EE was instructed that an exposed electrical cable was inactive. During the work operation (pumping out overflow rain water) EE contacted cable and received a shock. EE was taken to local ER, checked out and released.

An unplanned roof fall was located in the primary intake escapeway 5 breaks inby the fan at spad No. 1130. This was in the #2 entry and measured approx. 20 ft. long, 16 ft. wide and 6 ft. thick.

Received a hearing loss diagnosis for EE. EE quit to retire on 5/31/14.

Received a CWP diagnosis for employee. EE quit to retire on 5/31/2014.

Received a hearing loss diagnosis for employee. Employee was termed on 9/11/15 due to a reduction in force.

Received a CWP diagnosis for employee. No % disability at this time.

Employee was pulling muddy curtain out of face of #4 entry when he slipped and fell striking right knee on rock.

Employee was walking to the bolting station when he slipped on a rock covered with curtain. This caused him to fall onto a rock resulting in pain in his lower back area.

Employee arrived at the section to restock the CO#247 roof bolter. As he was walking away from the bolter after checking what supplies were needed, a piece of draw rock fell from the roof striking employee on his right foot. The resulting accident required the complete amputation of all 5 toes on the right foot due to their damage from the falling draw rock.

Received a hearing loss claim for employee. He retired on 10/1/2014.

Carrying 6 volt battery and putting it on the 10-20 outside. He felt a pop in his lower back.

After mining final lift and moving continuous mining machine, the roof fell approx. 50' in by survey station 15674 located in the #1 entry. The fall trapped 1 of the mobile roof supports.

Employee was removing nozzle from the end of a fire hose. It was too tight to remove by hand so the employee held it with one hand and hit the coupling with a hammer. On the third swing he missed the coupling and struck his left hand resulting in a fracture.

Received a hearing loss diagnosis on 8/12/2014. Employee last worked on 6/26/2013.

Employee was throwing a bag of dust on the roof bolter when he felt his back tighten up.

Bates Contractor stepped out of man trip and turned and walked into a piece of belt structure cutting his ear resulting in stitches.

Employee was adjusting forks on Lo-trac. When he slid the fork over, the fork fell catching his finger between the fork and flat car resulting in a fracture.

Employee was cutting second cut out of 4L when he felt a small piece of rock hit his left ear. This resulted in 4 stitches.

Employee was bolting on offside, started drilling hole and broke small rock loose. The spinning motion of drill steel caused it to spin off hitting him just below the nose on upper lip resulting in 5 stitches.

Employee lifted up draw head on locomotive and strained his back. He continued to work up until 9/16/13.

A roof fall was discovered at 1:30pm in the #4 mains one xcut in by SS 651. The fall measures approx. 20' x 20' x 5' high.

Employee was in the process of letting battery lid down while holding the lid by the handle. The lid slipped out of his hand catching the tip of his ring finger on his left hand resulting in 5 stitches.

Employee was lapping belt. The belt got fouled in structure and the belt clamp slipped off the belt striking him on the left leg on his shin. This resulted in a fracture (chip) of his tibia (shin bone).

Employee was changing hydraulic hose on the roof bolter when the wrench slipped causing him to strike his finger against the roof bolting machine resulting in a fracture.

Employee was in the outside surface shop when he tripped and fell on his left shoulder. He continued to work up until he had surgery on 4/18/2013.

Employee was putting pusher steel in chuck when a rock fell catching his ring finger and small finger on his right hand. The rock measured 1' wide x 1.5' long x 3"" thick. This resulted in a fracture of his pinky finger. Management did not become aware of the fracture until 11/28/12.

Employee was putting finisher steel in hole when a piece of draw rock fell hitting him on the middle finger on his left hand resulting in a small fracture and stitches.

Employee was getting out of his truck before his shift started. He stepped out of the truck onto a small rock causing his left ankle to twist. The truck measured 36"" off of the ground.

EE WAS CHANGING THE REAR-END ON A 10-10 MOTOR OUTSIDE WHEN HE MASHED HIS RING AND MIDDLE FINGERS BETWEEN THE STEARING JACK AND THE REAR FRAME.

