Fall from machine

IP was working on a project to clear trees that were fallen with an excavator and the IP was cutting off limbs to allow the excavator to safely move them to the burn pile. IP was cutting a limb approximately five feet off the ground when the tree rolled back towards IP and the limb IP had just cut hit IP in the chest, knocking IP to the ground.

Struck against a moving object

EE was operating a CAT 773 haul truck when EE hit the left side of head on the inside of the operator's cab after hitting a pothole/bump in the road. After seeking medical attention, EE was given medications in the ER but did not receive a written prescription and was released to full duty.

This was a truck driver driving a tandem dump truck. EE had a rock stuck in the tail gate and decided to open the tail gate with hands to get the stuck rock out. Then EE didn't move hand fast enough and the tail gate slammed closed on EE's finger.

Employee was walking past a cone crusher when a 10""-12"" rock bounced out of crusher striking the employee's hand against the handrail.

Employess of Barnette Contractors was a passenger in vehicle leaving jobsite; driver stopped to check on some equipment, leaving passenger in vehicle. When driver returned, contractor was out of vehicle. Driver found him lying in a ditch near vehicle. Autopsy has revealed a massive heart attack.

The employee was helping put the track back together on the D-10. The chain holding the come-a-long in place snapped allowing the track to roll backwards pinning his foot between the track and the leveling bar.

Fall onto or against objects

The ee was exiting the cab of the d-10 dozer. When his foot slipped off the step on the dozer arm blade. This cawed him to straddle the dozer arm blade, causing injury to his groin area.
