Fall down stairs

Fell down stairway. Talked to man that fell. He had no recogonition of how he fell. No metal skids on wooden steps and improper illumination. Was violation statements.

Caught in, under or between a moving and a stationary object

Employee was standing behind a 3-wheel ride and was told that it was going to be moved. The ride moved in the wrong direction and caught the employee between the ride and feeder.

Struck against stationary object

Employee was walking in the shop when he stepped on a piece of curtain that had a piece of wood under it and he tripped and twisted his left ankle.

Over-exertion in lifting objects

Employee was shoveling the #4 belt drive in a stooped position and when he stood up his back started hurting and left leg went numb.

An accumulation of methane was found from out gassing seals in a small isolated area. We consider this a non-inundation incident.

A rock fall was found at crosscut #45 blocking the primary escapeway (#7 entry) by the pre-shift foreman. The fall is approximately 35' long X 5' high X 20' wide.

Employee complained of his back hurting on 1-7-09 and started missing work on 1-14-09. Doctor's report shows that the employee is suffering from degenerative facet disease.

Employee was tramming the Miner away from the rib to service it when he got caught between the deck and rib.

Employee was operating a three wheel mantrip going down a slope, when he steered to make a turn the three wheeler turned over causing injury to his shoulder and neck. Due to complications he started missing work on February 05,2009.

On 6-4-08, employee had a piece of rock fall from the top and hit him on the right foot as he was bolting top and continued to work. His foot swelled up and he started missing work on 6-10-08 and returned 6-17-08.

A piece of draw rock fell and hit the employee in the upper back and neck area as he was operating the bolt machine.

Emp. was operating a bridge when his light got into the roof and it bent his head backward causing a strain to his neck.

A rock fall occured 2 breaks inby spad number 3176 in the crosscut between the number 4 & 5 entry and partly into the 5 entry. This was reported to MSHA at 9:30 PM. The fall was 60' long, 20' wide, and 5' thick.

Employee was building seals. He pulled on a water hose and the stick type valve came on and the hose whipped and the valve hit him on his left cheek (face).

EE was drilling in rock to install roof bolt. Piece of draw rock fell, hitting his upper left arm and cutting it. Took 17 staples to close wound.

Employee was on top scoop greasing drive line, when he got back off top of scoop he hit his right leg below knee cap. The intersection was 37 inch high- the scoop was 27 inch high, the intersection was 1 break out by spad #2911.

Employee was riding mac 8 mantrip, mantrip hit coal rib; coal rib rolled off hitting right side of employee.