Employee was dumping shuttle car on the feeder. As he was getting up to turn around the employee stated that he thought he got shocked.

EE WAS WORKING ON A TELEPHONE LINE, AND CUT HIS LEFT HAND.THIS REQUIRED SIX STITCHES.

EE WAS INSTALLING A CABLE BOLT. THE PLATE HIT HIS RIGHT ELBOW, HE CUT REQUIRED A COUPLE OF STITCHES.

EE PARKED HIS SCOOP. WHILE WALKING ALONG SIDE HIS Scoop, he SAID THAT HE STEPPED ON A ROCK AND TWISTED HIS KNEE.

EMPLOYEE WAS TRAVELING IN RETURN WHEN HE FOUND A ROOF FALL, 2 BREAKS INBY SPAD NO 1827 INNO 2SW MAINS. THE FALL IS 20 FT LONG BY 20 FT WIDE BY 7 FT HI. THE FALL IS 10,000 FT FROM THE FACE

ON 9/13/11 EE WAS IN NO. 1 SUB MAINS WHEN HE FOUND A ROOF FALL ONE BREAK IN BY SPAD NO 9683 PANEL NO 7. THE FALL WAS 80 FT LONG BY 20 FT WIDE BY 7FT HI. THIS AREA IS BOLTED WITH 6 FT TORQUE TENSION BOLTS. THIS FALL IS 13,400 FT FROM THE FACE.

THE OUT BY FORMAN WAS MAKING THE MAIN RETURNS WHEN HE FOUND A ROOF FALL ONE BRAKE INBY SPAD NO 673 ON THE MUD LICK SIDE. THE FALL WAS 40' LONG BY 19' WIDE BY 5' HI. THE TOP WAS BOLTED WITH 4FT GLUE BOLTS.

EE WAS CLOSING AIRLOCK DOORS BY HIMSELF. HE TRIED TO KEEP THE DOORS FROM SLAMMING SHUT AND WAS CAUGHT BETWEEN THE DOORS.

EE WAS TRAVELING IN RETURN WHEN HE FOUND A ROOF FALL, ONE BREAK INBY SPAD NO 1852 ON NO 2 SW MAINS.THE FALL IS 25 FT LONG BY 20 FT WIDE BY 7 FT HI.THE FALL IS 10,000 FT FROM THE FACE.

Member was spot bolting #3 entry across from feeder when a piece of loose draw rock fell, hitting his upper lip. Member was transported to local hospital, and given a couple of stitches to his lip.

While operating the shuttle car near #3 intersection near spad 3106, member struck his head on canopy when he ran over something. There were no witnesses, member was taking by EMTs on back board to local hospital where evaluation showed contusion on lower spine.

Hearing Test Results for EE: Standard Threshold Shifts (STS) was confirmed on EE.

Mantrip derailed due to loose fishplate in the track. One injury, bruised knee, with swelling.

Bent a bolt on the dust box and slipped off the bolt & hit his right eye on the edge of the pinner. He was putting up 2nd roll of bolts. There were some rocks on his side.

Employee stepped on belt that was not pinned together causing belt to slack between rollers, causing employee to stumble, striking his chin on top roller.

A roof fall measuring 30 L x 18 W x 8 H occurred in the #6 entry at spad 1785 in the No. 2 SW Mains off of No.1 SW Mains approximately 8,050 form active mining areas 005 and 003 MMUs. Area was bolted with 5 TT bolts.

EE was trying to straighten a 6 foot torque tension bolt to push up into the hole, when he felt a something pull in his lower back.

EE was scaling roof on Section 003 6B beltline and track, when a piece of drawrock fell, hitting him on the right foot. The rock measured 4.5 inches thick and 43 inches in length by 19 inches wide.

EE had hand positioned on loaded rails, while chain ratchet was being secured around the rail. The ratchet had not started to tighten up and a piece of rail rolled over EE's left hand.

