Employee was restacking a pallet of pulverized limestone 50 pound bags. EE said put one knee on the ground and picked up a bag and felt a burning sensation in the lower back.

EE was working on filling the newly installed greaser pump up on the Wa 600. When EE stepped back to see how full the tank was getting EE lost balance and fell hitting the floor. The rim of EE's glasses cut EE just above the eyebrow on the left side. Put a bandage on it from the first aid kit.

Performing close proximity maintenance on mill fan, hand contacted belt at pulley. Cut middle finger on right hand, needed 6 stitches.
Employee began working on the truck around 9:30 am. Employee performed a mental risk assessment. Removed the drive shaft from under the truck and put on tandems to work on. 1 of 2 caps were almost installed (with hammer) when driving the second cap in the drive shaft yoke employee struck their thumb with the hammer.

Employee was filling bulk bags of mine safety dust (ground limestone powder). After filling bag employee removed filling spout and dust splashed into his face. Employee washed out eyes but was unable to remove contact from 1 eye and needed medical attention to remove.

Maintenance worker was cutting metal from hopper when hot slag landed on boot and burned through. Worker suffered burns to top of foot before he could remove boot.
It was a reportable hearing loss, the standard threshold was more than 25db on the 2000Hz on his right ear.

Employee was performing fabrication (welding, grinding and chipping slag) and noticed irritation in eye. Employee was wearing PPE including welding helmet and safety glasses. Employee should have also utilized safety goggles.

EE and another employee were removing a guard off the tail section of C-10 belt conveyor. Guard shifted and pinned his left hand against a metal beam.
Slip and fall on frozen ground, hit face on ground, got 5 stitches in forehead and 1 in his lip.

Employee was removing a fuel tank on a mack truck and got his hand caught between the tank and the metal frame. Cut his ring finger on his right hand. He received six stitches and came back to work at his regular job.
Employee was helping unload a heavy leaf spring for a Mack Truck, the leave fell and smashed his index finger on his right hand against the ground.

This is a result of average hearing loss of 25 db or more in the range 2000, 3000 and 4000. His loss was 30 db in his left ear.
EMPLOYEE UNLOADING 50 LB PAIL OF GREASE OFF BACK OF TRUCK. HE ""TWISTED HIP, I THINK I JUST PULLED A MUSCLE"". REPORTED INCIDENT AT 4:45 PM TO ASHEVILLE OFFICE. HE REFUSED TO GO TO URGENT CARE TWICE- INSISTING ON SEEING HIS CHIROPRACTOR- WENT TO URGENT CARE ON 6/27/06 ON LIGHT DUTY FOR 5 WORKING DAYS TO BE REEVALULATED ON 7/7/06 FOR LOW BACK SPASM.
Employee was welding, starting and stopping a lot and during this time got several flashes of bright welding to his eyes. It was dark during this time also which made it worse. Eyes itched and burned later that night so employee went to doctor and got medicene for his eyes and that was the extent of the injury.
He was cleaning debris from an auger trough and pealed back open area of trough and his finger slipped into hole and hurt his finger. It cut the tip of his left middle finger off, took him to the doctor and they put stitches in it and sent him back to work. Not a lost time accident
Employee was adjusting plastic top sheeting inside caged area that was properly shut down. He turned and cut himself on a piece of metal.

Employee was dismounting the Stationary ladder on an open top railroad car, he lost his grip or footing and fell approx. six feet. He landed on his left wrist and left hip, this fall fractured his left wrist.
Co-worker was using a punch to drive a bearing off of a shaft; injured was standing near him when he hit the punch with the hammer. A small piece of metal (about the size of a BB0) broke off the punch and hit injured in the neck. This piece of metal stuck in his neck.

He was stepping up into loader when the bottom cable step broke, caused him to hit his right side ribs on the ladder. He was ask to go to the doctor and he felt he was alright. Then on 1/4/2005 he felt a need to go. The doctor said he had bruised only.Then contacted him later saying he had fractured a couple of ribs. No lost time, he put a wrap around his ribs for a few days.
Employee was to install a new spout on a bagger and it slipped from his grip and hit him in the mouth. It cut the upper lip on his mouth. He went to the doctor and got 3 stitches in his lip and a Tetanus shot.
Ee was using pry bar to loosen lid on Fuller Kenyon< his bar was placed, when the lid broke loose, his body was in an awkward position and he hurt his back. Dr said it was a pulled muscle and recommended physical therapy.

