Struck by falling object

Employee was traveling outby between a parked continous miner and the rib when a brow and rib fell striking him on the right shoulder, causing a slight separation of the shoulder.

Employee was attempting to reset a breaker on a scoop and his hand slipped and hit a bent metal tag on the panel, causing a cut to the ring finger of the right requiring 4 stitches.

Employee was assisting another employee remove a rock from underneath the roof bolter. The other employee was using a bar to strike the rock and the bar glanced off and hit the employee in the hand causing a gouge wound requiring 3 stitches. Gloves were being worn at the time.

The employee was helping pull cables during a power move and felt a pain in his abdominal area.

Struck by falling object

Employee was attempting to remove a tracking cable from under a piece of draw rock that had fallen on the cable. Another piece of draw rock fell and hit his right hand resulting in a laceration to his middle finger requiring stitches.

Accident type, without injuries

While building a brattice line, a small roof fall was discovered that had occurred some time in the past. I was in an area that was mined more than 20 years ago. It was not in a regularly traveled area. The fall was approximately 50' long, 20' wide and 3.5' thick. Located in #2 heading in #1 Mains i break inby spad #337.

Employee was using chain hoist to pull belt together and the handle of the chain hoist flipped around and hit him in the head. He required 3 stitches to close the laceration.

Accident type, without injuries

The mine examiner and an MSHA Inspector were traveling outby areas and discovered the fall, which was approximately 50' long by 20' wide and 3.5' thick. The area was timbereed off afterward. This area was mined approximately 30 years ago.

Employee was putting bolt in hole when a piece of rock fell striking his left middle finger, cutting the end of the finger and nail. He was wearing metacarpal gloves at the time.

Accident type, without injuries

A rock fall that was 4.5 feet thick by 16 feet wide by 20 feet long was found in the number two entry one cross cut inby the portal. This fall does not effect the ventilation or travel of miners. The fall was dangered and timbered off.

Struck by falling object

Employee was taking out an eye bolt out of a electric motor that he was changing and a piece of draw rock fell and hit his right hand and thumb causing a laceration that took sutures to close the wound

Struck by rolling or sliding object

Employee was pulling draw rock with a slate bar when a piece of rock slid down the bar and hit the back of his right hand making a laceration to the top knuckle of the pointer finger that took sutures to close. He was wearing gloves.

Struck by falling object

Employee was bolting top in the #2 heading. He had put the rib bolt in and as he swung the head to install the second bolt a piece of rock fell and hit him a glancing blow to his left shoulder. He continued to work and due to complication he was put on light duty on June 22, 2012. The restricted days will be reported when he returns to full duty.

Employee asked his helper for a pinner steel extension. As he tossed the extension across the front of the bolter it hit the t-bar and glanced hitting him in the lip that caused a laceration that took sutures to close. He returned to his bolter job the next shift.

Caught in, under or between a moving and a stationary object

Employee was installing a steering jack on a shuttle car when the jack slipped out of his hand and caught his thumb on his right hand between the jack and frame of the car causing an injury to the end of the thumb that required sutures.

Over-exertion in lifting objects

Employee complained that his back was hurting after he lifted cinder blocks while a brattice was being built.

Struck against a moving object

Employee was bringing a load of trash to the surface when he ran a scoop into the rib and broke 3 ribs and hurt his shoulder. He is working on restricted duty. The restricted days will be reported when he is able to work at full duty. He reported the scoop stuck on point. The scoop was inspected by company personnel and no defects could be found.

Over-exertion in wielding or throwing objects

Employee was on his knees in the #5 heading and threw the miner cable out of the haulway when he felt a pain in his lower back. He missed no work but was on restricted duty for 2 days.

Struck by falling object

Employee was tramming the bolter out of the #6 entry and ran into a corner. The corner fell into the operator's deck causing lacerations to the head and arm. His elbow also had a cracked bone. Employee is on restricted duty and the days will be reported when released for full duty.

Employee got on a work bench to reach a can of paint and as he exited his left thigh hit the edge of an metal door that caused a laceration that took 4 sutures to close.

Struck by falling object

Employee was bolting top in the number 3 entry when a piece of rock (slick siding) fell from the rib and hit him in the head, neck, right shoulder, rib, and hip areas. X-rays were negative. He had bruising in the areas listed. The piece of rock was 10'L X 3'W x 0-12"" thick.

Employee was drilling a hole next to a kettle bottom when it suddenly fell out hitting him on the left leg and thigh causing bruises. He was going to put a strap across the kettle bottom. He stated that he checked and sounded the top before starting to drill. He was put on light duty missing no work and returned to full duty on 3-6-2011. He was restricted for 5 days.

A spare tire lying on the mantrip caught a hanging cable and drug the tire, hitting the employee in the back of the head, pushing his neck into his chest causing a strained muscle. He continued to work but was on restricted work for 3 days.