EE hung line curtain across the heading in to the 5 to 6 X cut when a rock slipped out from the out-by corner catching ee on the left leg just above the ankle

EE was in the process of jacking a derailed track flatcar back on the track, the jack bar slipped, causing EE to twist his lower back and left hip.

A roof fall measuring 30L x 15W 8H occurred 100 inby survey station 1719 in the No. 2 SW Mains off of the the No. 1 SW Mains. The area was bolted with 5 TT bolts.

A roof fall measuring 25L x 18W x 6H occurred 2 breaks outby survey station 8811 in the 3rd LT Panel off of the SE NO 4 Panel. The area was bolted with 5 TT bolts.

EE was pulling a 6ft. roof bolt out of some scooped up gob in No. 3 left when he felt something pull in his lower back.

A roof fall measuring 95L X 18W X 8H occurred outby survey station 2908 in the No. 2 entry 3 breaks outby airshaft.

EE was working in the #2 intake entry close to the fan shaft. Cleaning up and re-bolting a slate fall, bolter had been backed out of the fall area, EE and a co-worker were under supported top evaluating the next step. Co-worker walked away, staying under supported top. As EE started to follow co-worker, he was struck by a rock originating from the fall area.

A roof fall measuring 27'L X 20'W X 6'H occurred in the #1 West Mains across from survey station 434 in the #2 Entry (Mudlick outby panels).

EE was putting a fork back on a fork lift when it droped and pinched his fingers between the fork and frame

EE was building a crib and was picking up a crib block and twisted around and felt pain in his lower back.

EE was replacing bit inserts on cutter head on Joy CM. A piece of rock measuring 9"" wide X 16"" long X 2"" thick fell from between the bolts striking his left hand between the rock and cutter head.

EE was trying to pry scoop batteries out of battery tray with a slate bar. Slate bar slipped causing ee to fall backwards and twist his right knee. Weather conditions were raining and wet.

A roof fall measuring 35'L X 25'W X 9.5'H occurred 1 break inby spad # 9965 in the 1ST LT Panel off or the 5TH RT Panel in the belt entry approximately 2,030' outby 002 MMU (#1 Section). Area was bolted with 6' torque tension bolts and 10' cable bolts.

A roof fall measuring 70'L X 20'W X 6'H occurred at spad # 8424 in the SE NO. 4 Panel approximately 4,503' outby 002 MMU (#1 Section). Area was bolted with 6' torque tension bolts.

A roof fall measuring 30' L X 20' W X 8' H occurred in the #2 entry of the NO. 1 NW Mains off of NO. 2 SW Mains. Area was bolted with 5' glue bolts. Area was mined approximately 3-1-00.

A roof fall measuring 75'L X 20'W X 12'H occurred in the #4 heading 1 break inby spad # 1527 in the NO. 3 NE MAINS off of the NO. 5 SE MAINS on the 005 MMU (003 Section). Area was bolted with 6' TT and 10 cable bolts.

Injury was doing mine rescue training on the mine rescue training field. He was exploring the No. 1 entery turned with a stretcher stepped on a rock an twisted his right ankle.

Employee lifted and was carrying a bucket of miner bits and twisted his back.

A roofall measuring (50'L X 18'W X 7'H) occurred at spad number 6375in the SE NO. 3 Panel off of the NO. 3 NE Mains (001 Section). Bloted with 5' bolts. Area was mined approximately 3/2005.

A roof fall measuring (40'L X 18"" W X 8'H) occurred at spad Number 6327 in the SE No. 3 Panel off of the MO. 3 NE mains (001 section). Bolted with 5' TT bolts. Panel was mined approximately 3/2005.

A roof fall measuring (40'L X 18"" W X 8'H) occurred 2 breaks inby spad number 9435 in panel A off of the SE NO 4 Panel. Bolted with 6' bolts and 10' cable bolts. Panel was mined approximately 6/2007

A roof fall measuring 100'L X 22'W X 6'-8'H occurred approximately 650' inby the portal in the slope mains. Roof was bolted with 6"" TT.

