Employee was removing a notch from a structural steel beam while holding the bottom flange with left hand. When the piece was separated, the notch dropped on employee's left hand causing lacerations to index & middle fingers. The employee received 8 sutures & was returned to work without restrictions.

The EE was walking up catwalk & ducked out of the way to avoid a water drip & struck his head against the water feed pipe to the manifold.EE lost his balance and begin to fall backward. EE struck his forearm on the water feed pipe in his attempt to catch himself. EE did not go to the doctor until 3/7/08.

REPLACING BUCKET TEETH ON LOADER, ONCE THE PINS WERE REMOVED THE EMPLOYEE USED A SMALL SLEDGE HAMMER TO KNOCK THE TEETH OFF. WHEN HE HIT THE TOOTH A SMALL PIECE OF METAL SPLINTERED FROM THE TOOTH. IT PUNCTURED AND LODGED INTO HIS RIGHT KNEE. THE WOUND WAS CLEANED AND DRESSED WITH A BANDAID. A WEEK LATER THE SITE BECAME PAINFULL AND SWOLLEN. HE WAS GIVEN ANTIBIOTICS.

EMPLOYEE WAS ATTEMPTING TO ALIGN RETAINER PIN HOLES ON LOCK COLLAR WITH HAMMER. HOLDING WITH LEFT HAND STRIKING WITH RIGHT HAND, HAMMER GLANCED OFF METAL AND HIT LEFT HAND BETWEEN THUMB AND IN DEX FINGER.
