EE was cleaning up spillage under #1 screen when a rock came off the chute and struck EE on the finger.

A piece of hot slag entered EE half unlaced safety boot and burned the upper part of his ankle. EE did not go to the doctor until 5/26/2006.

WHILE ASSISTING A SUBCONTRACTOR, EMPLOYEE, IN BURNING A PIN OUT OF A BOOM STICK ELECTED TO PLACE TORCH IN THE PIN HIMSELF AFTER WHICH THE BURNING ROD BURNT THROUGH THE SIDE OF THE PIN HITTING A GREASE CAVITY CAUSING PRESSURE TO RELEASE WHICH CREATED A FIRE PLUME. EMPLOYEE'S CLOTHES IGNITED & HE RECEIVED BURNS ON HIS FACE, WRIST AND LEGS.

Employee picked up a cylinder from off a pallet at the crusher station at the plant. The bands on the pallet had a jagged edge, causing the employee to cut his left hand, receiving stitches.

EMPLOYEE WAS WALKING DOWN THE MAIN HAUL ROAD RETURNING FROM CLEANING THE CONVEYOR BELTS. HE SLIPPED FACING FORWARD AND PUT HIS ARMS OUT TO CATCH HIS FALL.