EE was drilling holes in a rib roll to be cleaned up, when the hand drill he was using became stuck and turned, catching the right pinkie finger and twisting it upward then back.

Employee was removing a stuck top roller while changing out the belt structure. He caught his right middle finger between the top roller and rail.

EE was operating a scoop pulling belt. His hand was placed on the back of the scoops deck and bumped into the rib mashing two fingers.

Roof Bolter Operator was bending a roof bolt and felt a sharp pain in his lower back.

EE was putting a wrecked flat supply car back on the track, he was aligning the trucks of the flat car on the rail with a slate bar, when the car shifted and mashed his finger between the slate bar and rail.

A roof fall measuring 30'L X 18'W X 8'H occurred in the SE NO. 4 Panel off of the NO. 3 NE Mains at the left crosscut of spad # 8424, intersection of #1 entry into #2 entry approximately 5,400' outby the 002 MMU (001 Section).

Employee hanging miner cable in 3 heading out by right corner, a rock fell out between the roof bolt and rib, striking him on the left foot. X-rays were positive for fracture on left foot.

A methane ignition occurred on the 003-0 MMU (002 Section) in the face of the number 4 heading approximately 160 feet inby spad # 11072. Located in the 2nd LT. Panel off of the No. 5 Southeast Mains.

EE was operating a track mantrip, when his left arm came in contact with a timber that was set between the belt and track.

Changing out top belt cradle, pulled up cradle to lift out. Cradle had pressure on it from the weight of the belt causing it to mash his finger between the cradle and belt rail.

A roof fall measuring 20' L X 25' W X 7'H occurred in the No. 2 SW mains in the #3 entry at spad # 1322.

A rock fall occurred in the number 2 entry 3 breaks inby spad # 1934 at break number 25 in the No. 2 SE Mains. Fall measured 20 L X 30' W X 5' H and bolted back up with 5' TT bolts.

A roof fall measuring 20'L X 18'W X 6'H occurred in the #2 entry 1 break outby spad # 1810 in the No. 2 SE Mains inby the #5 drive at break #3. Area was cleaned and bolted up using 5"" TT bolts.

A roof fall measuring 70' L x 17'W x 17'H occurred in #4 heading 1 break inby spad # 10671 in the 4th SW mains off of the 3rd SW Mains on the 005 MMU (003 section). Area was cleaned/bolted up with 6' TT and 10' cable bolts.

EE was taking old bits out of the cutter drums, knocking them out with a bit hammer and mashed his finger

EE was standing near the roofbolter and had his hand placed on a roofbolter bit when a piece of rock fell and hit him on the 4th finger on the left hand.

Employee was making the fireboss run through the mine, while traveling in the mantrip, he hit a rock that had fallen on the No. 3 Belt. The rock turned into the operator's compartment striking employee in the right side.

Co-worker was attempting to put the mantrip back on the rails. A jack was set on crib blocks. While pushing the jack bar down, the jack gave out and hit employee in the face. He was cut in the face and lost 5 teeth.

A roofall occurred one break inby spad # 9317 in the #5 entry in Panel 5 off of Panel 1 Submains (005 MMU).

A roof fall two breaks inby spad # 9298 from #5 through # 6 entries occurred in panel 5 off of the Panel 1 Submains (005 MMU).

A roof fall measuring 25'L X 14'W X 6'H occurred 30' inby spad number 6082 in the 5th RT. Panel off of the SE No.3 Panel, 002 MMU. Area was bolted with 5' TT bolts.

Employee was running centers in the No. 3 entry. The ripperhead on the miner needed let down. Employee was standing behind the miner using the remote box to drop the head. As a result the miner rocked back, setting down on his left foot.

A roof fall measuring 30'L X 18""W X 6'H occurred in the 7th RT. Panel off of No. 4 Southwest Mains in the No. 2 heading 1 break inby spad No. 9463 on the 003 MMU (002 section). Fall was bolted back with 5' bolts.

A roof fall measuring 20'L x 18'W x 8'H occurred 1 break outby spad number 7331 in the No. 4 Southeast Mains, 003 MMU. Area was bolted with 5' TT bolts.

