Associate backed the haul truck under the conveyor to load Mineral filler. Preliminary findings indicate that the associate climbed to first level platform under the silo hopper. While standing on the platform, the upper section of the hopper detached from the silo. The entire hopper, along with contents, then collapsed onto the platform, engulfing the associate.

Associate was welding on skid steer bucket. Another associate came over to assist to complete the job. Both were welding back to back resulting in light reflecting on the inside of the welding shield. Associate noticed eye irritation the next day when the associate awoke.

While removing hand railing & using a wrench to loosen the bolt on the railing. As the associate with the ratchet loosened the bolt the wrench started to lose contact with the railing. EE reached in to reposition the wrench as the other associate pulled/pushed on the ratchet causing EE's finger to get pinched between the railing and the wrench.

EE was washing out the material in the bottom of a load out bin. EE slipped on some loose material causing EE to fall onto a shelf.
Associate was welding wear plates in the cavity below the head and bowl of the gyratory crusher. During the welding process some loose material from the feed hopper fell, with a softball size rock striking associate on the right side of head.
The area near the primary had the snow plowed off of it the day before in preparation for the work to be performed. Overnight this area had frozen. EE got out of the truck & walked to the back where EE got salt to spread on the walkway. While spreading salt EE slipped on the ice and fell hurting EE's right leg and knee.

While cleaning out ice build up in the socket sealing ring, the associate made contact with one of the edges on it, resulting in a laceration on the back of the wrist through the glove. The laceration resulted in 7 stitches.

While removing a zip tie from a hardhat, employee received a small puncture wound from the cut end of a zip tie. There was no immediate pain or even blood, so employee shook it off and went about the day. The next morning the finger had become very swollen and infected.
At the end of ee shift, EE was exiting the haul truck when ee felt a pain in back.

Employee had lifted the lid on the transfer box and climbed in to measure some wear metal. Lid was held open by a chain. When reaching to reposition self the lid fell down onto EE's hand. We believe that the wind, the employee touching the chain with EE's hand or vibration of the chute from EE's movement caused the lid to close.

Associate was working overnight to assist with the recovery mission with the trapped associate at our Leesburg Plant. The next morning the associate became disoriented.

Associate was using a shovel to clean up a small pile of material. As he started shoveling he felt a pain in his lower back.

A crew member was using a 5"" Dewalt grinder to cut off excess angle from a mid rail. The employee was holding the angle with his left hand and grinder in right. There was no handle on the grinder and when it got in a bind it jumped into the employee standing one step down, striking him in the Left Arm just below the elbow.
To avoid the rain, the associate was running from his pick-up truck to the shop. While running the associate suffered several sharp pains in his knee area.
While cutting and grinding, the employee became over heated and was treated the following day at a walk in clinic.
Associate was using a bar to clear fines from a chute when a significant amount of material broke free forcing his arms downward injuring his shoulder

While walking the associate stepped in the area where Conveyor catwalk and the stairway to Conveyor and the Crusher Platform meet. This is an area with a gap where two pieces of Grip Strut are side by side with one at a downward angle and the upward stairway. EE said that he apparently had stepped the area and heard his right knee pop.

Associate crawled across the frame/motor portion of the plant to access the stop switch. He slipped and cut his finger on the metal cover of the hydraulic tank and hoses.

Assoc was opening an underground valve at water truck fill station by turning a T-valve. Got it to the stop point, wiggled it a couple of times to make sure it was fully open. When he finished, he felt immediate pain in the left arm.

Troubleshooting the hydraulic adjustment system on the crusher. Installed a pressure gauge on the pump by attaching the pressure hose to a malleable black iron tee that had the pressure gauge threaded in the other end. Pump was turned on and hydraulic pressure split the tee that was in his hand. The hydraulic fluid, under 4000 psi, cut his thumb and damaged a tendon.
EE was preparing to use a cutting torch. His cutting goggles were on his hard hat. As he was pulling them down to put over his eyes, he got a foreign object in his eye. *This is being reported to MSHA late because it was not officially reported internally to Luck Stone until 5/24/07.

Assoc. using backhoe attachment of skid steer loader to dig ditch to uncover damaged wires that needed to be repaired at pug mill. Assoc. also digging ditch when closer to wires. After lunch complained of sore back, thought it was from the jerky motion of equip, combined with hand shoveling.

Associates were using steel tubing suspended from crane as a ram to strike a nut wrench. One assocate was guiding ram by holding the lifting eye, when swung ram missed nut wrench pinning associate's hand between wrench and ram.