EE WAS STACKING 50 LBS BAGS ONTO A PALLET.
EE WAS WALKING ACROSS THE LOT TOWARDS THE PLANT WHEN HE STOPPED TO HELP ANOTHER EE WHO HAD DROPPED A LOADED BULK SACK ONTO THE LOT. HE HELD ONE OF THE LOOPS OF THE BULK SACK WHILE THE OTHER EE ATTEMPTED TO PUT THE FORK OF THE FORKLIFT INTO THE LOOP. THE FORK CAME IN TOO FAR AND CAUGHT EE'S LEFT MIDDLE FINGER CUTTING IT, REQUIRING 3 STITCHES.
EE WAS DESCENDING A VERTICAL LADDER. WHEN HE STEPPED DOWN TO THE PLATFORM (MAY NOT HAVE BEEN ON BOTTOM STEP), HE LOST HIS BALANCE AND FELL AGAINST HANDRAIL CAUSING INJURY.
THE EMPLOYEE WAS WALKING ACROSS A CONCRETE FLOOR INSIDE OF A BUILDING, TO GET A SAMPLE BAG. HIS RIGHT FOOT SLIPPED AND HE FELL CAUSING AN INJURY TO HIS LEFT KNEE. THE EMPLOYEE HAD JUST COME BA CK TO WORK AFTER HAVING KNEE SURGERY ON THE SAME KNEE (PERSONAL INJURY). THE EXTENT OF INJURY IS UNCERTAIN, BUT EMPLOYEE MAY HAVE TORN LIGAMENTS.

WHILE ATTEMPTING TO ADJUST THE POWERED BAG CHAIR ON THE BAGGING MACHINE EMPLOYEE CONTACTED THE ACTUATOR BUTTON WHICH CLOSED THE BAG CHAIR ON HIS LEFT THUMB.
WHILE RE-STACKING DAMAGED PALLETS OF BAGGED MATERIAL EE STEPPED FROM AN EMPTY PALLET ONTO A CLEAN ASPHALT SURFACE AND APPARENTLY TWISTED HIS KNEE.
EE REPORTED ON MON. MORNING 3/12/01 THAT HE HAD HURT HIS BACK ON THURSDAY 3/8/01. HE WAS NOT SURE EXACTLY HOW OR WHAT TIME THE INJURY OCCURED. HE LATER SAID HE THOUGHT HE HURT IT MOVING SOME C ONCRETE FORMS. THE FIRST DAY MISSED DUE TO THIS INJURY WAS 3-31-01. NOTE THIS ACCIDENT CLAIM IS BEING DISPUTED DUE TO NOT REPORTING ON A TIMELY BASIS.
WHILE OPERATING A FORKLIFT ON A VERY RAIN DAY, EE GOT WET AND COLD AND DEVELOPED HYPOTHERMIA. FORKLIFT HAD A CANOPY BUT RAIN WAS BLOWING IN.

WHILE ATTEMPTING TO REPOSITION A PNEUMATIC BAG CHAIR EMPLOYEE BUMPED THE BAGGER START BUTTON & MADE THE BAG CHAIR CYCLE DOWN PINCHING & CUTTINGHIS RIGHT THUMB & FOREFINGER.
WHILE HELPING A MAINTENANCE MAN LOAD A SMALL SHEET OF METAL ONTO HIS TRUCK EMPLOYEE CUT HIS FINGER ON THE EDGE OF THE METAL. CUT WAS ON THE TIP OF HIS RIGHT RING FINGER AND REQUIRED 7 STITCHES .
WHILE ADJUSTING THE FILL SPOUT ON THE DECORATIVE STONE BAGGER, EE'S LEFT PANT LEG GOT HOOKE BY THE CONVEYOR SPLICE, PULLING THE MATERIAL INTO THE 1/4"" GAP BETWEEN THE GUARD AND THE CONVEYOR PU TTING PRESSURE ON HIS KNEE CAP. ALL NORMAL PINCH POINTS WERE GUARDED BY THE FACTORY. THE ROCK GUARD INVOLVED WAS ADDED ON SIT. WE DID NOT REALIZE WE WERE ACTUALLY CREATING ANOTHER PINCH POINT
