROVE OVER A 4'-LONG METAL ROOF JACK. THE METAL ROOF JACK FLIPPED UP AND STRUCK THE EE ON THE HEAD, RESULTING IN A MILD CONCUSSION. An investigation revealed that the alleged injury was falsified by the employee.

A CONT MINING MACHINE WAS BEING USED TO MINE PILLARS ON THE MMU 001, A SOUTH PANEL. THE SECOND PILLAR CUT OUT OF THE NO.5 ENTRY. A PILLAR FELL AND CAUGHT THE MACHINE. THE FALL MEASURED APPROX 30' LONG BY 20' WIDE BY 44' THICK. A RETRIEVER WAS USED TO RECOVER THE MINING MACHINE.

EMPLOYEE PULLED PINNER INTO PLACE TO START BOLTING, WHEN HE PLACED THE T-BAR THE FIRST TIME HE NOTICED LOOSE ROCK. HE LET THE T-BAR BACK OFF, RELOCATED THE PINNER, THEN PUT THE T-BAR UP AGAINS T LOOSE ROCK CAUSING ROCK FROM RIB TO ROLL OFF AND HIT EMPLOYEES FOOT.