A rock fall measuring 4'H X 20'L X 20'W was found at break number 75, one (1) break outby spad number 1305 in the crosscut between the number two and three intakes. This fall did not interrupt the air flow or interfere with travel.

Employee was trying to cut a hole in a piece of belt near the air lock doors when the knife slipped and hit him on the left hand at the heel of the thumb causing a laceration that took 6 sutures to close.

Struck against stationary object

Employee went to the section drive and started the drive. As he was turning the personnel ride he backed into a roof bolt with a pizza pan on it. He bent his neck down unto his chest causing a pulled muscle. He has been on restricted duty and has a full return to work slip.

Over-exertion in lifting objects

Employee was in the face of the #1 heading dusting. He was holding a bag of rock dust and throwing it on the rib, top and bottom; as he turned with the bag of dust in his hands, he felt pain in his lower back.

Caught in, under or between a moving and a stationary object

Employee got his index finger caught between the bit wrench and a piece of stuck pinner steel in the top. He had a small laceration to the index finger that took 2 sutures to close.

Struck against stationary object

Employee was hanging the Miner cable in the number 8 heading and as he got it near the roof his left elbow hit a roof bolt on the edge of a brow that caused a laceration to it that took 8 clamps to close.

Employee was putting a rib bolt in when he let the drill pot down onto his left knee. He finished his shift of work and ended up missing work on account of a deep bruise to his knee.

Employee was loading timbers into a mine car when he felt a pain in his left shoulder. He continued to work and went to the doctor on 4-5-10 and was put on restricted duty. He is continuing to work. Restricted days will be reported when full duty requirments are met.

On 12-27-2009 a employee was pulling the cable off of the roof bolter reel when he felt a pain in his right shoulder. He continued to work but a dull pain continued. He started missing work April 01, 2010 when he was diagnosed with muscle damage that required surgery.

A rock fall was found in the Southwest Panel #5, at BEP #5 in the number one entry. The fall was approximately 5' thick X 70' long X 20' wide. The fall did not block or hinder the ventilation.

A rock fall measuring approximately 19.5 feet wide X 35-45 feet long X 54-62 inches high was found in face of the number 6 entry. Located approximately ninety feet inby spad number 4066. Fall was found at approximately 6:20 PM. The fall did not block the escape or air ways.

A roof fall was discovered in the #5 entry between spads 3989 and 3991 on the 4 southwest panel. Approximately 100 foot long, 18 foot wide, 6 foot high. The fall was in an intersection and reached into the cut-throughs toward the number 4 and 6 ertries. The air flow was not effected.

Struck by falling object

On 5-1-09 employee was bolting top when a piece of rock fell and hit him on the left arm mid way between the wrist and elbow. X-rays at Buchanan Hospital showed negative. On 5-15-09 an MRI was conducted and a fracture was obsrved.(The orthopedic dr said that it was not borken-it was only a bone bruise by Misc Mail 6/2/09 #6)

Employee was loading a SC in the face when a brow rolled off and hit him in the right shoulder. Due to complications he had an operation on his shoulder on May 15, 2009 and is missing work.

Employee was exiting a 930 loader, when he lost his footing and fell off of the steps to the ground, landing on his right knee and bending it back under his body.

While hooking up the torch car, employee twisted to get the coupling pin and felt a popping noise in his neck. He started missing work on October 14th and returned to work on October 22, 2008.

Employee said he was pushing the miner cable out of the way with his feet, as he was crawling back to the miner and hurt his back.

Trying to get the drill steel unstuck, he got his left middle finger caught between the bolt head and the drill pot.

Employee was trying to get a piece of stuck drill steel out of the top, when the drill pot chuck broke and the drill steel hit him in the mouth, cutting his upper lip that took two sutures to close.

Employee was bolting top when he got his thumb on the right hand between two pieces of steel causing a laceration that took 3 sutures to close.

Fall to the walkway or working surface

Employee was carrying a bag of rock dust when he slipped and fell to the mine floor, hitting on his right hip, knee, and thigh. Due to complications He started missing work on January 21, 2008.

Employee was bolting top, when a piece of coal with rock fell from the rib/roof and hit him in the side/back, pushing him into the roof bolter controls.

Over-exertion in lifting objects

Employee picked up a bridge cable on 6/4/07 and reported pain in his groin, and continued to work. On 9/7/07 his doctor reported that he needed surgery for a rupture. He started missing work on 9/10/07. We are reporting this but don't feel that it is a related incident.

Employee lowered the bolter head down on his left knee, twisting his knee causing a sprain.

Employee slipped and fell as he was walking around the bridge unit, hitting on his left arm and shoulder. Left arm was diagnosed as broken.
