Employee was replacing the floor board on the flat bed section of the truck and was welding shims underneath the floor of the bed when he got debris in his eye. Employee did not seek medical treatment until 11/27/13.

Employee picked up a quarter keg grease container and carried it 8-10 feet into a building when he felt a tingle and pull in his shoulder.

Employee was changing the cylinder on a loader. While taking the pin out of the bottom using a two block system the block slipped catching the employees ring finger between the block and the pin causing a laceration to the knuckle requiring stitches.

THE AIR IMPACT WRENCH WAS PLACED ON THE ADJUSTMENT RING OF THE CONE CRUSHER WHEN ANOTHER EMPLOYEE ACCIDENTLY HIT THE AIR HOSE CAUSING THE WRENCH TO FALL APPROX. 10' STRIKING THE EMPLOYEE ON TO P OF HIS HARD HAT THEN GLANCING OFF HIS LEFT SHOULDER THEN STRIKING HIM IN THE LOWER BACK.
