Struck against a moving object

EE was operating a loader while working on a ramp in the plant yard. The bucket was fully loaded, and fully raised when the loader tipped over on its side.

Contact with hot objects or substances

Employee had finished using a cleaning solvent and lit a cigarette. Ashes from the cigarette caught residual solvent on the employee's shirt on fire.

IW was in chute above P4 tail pulley when EE's hand was impaled with a piece of steel that was sticking away from the wall causing a laceration to EE's right hand.

Struck by falling object

Employee was cutting a piece of wear metal out of a chute. As employee was cutting, the 42-inch x 21-inch x 1-inch plate popped free and smashed their left thumb between the chute and the plate.

Employee was onloading a small pipe bender by hand. When EE set the bender on the ground the handle came around and hit the employee in the ear, resulting in a laceration to the ear lobe.

On 6/6/19 at approximately 1330 hours WAH employee #11992 was using a metal pry-bar and the suction hose from a VAC-CON truck attempting to locate an existing water line. The pry-bar was sucked by the vac hose, pinching their left hand index finger and middle finger between the pry-bar and the hose.

Fall from machine

Employee slipped while attempting to get into track loader. Employee sprained their left ankle. Did not want to seek medical attention on injury date.

Struck by flying object

EE got a piece of concrete in EE's left eye while working in a man hole. Concrete flew onto EE's safety glasses and into EE's left eye causing 2 abrasions. EE was taken to the ER.

Employee was helping move a 150lb plate when right thumb got caught in between the two overlapped plates resulting in a laceration and fracture.

Rubbed or abraded

Employee was in the process of assisting in removing loader bucket hardware on a 988K loader. Employee reached under the bucket to push the bolt up as another employee spun the bolt. This resulted in a laceration to right middle finger. (3 stitches)

EE was welding and a piece of hot slag rolled into his boot, EE had his pant leg tucked in. EE worked the rest of his shift and part of the next before it started to swell. EE went back to his house and later to the hospital. EE had an infection set in and his Dr's. waited until the swelling went down before releasing him on 8/16/13 and back to full duty 8/20/13.

A splinter entered the EE's left forearm when he brushed against a rail tie while assisting another EE load a front-end loader onto a lowboy trailer.

Employee's right arm was pinned beneath the handrail and in the roller at the same time as his hand was drawn into the troughing roller while inspecting the #2 conveyor belt, receiving ten stitches and abrasions to multiple areas of right arm/hand while trying to free himself.

Employee was in the control booth and walked out the door, stepped with his left foot and twisted his ankle. He did not step on any obstruction.

Caught in, under or between a moving and a stationary object

Ee was removing the center rail hold down bolt with a ratchet wrench. The wrench slipped off the bolt head and pinched his finger between the socket and the angle bolt head protector causing the accident.

Struck against stationary object

Working on the middle deck of a creen shaker and the screen was handed into the shaker the wrong way. Ee then had to turn the screen around before sliding it into place. Also note that this did not become a reportable accident until September 1, 2004

WAS THROWING AWAY OLD ELECTRIC PUMP & OTHER STEEL INTO THE BUCKET OF A BOBCAT. STEPPED OVER THE METAL, GETTING READY TO TAKE IT TO THE DUMPSTER, WHEN HE NOTICED ANOTHER PIECE OF STEEL. HE WENT TO GET OUT AND CAUGHT HIS HEEL IN THE ELECTRIC CORD TRIPPING AND INJURING RIGHT ANKLE.

CHANGING THE SHAFT ON THE DRIVE HEAD PULLEY OF CONVEYOR 15, AFTER REMOVING THE SHAFT TO ADJUST IT SO WE COULD PULL IT OUT THROUGH THE ROOF OF THE TOWER, THE SHAFT SLID DOWN AND SMASHED THE LEF T INDEX FINGER BETWEEN THE SHAFT AND THE FRAME. IT REQUIRED FIVE STITCHES.

TRUCK DRIVER TOLD ROCKBREAKER OPERATOR THAT HE HAD AN OIL LEAK. THE OPERATOR PULLED OVER AND SWUNG MACHINE AROUND. IT WASN'T FULLY LEVEL. HE CALLED FOR A MECHANIC. MECHANIC & OPERATOR ADDED OI L TOT HE MACHINE. OPERATOR WAS ASKED TO START MACHINE. GOING DOWN TO START MACHINE, HE TRIPPED OVER THE OIL CAN LEFT ON THE CATWALK, FALLING ONTO THE TRACKS HURTING HIS RIGHT SIDE BACK AREA.

D AMAGE OR IMPAIRMENT WILL BE CONDUCTED. THIS REPORT IS BEING SUBMITTED TO SATISFY MSHA PROGRAM POLICY, LETTER P92-111-2.

EE WAS USING A 988 CAT LOADER TO SELECT ROCKS FROM A PILE ABOUT 15 FEET TALL. WHILE RETURNING TO THE PILE AFTER SETTING ASIDE ONE OF THE ROCKS. A ROCK ROLLED DOWN THE PILE AND HIT THE LOADER B UCKET WHICH WAS ABOUT A FOOT ABOVE THE FLOOR. WHEN THE ROCK MADE CONTACT WITH THE BUCKET IT JARRED THE OPERATOR AND INJURED HIS NECK.

EE WAS REMOVING A PIECE OF WEAR METAL FROM A WA-600 KOMATSU BUCKET. HE WAS USING A SLEDGE HAMMERTO HIT THE PIECE OF WEAR METAL IN ORDER TO KNOCK IT IN TO THE RELEASE POSITION. THIS WAS A SPARE LOADER BUCKET AND WAS SITTING ON THE YARD. IT WAS SLIGHTLY WEATHERED AND RUSTY. WHILE HITTING THE WEAR METAL WITH THE SLEDGE HAMMER A PIECE OF METAL BROKE OFF & PUNCTURED/LODGED IN HIS SHIN.

WHILE SERVICING EQUIPMENT IN THE SERV LANE IN THE SHOP EE WAS WALKING AROUND THE EQUIPMENT WHEN HE STEPPED IN SOME MUD THAT HAD DROPPED OFF SOMEOF THE EQUIPMENT THAT WAS BEING SERVICED. THE EE SLIPPED & FELL BACKWARDS AS HE TRIED TO BREAK HIS FALL WITH HIS RIGHT ARM. THE CONDITIONS OF THE GROUND OUTSIDE OF THE SHOP WAS MUDDY DU TO RAIN THE NITE BEFORE
