person was holding a tool while another person used a hammer to tap loose a shim. handle of hammer hit another object and cause the hammer head to miss the tool striking person's pinky finger. finger sustained fracture and required 6 stitches.

While moving the vulcanizer out from under the conveyor belt, a retaining bolt moved and caught the tip of the belt tech's finger, injuring the finger which required medical attention.
Person Injured themselves when conduction maintenance work on a haul truck brake system. Person was lifting a brake disc and injured their back muscles. The injured person went to the ER when back pain was not controlled by over the counter pain medication. ER documented injury as strained back muscles and instructed person to take 4 days off work to recover.

Employee's were involved in adjusting a motor base to replace the motor drive belts, when the motor mount bolts broke and struck employee in the lower right leg.
While standing with one foot on a catwalk and the other on a hyrdaulic hose bracket the employees foot slipped. He slipped about 18-inches down and hit his ribs onto the hydraulic lines. Left side ribs (6th & 7th) suffered a non-displaced fracture.

EE left index finger was lacerated while trying to remove a plow bolt on the cutting edge of the WA-900 loader with a impact wrench. The spinning bolt cut into his finger. Durabond was applied.
EE sprained his left knee after stepping off a locomotive. He went to the doctor on June 23.
Employee's tooth was broken after the Derrick hoist control box struck him in the mouth while he was pulling on it.

WHILE OPERATING A LOCOMOTIVE, EMPLOYEE STRUCK HIS RIGHT ELBOW WHEN THE LOCOMOTIVE CAME TO A SUDDEN STOP. INITIALLY, THE INJURY REQUIRED FIRST AID TREATMENT ONLY. HOWEVER, HE CONTINUED TO HAVE PROBLEMS WITH HIS ELBOW. HE WAS REFERRED TO AN ORTHOPEDIC SPECIALIST WHO RECOMMENDED SURGERY WHICH OCCURRED 12/19/03. EMPLOYEE RETURNED 1/5/04 AND RECEIVED FINAL RELEASE FROM DOCTOR 2/12/04.

EE WAS REPLACING THE METAL SKIRT BOARD ON CONVEYOR #1. THE PIECE OF METAL HE WAS WORKING WITH TILTED TO ONE SIDE AND PINCHED HIS RIGHT RING FINGER AGAINST THE IMPACT SLIDE BENEATH THE CONVEYOR BELT. THIS RESULTED IN A FRACTURE TO TIP OF FINGER.

EMPLOYEE DID NOT MAINTAIN 3-POINT CONTACT WHILE CLIMBING ON LOADER. EMPLOYEE FELL APPROX 7 FEET AND SUFFERED FRACTURED RIGHT ARM.
