Contact with electrical current

EE was instructed that an exposed electrical cable was inactive. During the work operation (pumping out overflow rain water) EE contacted cable and received a shock. EE was taken to local ER, checked out and released.

Employee was walking to the bolting station when he slipped on a rock covered with curtain. This caused him to fall onto a rock resulting in pain in his lower back area.

Employee arrived at the section to restock the CO#247 roof bolter. As he was walking away from the bolter after checking what supplies were needed, a piece of draw rock fell from the roof striking employee on his right foot. The resulting accident required the complete amputation of all 5 toes on the right foot due to their damage from the falling draw rock.

After mining final lift and moving continuous mining machine, the roof fell approx. 50' in by survey station 15674 located in the #1 entry. The fall trapped 1 of the mobile roof supports.

Employee was removing nozzle from the end of a fire hose. It was too tight to remove by hand so the employee held it with one hand and hit the coupling with a hammer. On the third swing he missed the coupling and struck his left hand resulting in a fracture.

Employee was adjusting forks on Lo-trac. When he slid the fork over, the fork fell catching his finger between the fork and flat car resulting in a fracture.

Employee was cutting second cut out of 4L when he felt a small piece of rock hit his left ear. This resulted in 4 stitches.

Employee was bolting on offside, started drilling hole and broke small rock loose. The spinning motion of drill steel caused it to spin off hitting him just below the nose on upper lip resulting in 5 stitches.

Employee lifted up draw head on locomotive and strained his back. He continued to work up until 9/16/13.

Employee was in the process of letting battery lid down while holding the lid by the handle. The lid slipped out of his hand catching the tip of his ring finger on his left hand resulting in 5 stitches.

Employee was lapping belt. The belt got fouled in structure and the belt clamp slipped off the belt striking him on the left leg on his shin. This resulted in a fracture (chip) of his tibia (shin bone).

Employee was changing hydraulic hose on the roof bolter when the wrench slipped causing him to strike his finger against the roof bolting machine resulting in a fracture.

Employee was in the outside surface shop when he tripped and fell on his left shoulder. He continued to work up until he had surgery on 4/18/2013.

Employee was putting pusher steel in chuck when a rock fell catching his ring finger and small finger on his right hand. The rock measured 1' wide x 1.5' long x 3"" thick. This resulted in a fracture of his pinky finger. Management did not become aware of the fracture until 11/28/12.

Employee was putting finisher steel in hole when a piece of draw rock fell hitting him on the middle finger on his left hand resulting in a small fracture and stitches.

Employee was getting out of his truck before his shift started. He stepped out of the truck onto a small rock causing his left ankle to twist. The truck measured 36"" off of the ground.

Member was spot bolting #3 entry across from feeder when a piece of loose draw rock fell, hitting his upper lip. Member was transported to local hospital, and given a couple of stitches to his lip.

While operating the shuttle car near #3 intersection near spad 3106, member struck his head on canopy when he ran over something. There were no witnesses, member was taking by EMTs on back board to local hospital where evaluation showed contusion on lower spine.

EE was trying to straighten a 6 foot torque tension bolt to push up into the hole, when he felt a something pull in his lower back.

EE was scaling roof on Section 003 6B beltline and track, when a piece of drawrock fell, hitting him on the right foot. The rock measured 4.5 inches thick and 43 inches in length by 19 inches wide.

EE had hand positioned on loaded rails, while chain ratchet was being secured around the rail. The ratchet had not started to tighten up and a piece of rail rolled over EE's left hand.

EE hung line curtain across the heading in to the 5 to 6 X cut when a rock slipped out from the out-by corner catching ee on the left leg just above the ankle

EE was working in the #2 intake entry close to the fan shaft. Cleaning up and re-bolting a slate fall, bolter had been backed out of the fall area, EE and a co-worker were under supported top evaluating the next step. Co-worker walked away, staying under supported top. As EE started to follow co-worker, he was struck by a rock originating from the fall area.

EE was replacing bit inserts on cutter head on Joy CM. A piece of rock measuring 9"" wide X 16"" long X 2"" thick fell from between the bolts striking his left hand between the rock and cutter head.

EE was trying to pry scoop batteries out of battery tray with a slate bar. Slate bar slipped causing ee to fall backwards and twist his right knee. Weather conditions were raining and wet.

Employee hanging miner cable in 3 heading out by right corner, a rock fell out between the roof bolt and rib, striking him on the left foot. X-rays were positive for fracture on left foot.

EE was operating a track mantrip, when his left arm came in contact with a timber that was set between the belt and track.

Changing out top belt cradle, pulled up cradle to lift out. Cradle had pressure on it from the weight of the belt causing it to mash his finger between the cradle and belt rail.

Employee was making the fireboss run through the mine, while traveling in the mantrip, he hit a rock that had fallen on the No. 3 Belt. The rock turned into the operator's compartment striking employee in the right side.

Co-worker was attempting to put the mantrip back on the rails. A jack was set on crib blocks. While pushing the jack bar down, the jack gave out and hit employee in the face. He was cut in the face and lost 5 teeth.

Employee was running centers in the No. 3 entry. The ripperhead on the miner needed let down. Employee was standing behind the miner using the remote box to drop the head. As a result the miner rocked back, setting down on his left foot.

A roof fall measuring 52'L x 20'W x 8'H occurred on 1b belt line at spad #8005 in the panel 1 submains. Fall was cleanded up and bolted back with 5' TT bolts.

Employee was operating the offside of the LRB. He had just finished the 2nd row of bolts and pullled the machine up to begin the third row. The ATRS was set and he walked forward to swing his drill station out when a rock fell striking him on the right foot. Seam Height was 98"". Pizza Pans were installed on every bolt for the last two cuts.

EE was assisting in replacing LCM Cat wear strips. The miner was elevated on cribs for the repairs to an approximate height of 5.5 feet. Repairs were completed. EE climbed on top of the miner to take down the work area lights. To get off the top of the miner, EE got on his hands and knees and jumped off backwards. Upon landing he twisted the left ankle.

Member was bolting in the No. 3 entry, installing inside bolts in 2nd row. A piece of drawrock fell measuring 56"" long x 21"" wide x 7"" thick, coming in under canopy and striking him on lower right leg. Bolt spacing was less than 48"". Seam height was approximately 10'.

EE was kneeling next to miner installing shims when a piece of draw rock measuring 25"" L x 15"" W x 1"" to 3"" thick struck him in left arm and left upper leg area. Roof was supported with 6' torque tension bolts with 8"" x 8"" bearing plates and pizza pans.

EE was helping to remove a guard from the miner. The guard fell between the cat and pan. When trying to retrieve the guard, EE cut his hand on sharp metal.