A roof fall measuring 18W x 18L x 7H occurred 3 breaks inby spad number 9189 in panel 5 off of panel 1 submains, 005 MMU. Area was bolted with 6' bolts.

Employee was knocking out a brattice when a piece of cinder block hit him on the left arm causing a laceration to the left arm.

A roof fall measuring 52'L x 20'W x 8'H occurred on 1b belt line at spad #8005 in the panel 1 submains. Fall was cleanded up and bolted back with 5' TT bolts.

Employee was operating the offside of the LRB. He had just finished the 2nd row of bolts and pullled the machine up to begin the third row. The ATRS was set and he walked forward to swing his drill station out when a rock fell striking him on the right foot. Seam Height was 98"". Pizza Pans were installed on every bolt for the last two cuts.

EE was assisting in replacing LCM Cat wear strips. The miner was elevated on cribs for the repairs to an approximate height of 5.5 feet. Repairs were completed. EE climbed on top of the miner to take down the work area lights. To get off the top of the miner, EE got on his hands and knees and jumped off backwards. Upon landing he twisted the left ankle.

After installing roofbolts in the 2nd right break, 120' inby SS#9017, employee heard the rib crack. As he turned to move from the drill station, he was struck by a rock measuring approximately 46"" x 45"" 16"".

A roofall measuring 20'L x 17'W x 6'H occurred in the #5 heading 40' inby spad No. 8984 of 003 MMU (002 Section). Area was bolted with 5' glue bolts.

Employee was hanging cable in the No. 5 Entry and stepped on a lump of coal, causing his ankle to twist. He was unable to walk as ankle began to swell.

Member was bolting in the No. 3 entry, installing inside bolts in 2nd row. A piece of drawrock fell measuring 56"" long x 21"" wide x 7"" thick, coming in under canopy and striking him on lower right leg. Bolt spacing was less than 48"". Seam height was approximately 10'.

A roof fall measuring 20' L x 30' W x 9'H occurred in the in the #4 entry in the 10th Rt. Panel at Spad # 5490 on 7C belt. Area was bolted with 5' TT Bolts.

A roofall measuring 20' L x 20' W x 6' H occured at spad number 8269 in the number 3 entry in the 4th LT Panel of off the SE No. 4 Panel, 001 Section (002 MMU). Area was bolted with 6' TT Bolts.

EE was maiking a belt splice and hit the index finger with a hammer.

A rockfall measuring 40'L x 18'W x 5'- 6'""H occurred in in the no. 4 entry of the No 3 NE Mains (No. 9 belt) 140 inby spad No. 5176.

A roofall measuring 75'L x 20'W x 8'H occured 3 breaks inby spad number 8331 in the 4th LT Panel, 001 section (002 MMU). Area was bolted with 6' TT bolts.

A roof fall measuring 20L x 20W x 7H occured in the No. 3 entry off 003 section (005 MMU) 2 breaks inby spad No. 8045 in the Panel 1 Submains. Area was dangered off.

A roofall measuring 20L x 20W x 7H occured in the No. 3 entry off 003 section (005 MMU) 1 break inby spad No. 8004 in the Panel 1 Submains. Area was dangered off.

A roof fall measuring 60'L x 20W x 7H occured in the No. 6 entry off 003 section (005 MMU) 2 breaks inby spad No. 8054 in the Panel 1 submains. Area was cribbed and dangered off.

Member was in the bathhouse getting dressed for work, taping pants around boots. His utility knife was hanging open from his mining belt. When his right hand came around with the tape the knife cut his wrist.

Employee was cutting the 1st cut out of 2nd left break when a rock approximately 15 inches in length x 6 inches wide x 2 inches thick fell out hitting the left hand. Rock fell out between bolts causing laceration.

Employee was using channel locks to break the metal bands holding 3x8""x4x4'angle together on the bed of the boom truck when one of his bands broke hitting his right wrist causing a laceration.

