Pulling on feeder cable to get slack to plug cat head up and felt a pain in his lower back.

Had waterline for miner over shoulder pulling up slack and struck a low hanging roof bolt with the top of his head. Later while using slate bar to pry conveyor chain around sprocket, felt pain in neck, between shoulder blades and upper back.

Employee was going under the #3 belt at cross under and his left foot slipped in the mud and he felt his left knee pop and after several minutes his knee started hurting.

Employee was moving miner cable out of the roadway when he felt a sharp pain in his lower back on the right side and down his right leg. Continued to work up until 4/24/2014 when he was taken off work by a doctor.

Employee was pulling on the feeder cable when he felt pain in his right arm and fell at the same time. He fell striking his elbow and shoulder.

Employee states he was dusting ribs by hand when he slipped and fell and dislocated his right shoulder.

Two employees were moving crib block out from under head on miner. Employee went to pick up crib when the other employee tossed another crib hitting him on the left pinky finger pinching it between the crib he was picking up and the crib thrown.

A non-injury roof fall occurred on the 003 MMU during pillar recovery covering the Joy continuous miner. The rock fell from the cut and did not pull permanent roof supports.

Employee was retrieving truck sheet from loader. As he climbed up the ladder he felt pain in his right shoulder. He continued to work up until he had shoulder surgery on 3/20/2014.

A roof fall was found on the South Mains 003 unit spad #15010 extending out by 210'. A MRS cable was caught by a piece of rock.

Employee saw the man trip but thought there was enough room to travel by on a 3-wheel ride beside of the man trip. He stated that he went through the fly and had his left foot lying sideways sticking out over the edge of the 3-wheeler. He got too close to the man trip under the fender wall where the wheel is located on man trip injuring his left foot.

Employee was walking across section to tighten curtain and strained his right knee. He continued to work up until he had surgery on 6/28/2013.

Employee was cutting a piece of grass rope with a utility knife. He was holding the rope in his right hand and cutting with left. The knife slipped and he cut his right hand below his index finger on his palm resulting in 5 stitches.

Employee was changing sign on the surface. He pulled hard with his right hand when it slipped loose and struck a metal rope that surrounds the pond cutting the back of his right hand resulting in 3 stitches.

Employee was loading plaster in bucket of scoop and twisted his back.

Employee was walking down the haul road when his left knee popped. After a subsequent doctors visit on 10/19, an MRI was ordered for 10/22/12. He was taken off work on 10/23/12. MRI revealed degenerative disease and abnormal softness of cartilage.

Employee was tramming bolter up when the canopy caught cable that was hung from the roof. He reached out from under the canopy pushing the cable up with his left hand. The canopy caught employee on the left hand mashing it against the top resulting in a fracture to the end of his left middle finger and a laceration requiring stitches.

Employee was trying to boost a Lo-trac off the batteries of a 4-wheeler. He hooked the boosting cable to the Lo-trac and then to the batteries on the 4-wheeler. He moved the booster cable leads on the 4-wheeler batteries and the top of the battery blew off. The employee got battery acid in his eyes and on his face.

Employee was plastering a brattice and got plaster in his eye.

Employee was loading roof plates from a supply hole when he felt pain moving from his back down his right thigh into his foot.

Employee was going to answer the phone and put his knee on pipe when it rolled and his knee popped and twisted.

A roof fall occurred at 11:54pm on 3/2/2012. The inby part of the fan canopy plus 1 row of bolts fell. It measured 5-6' in thickness x 18 to 20' wide x 4 to 7' long. The fan was manually turned off.

Employee was servicing the continuous miner in a crouched position. While trying to move around the side of the machine the employee slipped on a piece of flat rock that was laying in miner cable causing him to lunge forward resulting in a strain to his right knee. Employee returned to work as scheduled after seeing ER doctor. Became lost time on 1/29/12 after pain did not subside.

Employee was turning a turnbuckle to align 10B #2 belt drive when he felt a sharp pain in his shoulder. He returned to his regular job duties and on 11/14/2011, after a follow up doctor's visit, he was put on restricted duty.

Piece of draw rock dislodged and slid down the ATRS striking the employee on the right leg resulting in an abrasion and laceration.

While gathering tools after working on the miner section of the rib rolled off catching back of employee's right foot causing a simple fracture of the small distal toe. Maintenance crew had moved miner to intersection and checked area prior to performing work and no hazards were noticed.

