Fall from ladders

EE was climbing the ladder to hang a chain fall when the ladder slid from underneath him and he fell to the floor hitting his head on a piece of metal that resulted in a laceration to the back of his head.

Employee received a laceration to his left knee while cutting a tree down. He had notched one side of the tree and making the back cut, the tree started to and he was moving to a safe area when the saw hit his left knee.

Struck by powered moving object

EE was cutting bushing for pin on backhoe, when the grinder hung up and kicked back, striking EE below right elbow. Required 2 stitches just below right elbow.

Employee walked between line to hang halo spray bar. He started to rehang halo spray. When entering spot to reach chain, caught right boot toe on toe plate. Felt some knee pain but no medical attention needed at that time.

Employee was supervising removal of slide plate. Was attempting to hook 2nd come-a-long to ""I"" Beam when hooked come-a-long slid from ""I"" Beam falling approx. 4' hitting employee on right hand pointer finger causing possible fracture of finger.

EE WAS TAKING BOLTS OUT OF PLATE ON CRUSHER. HE WENT TO MOVE OUT THE THE WAY TO POSITION HIMSELF TO PULL THE PLATE. THE BOTTOM OF HIS SHOES CAUGHT ON THE BASE OF THE CRUSHER, RESULTING IN TWISTING OF LEFT KNEE. FIRST DAY OFF WORK 1/31/06.

EE OF MACK RD800 SHIFTING GEARS. TRUCK DID NOT GO COMPLETELY IN GEAR. EE HAD ENGINE BRAKE ON, WHEN APPLIED BRAKE, ENGINE STALLED OUT, LOST CONTROL, ROLLED ONTO SIDE.

EE STATES HE WAS COMING BACK FROM USING THE PORT-A JOHN & HE STEPPED ON A LARGE ROCK, TWISTING HIS ANKLE.

EMPLOYEE HAD POURED A BAG OF CHEMICAL INTO A FEEDER BOX. HE WANTED TO SEE HOW MUCH CHEMICAL WAS IN THE FEEDER. HE FOUND A METAL BOX TO CLIMB ONTO LOOK INTO THE FEEDER. HE LOST HIS BALANCE AND FELL TO THE FLOOR STRIKING HIS HEAD AND RIGHT HIP.

EMPLOYEE WAS WALKING TO A WORK AREA IN THE PLANT. HE WALKED UNDER A SUPPORT BEAM FOR A MOTOR AND RAISED UP BEFORE HE WAS CLEAR OF THE BEAM, STRIKING HIS HEAD. HE REPORTED STIFFNESS AND PAIN IN HIS NECK AND SHOULDERS.

EE STATES HE WAS CLEANING THE GUARD SHACK, SWEEPING THE FLOOR. HE HAD TO MOVE A SMALL COOLER TO SWEEP UNDER IT, HE STEPPED ON THE FLOOR & IT GAVE WAY W/HIM. HIS RT. FOOT WENT THROUGH THE FLOOR WHILE HIS LEFT FOOT/LEG WENT UNDERNEATH HIM.

ROAD WAS WASH BOARDED & DRIVER DEPLETED AIR OFF BRAKE SYSTEM. THE TRUCK BEGAN TO RUN AWAY & THE DRIVER JUMPED FROM THE TRUCK, STRIKING THE GROUND.

EE WAS USING A HAMMER TO DRIVE IN A WEDGE OVER THE COVER ON A DRAIN AND RINSE SCREEN. AS HE WAS MAKING A STRIKE, THE HAMMER STRUCK AN OVERHEAD BEAM, CAUSING THE HAMMER TO CHANGE DIRECTION AND STRIKE HIS LEFT INDEX FINGER. SUTURES WERE REQUIRED TO CLOSE THE WOUND.

EE WAS DESCENDING STAIRS FROM SECOND FLOOR WHEN HIS FEET SLIPPED, CAUSING HIM TO FALL. HE STRUCK HIS LEFT ELBOW, RESULTING IN A LACERATION REQUIRING SUTURES.

Absorption of radiations, caustics, toxic and noxious substances

THE EMPLOYEE DISTURBED A BEE'S NEST AS HE PLACED A BARREL OF OIL ONTO A RACK. HE WAS STUNG ON HIS RIGHT WRIST. HE IS ALLERGIC TO BEE STINGS BUT DID NOT HAVE HIS STING KIT WITH HIM. HE WENT TO THE LOCAL HOSPITAL EMERGENCY ROOM FOR INJECTION OF MEDICATION.

Struck by flying object

EE WAS INFLATING A TRUCK, WHILE IN THE PROTECTIVE TIRE CAGE. WHEN SOMEHOW THE LOCK RING BLEW OFF AND CAME OUT OF THE CAGE AND STRUCK HIM IN THE FACE AND HEAD.

EMPLOYEE WAS GOING TO HIS CAR, SLIPPED ON SOME ICE ON SOME STEPS, FELL INJURING HIS BACK.

WHILE SHOVELING COAL OFF BELT ABOVE HEAD LEVEL, EE FELT ""PULL"" AND PAIN IN LOW ABDOMEN. DIAGNOSED AS HERNIA. (NOTE: EE HGAD DR. APPT. AT 1400 ON 1-10, BUT WAITED MORE THAN 4 HOURS AND DR. NEVE R ARRIVED. HE SAW ANOTHER DR. DURING AFTERNOON OF 1-11 AND WAS RETURNED TO REGULAR DUTY.)
