Over-exertion in lifting objects

Employee was working with another employee to change the idler rack when he felt a pop in his shoulder that caused him pain and limited mobility in his shoulder. He did not seek medical treatment until 6/16/2015.

Employee was found unconscious sitting in parked welding truck while the engine was running. It is believed the employee died of natural causes.

Contact with heat

While performing maintenance during hot weather the employee did not realize he had become dehydrated and at the end of the job showed heat related symptoms and was later diagnosed with heat exhaustion.

Struck against stationary object

Employee received laceration to his left pointer finger from a roll pin inserted into the impact extension.

Employee was removing a piece of wear metal from feed chute when metal slipped out of his hand causing a laceration to finger.

Two employees were replacing UMHW Chute Liner at Vulcan Materials Stafford Plant. One employee was holding the liner while the other employee drilled a 1/2"" hole in it. While the hole was being drilled the liner moved causing the employee holding it to move it back in place at which time his glove got caught in the drill bit pulling his hand into the moving bit.

Struck against a moving object

A Cat 769 D water truck owned by Vulcan Materials and a 2003 Sterling dump truck owned by a customer, collided on the plant entrance road. The customer dump truck operator later complained of back pain and was advised to stay out of work.

Lost foot grip on ladder of haul truck resulting in twisted ankle which caused fracture of ankle.

Fall to the walkway or working surface

Driver got out of his truck to approach dispatcher's window to see if he was needed for a load, he made 2-3 steps, slipped and fell on icy road.

Employee was using 1/2"" pull handle to remove bolt from exhaust manifold, socket slipped and caught left ring finger between exhaust stud and pull handle.