Employee was watering Belt entry with a 1 inch water line with a metal cutoff valve end. While pulling up slack on the water line, the water hose caught on something, pulling the line through his hand cutting the area between his index finger and thumb requiring Seven (7) stitches.

Employee was lifting a crib block on uneven ground that had some thin ice in the surrounding area. The ice broke loose causing employee's foot to slip through resulting in a pull to the left calf muscle.

Employee was jacking a chain hoist when the chain broke causing him to jerk and strain his right shoulder. (Employee did not lose any work days until he was taken off work on 11/29/2010 by Dr.)

Employee was preparing to install bolt into hole in the #5 belt heading, while straightening bolt a rock fell hitting him on the left wrist. Employee received a fractured left Ulna.

Employee was in the process of closing the track entry airlock door. Employee had right hand on a chain attached to door when watch on other wrist got caught on door and employee tried to catch door with his hand. Air pressure caused the door to close catching his left index finger to get caught between the door and roof bolt, cutting and mashing his finger.

Employee stepped off a Tram that was moving and twisted his right leg.

Employee was running tram when he stoped to set timbers. He twisted his left knee while exiting out of tram. Employee continued to work his regular job duties until surgery. Began losing time 02/08/2010.

Employee was using a hammer and chisel to remove a piece of weld when the chisel slipped in his hand causing a laceration that required stitches.

Employee was helping anchor tailpiece and while holding one of the hydraulic jacks got his right hand caught between the jackbase and roof pad.

While moving a rock to a belt, employee stated that he twisted his right knee.

While unloading miner bits that were in buckets, employee felt pain in his back and numbness down his left leg. The doctor took him off work on 3/26/09. He began losing time as of that date.

Employee picked up a box of resin and turned to the right and at such time he felt pain in his lower back. Went to the doctor on 3/12 and was taken off work. Started to lose time as of 3/13/09.

Employee's feet got caught by the feeder cable as it was being pulled, injuring his right knee.

While tramming diesel mantrip down slope track, brakes failed causing mantrip to derail and wreck at bottom of slope. Employee was thrown out and complained of back pain.

Employee was struck by a rock brow on his shoulder and back when it fell out, driving him to the ground. The rock measured 5' x 28"" x 16"".

Employee ws tramming feeder off of tailpiece. He crawled over a rock and hurt his left knee. Employee continued to work until 2/16/09, when surgery was performed on his knee. He started to lose time as of that date.

Employee was in process of connecting finisher and pusher steels when he inadvertently hit the rotation causing the pusher to fly out of the chuck. It hit him below his left eye resulting in a cut which got infected due to the type of sutures used.

Employee knelt to position body in order to hang curtain.

Moving miner, a loop of cable slack pulled across, striking him and knocking him to the mine floor.

Shuttle car operator bumped head on top.

Employee was on his knees loading bags of rock dust into a scoop bucket. A piece of coal got between his knee and knee pad he was wearing. The rubbing action of the coal against the knee caused a soft tissue injury and possible chipped knee bone. He started missing time on 05/19/2008.

East Mains 002 unit, a roof fall occurred in the # 3 entry 130' inby spad 10025 measuring approx. 25' x 20' x 4'. A portion of the Joy continuous mining machine was caught under the rock. No disruption to ventilation, no travelways blocked, and no injuries.

An evening shift electrician was loading tool box on 3-wheeler. He alleges that he felt sharp pain in his lower back. Employee began losing time on 3/10/08.

A roof fall was discovered in the #4 entry at the mouth of a previously pillared section beginning at spad 5342 and extending to spad 9173. The fall measured approx. 200' x 22' x 7'.

Employee was hanging a trailing cable, he turned slightly and felt pain in his left knee. He began losing days on 4-1-08.

Retreat mining in the #5 entry pillar block when the roof started working in the previously mined #4 entry block. The roof fell in the #4 entry and took out the breaker timbers. The timbers were replaced and mining continued. The roof began working again, mining ceased. The roof in the #4 entry fell again, the fall maesured approx. 80'L x 22'W x 5'H. The entry was timbered off.

Employee stated he was exiting the mantrip; when he turned he felt pain in his right knee. Doctor removed him from work on 1-24-2008.

Employee was closing track door, catching his finger between door and roof bolt plate. Went to doctor on 12-13-07 Returned to work on 12-14-2007.

Employee was moving the stinger (4-Wheel Ride) from the Power Center to the track to load on flat car to bring outside. While bringing stinger to track, front tire hit a rock in haulway causing steering to jerk hitting his hand and wrist.