A roof fall measuring 20'L x 30'W x 8'H occurred in the #4 entry 3 breaks inby spad no. 1818 in no. 2 SW mains. Area was bolted with 6' TT bolts.

EE was kneeling next to miner installing shims when a piece of draw rock measuring 25"" L x 15"" W x 1"" to 3"" thick struck him in left arm and left upper leg area. Roof was supported with 6' torque tension bolts with 8"" x 8"" bearing plates and pizza pans.

EE was helping to remove a guard from the miner. The guard fell between the cat and pan. When trying to retrieve the guard, EE cut his hand on sharp metal.

EE tripped on a rock measuring 8""l x 6"" w X 14"" to 6"" thickness and struck his left knee on a cable hanger on the operator side of the miner. He was transported to Clinch Valley Medical Center for testing and was released.

A roof fall measuring 17'L x 17'W x 7'H occurred in the #2 entry off No. 2 Section at Spad No. 7503, No. 4 SE Mains. Area was bolted with 5' bolts.

EMPLOYEE WAS PULLING CABLE WHICH HAD YELLOW ROPE HANGER WRAPPED AROUND THE CABLE WITH TENSION ON HANGERS. EE TOOK ONE HANGER LOOSE; THE OTHER HANGER WAS BENT SOMEWHAT, HOOK-END CAME LOOSE & STRUCK EE IN THE NOSE.

A roof fall measuring approximately 12'Wx14'Lx6'H was bolted with 6' bolts.

A roofall measuring 20'Wx25'Lx6'H occured in the No. 5 entry off No. 8 belt one break in by spad nO. 2345. Area was bolted with 6' bolts.

A rooffall measuring 20'Wx40'Lx8'H 25' out by spad no. 2368. Area was bolted with 6' bolts.

A roof fall measuring 24'L X 28'W X 7'H occured at the number No.3 NW mains No. 2 entry spad number 5099 area was bolted with 6' bolts.

A roof fall measuring 18' W x 40' L x 8' H occurred one break inby spad no. 3057 No. 1 NE mains. Area was bolted with 6' bolts.

A ROOF FALL MEASURING APPROXIMATELY 18' W X 50' L X 5' H OCCURRED IN #6 AND #7 ENTRY IN INTAKE AT SPAD #1928. AREA WAS BOLTED WITH 5' BOLTS.

A ROOF FALL MEASURING 40'LONG X 18'WIDE X 8'HIGH OCCURRED IN #0 ENTRY, 1300' OUTBY 003 SECTION AT SPAD NO.6570. AREA WAS BOLTED WITH 5'BOLTS.

A ROOF FALL MEASURING APPROX. 40'L X 18'W X 10'H, ONE BREAK INBY SPAD# 7180 IN NO.6 ENTRY ON OO1 SECTION OCCURRED. AREA WAS BOLTED WITH 5'ROOF BOLTS.

A ROOF FALL MEASURING 80' L X 18' W X 10' H OCCURRED 1 BREAK INBY SPAD NO 7179 IN THE NO 4 ENTRY ON 001 SECTION. AREA WAS BOLTED WITH 5' BOLTS.

A roof fall measuring approximately 22' L x 34' W x 13' H occurred at the no. 4 entry, spad no. 7216, section 001. Area was bolted with 5' bolts. Roof fall will be cleaned up.

A rock measuring 2' x 2' x 4"" was on top of the canopy of the roof bolter. When member swung the canopy, rock fell and hit his right forearm. Member received a cut requiring stitches.

A roof fall measuring 30'L x 20'W x 6'H occurred 1400' outby 001 section in the no. 1 entry. Location spad 5667. Area was bolted with 5' bolts.

EE WAS SETTING UP ROOF BOLTER TO START BOLTING A PIECE OF DRAWROCK MEASURING 16X24X1"" FELL & HIT THE EE ON LEFT FOREARM

A roof fall measuring approximately 70'L x 19'W x 9'H occurred 1500' outby 001 section in the NO. 1 entry. Area was bolted with 5' bolts. Location - spad 6350.

