EMPLOYEE WAS MOVING OXYGEN TANKS, HAD AN OXYGEN TANK SLIDE FORWARD AND EMPLOYEE PUSHED IT BACK IN THE BRACKET TO HOLD BOTTLE AND SMASHED MIDDLE FINGER ON LEFT HAND, LACERATING THE FINGER.

EMPLOYEE WAS CLIMBING ON LOADER AND REACHED FOR TOP OF HANDLE TO PULL SELF UP. EE FELT SOMETHING POP IN LEFT FOREARM. WENT TO ER, THEY TOLD EE IT WAS A STRAIN, EE WASNT GETTING ANY BETTER HAD MRI DONE AND RESULTS WAS A SMALL TEAR. HAD OUTPATIENT SURGERY ON 04/10/2023, HASNT MISSED ANY WORK

Employee was standing beside another employee that was using a hammer to loosen a valve when a piece of metal came off valve and struck said employee in right eye.

Changing vibrator spring, the spring slipped caught left thumb between spring and vibrator frame.

Employee stepped out of sweeper truck, when their right foot made contact with ground their ankle turned over, twisting it. They returned to work on their next scheduled shift and started missing work on Monday July 24.

Employee bent over to pick up ratchet and his back started giving him trouble.

Struck against a moving object

Conditions outside were foggy, truck driver miscalculated the end of the scale ramp causing the truck to overturn onto the passenger side. This resulted in the driver having a fracture of the right knee

Employee had been working on cutting out deck on vibrator. While he was going in and around deck to cut bolts he strained his back. Employee did not report accident until 05/19/2014, he worked 5/20 and 5/21 then went to the doctor which scheduled him off work for 5/22 and 5/23. Due to holidays there was no work scheduled for this employee until 5/26/14.

EE was removing metal Turbine (fan) from box at warehouse when he knocked the fan over. When he reached to grab the fan to keep it from hitting the floor he cut the outside of his little finger on his left hand.

While in the process of draining thickener, employee stepped in mud that went over his boots. Apparently there was caustic in the mud causing burn to left foot. Employee tried to work but burn got worse. Started missing 2-21-05.
