Caught in, under or between a moving and a stationary object

While trying to roll the old drive belt off the sheave, EE's finger got caught in-between the belt and sheave pinching their right index finger.

EE went to throw the rail switch when EE felt a pop in right arm. Did not seek medical treatment until 4/8/2023

EE was filling a blast pot when EE's finger became pinched between lid and pot when someone put the lid back on. EE went to the emergency room, no stitches and not broken. Took off the day after but went back to work 12/3/21.

Fall to the walkway or working surface

A repair supervisor was injured after obtaining the location of a hydraulic hose on a haul truck. The location was needed to facilitate the repair of a similar truck. After obtaining the information, the employee stumbled and fell while returning to the step ladder. The fall resulted in a fractured collar bone.

Employee was working on pouring a concrete pad for C-1 below primary crusher. While floating and finishing the pad, employee was standing in the concrete when his feet got wet.

Employee operating man basket lift in close to chute being dismantled. Man lift became hung on metal angle iron. Employee had his head between the control station and chute when it dislodged catching employee's head.

The employees lower back was strained while repairing a Euclid R-65 hauler using a 3/4 inch drive ratchet with a four foot handle. The employee went to the doctor on 3/21/04.

EE WAS REPLACING THE BACK, RIGHT FEEDER BOLT ON THE SYNTRON FEEDER. A COME-ALONG WAS BEING USED: ONE HOOK ATTACHED TO A CLEVIS ON THE FEEDER, THE OTHER HOOK ATTACHED TO THE FEEDER ROOF HANGER BOLT. EE WAS HAND-TIGHTENING THE FEEDER BOLT WHILE ANOTHER EE WAS LIFTING WITH THE COME-ALONG. THE FEEDER ROOF HANGER BOLT BROKE CAUSING THE SPRING CLUSTER TO FALL. HIS HAND WAS CAUGHT IN IT.

EE WAS IN THE PROCESS OF REMOVING THE DISTRIBUTOR PLATE FROM THE HEAD OF A 1560 OMNICONE CRUSHER WHEN HIS RT FOOT SLIPPED DOWN INTO THE CAVITY CAUSING HIM TO STRAIN LIGAMENTS IN HIS KNEE.

EMPLOYEE WAS IN THE PROCESS OF REMOVING A FILTER HOUSING FROM THE DRILL, AS EMPLOYEE WAS REMOVING ONE OF THE LAST BOLTS FROM THE HOUSING IT PIVOTED ON THE REMAINING BOLT CAUSING HIS FINGER TO BECOME WEDGED BETWEEN THE HOUSING AND THE COMPRESSOR TANK. THE BOTTOM PART OF THE HOUSING HAD BEEN REMOVED EXPOSING A SHARP EDGE.

EMPLOYEE WAS OPENING A BUCKET OF GREASE, CUTTINGTHE PLASTIC LID ON TOP; HE WAS EXERTING PRESSUREWITH BOTH HANDS WHEN HIS HAND SLIPPED, CAUSING THE BLADE TO FLIP OVER, CAUSING LACERATION.
