Inhalation of radiations, caustics, toxic and noxious substances

The sub-contractor was found unconscious in EE's vehicle by EE's co-worker. The car was locked, co-worker attempted to wake EE. Co-worker called 911, then got mine superintendent, who came to the scene, broke the window, removed EE from EE's car, and started CPR. EE was then pronounced deceased at the hospital.

Struck against a moving object

A contracted fabricator was using a man lift on the 2nd floor of the plant to weld on the bottom of the froth cells. A section of the 2nd floor fell causing the manlift and contracted fabricator to fall to the first floor.

Flash burns (electric)

An electrician was troubleshooting and checking connections on the #1 Magnetic Separator circuit breaker. A screwdriver EE was using came in contact with energized leads causing an arc that resulted in first degree burns to EE's neck and right hand and second degree burns to EE's left hand.

Accident type, without injuries

The #3 clean Coal belt line caught fire from the take up to the head drive. The fire could not be extinguished within 30 minutes.

Caught in, under or between a moving and a stationary object

An employee had climbed up the ladder to check the feeder at the #1 hopper. While climbing back down the ladder, coal was loaded in to the hopper which caused the safety wire rope slack to tighten toward the handrail. The wire rope pinched employee's right ring finger against the handrail.

An employee received a cut on the back of the head that required sutures. The employee stated that EE leaned down to raise the elevator door on the 1st floor, then hit head on the door.

The employee was welding under the bypass crusher when a rock was washed down from above. The rock broke the lens in EE's helmet which caused a cut to EE's forehead.

A group of employees were working together to put a speed reducer chain back on that drives the rotary breaker. They were working the chain back on by turning the dodge coupling with a pipe wrench. The wrench slipped and starting spinning around in the opposite direction striking an employee on the right leg above the knee cap resulting in a deep bruise that required drainage.

An employee was attempting to free multiple small wires by cutting the zip tie that was holding them together with a utility knife. Employee received a cut below the left thumb that required three stitches.

Fall from machine

An employee lost EE's grip while attempting to go up the steps of the CAT 980M Coal Loader. EE's right hand slipped off of the hand hold causing EE to fall backwards and break EE's left wrist.

Fall down stairs

An employee lost footing while going down the set of steps from the 3rd floor MCC room to the 3rd floor of the plant. The fall resulted in a strained left shoulder that required surgery on 11/22/19.

Struck by falling object

The employee was moving a section of screen deck with a series of chains. One of the chains slipped causing a corner of the screen deck to drop on the employee's left foot. Employee broke left big toe.

Employee was driving a clip onto master link on a chain and the hammer glanced off and hit employee's left index finger, resulting in a broken bone in the finger.

Over-exertion in lifting objects

An electrical motor, wrapped in plastic, was lying on the tailgate of a pick up truck. The top of the plastic was cut and as he lifted the motor to get the plastic from underneath the motor he felt pain in his back. Due to pain in his back he started missing work on 8-23-2012.

Struck against a moving object

Driver stated while sitting in the truck and dumping a load of coal, he had his arm rested on the window of the tractor. The dump trailer was mid way up when the 2nd stage locking ring broke and allowed the trailer to fall. This caused a jolt to the truck which bruised the driver around and under his shoulder. Damage to trailer but not to truck.

Struck by falling object

Employee below was lowering materials, as the employee above cut the metal plates that they were replacing. The Plates were corroded and the concrete the plate was attached to was cracked and falling apart. The employee below grabbed the rope to lower the plate and when he did that the concrete block broke off the building resulting in hitting the employee below in the back.

Employee was tying rebar in a bent over position. Another contractor working in a chute above our work area had removed a temporary cover covering the chute opening. He then dropped his 3lb hammer and it struck our employee on the elbow. After x-rays he returned to work. He worked until 12/1/11 and then decided he couldn't work at his normal rate.

Employee walked around a trough to unhook a chain fall and stepped up onto a piece of pipe to release hook, foot slipped and twisted leg.

As employee was pulling a piece of floor grating to stand on to work on a dryer unit when he felt a sharp pain in his back.

EE states he was injured when the sledge hammer he was using deflected off a bull pin and hit him on the right shin. No medical treatment was needed at time of injury. EE developed an infection and sought med treatment on 5/14/10 where he was placed on modified duty and lost one day. RTW 5/25/10.

On February 2, 2010 an employee was helping pick up a pump barrel when he felt a pain in his left groin. He continued to work and on March 3, 2010 due to complications he started missing work.

Employee got his right hand cut when the nozzle blew off of a water hose. The tail on the ideal clamp cut his hand. It took 10 sutures to close the wound.

Employee was putting belts on a cyclone pump sheave wheel when he got the tip of his thumb on the right hand between the belt and the sheave causing a laceration that took sutures to close.

On Sept. 24, 2007 EE stained his back when he moved a bearing assembly he was hooking a chain hoist up to as it was lying in a pick-up truck bed. He started missing work on March 3, 2008.

Employee was changing out a sieve box and as he was pulling on it he felt pain in his lower back. Due to complication this employee started missing work on August 13, 2007.

Employee was lifting an air compressor frame by means of a hoist truck and the cloth strap slipped and the frame caught middle finger on right hand between frame and side of locomotive.

Employee was in the rotor breaker coal chute prying on a piece of frozen coal material when it broke loose and fouled him against the side of the chute. Coal material was approximately 1 to 1.4 tons.

Employee stepped on a piece of grating that was over a drain and it flipped up and he fell approximately 16 inches into the drain channel.*The reason the form was late was the time involved from the injury date (3/27/2004) & first day missed (5/12/2004) was questionable and we waited on a doctor report to confirm the injury*.

THE ROTOR BREAKER HAD A BLOCKAGE AND WAS SPILLING OIL. EE WAS TRYING TO UNSTOP THE BLOCKAGE AND AS HE WAS GOUGING AT THE BLOCKAGE WITH A SHOVEL HE HIT HIS RIGHT KNEE ON A PIECE OF METAL.

EE WAS OPERATING A TRUCK WHICH WAS LOADED WITH SLUDGE. HE ROUNDED A SHARP TURN AND AS HE WAS PARTIALLY ON A SET OF SCALES, THE TRUCK STARTED SPINNING AND SLID SIDEWAYS. THE REAR WHEELS SLID OV ER A 3' DROP AND THE TRUCK TURNED OVER. THE WEATHER WAS SLEET, RAIN AND SNOW MIXED AT THE TIME OF THE ACCIDENT.

EE HAD A DRYER BASKET HANGING FROM A CHAIN HOIST AND AS HE WAS PUSHING IT INTO PLACE THE BASKET TWISTED AND FELL HITTING HIM ON THE SIDE OF HIS LEFT FOOT.

EMPLOYEE WAS HELPING CHANGE AN IMPELLER OUT ON A DELUTE PUMP. HE WAS HOLDING THE IMPELLER ON THE PUMP SHAFT AND IT STARTED FALLING OFF. HE PUSHED HARDER ON THE IMPELLER AS IT WAS COMING OFF AN D FELT A PAIN IN HIS LEFT SHOULDER AND ALSO HEARD A POPPING NOISE IN HIS SHOULDER.