Employee was lifting a jack to put a porta bus back on track. The jack slipped out and the jack bar struck the employee on finger.

Employee was operating a shuttle car, ran over a rock in haulway, bumping his head against roof. Went to doctor on 7/12 & 7/18; started loosing time 7/18/07.

Employee had made up some roof bolts & was crawling back toward front end of roof bolter-twisting his right knee. Went to doctor 5/31/07. Started losing time 6/1/07.

Employee was duck walking down #4 entry about 2 breaks outby face area. Bumping his head against some low top in the top. Went to Dr. 5/21/07, follow-up 5/22/07. Taken off of reg.schedule (EE works 4 on 4 off) started losing time 5/27/07.

The continuous miner (Joy) was mining in the #5 pillar block. A roof fall from the pillar line fell catching the ripper head on the continuous miner. No injuries, no blockage of ventilation, no blockage of escapeway. Fall not cleaned up. Breakered off & dangered, plotted on map.

Employee was putting the forks back on the LoTrack - lifting a fork he felt pain in his lower back. Went back to doctor on 3/6/07. Started losing time on 3/7/07.

Employee was operating a shuttle car. Shuttle car got stuck on feeder. Used scoop to push shuttle car off feeder. Employee got his SCSR caught between rib & the shuttle car, bruising left hip & lower left side of back. Kept working. Went back to doctor 1/18/07. Losing time 1/18/07.

Employee was pulling slack out of miner cable feeling sharp pain in lower part of his back. Went to doctor on 12-4-06, continued to work, went back to doctor on 12-11-06 and started losing time on 12-12-06.

Spad #7504 #4 Hd. #4 Block: Bridge Unit (003-0) was on retreat mining cycle when some rock fell from Gob line on the ripper head of the continuous mining machine pushing the ripper head down in the bottom. Had to use the retriever to pull the miner out. No injuries & no damage to equipment.

Employee was checking pump, walking beside the pipe line & he stepped on a block & turned his ankle, fracturing his leg.(started losing time on 6-28-06 by 220061910063)

Laceration & Fracture above Rt thumb - 3 stitches and immobillzation. Struck by pin from impact wrench. No lost time - employee performing light duty since date of accident. Total duration of LD may be up to 5 weeks.

Employee was loading a tool box onto a mac 8 rubber tire mantrip-corner of box slipped catching employee's finger between tool box handle & frame of the mac 8.

EMPLOYEE WAS OPERATING THE CONTINUOUS MINING MACHINE. A PIECE OF ROCK SLID OFF RIB. EMPLOYEE MOVED OUT OF WAY OF ROCK, TWISTING KNEE. WENT TO DOCTOR ON 8-24-05. STARTED MISSING WORK ON 8-29-05.

EMPLOYEE WAS SAWING A TIMBER WITH A BOW SAW, SAW CAME OUT AND CUT HIM ON THE FINGER. HAD DR APPT ON 8/15/05, RETURNED TO WORK 8/16/05

EE WAS PREPARING TO DRILL BOLT HOLE, PLACED DRILL STEEL AGAINST TOP, PIECE OF ROCK FELL STRIKING HIM ON END OF FINGER

EE WAS WALKING DOWN HAULWAY-STEPPED IN A HOLE TWISTIG HIS LEFT KNEE. CONT'D TO WORK-KNEE KEPT GIVING HIM PROBLEMS. WENT TO DR. 3/4/05 HAD SURGERY 3/7/05.

Injured was removing a crib block which was stuck in some mud, causing a strain to his lower back. Note: Employee started losing time as of 11-15-04.

EMPLOYEE STEPPED FROM A FLATCAR WHILE UNLOADING TIMBERS AND STEPPED ON A ROCK, TWISTING HIS KNEE. STARTED LOSING TIME ON 1/24/05. HAD SURGERY 1/26/05.

In by spad #5539 fall of pillar line occured on the second lift off of #1 block, no injuries, no blockage of escapeway or ventilation, caught the 1415 cont mining machine (Minor Damage) Fall will not be cleaned up, breakered off and dangered, after the removal of the mining machine- plotted on mine map.

EE WAS MOVING THE CONTINUOUS MINER TO THE OUTSIDE AND NEEDED TO MOVE THE DB BOX OVER. EE PLACED A TIMBER BETWEEN DB BOX AND MINER BOOM TO MOVE BOX OVER. TIMBER SLIPPED OUT AND EE STRUCK THE MINER BOOM WITH HIS THUMB. WENT TO DOCTOR ON 6/7/04. RETURNED TO WORK ON 6/8/04.

