He stated that he went through the fly and had his left foot lying sideways sticking out over the edge of the 3-wheeler. He got too close to the man trip under the fender wall where the wheel is located on man trip injuring his left foot.

Employee was walking down the haul road when his left knee popped. After a subsequent doctors visit on 10/19, an MRI was ordered for 10/22/12. He was taken off work on 10/23/12. MRI revealed degenerative disease and abnormal softness of cartilage.

Employee was tramming bolter up when the canopy caught cable that was hung from the roof. He reached out from under the canopy pushing the cable up with his left hand. The canopy caught employee on the left hand mashing it against the top resulting in a fracture to the end of his left middle finger and a laceration requiring stitches.

Employee was trying to boost a Lo-trac off the batteries of a 4-wheeler. He hooked the boosting cable to the Lo-trac and then to the batteries on the 4-wheeler. He moved the booster cable leads on the 4-wheeler batteries and the top of the battery blew off. The employee got battery acid in his eyes and on his face.

A roof fall occurred at 11:54pm on 3/2/2012. The inby part of the fan canopy plus 1 row of bolts fell. It measured 5-6' in thickness x 18 to 20' wide x 4 to 7' long. The fan was manually turned off.

While trying to move around the side of the machine the employee slipped on a piece of flat rock that was laying in miner cable causing him to lunge forward resulting in a strain to his right knee. Piece of draw rock dislodged and slid down the ATRS striking the employee on the right leg resulting in an abrasion and laceration.

While gathering tools after working on the miner section of the rib rolled off catching back of employee's right foot causing a simple fracture of the small distal toe. Maintenance crew had moved miner to intersection and checked area prior to performing work and no hazards were noticed.

Employee ws tramming feeder off of tailpiece. He crawled over a rock and hurt his left knee. Employee continued to work until 2/16/09, when surgery was performed on his knee. He started to lose time as of that date.

Employee was in process of connecting finisher and pusher steels when he inadvertently hit the rotation causing the pusher to fly out of the chuck. It hit him below his left eye resulting in a cut which got infected due to the type of sutures used.

East Mains 002 unit, a roof fall occurred in the # 3 entry 130' inby spad 10025 measuring approx. 25' x 20' x 4'. A portion of the Joy continuous mining machine was caught under the rock. No disruption to ventilation, no travelways blocked, and no injuries.

Retreat mining in the #5 entry pillar block when the roof started working in the previously mined #4 entry block. The roof fell in the #4 entry and took out the breaker timbers. The timbers were replaced and mining continued. The roof began working again, mining ceased. The roof in the #4 entry fell again, the fall maesured approx. 80'L x 22'W x 5'H. The entry was timbered off.

Employee was moving the stinger (4-Wheel Ride) from the Power Center to the track to load on flat car to bring outside. While bringing stinger to track, front tire hit a rock in haulway causing steering to jerk hitting his hand and wrist.

The continuous miner (Joy) was mining in the #5 pillar block. A roof fall from the pillar line fell catching the ripper head on the continuous miner. No injuries, no blockage of ventilation, no blockage of escapeway. Fall not cleaned up. Breakered off & dangered, plotted on map.

Employee was putting the forks back on the LoTrack - lifting a fork he felt pain in his lower back. Went back to doctor on 3/6/07. Started losing time on 3/7/07.

Employee was operating a shuttle car. Shuttle car got stuck on feeder. Used scoop to push shuttle car off feeder. Employee got his SCSR caught between rib & the shuttle car, bruising left hip & lower left side of back. Kept working. Went back to doctor 1/18/07. Losing time 1/18/07.

Spad #7504 #4 Hd. #4 Block: Bridge Unit (003-0) was on retreat mining cycle when some rock fell from Gob line on the ripper head of the continuous mining machine pushing the ripper head down in the bottom. Had to use the retriever to pull the miner out. No injuries & no damage to equipment.

HIS KNEE STARTED SWELLING UP. ON 1-24-00 WAS HISSCHEDULED SHIFT TO RETURN TO WORK. HE BEGAN MISSING TIME ON THIS DAY.