A roof fall of approximately 30'L x 20'W x 8' - 10'H occurred 2 breaks inby spad no. 5737 in the No. 4 entry 3000' outby No. 1 section. Area was bolted with 5' bolts. Plans are to clean up roof fall.

EE WAS OPERATING CMO IN NO.5 RT CROSS CUT. HE LOADED SHUTTLE CARS AND WAS IN PROCESS OF GETTING CABLE IN PLACE FOR REMAINDER OF CUT. AS HE BENT DOWN TO MOVE CABLE A ROCK FELL FROM MINING ROOF PINNING HIM TO MINE FLOOR. ROCK MEASURED 80"" X 24"" X 16"". ROCK FELL BETWEEN RT RIB AND MIDDLE ROW OF BOLTS.

A roof fall of approximately 60' l by 17' w by 7' h occurred at spad no. 5561 6000' outby no. 1 section in #2 entry. Area bolted with 5' bolts.

EE was pulling one piece of metal that laying against the rib of the mine between forklift and mine rib of the metal fell and he tried to stop them metal fell on rt hand and left leg fracturing finger bruising leg.

EE WAS OPERATING A CONTINUOUS MINER. DURING CLEANUP OF CUT, A PIECE OF ROCK (MEASURING 4' LONG X 21"" WIDE X 3"" DEEP) FELL OUT BETWEEN THE RIB BOLTS AND THE RIGHT RIB, STRIKING HIM ON THE BACK.

EE & co-worker were setting beams & jacks. Beams were on a flat car approximately 20"" from mine floor. Beam was raised & chained. EE moved to the other end of beam to pick up & set jack. First jack & beam became dislodged. To get out of way, EE proceeded to end of flat car & fell head first, contacting skull with sharp rock.

Member was welding belt take-up, moved across beam and twisted his ankle at 003 section belt drive.

A SMALL PIECE OF STEEL THE APPROX. SIZE OF A BB BROKE OFF OF THE FACE OF ORE OF THE HAMMER, STRIKING THE INJURED IN THE LT. FOREARM AND PENETRATING INTO THE TISSUE.

INJURED WAS HELPING LAY TRACK. A FORKLIFT WAS USED TO RAISE A RAIL IN ORDER TO PLACE TIES ON THE RAIL AND LUG THE TIE, INJURED HAD HIS FOOT UNDER THE RAIL AS HE WAS LUGGING TIES. THE RAIL SLIP PED OFF THE FORK AND STRUCK ON HIS RIGHT FOOT.

EE WAS INSTALLING THE LAST ROOF BOLT IN A PLACE WHEN A PIECE OF DRAW ROCK FELL OUT AROUND THE BOLT STRIKING HIM IN THE EAR & LEFT WRIST. HE RECEIVED 4 STITCHES IN HIS WRIST & 2 STITCHES IN HIS EAR.

TWO EMPLOYEES WERE GETTING A PUMP THAT WAS ON A PALLET BESIDE AN ELECTRIC MOTOR. AS THEY WERE PULLING THE PUMP FROM THE PALLET, INJURED'S RIGHT THUMB WAS CAUGHT BETWEEN THE PUMP & MOTOR RESULT ING IN A CONTUSION TO HIS LEFT THUMB THAT REQUIRED 3 SUTURES.

EE WAS OPERATING A ROOF RANGER II AND HAD JUST STARTED A HOLE WHEN A PIECE OF DRAW ROCK CAME LOOSE AND HIT HIS HAND. THIS ACCIDENT WAS REPORTED ON 9/14/01.

EE WAS LOADING HIS SHUTTLE CAR IN THE SECOND CUT OUT OF #6 HEADING FROM THE CROSS-CUT. HE GOT OUT AND TURNED AROUND AND WAS LOOKING UP THE CONVEYOR CHAIN OF THE MINER WHEN A PIECE OF ROCK 6"" X 10"" X 1/2""THICK HIT HIM ON THE FOREHEAD. 5 STITCHES WERE REQUIRED & A DAY LATER ON TUESDAY EVENING, A CAT SCAN.