EMPLOYEE STEPPED ON A PIECE OF BANDING MATERIAL WHICH FLIPPED UP AND STRUCK HIS TOOTH CAUSING CHIPPING OF THE LEFT FRONT TOOTH.

EMPLOYEE WAS PLACING ROOF BOLTS INSIDE THE AIR LOCK DOORS OF FAN. AIR LOCK DOOR OPENED. AIR CURRENT PUSHED HIM BACK AGAINST A BUNDLE OF ROOF BOLTS.

EE WAS WORKING ON THE AUTO-TRACK DOORS, THE DOOR CLOSED UP CATCHING HIS LEFT FOOT.

EE STATED HE WAS PUTTING (2) PIECES OF STEEL TOGETHER, CATCHING HIS GLOVE & LITTLE FINGER ON RT.HAND, BETWEEN THE COUPLING POINT OF THE STEEL.

EE STATED HE WAS OPERATING 3-WHEELER. HIT HOLE IN HAULWAY, BUMPING HIS BACK AGAINST FRAME OF 3- WHEELER AND TOOL POUCH ON HIS MINING BELT. STARTED LOOSING TIME ON 11/19/02.

EE STATED HE WAS SETTING TIMBERS ON THE PILLAR SECTION, PIECE OF ROCK FELL, STRIKING HIM ON LEG. WENT TO DR. 11-10-02, WENT BACK NEXT DAY. STARTED LOSING TIME ON 11-25-02.

EE STATED THAT HE BENT OVER TO MOVE MINER CABLE OUT OF HALLWAY, FEELING PAIN IN LOWER BACK. WENT TO DOCTOR ON 9-12-02 STARTED MISSING WORK ON 9-18-02.

EMPLOYEE WAS REMOVING BELT STRUCTURE THAT WAS HUNG. STRUCTURE CAME LOOSE ON OFFSIDE STRIKING EMPLOYEE ON LEG. LEG KEPT BOTHERING HIM. WENT TO DOCTOR ON 7/28/02. STARTED MISSING WORK ON 7/28/02 .

THIS EE HAS BEEN DIAGNOSED AS HAVING CARPAL TUNNEL SYNDROME IN HIS HANDS-DUE TO REPETITIVE TRAUMA. CORRECTIVE SURGERY HAS BEEN DONE ON HIS RIGHT HAND AND SURGERY ON THE LEFT HAND IS SCHEDULED FOR 6-10-02.

EE HAD PUT UP RIB BOLT AND WAS MOVING OVER TO INSTALL SECOND BOLT. HE HEARD ROCK STARTING TO BREAK AND FALL. HE STARTED MOVING TOWARD BACK OF PINNER. FALLING ROCK STRUCK HIS FOOT.

EMPLOYEE STATED HE WAS OPERATING CONTINUOUS MINER, TOP BUMPED, PIECE OF ROCK FELL STRIKING HIM ON THE LEG.

EE WAS SETTING A TIMBER AND A PIECE OF ROCK FELLSTRIKING HIM ON BACK OF NECK AND SHOULDERS. WENTTO DOCTOR ON 11/18 AND AGAIN ON 11/19, STARTING RESTRICTED DUTY ON 11/19/01.

EE STATED THAT HE WENT TO CHECK WATER PUMP, STEPPED IN MUD AND TWISTED HIS KNEE. WENT TO DOCTOR 11/12. STARTED LOSING TIME 12/8. WENT BACK TO DOCTOR 12/4 & HAD SURGERY.

EMPLOYEE STATED HE WAS HOLDING A TIMBER AND SAW-ING THE TIMBER AT THE SAME TIME- SAW SLIPPED ANDSTRUCK EMPLOYEE ON BACK OF HAND.

EE WAS DRAGGING A 5 GALLON BUCKET OF GEAR OIL AROUND CONTINUOUS MINER - FEELING SHARP PAIN IN LOWER PART OF HIS BACK.

EE STATED HE WAS EXITING THE SCOOP-STEPPED ON ROCK TWISTING HIS KNEE.

EE STATED THAT A SMALL PIECE OF ROCK GOT UNDER HIS KNEE PAD AND HE BENT DOWN ON HIS KNEES AND BRUISED KNEE.

EMPLOYEE STATED HE WAS HELPING TO REMOVE A PIECE OF ROCK OFF MINER WITH SLATE BAR, FEELING PAIN\IN LOWER BACK. WENT TO DOCTOR 5-8-01. STARTED RESTRICTED DUTY 5-9-01

WITH REFERENCE TOT HE ATTACHED NOTICE DATED APRIL 26, 01, THERE IS NO ADMIT THAT THE EMPLOYEE HAS THE DISEASE AND WE RESERVE THE RIGHT TO CONTEST THE ISSUE.

EE WAS HOLDING SLEDGE HAMMER ON OFF SIDE OF MINER CHAIN. ANOTHER EMPLOYEE WAS DRIVING PIN INTO CHAIN. HAMMER GLANCED AND HIT INJURED ON FINGER.

EE STATED HE WAS CUTTING PLASTIC OFF OF SUPPLIES. KNIFE SLIPPED AND CUT HIS FINGER.

EMPLOYEE STATED WAS IN A TIGHT POSITION BENDING A ROOF BOLT TO INSTALL- FEELING PAIN IN HIS KNEE. WENT TO DOCTOR ON 4-19-01. STARTED RESTRICTED DUTY ON 4-20-01.

EE STATED HE WAS HELPING TO UN-ROLL SOME HIGH VOLTAGE FROM A SPOOL HANGING BY CHAINS. ONE OF THECHAINS BROKE, SPOOL STRIKING EE ON SHOULDER.

FALL OCCURRED IN #3 ENTRY (NEUTRAL) INTERSECTION, FALL APPROX 22' LONG APP 40' WIDE 4 1/2' THICK, NO INJURIES, NO EQUIP INVOLVED, NO BLOCKAGE OFESCAPEWAY OR VENTILATION. TIMBERED AND DANGERED OFF. WILL NOT BE CLEANED UP.

FALL OCCURRED IN #3 HEADING - 20' INBY SPAD #1726. FALL APP 30'LONG, APP 18'WIDE, APP 0 TO 4' THICK. NO BLOCKAGE OF VENTILATION, ESCAPEWAY, NO INJURIED, NO EQUIP INVOLVED, FALL WILL BE CLEANED UP. CLEAN UP PLAN PASTED AT FALL SITE.

EMPLOYEE STATED HE WAS LOADING A GREASE BUCKET INTO THE MACH 8; FEELING PAIN IN SHOULDER. WENT TO DOCTOR ON 7/22/00. STARTED RESTRICTED DUTY ON 7/23/00.

EMPLOYEE STATED HE WAS GETTING OUT OF SCOOP DECK, HIS KNEE PAD CAUGHT BETWEEN SEAT AND FRAME OF SCOOP, HE ROLLED OUT LANDING ON HIS SIDE, HEARD POP IN RIGHT HIP AREA.

EE WAS SPLICING PUMP CALBE STRIKING ANVEIL WITH HAMMER WHEN PIECE OF METAL BROKE OFF EDGE OF ANVIL STRIKING EE IN LEFT THUMB

EE STATED THAT HE WAS LOADING BOLTS ONTO A ROOF BOLTER BAND THAT HOLDS BOLTS TOGETHER WAS HANGING LOOSE HIS HAND CAUGHT END OF STRAP

EMPLOYEE STATED HE WAS TRAMMING THE #4 MBC INTO THE FACE OF #1 HEADING- THERE WAS A ROCK IN THE ENTRY THAT THE MBC TRAMMED UPON- THEREFORE CAUSING THE DOCK TO RAISE- BUMPED HIS HEAD AGAINST TH E TOP AND BACK OF HIS HEAD AGAINST THE DECK, CUTTING THE BACK OF HIS HEAD BELOW HIS HARD HAT.

EMPLOYEE STATED HE WAS MOVING A PIECE OF 6"" WATERLINE, EMPLOYEE BASCKING UP, TRIPPED OVER TRACK FALLING AND PIPE FELL ON HIS FOOT.

THE EE WAS ASSISTING OTHERS GETTING BELT OUT OF THE STORAGE UNIT TO MAKE A BELT SPLICE. HE WAS STRADDLE THE STORAGE UNIT RAIL AND TURNED TO GET OUT WHEN HE FELT SUDDEN PAIN IN HIS RIGHT KNEE. HIS KNEE STARTED SWELLING UP. ON 1-24-00 WAS HISSCHEDULED SHIFT TO RETURN TO WORK. HE BEGAN MISSING TIME ON THIS DAY.
